1.Expression of IRS-2 in the Liver of High Fat and Sugar Diet Induced NAFL Mice

Objective To investigate the expression of insulin receptor substrate-2(IRS-2)in the liver of nonalcoholic fatty liver(NAFL)mice.Methods Ten female C57BL6J mice fed with normal diet were served as controls,while fifteen fed with high fat and sugar diet for either 8 weeks(group-A)or 16 weeks(group B)were as NAFL mice models.Abdominal fat mass,total cholesterol(TC),triglycerides(TG),high density lipoprotein cholesterol(HDL-C),fasting blood glucose(FBG)insulin(INS)and liver function,liver weight were measured.And the expression of IRS-2 and the content of lipid in the liver were also detected.Results The abdominal fat mass serum TC,and FBG were significantly higher in high fat and sugar diet fed mice than that in controls(P

(3) The major side effects included Cushing's reaction and the infection of the upper respiratory tract.Conclusion Steroids can quicken the recovery of GBS, and shortened average hospitalization in patients with GBS, but steroid doesn't alter the incidence rate of the sequel and the severity of the disability at 1 year in the course.It has no side reaction to use steroid reasonably.

5.Study of platelet aggregation induced by shear stress using flow cytometry

Objective To establish a flow cytometric method for assay of platelet aggregation induced by shear stress(SIPA) and investigate its clinical value. Methods After the whole blood was subjected to a certain shear stress by a rotational viscometer, the platelets were activated and aggregated. Then the fluorescence antibody CD61 PerCP was added to stain the platelets. The free platelets and aggregates were counted respectively by flow cytometry, thereby the ratio of platelet aggregation was calculated. Results Shear stress-induced whole blood platelet aggregation varied with the increase of shear stress and time of stress. Only a small amount of platelet aggregation was detected at relatively low shear stress of 100S -1, and it was increased at 500S -1. However, a significant increase occurred with 2 000S -1. It approached to the maximum value (58.4%?5.3%) at 3 000S -1 with exposure of 3min, which was higher than the values at 100S -1(9.4%?1.3%)and 500S -1(26.4%?3.2%). At the same time, the aggregates increased in size and became more compact with increase in the shear stress and prolongation of time. High shear stress induced platelet aggregation(H-SIPA) was significantly enhanced in patients with AMI, cerebral infarction, TIA or diabetes, all of them were susceptible to coagulopathy, compared with normal control. Conclusion The results indicated that shear stress could induce platelet aggregation directly, and the degree of SIPA had positive correlation with the stress stength and exposure time.The measurement of H-SIPA was of some clinical value in the diagnosis of arterial thrombosis.
6.Predictors of catheter-related bladder discomfort after urological surgery.

The aim of this study was to figure out the predictors of early postoperative catheter-related bladder discomfort (CRBD) after urological surgery. We designed a prospective observational study in our hospital. Consecutive adult patients undergoing surgery under general anaesthesia or epidural anaesthesia necessitating urinary catheterization were included during a 3-month period. severity of bladder discomfort was assessed on a 4-point scale: (1) no pain, (2) mild pain (revealed only by interviewing the patient), (3) moderate (a spontaneous complaint by the patient of a burning sensation in the urethra and/or an urge to urinate and/or sensation of urethral foreign body without any emotional agitation) and (4) severe discomfort (agitation, loud complaints and attempt to remove the bladder catheter associated with a burning sensation in the urethra). Predictors of CRBD were identified by univariate and multivariate analysis. Totally, 116 patients were included, of which 84.5% had CRBD (mild CRBD: 40.5%; moderate or severe CRBD: 44.0%) at day 1, while 31.9% developed CRBD (mild CRBD: 29.3%; moderate or severe CRBD: 2.6%) at day 3. We evaluated 9 potential forecast factors of CRBD, and univariate Chi-square test showed male gender [OR=2.4, 95%CI (1.1-5.6), P<0.05], abdominal open surgery compared with transurethral surgery [OR=0.3, 95%CI (0.1-0.6), P<0.05], abdominal surgery compared with laparoscopic surgery [OR=3.3, 95%CI (1.2-8.9), P<0.05] and history of catheterization [OR=0.5, 95%CI (0.2-0.9), P<0.05] were independent predictors of moderate or severe CRBD in the patients after surgery. While multivariate logistic regression analysis showed that the abdominal open surgery [EXP(B)=3.074, 95%CI (1.3-7.4), P<0.05] and the history of catheterization [EXP(B)=2.458, 95%CI (1.1-5.9), P<0.05] might contribute more to the occurrence of moderate or severe CRBD. In conclusion, this observational study identified that the type of surgery and the history of catheterization might be predictive factors of moderate and severe CRBD after urological surgery.
7.Therapeutic effect of photodynamic therapy and intravitreal injection with ranibizumab for choroidal neovascularization of pathologic myopia

