1.Influence of nursing intervention on anxiety and pains of out-patients who had accepted operation

Objective To know the correlation of anxiety and pains of out-patients who had accept-ed operation and the effect of nursing intervention. Methods Divided 173 out-patients with operation in-to the intervention group(83 cases) and the control group(90 cases) randomly. Routine nursing cares was used in the control group, preoperative health education was used in the intervention group in addition. Us-ing the Me-Gill and SF-MPQ to evaluate pains between the 2 groups. Using the S-AI and the T-AI to evaluate the emotional condition between the 2 groups. Results The indexes which can indicated the pains and emotional condition in the intervention group after the health education were significant better than those of in the control group and themselves before the nursing intervention. Conclusions Preoperative pains and emotional condition should be evaluated among out-patients, and then carry out certain counter nursing measures to release patients' pain and negative emotion.
2.Efficacy analysis of different bismuth-based quadruple therapies for two hundred and forty cases of Helicobacter pylori eradication

Objective To evaluate the efficacy and safety of different bismuth-based quadruple therapies for Helicobacter pylori (H.pylori) eradication.Methods From December 2012 to October 2013,240 patients with H.pylori infection were collected and evenly divided into clarithromycin group,levofloxacin group,furanzolidone group and metronidazole group.Each group received bismuth potassium citrate 220 mg,esomeprazole 20 mg and amoxicillin 1 000 mg twice daily.In addition,each group received clarithromycin 500 mg,levofloxacin 200 mg,furanzolidone 100 mg,and metronidazole 400 mg,respectively.The course of treatment was 10 days.At least four weeks after the end of therapy and withdrawal the medicine,patients underwent fasting 13C-urea breath test or 14C-urea breath test.The negative result indicated as successful H.pylori eradication.The adverse effects were observed and recorded during treatment.The rate of H.pylori eradication was analyzed by the intention to treat (ITT) analysis and per protocol (PP) analysis.Chi-square test was performed for eradication rate comparison among groups.Results According to ITT analysis,the eradication rate of clarithromycin group,levofloxacin group,furanzolidone group and metronidazole group was 81.67% (49/60),88.33% (53/60),93.33% (56/60) and 73.33% (44/60),respectively,and according to PP analysis which was 85.96% (49/57),89.83% (53/59),94.92% (56/59) and 75.86% (44/58),respectively.The differences among four groups were statistically significant (x2 =10.13 and 9.89,both P<0.05).The differences between furanzolidone group and metronidazole group were statistically significant (x2 =8.64 and 8.55,both P<0.01).There were no statisticaly significant differences in adverse effects among the four groups (x2 =0.47,P>0.05).Conclusion The H.pylori eradication rate is high in furanzolidone contained bismuth based quadruple therapy and with good safety,which could be the first line treatment for H.pylori eradication.
3.Maximum entropy model versus remote sensing-based methods for extract-ing Oncomelania hupensis snail habitats

Objective To explore the technique of maximum entropy model for extracting Oncomelania hupensis snail habi?tats in Poyang Lake zone. Methods The information of snail habitats and related environment factors collected in Poyang Lake zone were integrated to set up the maximum entropy based species model and generate snail habitats distribution map. Two Land?sat 7 ETM+remote sensing images of both wet and drought seasons in Poyang Lake zone were obtained,where the two indices of modified normalized difference water index(MNDWI)and normalized difference vegetation index(NDVI)were applied to ex?tract snail habitats. The ROC curve,sensitivities and specificities were applied to assess their results. Furthermore,the impor?tance of the variables for snail habitats was analyzed by using Jackknife approach. Results The evaluation results showed that the area under receiver operating characteristic curve(AUC)of testing data by the remote sensing?based method was only 0.56, and the sensitivity and specificity were 0.23 and 0.89 respectively. Nevertheless,those indices above?mentioned of maximum en?tropy model were 0.876,0.89 and 0.74 respectively. The main concentration of snail habitats in Poyang Lake zone covered the northeast part of Yongxiu County,northwest of Yugan County,southwest of Poyang County and middle of Xinjian County,and the elevation was the most important environment variable affecting the distribution of snails,and the next was land surface tem?perature(LST). Conclusions The maximum entropy model is more reliable and accurate than the remote sensing?based meth?od for the sake of extracting snail habitats,which has certain guiding significance for the relevant departments to carry out mea?sures to prevent and control high?risk snail habitats.
4.Endemic situation trend of schistosomiasis in Guichi District of Chizhou City,Anhui Province,1991-2011

