1.The Effect of Acute Anemia on The Healing of Intestinal Anastomoses in Rabbits.

In consideration of the potential deleterious effects of blood transfusion, the effect of acute anemia on the healing of both small (SI) and large intestinal (LI) anastomoses was evaluated. Thirty-six white rabbits underwent resection of both the mid-ileum and the distal colon with primary anastomosis of each. One and two weeks post operation, all anastomoses were assessed by anastomotic bursting pressure (ABP) and histologic evaluation utilizing a modified Erhlich/Hunt scale with grading of the WBC influx, blood vessel ingrowth, fibroblast proliferation, and collagen deposition. Results were analyzed using oneway analysis of variance (ANOVA) and the Chi-Square approximation with p<0.05 being considered significant. The SI ABP, reported in mmHg SE, of the anemic rabbits was decreased at one week vs. the controls (234 mmHg 17 vs. 201 mmHg 16), reaching significance at two weeks (190 mmHg 6 vs. 257 mmHg 17). A decreased LI ABP was found at one week, though not quite reaching singnificance (p=0.06). Recovery to control values occurred by two weeks. Histologic assessment (anemia vs. control) demonstrated a uniform decrease in WBC infiltration, reaching significance in the LI group at two weeks. Blood vessel ingrowth was likewise depressed, but did not achieve significance. The collagen contents of both the SI and the LI groups at two weeks decreased, as were the SI fibroblast proliferation at one week (p<0.05). This study demonstrates that severe, acute anemia in rabbits undergoing intestinal anastomoses severely alters histologic parameters in both the SI and the LI anastomoses while decreasing the overall strength of the SI anastomoses.

2.Comparative Analysis of the Extracellular Matrix Composition in Proliferating and Involuted Infantile Hemangiomas.

BACKGROUND: Changes in the composition of the extracellular matrix (ECM) occur between the proliferating and involuted phases of infantile hemangiomas (IH), and are associated with angiogenic growth. We examined the composition of the ECM in proliferating and involuted IHs and assessed correlations between the composition of the ECM and whether the IH was in the proliferating or the involuted phase. METHODS: We evaluated IH samples from a cohort of patients who had five proliferating IHs and five involuted IHs. The following ECM molecules were analyzed using enzyme-linked immunosorbent assays and immunohistochemistry: laminin, fibronectin, collagen type I, collagen type II, and collagen type III. RESULTS: The involuted IHs had higher levels of deposition of collagen type III than the proliferating IHs. The median values (interquartile ranges) were 1.135 (0.946-1.486) and 1.008 (0.780-1.166) (P=0.019), respectively. The level of laminin was higher in involuted IHs than in proliferating IHs, with median values (interquartile ranges) of 3.191 (2.945-3.191) and 2.479 (1.699-3.284) (P=0.047), respectively. Abundant collagen type III staining was found in involuted IHs. Laminin alpha4 chain staining was clearly present within the basement membrane adjacent to the blood vessels, and was significantly more intense in involuted IHs than in proliferative IHs. CONCLUSIONS: Involuted hemangiomas showed extensive deposition of collagen III and laminin, suggesting that differences in the composition of the ECM reflect stages of the development of IHs. This pattern may be due to the rapid senescence of IHs.

OBJECTIVETo evaluate the detection method of ELISA and Enhanced Chemiluminescence Immunoassay (ECLIA) in use to determine serum hyaluronate acid (HA), laminin (LN), type IV collagen (IV-C) and type III procollagen (PC III).
METHODS253 patients with chronic hepatitis B were determined the four liver fibrosis serum markers with both the ECLIA and ELISA, and then compared with pathology results separately.
RESULTSBoth the detection results of ELISA and ECLIA can reflect that the patient's liver fibrosis from hepatitis to liver cirrhosis aggravated gradually. Compared with ELISA, the results of ECLIA and pathology have a better correlation.
CONCLUSIONSThe detection of four liver fibrosis serum markers by ECLIA could indicate the better the response of the state of live fibrosis.

5.A Case of Pseudoangiomatous Spindle Cell Lipoma on the Abdomen.

