1. Chemical constituents from Cardamine leucantha

Objective: To investigate the chemical constituents in the whole herbs of Cardamine leucantha. Methods: The chemical constituents were isolated and purified via silica gel, polyamide, D-101 macroperous resin, and Sephadex LH-20 column chromatography. Their structures were determined on the basis of spectral analysis and physicochemical properties. Results: Fourteen compounds were isolated and identified, covering three simple indoles: indole-3-carboxylic acid (1), 6-hydroxyindole-3-carboxylic acid (2), and 6-hydroxyindole-3-carboxylic acid-6-O-β-D-glucopyranoside (3); two coumarins: esculetin (4) and esculin (5); seven steroids: 3-O-(p-coumaroyl)-β-sitosterol (6), β-sitosterol (7), 5α, 8α-epidioxyergosta-6, 22-dien-3β-ol (8), 5α, 8α-epidioxyergosta-6, 9 (11), 22-trien-3β-ol (9), sitost-5-ene-3β, 7α-diol (10), β-sitosteryl-3β-glucopyranoside-6'-O-palmitate (11), and daucosterol (12); and two phenolic compounds: p-hydroxybenzoic acid (13) and vanillic acid (14). Conclusion: All the compounds are isolated from the plants in Cardamine L. for the first time.
2.Application of Fast Track Surgery in Perioperative Nursing Care of Laparoscopic Cholecystectomy

Objective To investigate the clinical effects of fast track surgery ( FTS ) in perioperative nursing care of laparoscopic cholecystectomy . Methods A total of 200 cases of laparoscopic cholecystectomy between January and December 2012 were randomly divided into two groups , with 100 cases of each group , receiving either FTS or conventional treatment .Comparison of the two groups in patients rehabilitation and complications were observed . Results The postoperative anal flatus time was significantly shorter in the FTS group (12.0 ±4.4) h than that in the control group (24.9 ±5.4) h (t=-18.519, P=0.000).The postoperative hospital stay was significantly shorter in the FTS group (3.2 ±1.3) d than that in the control group (4.2 ±2.1) d (t=-4.049, P=0.000).The patients’ weight loss at the 5 postoperative day was significantly less in the FTS group (3.2 ±0.3) kg than that in the control group (5.2 ±0.6) kg (t=-29.814, P=0.000). Conclusion FTS measures applied in laparoscopic cholecystectomy can significantly accelerate postoperative rehabilitation .

5.Effect of various surface conditionings on the shear bond strength of orthodontic brackets to porcelain

Objective:To evaluate the effect of various surface treatments with different water bath time and thermocycling on the shear bond strength of orthodontic brackets to the feldspathic porcelain surfaces.Methods:Four surface conditioned methods were applied on 128 disc-shaped porcelain specimens.Metal or ceramic brackets were bonded onto each treated porcelain disc with light-cured resin.The samples were randomly assigned and bathed in 37 ℃ water 1 day or 7 days respectively,and thermocycled 500 times from 5 ℃ to 55 ℃.The shearing strengths were measured and bond failure sites were classified according to the adhesive remnant index system.The treated porcelain surfaces were examined with scanning electronic microscopy.Results:Almost the same effects of the four conditionings on the two types of brackets were obtained.All groups achieved adequate bond strengths to withstand the application of orthodontic forces.Conclusion:The optimal method for orthodontic brackets bonding to feldspathic porcelain is to apply orthophosphoric acid combined with silane.

7.The treatment of liver trauma in 106 cases

Objective To improve the diagnosis and treatment of liver trauma. Methods We retrospectively analyzed different therapeutic means on liver trauma including 79 cases with operative treatment and 27 cases with nonoperative treatment. Results In nonoperative treatment group, 25 cases were cured, 2 cases died. In operative treatment group, 73 cases were cured, 6 cases died. Cure rate was 92.5%(98/106), mortality was 7.5%(8/106). 5 cases died of major blood vessels rupture, 3 cases died on multiple organ function failure. Postoperative complications included 3 cases of subphrenic infection, 10 cases of hepatic abscess, 8 cases of pleural hydrops, 7 cases of incisional infection. Conclutions Type I of liver trauma can be treated by nonoperation, type Ⅱ~Ⅳ of blunt liver trauma can be treated by nonoperation on the condition of intensive monitoring. Type Ⅱ~ Ⅵ of liver trauma should be operated on emergently in case of massive intraabdominal bleeding and combined organ injury.
8.Predictors of catheter-related bladder discomfort after urological surgery.