Objective To understand the epidemical patterns of schistosomiasis in Guichi District of Chizhou City,Anhui Province,in order to provide the reference for schistosomiasis control. Methods The annual data of schistosomiasis endemic situation in Guichi District from 1991 to 2011 were collected. The descriptive analysis was first conducted to describe the changes of schistosomiasis in local residents and the status of Oncomelania hupensis. An autoregressive integrated moving average (ARIMA)model was applied to fit and predict the tendency of schistosomiasis incidence in this region. Results The humanmorbidity increased with the increasing of the areas with snail habitats(P < 0.05),and four peaks(in 1992,1995,2005,2008)were detected. The difference of the area with snails among different types of snail habitats was statistically significant(F = 256.79,P < 0.05). ARIMA(1,1,1)was determined to be the optimal model for analyzing the morbidity of schistosomiasis,and the short⁃term forecast of the morbidity in Guichi District from 2012 to 2015 showed that the predicted values were 0.017%,0.007%,0.012%,and 0.010%,respectively. Conclusions The endemic situation of schistosomiasis in Guichi District is controlled relatively well in the past two decades. However,the surveillance in the lake and marshland regions should be
strengthened continuously to prevent the rebounding of the schistosomiasis endemic situation.
5.Correlation analysis between drug resistance of Helicobacter pylori and the clinical eradication efficacy of bismuth-based quadruple therapies

Objective To investigate the correlation between the drug resistance of Helicobacter pylori (H .pylori )and clinical eradication efficacy of bismuth-based quadruple therapies,and to guide clinical rational drug use in the region.Methods A total of 260 patients with H .pylori infections were collected.H .pylori from biopsied gastric mucosa tissues were isolated and cultured.Drug resistant rates of isolated H .pylori to metronidazole,clarithromycin,amoxicillin,levofloxacin and furanzolidone were tested.Patients were randomly divided into clarithromycin,levofloxacin,furanzolidone and metronidazole groups by completely randomized design.All patients received bismuth potassium 220 mg,esomeprazole 20 mg and amoxicillin 1 000 mg twice daily,and according to group received clarithromycin 500 mg, levofloxacin 200 mg,furanzolidone 100 mg and metronidazole 400 mg,twice daily,espectively.The treatment course was 10 days.At least four weeks after treatment,13 Curea breath test or 14 Curea breath test was taken.According to the intention to treat (ITT)and per-protocal (PP),the eradication rate of each group was caculated.Chi square test was performed to compare the differences between groups. Results The drug resistant rate of H .pylori to metronidazole,clarithromycin,amoxicillin,levofloxacin and furanzolidone was 94.2% (146/155 ), 21 .3% (33/155 ), 2.6% (4/155 ), 5 .8% (9/155 ) and 1 .9%(3/155),respectively.According to ITT analysis,the eradication rate of clarithromycin group, levofloxacin group,furanzolidone group and metronidazole group was 81 .5 %(53/65 ),90.8%(59/65 ), 93.8% (61/65 )and 75 .4%(49/65),respectively.And according to PP analysis which was 84.1 %(53/63),92.2%(59/64),95 .3%(61/64)and 79.0%(49/62 ),respectively.The differences between furanzolidone group and metronidazole group,clarithromycin group were staistcally significant (χ2ITT =8.509 and 4.561 ;χ2PP = 7.592 and 4.323,all P < 0.05 ).There was no statistical significance in the H .pylori eradication rate between resistant strains and sensitive strains of each group.Conclusion Bismuth-based quadruple therapy can overcome antibiotic resistance,the eradication rate of protocal with furanzolidone is higher and with good safety,which can be the first-line treatment for H .pylori eradication.
6.Application of time series model in prediction of nosocomial infection for cancer patients

