1.Intravitreal injection with Ranibizumab combined with laser therapy for macular edema caused by branch retinal vein occlusion

AIM: To investigate the efficacy of intravitreal ranibizumab combined with laser therapy in the treatment of macular edema caused by branch retinal vein occlusion ( BRVO) .
METHODS:There were 78 patients (78 eyes) who were diagnosed with macular edema caused by BRVO using fundus fluorescence angiography ( FFA ) and optical coherence tomography ( OCT ) . Group A: randomly selected 26 cases ( 26 eyes ) were given grid laser photocoagulation ( GLP) . Group B: randomly selected 26 cases (26 eyes) were given GLP first, and then received intravitreal ranibizumab 1wk later. Group C: randomly selected 26 cases ( 26 eyes ) undergone intravitreal ranibizumab first, and then given GLP 1wk later. There was no significant difference in macular edema. We analyzed the changes in the best corrected visual activity ( BCVA ) , central macular thickness ( CMT ) before and 1wk, 1,6mo after treatment.
RESULTS: Compared with before treatment, 1wk after treatment: mean value changes of BCVA and CMT were no significant difference in group A (P>0. 05);mean value changes of BCVA was improved and mean value of CMT was decreased in groups B and C, the difference was statistically significant (P<0. 05);BCVA and CMT changes between groups had statistical significance ( P<0. 01 );After treatment 1, 6mo:compared with before treatment, mean BCVA was improved in three groups, mean CMT was lower, the differences were statistically significant (P<0. 01);BCVA and CMT changes between groups were statistically significant ( P<0. 05 ); compared with 1mo after treatment, mean BCVA and CMT were no significant difference (P>0.05) between groups A and B after 6mo treatment;mean BCVA improved and CMT average value was decreased in group C, the difference was statistically significant ( P<0.01 ); BCVA and CMT changes between groups were statistically significant (P<0. 01).
CONCLUSION: Intravitreal injection ranibizumab combined laser therapy can effectively reduce BRVO induced macular edema, enhance vision acuity. Compared with GLP, combination therapy has more rapid onset of treatment, and reduce macular edema better; Intravitreal ranibizumab should be given in front of the GLP, and the treatment effect is more precise, more stability.

Objective To investigate the relationship between hepatitis B virus (HBV) basic core promoter(BCP)/precore(PreC) mutations and severity of liver dis ease. Methods In 113 patients chronically infected with HBV, do uble mutations in BCP(T1762/A1764) and PreC mutation(A1896) were determined by INNO-LiPA and HB V genotype was determined by S gene sequencing. Results Whether in all patients or in patients infected by single genotype C, compared with AsC, the prevalence of double mutations in BCP(T1762/A1764) was higher in patients with CHB, LC and HCC[(24.1% vs 2.8%,? 2=5.93, P0.05). Conclusions The doubl e mutations in BCP(T1762/A1764 ) may be related to progressive liver disease in patients with chronic HBV infec tion.
4. HPLC-ELSD in determination of AR-I content in total furostanol saponins of Asparagus cochinchinensis

Objective: To determine AR-I content in total furostanol saponins of Asparagus cochinchinensis. Methods: High performance liquid chromatogra phy (HPLC) was used and the condition was: Diamonsil C18 column (5 mm×250 mm, 5 μm); mobile phase: CH3CN-H2O (36 : 64); flow rate: 0.8 ml/min; injector volume:20 μl;colum temperature: room temperature. Evaporative light scattering detector(ELSD) condition was: drift tube temperature: 40°C; and nebulizer gas pressure (N2): 2.0×105 Pa. Results: The linearity was good when AR-I was within the range of 0.094-0.940 0 g · L-1 (r=0.999 2). The test had higher sensitivity and good stability. The average recovery was 101.3% (n=6)and RSD was 2.36%. Conclusion: HPLC-ELSD is practical and reliable for the determination of the furostanol saponins of Asparagus cochinchinensis.
5.Therapeutic effect of photodynamic therapy and intravitreal injection with ranibizumab for choroidal neovascularization of pathologic myopia

