1.Observations on iris melanocytes implanted in the cornea.

The pattern and morphology of cellular infiltration of iris melanocytes implanted into the corneal stroma were studied with a rabbit corneal model. Iris melanocytes are transformed into fibroblast-like cells with a loss of pigment granules, which may reflect the in vivo characteristics of iris melanocytes under pathologic conditions. The metaplastic chararter of iris melanocytes appears to be related to the formation of retrocorneal pigmentation and fibrous membrane.

2.Epithelial healing time and rate of the cornea after myopic epikeratoplasty.

This study was performed to investigate the corneal epithelial healing time and rate according to the method for promoting the reepithelization after myopic epikeratoplasty. A prospective study was conducted on 30 myopic epikeratoplasties which were divided into 3 groups according to the method for promoting the epithelial healing. The groups consisted of 10 eyes with pressure patches, 10 eyes with Acuvue(R) disposable contact lens (CL) and 10 eyes with SeeQuence(R) disposable CL. The cornea epithelial healing time were 3.4, 3.5 and 3.4 postoperative days for the pressure patch, Acuvue(R) CL and SeeQuence(R) CL groups, respectively. The corneal epithelial healing rates during postoperative 1, 2, and 3 days were 0.33, 0.78, and 0.44 mm2/hour for the pressure patch group; 0.24, 0.92 and 0.37 mm2/hour for the Acuvue(R) CL group; and 0.30, 0.79 and 0.38 mm2/hour for the SeeQuence(R) CL group. These results suggest that a disposable contact lens may not hinder epithelial healing compared with a pressure patch.

3.Comparison between Anterior Corneal Aberration and Ocular Aberration in Laser Refractive Surgery.

PURPOSE: To compare changes of anterior corneal aberration (Pentacam(R)) and ocular aberration (aberrometer, LADARWave(R)) after laser refractive surgery. METHODS: Sixty-six eyes underwent laser refractive surgery and were retrospectively reviewed. Anterior corneal aberration and ocular aberration were measured by Pentacam(R) and an aberrometer (LADARWave(R)) respectively. Changes of root mean square (RMS) values of coma, spherical aberration, and total high order aberration (HOA) were evaluated before, 1 month, and 3 months after surgery RESULTS: Ocular aberrations displayed low preoperative values, but after laser refractive surgery, anterior corneal aberration and ocular aberration increased equally. There were no statistically significant differences of internal optics aberration values (ocular aberration minus anterior corneal aberration) in coma, spherical aberration, and total HOA. Anterior corneal aberration and ocular aberration showed statistically significant correlations at 1 and 3 months after surgery. CONCLUSIONS: Internal optics aberration compensated the anterior corneal aberration effectively before surgery, but the increase of anterior corneal aberration after laser refractive surgery exceeded the compensation of internal optics. As a result, anterior corneal aberration and ocular aberration increased equally. The correlation between anterior corneal aberration and ocular aberration after surgery was statistically significant due to the increased proportion of anterior corneal aberration in ocular aberration.

4.Role of dendritic cells in graft rejection after penetrating keratoplasty.

OBJECTIVETo gain insight into the role of dendritic cells in graft rejection following penetrating keratoplasty by investigating their distribution in rat cornea.
METHODSOrthotopical corneal transplantation was performed and immunohistochemical staining of the whole-mount cornea and the spleen tissue specimen employed to determine the distribution of the dendritic cells in the cornea.
RESULTSGraft rejection occurred in all rats following the transplantation. No OX-62(+) dendritic cells were found in normal cornea but they were present in the epithelium of the cornea graft with allograft rejection.
CONCLUSIONOX-62(+) dendritic cells presenting in the rejected cornea may be related to acute graft rejection after penetrating keratoplasty.

5.Comparison of the Astigmatic Power of Toric Intraocular Lenses Using Three Toric Calculators.