AlM: To investigate the efficacy of photodynamic therapy ( PDT) and intravitreal injection with ranibizumab on macular choroidal neovascularization ( CNV ) of pathologic myopia ( PM) .
METHODS: There were patients ( 32 eyes ) who were diagnosed as PM with CNV. Randomly selected 16 cases ( 16 eyes ) which were given the PDT treatment ( PDT group ) . The remaining were given both PDT and intravitreal injection with ranibizumab ( combination group) . There is no significant difference on macular edema between two groups. We analyzed the changes in the best corrected visual activity ( BCVA) , optic coherence tomograph ( OCT ) and fundus fluorescein angiography (FFA) before and 1, 6mo after treatment.
RESULTS:One month after the treatment in PDT group:the BCVA increased while the CMT decreased compared with that of pretreatment (P<0. 05). One month after the treatment in combination group: the BCVA increased while the CMT decreased significantly compared with that of pretreatment (P<0. 01);the changes of BCVA and CMT showed statistically significant between the two groups ( P<0. 05). Six month after the treatment in PDT group: the BCVA increased while the CMT decreased compared with that of pretreatment ( P <0. 05 ). Six month after the treatment in combination group: the BCVA increased while the CMT decreased significantly compared with that of pretreatment ( P <0. 01 ); compared with changes of BCVA and CMT in two groups, the difference was significant after treatment (P<0. 05). Compared 1mo with 6mo after treatment:there was no significant difference in the BCVA and CMT changes (P>0. 05). One month after treatment: in PDT group, FFA showed no leakage or reduced leakage of CNV in 11 eyes (69%), and the fundus remained leaky in 5 eyes ( 31%); in combination group, FFA showed no leakage or reduced leakage of CNV in 13 eyes (81%);the fundus remained leaky in 3 eyes (19%). Six month after treatment:in PDT group, FFA showed no leakage or reduced leakage in 10 eyes ( 62. 5%); the fundus remained leaky in 4 eyes ( 25%); two eyes ( 12. 5%) relapsed leakage; in combination group, FFA showed no leakage or reduced leakage of CNV in 15 eyes (94%);the fundus remained leaky in 1 eye (6%).
CONCLUSlON: Not only PDT but also PDT and intravitreal injection with ranibizumab can block CNV of pathologic myopia completely or partly, and reduce the danger causing descent of vision. Effects and the stability of the combination therapy is superior to PDT treatment.
8.Role of endoplasmic reticulum stress in diabetic atherosclerosis

The diabetic patients died of cardiovascular disease mainly. However, the molecular and cellular mechanisms of diabetes complicated with atherosclerosis were still unclear. Endoplasmic reticulum (ER) stress may play a vital intermediate role in the progression of diabetic atherosclerosis. Both of diabetic hyperglycemia induced hexosamine pathway and diabetes induced macrophage insulin resistance can lead to ER stress. The unfolded protein response (UPR) produced by ER stress can be observed throughout the whole development course of atherosclerosis. In addition, ER stress also induced lipid deposition, cell apoptosis and inflammatory response, all of which was important for the progression of atherosclerosis. Though ER stress may be key for diabetes induced atherosclerosis, which pathway played the central role and the interaction between cytokines still need to be investigated. So that we could provide individual therapeutic scheme for treatment as well as prevention of diabetic complication.
9.Two dimensional finite element analysis of ceramic veneer bonding layer

Objective: To intimately assess the internal stress distribution in ceramic veneer bonding layer. Methods:Six buccolingual cross section models of the incisor restored with IPS-Empress ceramic veneer were built with the MSC/NASTRAN software. 2D-finite element method was used to analyze the stress distribution regularity of the bonding layer on the models under different functional load. Results: Stress concentration areas of ceramic veneer bonding layer were regularly located on the incisal margin and cervical margin. Under the same condition, incisal load caused highest stresse on the adhesive layer. Following the load shift from incisal to gingival area, the stress was gradually decreased.When the functional load acted on the palate margin of the restoration, the compressive stress on the bonding layer was significantly increased. Different load angle generated stress change in the bonding layer. The shear stress on adhesive layer increased following the horizontal load angle increasing close to the debonding level. Among the different incisal reduction designs there was no significant Von Mises stress variation under the same load condition. However, when the incisal edge was reduced by more than 4 mm, the tensile stresses in the bonding layer greatly increased. Conclusion: Restoration margin should be kept away from the tooth contact area, be exposed to minimal occlusion contact. The functional load should be induced to follow the tooth long axis. Edge to edge occlusion should be avoided. When incisal areas need to be altered, the incisal overlap or butt margin design will not cause any stress problem in the bonding layer. When restoring the incisal defect for more than 4 mm, the bonding agents must be selected carefully.
10.Cutoff Waist?to?height and Waist?to?hip Ratios for Metabolic Syndrome in Chinese Children and Adolescents

Objective This study aimed to identify high?risk groups for metabolic syndrome(MS)among Chinese children and adolescents by calculating the cutoff values of adiposity measures and insulin resistance(IR). Methods From December 2010 to January 2011,data from 933 Chinese subjects(492 boys and 436 girls,aged 11?16 years)from Liaoyang City,China,were included. Questionnaires were administered,and da?ta such as anthropometric parameters(height,waist circumference,and hip circumference),blood pressure,fasting blood glucose level,blood lip?id level,serum uric acid level,and fasting insulin level were collected. Receiver?operating characteristic(ROC)curve analysis was used to deter?mine the optimal cutoff waist?to?height ratio(WHtR),waist?to?hip ratio(WHR),homeostasis model assessment?insulin resistance(HOMA?IR) index,triglyceride level,glucose index(TyG index),and serum uric acid(SUA)level for identification of children and adolescents with MS. Results The AUC of WHtR,WHR,TyG index,HOMA?IR index,and SUA level were larger than 0.5,with P values of<0.01. The optimal cutoff WHtR and WHR derived from the ROC analysis were respectively 0.48(87.0%,84.1%)and 0.84(83.3%,70.3%)for boys and 0.46(95.1%, 81.8%)and 0.80(90.5%,71.8%)for girls. Conclusion This study demonstrated that WHtR and WHR are useful predictive markers of MS , while HOMA?IR,TyG index,and SUA level are not. The usefulness of these criteria needs to be verified by further evaluation.
