1.Malnutrition assessment and nutrition support therapy for end-stage liver disease

Malnutrition is a common complication in patients with end-stage liver disease and is associated with patients' clinical prognosis and quality of life,but it has not been taken seriously by clinical physicians.This article elaborates on the pathogenesis of malnutrition in patients with end-stage liver disease and introduces the latest research advances in malnutrition assessment and nutrition support therapy for end-stage liver disease in China and foreign countries.Early nutrition support therapy for patients with end-stage liver disease may help to improve their comprehensive clinical outcome.
2.Application of teaching based on task-driven joint feedback mechanism in the teaching of practical nursing students in department of obstetrics and gynecology

Objective:To explore the effect of teaching based on task-driven joint feedback mechanism on improving the quality and satisfaction of teaching practical nursing students in department of gynaecology and obstetrics.Methods:A total of 203 practical nursing students who had internship in the Department of Gynaecology and Obstetrics, The First Affiliated Hospital of Hainan Medical University from June 2019 to June 2020 were selected as the research objects. They were divided into a control group ( n=96) and a study group ( n=107) according to the order of admission. The control group used traditional teaching, and the study group used task-driven joint feedback mechanism-based teaching. After the teaching, the teaching effect of the two groups of practical nursing students was evaluated through theoretical assessment, skill operation assessment and teaching satisfaction. SPSS 22.0 was used for t test and non-parametric rank sum test. Results:The theoretical assessment scores [(93.51±4.22) vs. (89.62±4.08)] and the total scores of the skill operation assessment [(93.20±4.14) vs. (82.06±3.82)] of the practical nursing students in the study group were better than those of the control group, and the difference was statistically significant ( P<0.05). The results of teaching satisfaction evaluation (teacher's ability and execution ability, teaching content and task design, teaching task completion, feedback mechanism execution and improvement after feedback) of the study group were better those of the control group, and the difference was statistically significant ( P<0.05). Conclusion:The teaching based on the task-driven joint feedback mechanism can effectively improve the teaching quality and teaching satisfaction of the practical nursing students in the department of obstetrics and gynecology.
3.Clinical therapeutic effect of surgery on upper cervical spinal cord tumors.

OBJECTIVE:
To evaluate the long-term clinical therapeutic effect of polyaxial screw-rod system for posterior cervical arthrodesis on patients with upper cervical spinal cord tumors.
METHODS:
From March 2007 to May 2013, 22 patients with upper cervical spinal cord tumors underwent tumor resection and posterior cervical arthrodesis in our institution. The medical records of these patients were reviewed respectively. There were 10 males and 12 females with ages ranging from 16 to 60 years old. Posterior cervical arthrodesis by polyaxial screw-rod was performed at the upper cervical spine (C1-3). All patients were followed-up clinically and radiographically.
RESULTS:
The average follow-up was 65.5 months. Twenty-two patients were enrolled and a total of 114 screws were placed in this study. Histopathology revealed neurinoma, meningioma, ganglioneuroma and ganglioglioma in 16, 3, 1 and 1 case (s), respectively. The mixed tumor with component of ganglioneuroma and neurinoma was observed in 1 case. All patients received tumor resection and posterior athrodesis by polyaxial screw-rod system. Cervical kyphosis was encountered in one patient and this patient suffered the recurrence of tumor. Solid fusion was achieved in all patients. The average postoperative Japanese Orthopaedic Association (JOA) score was 13.9 and the average recovery rate was 51.4%. Neurologic deterioration was found in 2 patients. No complications, such as spinal cord or vertebral artery injury, postoperative radiculopathy or instrumentation failure, were observed.
CONCLUSION
The long-term clinical therapeutic effects of posterior cervical arthrodesis using polyaxial screw-rod system on upper cervical spinal cord tumors are satisfactory, with no severe complication.

4.Clinical Observation of Recombinant Human Interferon Gel Combined with Baofukang Suppository in the Treat- ment of Cervical High-risk HPV Infection

OBJECTIVE:To inv estigate therapeutic efficacy and safety of recombinant human interferon gel combined with Baofukang suppository in the treatment of cervical high-risk human papillomavirus (HPV)infection. METHODS :Totally 259 patients with persistent high-risk HPV infection diagnosed and treated in gynecology department of the First Affiliated Hospital of Hainan Medical University from Aug. 2017 to Sept. 2019 were selected and divided into interferon group (n=82),Baofukang suppository group (n=86)and combination group (n=91)according to random number table. The patients in interferon group and Baofukang suppository group were given Recombinant human interferon α2b gel 1 g, qd or Baofukang suppository 1 capsule,qd; the patients in combination group were given Recombinant human interferon α2b gel and Baofukang suppository 1 capsule,qd;for 3 months. Then the clinical efficacy ,negative time of HPV ,duration of abnormal secretion ,LCT test results ,cervical inflammation score ,HPV relative light unit/critical value (RLU/CO)and the incidence of ADR were recorded. RESULTS :The total effective rate of combination group was significantly higher than that of interferon group and Baofukang suppository group , the negative time of HPV and duration of abnormal secretion in combination group were significantly shorter than interferon group and Baofukang suppository group (P<0.05). Before treatment ,the normal rate of LCT of 3 groups were 0,and there was no statistical significance in cervical inflammation score and HPV RLU/CO among 3 groups(P>0.05). After treatment ,normal rate of LCT was increased in 3 groups,compared with before treatment (P<0.05),and normal rate of LCT in combination group was significantly higher than interferon group and Baofukang suppository group. The cervical inflammation score and HPV RLU/CO were significantly lower than before treatment ,and the combination group was significantly lower than interferon group and Baofukang suppository group (P<0.05). There was no statistical significance in above indicatora after treatment betwent interferon group and Baofukang suppository group and the incidence of ADR among 3 groups during medication (P>0.05). CONCLUSIONS:The application of recombinant human interferon gel combined with Baofukang suppository is effective and safe way in the treatment of cervical high-risk HPV infection.
