A single coronary artery is a rare congenital anomaly. The right coronary artery, originating from distal left circumflex, is an extremely rare variety of a single coronary artery. Our report is accompanied by a brief review of the literature.

Spontaneous coronary artery dissection (SCAD) and woven coronary artery anomaly (WCAA) are relatively rare. A few of the previously reported woven coronary artery cases have involved in a single coronary artery. We present an unusual woven case involving all coronary arteries and two patient with SCAD. We have also reviewed the literature related to these disease, as they resemble one another.

Single anomalous coronary artery is a rare congenital anomaly of the coronary circulation. Right coronary artery (RCA) arising from the left anterior descending artery is an extermely rare variety of single coronary artery. We report a 68-year-old patient with a single coronary artery system, in whom the right coronary artery originated from the mid left anterior descending artery.

Korean Circulation Journal 2004;34(10):992-996
Of coronary anomalies, a coronary arteriovenous fistula from the coronary artery to the left ventricle is rare. Five consecutive cases of coronary arteriovenous fistulae, from the coronary artery to the left ventricle were experienced in cath lab over a 2 year period, and are reported, with a review of the literature.

This case report we presented is that the anomalous left main coronary artery (LMCA) originates from the proximal segment of right coronary artery. In order to confirm the origin and course of the anomalous LMCA, a multi-slice computed tomography (MSCT) of the heart was performed on a 64-slice machine (Philips 64 Slice, Philips, USA) after 6 months of coronary angiography operation. The results showed that the anomalous LMCA originates from the proximal segment of right coronary artery, lies posteriorly to the aorta before taking acute sharply to go between the aorta and left atrium. It was classified as R-II P subtype according to Lipton’s classification. It is a rare case in the clinical practice.

An isolated single coronary artery is rare but often associated with other congenital cardiac malformations and myocardial ischemia. We report a rare case of right ventricular myocardial infarction due to total occlusion of the right coronary artery originating from the distal left circumflex artery.