The aim of this study was to figure out the predictors of early postoperative catheter-related bladder discomfort (CRBD) after urological surgery. We designed a prospective observational study in our hospital. Consecutive adult patients undergoing surgery under general anaesthesia or epidural anaesthesia necessitating urinary catheterization were included during a 3-month period. severity of bladder discomfort was assessed on a 4-point scale: (1) no pain, (2) mild pain (revealed only by interviewing the patient), (3) moderate (a spontaneous complaint by the patient of a burning sensation in the urethra and/or an urge to urinate and/or sensation of urethral foreign body without any emotional agitation) and (4) severe discomfort (agitation, loud complaints and attempt to remove the bladder catheter associated with a burning sensation in the urethra). Predictors of CRBD were identified by univariate and multivariate analysis. Totally, 116 patients were included, of which 84.5% had CRBD (mild CRBD: 40.5%; moderate or severe CRBD: 44.0%) at day 1, while 31.9% developed CRBD (mild CRBD: 29.3%; moderate or severe CRBD: 2.6%) at day 3. We evaluated 9 potential forecast factors of CRBD, and univariate Chi-square test showed male gender [OR=2.4, 95%CI (1.1-5.6), P<0.05], abdominal open surgery compared with transurethral surgery [OR=0.3, 95%CI (0.1-0.6), P<0.05], abdominal surgery compared with laparoscopic surgery [OR=3.3, 95%CI (1.2-8.9), P<0.05] and history of catheterization [OR=0.5, 95%CI (0.2-0.9), P<0.05] were independent predictors of moderate or severe CRBD in the patients after surgery. While multivariate logistic regression analysis showed that the abdominal open surgery [EXP(B)=3.074, 95%CI (1.3-7.4), P<0.05] and the history of catheterization [EXP(B)=2.458, 95%CI (1.1-5.9), P<0.05] might contribute more to the occurrence of moderate or severe CRBD. In conclusion, this observational study identified that the type of surgery and the history of catheterization might be predictive factors of moderate and severe CRBD after urological surgery.
9.The therapeutic effect of 3D-conformal radiotherapy on metastatic hepatic cancer

Ojective To evaluate the therapeutic effect of conformal radiotherapy on metastatic hepatic cancer. Methods 10 cases of metastatic hepatic cancer (1 to 5 metastatic focus respectively) were treated with stereotactic radiotherapy from Septermber 1997 to April 2000. The clinical target volume(CTV) of tumor ranged from 0.6cm 3 to 232cm 3 (the mean 41.9cm 3), peripheral irradiation doses of planning target volume (PTV) ranged from 3.96Gy to 7.00Gy(the mean 5.26Gy)each time, cential irradiation doses of PTV were 6.3Gy~10.88Gy (the mean 7.89Gy) each time, once a day for 5~8 times totally. Results No patient died during therapy. KPS of patients normal condition were 20~90(54?25.90) before treatment and 30~100 (74?23.90) after treatment. Effect of entity tumor were as follows completely relaxed (CR) in two cases, partly relaxed (PR) in four cases, no change (NC) in three cases, progressed (PD) in one case. During follow-up period tasting 2 to 28 months 90% of the tumors were controlled (no progressive disease). 60% fo the tumors decreased in size or disappeared. Conclusions Stereotactic radiotherapy has better therapeutic effects on cases of metastatic hepatic cancer.
10.Determination of benzyl glucosinolate in Lepidium meyenii from different regions by HPLC.

The content of benzyl isothiocyanate (BITC) which as the enzymatic hydrolysis product of benzyl glucosinolate through thioglucosidase was determined by HPLC. The content of benzyl isothiocyanate (BITC) which as the enzymatic hydrolysis product of benzyl glucosinolate through thioglucosidase was determined by HPLC. The chromatography condition was as follows: Kaseisorb LC ODS 2000 (4.6 mm x 150 mm, 5 min) column with the mobile phase of acetonitrile(A)-water( B) under gradient elution (0-5 min, 3%-8% A; 5-9 min, 8%-48% A; 9-23 min, 48%-62% A; 23-28 min, 62%-99% A); the flow rate was 1.0 mL x min(-1) with 10 microL injection volume; detection wavelength was 246 nm and temperature of column was 40 degrees C. The content of benzyl glucosinolate was in the range of 10.76-17.91 g x L(-1). The method is simple, accurate and good reproducibility which can be used for the determination of benzyl glucosinolate in Lepidium meyenii, effectively.
