Macular corneal dystrophy is a heredo-familial disease inherited as an autosomal recessive trait and characterized by corneal opacities due to localized mucopolysaccharidosis. One case of macular corneal dystrophy was reported by Cho et al in Korea in 1976, but it was a case without a family history. We report 3 cases of macular corneal dystrophy with a family history including more specific histopathologic findings and the review of available literatures.

IOP was measured with 3 different tools, namely, the Pulsair peumotonometer(A), the Oculab Tono-pen(B), and the Goldmann applanation tonometer(C), in order to evaluate the clinical usefulness of each. 174 eyes without corneal opacity and corneal edema were measured. The results were as follows; 1. The correlation coefficient was 0.85(P<0.005) between A and C, 0.82(P<0.005) between B and C and 0.83(P<0.005) between A and B. The correlative formulas between the measurements of IOPs were produced to be: A = 0.73 X C + 5.49, B = 0.76 X C + 5.33, A = 0.78 X B + 3.87. 2. At low IOPs(<10 mmHg) as Measured by C(11 eyes), the correlation Coefficient was 0.81(P<0.05) between B and C. However, A and B valuse tended to be highter than C value. 3. At relatively high IOPs(>20 mmHg)as measured by C(56 eyes), the correleation coefficient was 0.83 between A and C(P<0.0001) and 0.82(P<0.0001) between B and C. However, A and B values tended to be lower than c value. 4. At relatively high IOPs(>20 mmHg)as measurde by C, the sensitivity of A was 60.53% and that of B was 73.68%. According to this study, no significant difference could be found among these three tools. However, at low IOPs(<10 mmHg), the Oculab Tono-pen and Pulsair pneumotonometer tended to show higher values than the Goldmann applanation tonometer. In case of relatively high IOPs(>20 mmHg as measured by the Goldman tonometer), the Oculab Tono-pen and Pulsair pneumotonometer tended to show lower values than the Goldmana applanation tono meter and the sensitivity of the Oculab Tono-pen showed higher than that of the Pulsair pneumotonomer.

Some species of venomous snakes spit venom in human eyes as a defence mechanism when threatened. If not detected and treated appropriately early, this can result in severe toxic ocular injury leading to potential blindness (snake venom ophthalmia). Not much is known of the clinical course and treatment guidelines due to the rarity of such occurrences. We present a case of isolated severe toxic ocular injury in one eye who reported to us with very poor vision following venom spit, which was promptly treated leading to a successful visual recovery. This is the first documented case of snake venom ophthalmia from Kuala Belait, Brunei Darussalam.

OBJECTIVE: According to the digital image features of corneal opacity, a multi classification model of support vector machine (SVM) was established to explore the objective quantification method of corneal opacity. METHODS: The cornea digital images of dead pigs were collected, part of the color features and texture features were extracted according to the previous experience, and the SVM multi classification model was established. The test results of the model were evaluated by precision, sensitivity and RESULTS: In the classification of corneal opacity, the highest CONCLUSIONS The SVM multi classification model can classify the degree of corneal opacity.

PURPOSE: To introduce a new genetic method for the diagnosis of Avellino corneal dystrophy (ACD), which is non-invasive and can be easily performed on an outpatient basis, and to evaluate the relationship between the degree of corneal opacity and age or sex. METHODS: A genetic study was performed on 11 patients who had a specific corneal opacity by slit-lamp examination and on four normal patients by using a specific adhesive tape to obtain epidermal keratinocytes. Corneal dystrophy was diagnosed according to the genetic study. RESULTS: All 11 patients were confirmed as having heterozygous ACD. Heterozygous ACD patients were classified into five stages: trace, mild, moderate, severe, or very severe, based on slit-lamp photography status. Corneal stages had no relationship with sex (p=0.982), but the severity of ACD increased with age (p=0.005). CONCLUSIONS: A non-invasive sticker-type genetic study kit, the "U-gene test" is a good method to diagnose corneal dystrophy genetically. Avellino corneal dystrophy becomes more severe over time but has no relationship with sex.

PURPOSE: To evaluate the effects of different corneal epithelial manipulations in PRK procedure on keratocyte apoptosis. METHODS: Rabbit cornea was examined qualitatively and quantitatively after epithelial scraping(Group I), epithelial scraping followed by PRK(Group II) and transepithelial PRK(Group III). Each of them were compared with that of unwounded control. RESULTS: The number of anterior stromal keratocytes had decreased significantly and TUNEL-positive cells were significantly increased in the anterior stroma in group I and group II on POD 3 days compared with control group(p<0.05). There was a statistically significant increase in the number of anterior stromal keratocytes in group II compared with group III on POD 1 month(p<0.05). In group I and II, keratocytes that had morphologic changes consistent with apoptosis were detected with transmission and scanning electron microscopy on POD 3 days. But in group III, there were many normal appearing keratocyte in the anterior corneal stroma, and few keratocytes undergoing apoptosis. CONCLUSIONS: Transepithelial PRK induced lower levels of keratocyte apoptosis in rabbit cornea compared to those induced from epithelial scraping followed by PRK. This suggests that transepithelial PRK could be useful in preventing or minimizing refractive regression and corneal opacity, especially in cases of myopia or retreatment after regression.

Reis-Bucklers' dystrophy is an autosomal dominant disorder involving Bowman's membrane. It usually appears as a recurrent corneal erosion in early childhood and is characterized by an irregular, semi-opaque accumulation of fibrous connective tissue in the area of Bowman's membrane and symmetrical progressive opacity of superficial central cornea. Eventually, it causes visual disturbance due to re-ticulated gray-white corneal opacity and corneal surface irregularity. The ophthalmic surgeons have met difficulties in the management of Reis-Bucklers' corneal dystrophy because of poor long-term results following conventional penetrating and lamellar keratoplasties. We experienced a case of Reis-Bucklers' dystrophy in 25 years old man which was confirmed by clinical and pathological evidences and successfully treated by excimer laser phototherapeutic keratectomy(PTK) with VISX 20/20 B(R). In conclusion, we would suggest that PTK could be another modality of management in this anterior corneal disease when surgical intervention is required.

PURPOSE: To report a case of neurofibromatosis of the orbit and ocular surface. METHODS: A 26-year-old man was admitted complaining of conjunctival mass and exophthalmos. Lisch nodules on the iris were observed in the right eye, and corneal opacity with neovascularization and conjunctival mass were detected in the left eye. Orbital computed tomography revealed diffuse infiltrating facial tumors including the left orbit, suggestive of plexiform neurofibromatosis. RESULTS: We performed evisceration of the left eye and excision of the left orbital mass. Histopathologic examination revealed neurofibroma which was positive for S-100. No recurrence was detected during a 6-month post-operative period.
