It was suggested that temporary vascular insufficiency within the wall of the colon causes iachemic colitis. So, motivated by this fact, we studied and analyzed 15 cases of iachemic colitis, which were confirmed by the diagnostic criteria of Nagasako(1982), through 32 months from January, 1993 to August, 1995 at Our Lady of Mercy Hospital, Incheon.(continue...)

Pseudopolyp is a common association of ulcerative colitis but may also occur in Crohn's disease, ischemic colitis and infective colitis. Pseudopolyps are results of the repair process, representing islands of granulation tissue surrounded by mucosa with ulceation. When a pseudopolyp is larger than 1.5 cm, it is often called a "giant pseudopolyp". Such lesions of inflammatory origin are often confused with polypoid carcinoma or infiltrating submucosal malignancy. We report a case of asymptomatic Crohn's disease as giant pseudopolyposis mimicking malignancy on sigmoid colon, with a review of relevant literatures.

Ischemic colitis is an uncommon disease in Korea, but nowadays, the incidence of this disease is increasing in Korea. The reason is that the number of old patients is increasing and colonoscopic diagnosis is performed more frequently. It is especially important to differentiate it from other inflammatory bowel disease, such as infectious colitis, ulcerative colitis, Crohn's disease, and drug-induced colitis, because ischemic colitis is similar to other inflammatory bowel disease in symptoms and colonoscopic findings. However, the authors were able to differentiate ischemic colitis from other inflammatory bowel disease by close examination of a patient's history, microscopic examination and culture studies of the bacteria in the stool, and close observation of the changes in the colonoscopic findings with time. We experienced six cases of ischemic colitis and report them along with a brief review of the literature.

Ischemic colitis can be caused either by vascular or colonic wall factor or by both. Shunting of blood away from the mucosa may contribute to ischemia of the colon, but the mechanism of ischemia is not known. Ischemic colitis precipitated by colonoscopy has rarely been reported. Potentially air insufflation or mechanical effect during colonoscopic examination can cause ischemic colitis as a colonic wall factor. Recently, we experienced a case of ischemic colitic, which had been developed several hours after colonoscopic polypectomy. So we report this case with brief review of literature.

Ischemic colitis is the most common form of intestinal ischemia. Although there are a variety of causes, the most common mechanism is acute, self-limited compromise in intestinal blood flow. Most cases of ischemic colitis are transient and resolve spontaneously, but right colon involvement is associated with poor outcome. Colonoscopy is the gold standard for the identification of colonic ischemia; however, very rarely, colonoscopy itself may induce ischemic colitis in patients with underlying predisposing factors, such as connective tissue disorders. We experienced a case of ischemic colitis of the right colon, following colonoscopy in a patient without risk factors.

Ischemic colitis is generally considered a disease of the elderly. The causes of ischemic colitis include low-flow states due to cardiac dysfunction or hypovolemia and certain medications including estrogen. Here we report a case of ischemic colitis in a 26-year-old woman. She had no specific medical history except taking oral-contraceptives for a long time. The mechanism of estrogen-induced ischemic colitis is not clearly understood. But we recommend that oral-contraceptives should be considered as a cause of ischemic colitis in young women.
