BACKGROUND:Studies have shown that the finite element method could preferably simulate the biomechanical analysis for the object with complicated structures and irregular shapes. The similarities for the finite element model have great influences on the results of the analysis. However, to construct an ideal model is the most time-consuming and complicated portion for the finite element analysis. OBJECTIVE:To construct a finite element model for the maxilary first molar and the periodontal tissue, and to provide a basis of biomechanical researches of the maxilary first molar. METHODS: A volunteer with complete mandibular dentition and healthy periodontal tissue was selected in this study. Cone-beam CT was scanned. The images were saved as DICOM format. These images were imported to the medical modeling software Mimics. The surface model for the maxilary first molar and the alveolar bone was constructed. The model was then imported to GiD for pre-processing. Thus, the complete three-dimensional finite element model for the maxilary first molar and the periodontal tissue was constructed. RESULTS AND CONCLUSION:A finite element model for bilateral maxilary first molar, periodontal ligament and maxilary alveolar bone was constructed, including 896 035 nodes and 4 881 067 elements. This model has restored the geometric shape and the structure of the research object. This study successfuly constructed finite element models of maxilary first molar and the periodontal tissue, which can be a basis of biomechanical researches for the maxilary first molar and the periodontal tissue under the effect of different clinical orthodontic forces.

Objective To identify differences in the anal sphincter surface electromyography (sEMG)variables in spinal cord injury (SCI) subjects with different severities. Methods One hundred and ten SCI patients' impairments were classified as ASIA A,B,C or D using the International Standards for the Neurological Classification of Spinal Cord Injury. The evaluation was pedormed using sEMG equipment with an inserted anal sensor electrode and the Glazer pelvic floor muscle sEMG protocol. The sEMG variables were recorded and compared. Results There was no significant difference in sEMG variables between groups A and B,but the mean and maximum sEMG values of groups C and D in flick contractions ( 1 s),tonic contractions ( 10 s),and endurance contractions (60 s) were significantly higher than those of group A.Compared with group C,the mean and maximum sEMG values of group D were all significantly higher. Conclusions The sEMG data from the anal sphincter during contractions decreases significantly after SCI.Glazer's pelvic floor muscle sEMG protocol is a noninvasive and convenient real-time assessment. It is a useful complementary tool for quantitative assessment of the pelvic floor muscles of SCI patients.

Objective:To analyze the mean levels of skeletal muscle mass and strength in elderly patients with type 2 diabetes mellitus(T2DM), and to investigate the effects of chronic inflammatory factors and oxidative stress on them. Methods:A cross-sectional study was conducted on 120 patients with T2DM aged over 60 years and 126 elderly patients without diabetes(the control group). Skeletal muscle mass, strength and serum levels of chronic inflammatory factors interleukin-6(IL-6), tumor necrosis factor-α(TNF-α)and 8-hydroxy-2′-deoxyguanosine(8-OHdG)were determined, and their effects on skeletal muscle mass and strength in elderly patients with T2DM were analyzed. Results:Compared with the control group, grip strength decreased in elderly patients with T2DM(25.03±7.85)kg vs.(29.52±7.73)kg( P<0.01), and skeletal muscle mass decreased(21.36±5.46)kg vs.(22.01±5.22)kg with no significant difference( P>0.05). Serum levels of 8-OHdG were higher in elderly patients with T2DM than in the control group(3.08±0.26)ng/L vs.(2.59±0.16)ng/L( P<0.01). Correlation and regression analysis results showed that 8-OHdG was an influencing factor for muscle strength in elderly patients with T2DM( R2=0.457)and that height and weight could be influencing factors for skeletal muscle mass in elderly patients with T2DM( R2=0.822). Conclusions:Skeletal muscle mass and strength decline in elderly T2DM patients, probably as a result of increased levels of oxidative stress. These findings may serve as evidence for sarcopenia intervention in elderly T2DM patients.

Chinese Journal of Organ Transplantation 2003;0(05):-
Objective To investigate whether the signaling pathway of Akt/protein kinase B (PKB)is involved in the pathogenesis in the early stage of chronic allograft nephropathy(CAN). Methods The kidneys of Fisher(F344)rats were orthotopically transplanted into Lewis recipients. Animals were harvested respectively at 4th,8th,12th and 16th week after transplantation for renal function and histological examination. The expression of p-Akt and MMP-9 was detected by immuno- histochemical assay, and that expression of p-Akt mRNA by RT-PCR.Results At the 16th week after kidney transplantation, the levels of 24-h proteinuria and serum creatinine in transplant rats were higher significantly than those in LEW controls and F344 controls. The expression of p-Akt protein and mRNA was up-regulated,and there was a significantly negative correlation between the expression of p-Akt and the quantity of smooth muscular cells(SMCs)in arteriolar wall, the quality of tubuloin- terstitial infiltrated mononuclear cells and also MMP-9 level. Conclusion The up-regulation of p-Akt played an important role in the pathogenesis such as proliferation and migration of SMCs,infiltrated mononuclear cells in tubulointerstitial in CAN,and may be the signaling pathway leading to the up- regulation of MMP-9 expression in the early stage of CAN.