PURPOSE: To compare the astigmatic power of toric intraocular lenses (IOLs) obtained from the AcrySof, TECNIS, and iTrace toric calculator in patients with preoperative with-the-rule (WTR) or against-the-rule (ATR) corneal astigmatism. MATERIALS AND METHODS: Fifty eyes with cataract and corneal astigmatism greater than 0.75 diopters were enrolled in each group (WTR and ATR). Keratometric values were measured using autokeratometry, an IOLMaster, and an iTrace, which incorporated corneal topography and ray-tracing aberrometry. Based on measured keratometric values, the astigmatic power of each toric IOL was calculated using three toric calculators. RESULTS: Bland-Altman plots showed good agreement between six pairwise corneal astigmatism values in both groups. The TECNIS calculator tended to suggest a higher astigmatic power of the toric IOL than the AcrySof calculator. With the higher astigmatism and keratometric values from the IOLMaster, in both groups, calculations from the AcrySof and TECNIS calculators resulted in higher calculated astigmatic powers than those from same calculators with autokeratometry-measured values, demonstrating good agreement. With the higher calculated astigmatic power values, the values from the iTrace toric calculator using keratometric values obtained from iTrace ray tracing wavefront aberrometry or iTrace simulated keratometry showed fair to moderate agreement with those from the other calculator-keratometry pairs in both groups. CONCLUSION: To achieve the best refractive outcome after toric IOL implantation, understanding the differences in keratometric values between instruments and in calculated astigmatic power among toric calculator programs is necessary. Moreover, systemic analysis of each toric calculator in conjunction with postoperative data is required.

6.A Novel Tectonic Keratoplasty with Femtosecond Laser Intrastromal Lenticule for Corneal Ulcer and Perforation.

BACKGROUNDSmall incision refractive lenticule extraction (SMILE) is an effective laser procedure that treats myopia. This research was to describe a novel approach to treat corneal ulcer or perforation using the corneal lenticules obtained from SMILE and to evaluate the safety and effectiveness of tectonic keratoplasty with femtosecond laser intrastromal lenticule (TEKIL).
METHODSA total of twenty patients (22 eyes) were monitored for at least 6 months and were assessed using slit lamp microscopy, optical coherence tomography, and best-corrected visual acuity (BCVA). Postoperative complications throughout the study period were recorded.
RESULTSCorneal ulcer in 14 patients (16 eyes) and corneal perforation in six patients (6 eyes) were treated with TEKIL. The patients were ten females and ten males, with a mean age of 58.5 ± 16.3 years (range: 16-81 years). In this study, the most causes of corneal ulcer or perforation were immunologic causes (54.5%). After TEKIL procedure, global integrity was achieved in all cases. No immune rejection or perforation was detected. The mean BCVA improved from 0.17 ± 0.20 preoperatively to 0.27 ± 0.25 postoperatively at the final follow-up (t = 2.095, P = 0.052). The postoperative BCVA improved in 12 eyes (54.5%) and maintained in nine eyes (40.9%). Vision function successfully maintained in all eyes (100%). In three cases, corneal ulcers were treated by reoperation of TEKIL at 3 months after the initial surgery for the reason that the residual corneal thickness was <250 μm.
CONCLUSIONSTEKIL seems to be an effective treatment for corneal ulcer and perforation in the condition of emergency and donor shortage.

7.Evaluation of biocompatibility of modified gelatin composite membranes for corneal regeneration.

In order to investigate the feasibility of the modified chitosan-gelatin crosslinked membrane (MC-Gel) and chitosan-gelatin crosslinked membrane (CS-Gel) to be a potential biomaterial for corneal regeneration, we evaluated their physicochemical properties and intraocular biocompatibility in this study. White light transmission and permeability of these membranes were detected. Results showed that white light transmission of both membranes was above 90% at 500 nm, which was similar to that of human cornea. The glucose, tryptophan and NaCl permeability of MC-Gel membrane and CS-Gel membrane was better than or similar to those of human cornea. The methylthiazol tetrazolium (MTT) assay was used to assess cell viability and proliferation. Also, interlamellar corneal transplantation was carried out to evaluate ophthalmic biocompatibility of MC-Gel membrane and CS-Gel membrane. Results indicated that MC-Gel membranes could support the proliferation of HCEC and displayed good intraocular biocompatibility when implanted into rabbits. No severe inflammatory reaction occurred after transplantation and the implanted MC-Gel membrane degraded completely 16 weeks post-operation. Due to its good physicochemical properties and biocompatibility, MC-Gel membrane could be a promising candidate material for corneal regeneration.
