Endoscopic symphathicotomy is an effective treatment for hyperhidrosis. Chylothorax is a rare but life-threatening complication. We recently encountered a case of chylothorax after endoscopic symphathicotomy at T2-T4 symphathetic nerve. We successfully treated chylothorax by electric cauterization with thoracic endoscopy.

7.Thoracic Duct Cyst in Mediastinum: A case report.
The Korean Journal of Thoracic and Cardiovascular Surgery 2011;44(1):83-85
The thoracic duct cyst is an extremely rare cystic lesion in the mediastinum. Surgical treatment of the cyst is necessary to confirm histologic diagnosis and prevent potential complications such as spontaneous or traumatic rupture of the cyst and chylothorax.

Endoscopic sympathicotomy is the choice of the treatment for hyperhidrosis. There are some recognized risks such as Honer's syndrome and Hemopneumothorax; however, Chylothorax has very rarely been reported after thoracic sympathicotomy. We recently encountered a case of chylothorax. Early diagnosis and prompt treatment is noteworthy.

9.Ligation of Thoracic Duct Via Extrapleural Approach for the Treatment of Chylothorax: Report of 1 case.
The Korean Journal of Thoracic and Cardiovascular Surgery 2003;36(11):883-885
Traditional surgical management of chylothorax refractory to conservative treatment is thoracic duct ligation through a right open thoracotomy. A new surgical procedure of ligation of the thoracic duct via extrapleural approach is presented. Its advantages are less invasive, no need of a new drainage tube, and easier to expose the thoracic duct than the conventional transthoracic approach. We experienced a new surgical method in the treatment of the chylothorax and report it with literatures.

Chylothorax is a debilitating condition to the point of threatening life. Diagnosis and subsequent management present significant problems for the clinician and the adverse effects of chyle loss on nutrition status and immune function result in devastating consequences for the patient. prompt diagnosis is essential to institute an effective therapeutic regimen. we reported a patient of idiopathic bilateral chylothorax treated by conservative approach with review of literatures.