Objective:To investigate the applicability of time series model in predicting incidence of nosocomial infection in a cancer center in Shanghai, and to provide the references for early warning and prevention.Methods:The nosocomial infection data of inpatients of a tertiary oncology hospital in Shanghai from 2013 to 2018 were collected. The autoregressive integrated moving average (ARIMA) model and the exponential smoothing model were established by SPSS 22.0 expert modeler. The fitting predictions were compared between these two time series models to select the optimal one. The nosocomial infection data from January 2019 to June 2019 were used to test the predictive effect of the model.Results:A total of 379 477 cancer inpatients were studied, 3 170 of which acquired nosocomial infection and the incidence was 0.84% from 2013 to 2018. Additive Holt-Winters method exponential smoothing model was the better model with R2of 0.82. Using this model, the predicted value fitted well with observed value from January 2019 to June 2019, and the mean relative percentage error was 15.22%. Conclusion:Additive Holt-Winters method exponential smoothing model could be used to fit and predict the tendency of nosocomial infection among cancer patients, which can provide reference for surveillance of nosocomial infection in oncology hospitals.
7.Creation and translational relevance of abdominal aortic aneurysm animal models

Population aging in China has led to an increase in the incidence of abdominal aortic aneurysm(AAA).AAA rupture is one of the most severe life-threatening diseases,with high mortality.The main histopathological features of AAA include elastin degradation,smooth muscle cell depletion,extracellular matrix digestion,and mural leukocyte accumulation.Clinically,drug therapy is still lacking,and open/endovascular repair remains the most effective treatment strategy for AAA management.Notably however,the detailed molecular mechanism of AAA remains unclear,representing an important bottleneck affecting the development of potential drug targets.Animal models are the most powerful tools for clarifying the pathogenesis of AAA,and although some medium-to-large laboratory animal models(e.g.,rabbits,guinea pigs,dogs,pigs)have been established for AAA studies,rodent models(mice and rats)are still the main models used in this field.Current method of inducing AAA include intra-infrarenal aortic infusion of elastase,subcutaneous infusion of angiotensin Ⅱ,periaortic calcium chloride painting,and decellularized aortic xenografting;however,AAA tends to stabilize in most models after ceasing pre-induced stimulation(medical or surgical),and there remains a need for ideal animal models that maintain continuous aortic dilation and even rupture.AAA animal models are helpful for elucidating the pathogenesis of AAA,screening new drug targets,and promoting clinical translation.This review aims to discuss the application of current AAA modeling method and their translational relevance.
8.Application of gene screening technology in screening common newborn genetic diseases