AlM: To investigate the efficacy of photodynamic therapy ( PDT) and intravitreal injection with ranibizumab on macular choroidal neovascularization ( CNV ) of pathologic myopia ( PM) .
METHODS: There were patients ( 32 eyes ) who were diagnosed as PM with CNV. Randomly selected 16 cases ( 16 eyes ) which were given the PDT treatment ( PDT group ) . The remaining were given both PDT and intravitreal injection with ranibizumab ( combination group) . There is no significant difference on macular edema between two groups. We analyzed the changes in the best corrected visual activity ( BCVA) , optic coherence tomograph ( OCT ) and fundus fluorescein angiography (FFA) before and 1, 6mo after treatment.
RESULTS:One month after the treatment in PDT group:the BCVA increased while the CMT decreased compared with that of pretreatment (P<0. 05). One month after the treatment in combination group: the BCVA increased while the CMT decreased significantly compared with that of pretreatment (P<0. 01);the changes of BCVA and CMT showed statistically significant between the two groups ( P<0. 05). Six month after the treatment in PDT group: the BCVA increased while the CMT decreased compared with that of pretreatment ( P <0. 05 ). Six month after the treatment in combination group: the BCVA increased while the CMT decreased significantly compared with that of pretreatment ( P <0. 01 ); compared with changes of BCVA and CMT in two groups, the difference was significant after treatment (P<0. 05). Compared 1mo with 6mo after treatment:there was no significant difference in the BCVA and CMT changes (P>0. 05). One month after treatment: in PDT group, FFA showed no leakage or reduced leakage of CNV in 11 eyes (69%), and the fundus remained leaky in 5 eyes ( 31%); in combination group, FFA showed no leakage or reduced leakage of CNV in 13 eyes (81%);the fundus remained leaky in 3 eyes (19%). Six month after treatment:in PDT group, FFA showed no leakage or reduced leakage in 10 eyes ( 62. 5%); the fundus remained leaky in 4 eyes ( 25%); two eyes ( 12. 5%) relapsed leakage; in combination group, FFA showed no leakage or reduced leakage of CNV in 15 eyes (94%);the fundus remained leaky in 1 eye (6%).
CONCLUSlON: Not only PDT but also PDT and intravitreal injection with ranibizumab can block CNV of pathologic myopia completely or partly, and reduce the danger causing descent of vision. Effects and the stability of the combination therapy is superior to PDT treatment.
6.Association between HbA1C level and severity of coronary artery lesion in non-diabetic patients with coronary heart disease

To investigate the association between HbA1C level and severity of coronary artery lesion in non-diabetic patients with coronary heart disease. This study was a retrospective and controlled study in a single center. 302 patients were enrolled. Coronary arterial lesion was confirmed by coronary angiography carried out in Tianjin Chest Hospital between January 2008 and November 2011. The patients enrolled should be non-diabetic with normal fasting blood glucose and HbA1C levels. The patients were divided into 3 groups according to the HbA1C levels, Group 1 (≤5. 5%, n=92),Group 2(5. 6%-5. 8%, n=94),Group 3(5. 9%-6. 4%, n=116), and the SYNTAX scores between the groups were compared. The predictive value of HbA1C in groups with intermediate and high SYNTAX score was identified by Logistic regression analysis while common risk factors such as sex, age, hypertension, lipid, and fasting glucose were adjusted. The SYNTAX score from Group 1 to Group 3 had statistically significantdifference(P<0.01). Simpleregressionwasusedtocalculatethecorrelationcoefficient(r=0.335,P<0. 01). Binary Logistic regression was used to confirm that HbA1C level was also associated with intermediate and high SYNTAX score(OR=5. 089, P<0. 05). In non-diabetic patients, the HbA1C level is associated with the severity of the lesion of coronary artery. The results indicate that a higher level of HbA1C seems to be an independent predictor to the prevalence of complex coronary lesions.
7.The analysis of the sacroiliac joint CT examination in the detection rate and missed diagnosis of sacral canal cyst

