An evaluation of combination of scar removal surgery by laser CO2 with steroid injection triamcinolone was carried out in 10 patients with the keloid and hypertrophy scars according to the criteria of Cosman 1996, Peacok 1970 and Cohen 1976. The results have shown that the combination was a simple method with easy implementation and convenience for patients. There were no complications or accidents during treatment period.

Study on 100 patients with contractile scars on faciocervical area treated in Central Military Hospital No.108 from January 1995 to January 2004. The subjects were divided into groups with different mentocervical angle reconstruction methods such as using local flaps, skin graft or fasciocutaneous flaps. The results: the scars on faciocervical area often changed mentocervical angle, which affected greatly to aestheticism and function. The reconstruction of mentocervical angle needed to base on lesion’s levels to find out an appropriate treatment. The best results was achieved by using local flaps, however, it was only suitable for small lesions, less cervical deformations. Using skin graft and fasciocutaneous flaps was applied for serious lesions and could still make a rather standard mentocervical angle .

This paper reviews the new progress in the research of fractional carbon dioxide laser in treating hypertrophic scar after burn injury, which remains a challenging problem for burn care surgeons. There have been many traditional therapeutic approaches, such as compression remedy, operation, and so on. However, a satisfactory method is lacking to date. In recent years, the newly developed fractional carbon dioxide laser has been employed to treat different kinds of scars, and it has been proved to be effective in terms of an improvement of scar color, texture, and rigidity. It seems to be a promising method for scar treatment in future.

OBJECTIVETo explore the application of liposuction in scar repair.

METHODSLiposuction using tumescent technique was performed at the scar region. While the local skin was relaxed and tension decreased, scar resection and wound closure followed. The indications and key points of this operation are discussed in the article.

RESULTSThis method has been used in 27 patients for scar repair since 2000. The biggest area of excised scar was 40 x 9.5 cm. The width of the excised scar reached 11 cm. All the patients got satisfactory results.

CONCLUSIONThe operation was simple and safe with little blood loss. After liposuction, a larger area of scar at a fatty region could be excised, and meanwhile, body sculpture was achieved.

OBJECTIVETo explore the reasonable therapeutic strategy for different types of epicanthus.

METHODSPatients with epicanthus were classificated according to the shape, extent and inner canthal distance and treated with different methods appropriately. Modified asymmetric Z plasty with two curve method was used in lower eyelid type epicanthus, inner canthus type epicanthus and severe upper eyelid type epicanthus. Moderate upper epicanthus underwent '-' shape method. Mild Upper epicanthus in two conditions which underwent nasal augumentation and double eyelid formation with normal inner canthal distance need no correction surgery. The other mild epicanthus underwent '-' shape method.

RESULTSA total of 66 cases underwent the classification and the appropriate treatment. All wounds healed well. During 3 to 12 months follow-up period, all epicanthus were corrected completely with natural contour and unconspicuous scars. All patients were satisfied with the results.

CONCLUSIONSClassification of epicanthus hosed on the shape, extent and inner canthal distance and correction with appropriate methods is a reasonable therapeutic strategy.

BACKGROUND: Minimizing scarring has long been a challenge in plastic surgery. Factors affecting scar formation are well known, but the effect of some patient-specific factors such as dermal thickness remains unverified. Management of factors predictive of scarring can improve postoperative patient satisfaction and scar treatment. METHODS: For 3 years, we used ultrasonography to measure dermal thickness in female patients who had undergone thyroidectomy for cancer at our hospital. We confirmed the influence of dermal thickness on hypertrophic scar formation and the Patient and Observer Scar Assessment Scale scar score 6 months after surgery. RESULTS: There was a positive correlation between dermal thickness and scar score (p < 0.05), and dermal thickness appears to be a cause of hypertrophic scar formation (p < 0.05). CONCLUSION: Thick dermis was found to cause poor scar formation and hypertrophic scarring. Prediction of factors that can influence scar formation can be used to educate patients before surgery and can help in scar management and improvement in patient satisfaction.

A technique for the chemical fixation of cutaneous tumors followed by serial excision using microscopic control has been developed since the mid 1930s. Mohs micrographic surgery is the most reliable and cost effective treatment modslity, offering maximal preservaticn of normal tissue and therefore the lowest functional and cosmetic morbidity in the cancer treatments. We report a case of basal cell carcinoma treated with Mohs micrographic surgery and reconstructed with nasalis myocutaneous sliding flap. The pstient was a 56-year-old woman, who had a been sized basal cell carcinoma on her nasal tip for the previous 3 years. The lesion was removed under complete microscopical control using fresh tissue technique. The defect healed with atrophic scar formation ore and half months later. To remove scar tissue and to gsin proper cosmetic results, reconstruction with nasalis myocutaneous sliding flap was performed successfully.

Recently, minimally invasive approach via limited incision becomes one of the rapidly developing strategy in cardiac surgery as it gives less surgical trauma and is more satisfactory to the patients in respect to operative scar although the surgical technique is more demanding. Here, we report two cases of aortic valve replacement via limited transsternal incision. The methodology is described with review of the relevant literature.

Large surgical defects of the face can often be difficult to repair. And so, extensive adjacent tissue transfer may be necessary and may result in significant scars and possible flap compromise. Often there may be less donor tissue available than is necessary to achieve closure. Four cases of malignant tumors on the face were removed by Mohs micrographic surgery, followed by reconstruction of large defects utilizing a purse-string suture and a full-thickness graft. This method enabled dermatologist to reconstruct large surgical defects of the face with a relatively small full-thickness graft. In our cases, there were no significant complications and the results were cosmetically acceptable.

Dermatofibrosarcoma protuberans is a dermal spindle cell neoplasm of intermediate malignancy, and is characterized by a high local recurrence rate after surgical excision due to its aggressive, finger-like outgrowth pattern. Mohs micrographic surgery, which ensures complete excision of the tumor and minimizes normal tissue loss, is accepted as the treatment of choice for dermatofibrosarcoma protuberans. However, the use of frozen sections for margin control in dermatofibrosarcoma protuberans is controversial because it could be difficult to differentiate minimal residual tumor from normal skin on the frozen sections, and is more challenging in recurrent disease because of the scar tissue. We report a case of a recurrent dermatofibrosarcoma protuberans in which the tumor was excised using the Mohs surgical technique and margin control was achieved with horizontally-cut paraffin-embedded permanent sections, named as rush permanent sections.