Objective:To detect the genes of common genetic diseases in newborns with the high-throughput sequencing technology based on target gene capture, to study the incidence rate of such diseases, the carrying rate and variant types of pathogenic mutations related to such diseases, and to explore the application value of the high-throughput sequencing technology in screening genetic diseases of newborns.Methods:The heel blood of 1 793 newborns born in Guangdong province from June 2019 to April 2020 were collected, and the exon regions of 138 common genetic disease-related genes in neonates were detected using the high-throughput sequencing technology based on target gene capture.The pathogenicity of the mutations was interpreted according to the " Classification Criteria and Guidelines for Genetic Variation(2017)" , in which known disease and probable disease were considered as positive mutations.The positive mutations were verified by Sanger sequencing technology, and the test results were analyzed with statistical methods.Results:Among the 1 793 newborns, 978 were male and 815 were female.A total of 158 positive cases were screened(8.81%), and 11 positive diseases were detected.Among the positive diseases, there were 41 cases(2.29%)of autosomal recessive deafness type 1A, 40 cases(2.23%)of Gilbert syndrome or Crigler-Najjar syndrome, and 33 cases(1.84%)of glucose-6-phosphate dehydrogenase deficiency(1.84%), 19 cases(1.06%)of familial hypercho-lesterolemia, 18 cases(1.00%) of sodium taurocholate cotransporter peptide deficiency disease, 2 cases(0.11%)of mitochondrial non-syndromic deafness, 2 cases(0.11%)of Citrin deficiency, 1 case(0.06%)of holocarboxylase synthase deficiency, 1 case(0.06%)of β-thalassemia and 1 case(0.06%)of metachromatic leukodystrophies.Of all studied cases, 972 carried one or more positive mutations, involving 85 kinds of diseases in total.The diseases with a high carrying rate were Gilbert syndrome or Crigler-Najjar syndrome(359 cases, 20.02%), autosomal recessive deafness type 1A(302 cases, 16.84%), and sodium taurocholate cotransport peptide deficiency disease(291 cases, 16.22%). The high-frequency mutation sites were UGT1A1 gene c. 211G> A, GJB2 gene c .109G> A and SLC10A1 gene c. 800C> T. Conclusions:The common genetic diseases detected in neonates from Guangdong province are autosomal recessive deafness type 1A, Gilbert syndrome or Crigler-Najjar syndrome, glucose-6-phosphate dehydrogenase deficiency, familial hypercholesterolemia, and sodium taurocholate cotransport peptide deficiency.There are high-frequency carrying mutation sites in the population.Preliminary genetic screening of common neonatal genetic diseases can accumulate data and experience for the development of newborn genetic screening.
9. Current advances in research of burning mouth syndrome: biological nerve and social psychology patterns

Burning mouth syndrome (BMS) is a complex chronic disorder characterized symptomatically by burning sensation mainly localized in the tongue with no specific lesions. Its etiology is not fully understood. Due to the persistent pain and unsatisfactory outcome of treatment, patients with high expectation when seeking medical advice easily feel discontent after treatment, which might cause medical dispute. This review introduces the epidemiology, manifestations, etiology, diagnosis, classification and treatment of BMS. The authors hope the current advances about BMS, mainly focus on the etiology of biological nerve and social psychology, may help the clinicians in treatment of BMS.
10.Effect of dexmedetomidine on postoperative outcomes in pediatric patients undergoing open reduc?tion and internal fixation following upper limb fracture performed under sevoflurane anesthesia combined with brachial plexus block

Objective To evaluate the effect of dexmedetomidine on postoperative outcomes in ped?iatric patients undergoing open reduction and internal fixation following upper limb fracture performed under sevoflurane anesthesia combined with brachial plexus block. Methods Sixty American Society of Anesthe?siologists physical status Ⅰ pediatric patients of both sexes, aged 6-12 yr, with body mass index <24 kg∕m2, scheduled for elective open reduction and internal fixation following upper limb fracture, were ran?domly divided into ropivacaine group(group R)and dexmedetomidine mixed with ropivacaine group (group DR), with 30 cases in each group. Interscalene brachial plexus block was performed under ultra?sound guidance after sevoflurane anesthesia, 0.25% ropivacaine plus 1 μg∕kg dexmedetomidine(0.3 ml∕kg)was given in group DR, and 0.25% ropivacaine 0.3 ml∕kg was given in group R. The emergence time was recorded, and the development of agitation was observed immediately after emergence from anes?thesia. Sensory block duration, motor block duration, consumption of sedatives and analgesics during post?anesthesia care unit(PACU), consumption of rescue analgesics within 24 h after operation, length of stay in PACU and adverse reactions in the perioperative period were observed and recorded. Results Compared with group R, the incidence of agitation immediately after emergence from anesthesia was significantly de?creased, sensory block duration was prolonged, the consumption of sedatives and analgesics during PACU and rescue analgesics within 24 h after operation was reduced, and the emergence time and length of stay in PACU were prolonged in group DR(P<0.05). There was no significant difference in the incidence of bradycardia between the two groups(P>0.05). No adverse reactions were found in the perioperative period in the two groups. Conclusion Dexmedetomidine can promote postoperative outcomes in pediatric patients undergoing open reduction and internal fixation following upper limb fracture performed under sevoflurane anesthesia combined with brachial plexus block.