Objective To investigate the detection rate and missed diagnosis reason of sacral canal cyst with sacroiliac joint CT scan.Methods Retrospectively analyzed the changes of sacral canal in 1 286 cases with sacroiliac joint CT scan.CT scanning included bone algorithm reconstruction and standard algorithm reconstruction.Results Among 1 025 patients who had negative sacroiliac joint CT scan were found in 36 cases of sacral canal cyst,the detection rate was 3.5% (36/1 025).Single cyst in 34 cases and multiple cysts in 2 cases.Cyst in S1 level 5 cases,S1-2 level 4 cases,S2 level 22 cases,S2-3 level 4 cases,S3 level 3 cases.The minimal short diameter of the cysts was 0.5 cm and the maximal diameter was 3.2 cm,average 1.2 cm.CT scan showed a round uniform cystic low density,and combined with sacral canal expansion in 19 cases.CT scan used bone algorithm reconstruction in 29 cases,used standard and bone algorithm reconstruction in 7 cases.Clinical findings and correctly diagnosed in 11 cases,accounted for 30.6% (11/36),about 69.4% (25/36) cases were missed diagnosis.Conclusions About 3.5% patients are found sacral canal cyst who have negative sacroiliac joint CT scan.Both the standard and bone algorithm reconstruction should be used to avoid the missed diagnosis of sacral canal cyst.
8.Healing effect of porcine acellular dermal matrix combined with split-thickness autologous skin composites on full-thickness cutaneous wounds in SD rats

Objective To prepare a new type of micropore porcine acellular dermis matrix with the aid of laser (LPADM),and to validate the healing effect of the LPADM through the phrase Ⅰ composite transplanting on the back of the full- thickness skin defects in SD rats.Methods In vitro,the allogeneic fibroblasts were separately cultured with the LPADM (LPADM group) or the non-pore PADM (non-pore LPADM group),while fibroblasts cultured by pure medium were used as control.After culture of 1 day,3 days and 5 days,the contents of IL-10,IL-6,TGF-β1,LN,VEGF expressed by fibroblasts were determined by double-antibody sandwich ELISA method.In vivo,the phrase Ⅰ transplantations of LPADM graft with split-thickness autologous skin were carried on the backs of the full-thickness cutaneous defects of SD rats.The healing condition was observed and analyzed by histological tests.Results The differences of the absorbance value between the LPADMgroup,PADM group and control group in each day were not statistically significant (F=0.050-1.763,P>0.05).The transplantation of LPADM graft with split-thickness autologous skin graft resulted in high rate of surviving without signs of rejection 3 weeks later.After 1-month of transplantation,the regenerated skin was well enough to be lifted without any serious scars.Conclusions The phrase Ⅰ transplantation of LPADM graft with split-thickness autologous skin graft can accelerate the healing process of full-thickness skin wounds with high biological safety.
9.Coping strategies and influence factors among primary glaucoma patients

Objective To explore coping strategies and influence factors in primary glaucoma patients through describing characteristics of their coping strategies.Methods From November 2007 to June 2008,data were collected from 120 primary glaucoma patients in three hospitals in Guangzhou and Shenzhen.The final sample consisted of 100 patients and the data underwent analysis.Results Among different coping styles of primary glaucoma patients,facing was lower than norm,avoiding and yielding were higher than norm.The factors related to coping style were age,different cohabitant,different economic condition,different occupation,different educational level and health payment mode.Conclusions Primary glaucoma patients usually took avoiding or yielding coping styles.Nurses should help patients with low income and education background to face the disease positively.Besides,fully consideration should be given to patients with different age,at the same time,marriage and family support should also exert active function.
10.Effect of CCK-8 on expression of MMPs/TIMP-1 in TNF-α-induced RSC-364

Aim To observe the influence of CCK-8 on expression of MMPs/TIMP-1 in TNF-α-induced rat fibroblast-like synovial cell line RSC-364.Methods The secretion levels of MMP-1, MMP-3, MMP-9 and TIMP-1 were determined using ELISA;MMP-3 and MMP-9 mRNA expressions were detected by RT-PCR.Results MMP-3 and MMP-9 could not be examined in RSC-364 incubated with CCK-8 and unstimulated RSC-364, which was able to product a little MMP-1, TIMP-1 and express even less MMP-3,-9 mRNA.CCK-8 inhibited the increase in MMP-1, MMP-3, MMP-9 secretion and MMP-3,-9 mRNA expression in TNF-α-induced RSC-364.TIMP-1 production was also increased in TNF-α-induced RSC-364.CCK-8 had no effect on TIMP-1 production in TNF-α-induced RSC-364, but was able to reduce the ratios of MMP-1, MMP-3, MMP-9 to TIMP-1.Conclusion The inhibitory effect of CCK-8 on MMPs activity may be related to the decrease of MMPs mRNA expression, MMPs secretion and the ratios of MMPs to TIMP-1 in TNF-α-induced RSC-364, which indicates that CCK-8 might be a possible regulator in the pathogenesis of rheumatoid arthritis.