Spindle cell lipoma is a benign, subcutaneous neoplasm typically located on the neck, back and shoulder of men. Pseudoangiomatous spindle cell lipoma is a rare variant of spindle cell lipoma that is characterized by non-vascular branching spaces. A 64-year old man presented with 2 masses on the abdomen. Although 1 tumor had a very low fat component, it could be histologically diagnosed as pseudoangiomatous spindle cell lipoma due to CD 34 positive spindle cells, ropey collagen and blood vessels. We report a case of pseudoangiomatous spindle cell lipoma on the abdomen, which was diagnostically difficult due to a scanty fat component.

8.Effects of oral aspirin on Platelet function analyzer-100 collagen-epinephrine closure time in patients undergoing elective surgery.

BACKGROUND: There is a growing need for precise blood coagulation tests in preparation for an operation, as a growing number of patients take aspirin during the run-up to their operation. Against this backdrop, we planned to find what effect aspirin has, if taken regularly prior to an operation, on primary hemostasis. We also attempted to determine the possibility of regional anesthesia in cases which the Platelet function analyzer-100 collagen-epinephrine closure time (PFA-100 C/EPI CT) is prolonged. METHODS: We examined three groups the impact of aspirin on the result of the PFA-100 C/EPI CT: Group A (n = 60) consisted of control; Group B (n = 54) who taking aspirin every day until surgery; and Group C (n = 60) was composed of patients who stopped taking aspirin for 7 days before surgery. PFA-100 collagen adenosine-5-diphosphage closure time were only performed on those with abnormal PFA-100 C/EPI CT. RESULTS: PFA-100 C/EPI CT of Group B was significantly longer than Group C and Group A. There was no distinction between Group A and Group C in terms of the PFA-100 C/EPI CT. CONCLUSIONS: PFA-100 C/EPI CT has better sensitivity and specificity than a bleeding test; moreover, it is an easy-to-use and point-of-care test. We concluded that regional anesthesia can be performed for those who taking aspirin until their surgery despite their prolonged collagen-epinephrine closure time except prolonged collagen adenosine-5-diphosphate closure time.

9.Immuno-expression of Type IV Collagen in the Intervertebral Disc.

STUDY DESIGN: In vitro studies using human intervertebral disc for the localization of the type IV collagen. OBJECTIVE: 1) To study the distribution pattern and immunoexpression of type 4 collagen in the intervertebral disc, 2) To study the function of type IV collagen in the intervertebral disc. SUMMARY OF BACK GROUND: The correlations of degeneration changes and collagens in the dics have not been determined. The reports for type IV collagen were few. So far, the histologic analysis for the expression of type IV collagen in the intervertebral disc has not been done. There was no report to study the function of the type IV collagen in the intervertebral disc. METHODS: Fifty-four disc blocks obtained during anterior interbody fusion of the lumbar spine were used to observe the expression pattern of the type IV collagen with immunochemical stain. For the observation of the myxomatous degeneration in the intervertebral disc, the alcian blue stain with periodic acid-schilff was done. For the control group, 22 neonate intervertebral disc blocks were obtained at autopsy. RESULTS: The immunoreactions for type IV collagen were associated blood vessels in the anulus fibrosus in the disc. There was no statistical significant difference of the type IV collagen expression between the control and disease groups. Myxomatous degenerations were observed as the irregular form in the degenerative intervertebral disc. CONCLUSION: The immunoreactions for the type IV collagen were observed in the intervertebral discs and associated with the formation of the blood vessels, especially in anulus fibrosus.

10.Liver fibrosis indexes in nonalcoholic fatty liver disease.

OBJECTIVETo assess the clinical utility of liver fibrosis indexes determination in patients with nonalcoholic fatty liver disease.
METHODSLiver fibrosis indexes of 79 patients with fatty liver, diagnosed by B ultrasonic echography, 37 patients with posthepatitic cirrhosis patients and 32 healthy subjects were determined.
RESULTSThe level of procollagen III(PC III), hyaluronic acid (HA), collagen IV(C IV), laminin (LN) in the fatty liver cases were significantly higher than those of healthy subjects (P<0.05 - 0.01), but lower than those in the liver cirrhosis cases (P<0.01). The difference of PC III, C IV, LN between the severe and early fatty liver cases was significant (P<0.05 - 0.01). Positive rate of these indexes was higher in severe cases.
CONCLUSIONThere is a trend of liver fibrosis in severe fatty liver patients. Combined determination of liver fibrosis indexes may help the diagnosis of liver fibrosis in the fatty liver patients.
