1.Procedural analysis of acid-base balance disorder: case serials in 4 patents

Objective To establish the standardization process of acid-base balance analysis, analyze cases of acid-base balance disorder with the aid of acid-base balance coordinate graph.Methods The acid-base balance theory were reviewed systematically on recent research progress, and the important concepts, definitions, formulas, parameters, regularity and inference in the analysis of acid-base balance were studied. The analysis of acid-base balance disordered processes and steps were figured. The application of acid-base balance coordinate graph in the cases was introduced.Results The method of four parameters-four steps analysis was put forward to analyze the acid-base balance disorders completely. Four parameters included pH, arterial partial pressure of carbon dioxide (PaCO2), HCO3- and anion gap (AG). Four steps were outlined by following aspects: ① according to the pH, PaCO2 and HCO3-, the primary or main types of acid-base balance disorder was determined; ② primary or main types of acid-base disorder were used to choose the appropriate compensation formula and to determine the presence of double mixed acid-base balance disorder; ③ the primary acid-base balance disorders were divided into two parts: respiratory acidosis or respiratory alkalosis, at the same time, the potential HCO3- should be calculated, the measured HCO3- should be replaced with potential HCO3-, to determine whether there were three mixed acid-base disorders; ④ based on the above analysis the data judged as the simple AG increased-metabolic acidosis was needed to be further analyzed. The ratio of ΔAG↑/ΔHCO3-↓ was also needed to be calculated, to determine whether there was normal AG metabolic acidosis or metabolic alkalosis. In the clinical practice, PaCO2 (as the abscissa) and HCO3- (as the ordinate) were used to establish a rectangular coordinate system, through origin (0, 0) and coordinate point (40, 24) could be a straight line, and all points on the straight line pH were equal to 7.40. The acid-base balance coordinate graph could be divided into seven areas by three straight lines [namely pH = 7.40 isoline, PaCO2 = 40 mmHg (1 mmHg = 0.133 kPa) line and HCO3- = 24 mmol/L line]: main respiratory alkalosis area, main metabolic alkalosis area, respiratory+ metabolic alkalosis area, main respiratory acidosis area, main metabolic acidosis area, respiratory+ metabolic acidosis area and normal area. It was easier to determine the type of acid-base balance disorders by identifying the location of the (PaCO2, HCO3-) or (PaCO2, potential HCO3-) point on the acid-base balance coordinate graph.Conclusions Four parameters-four steps method is systematic and comprehensive. At the same time, by using the acid-base balance coordinate graph, it is simpler to estimate the types of acid-base balance disorders. It is worthy of popularizing and generalizing.

Objective:In order to achieve the use of community medical care and branch doctor resources, to provide monitoring of remote diagnosis, providing more convenient, safe and efficient medical service for patients and hospital objective. Methods:Multi parameter remote monitoring system design of special model of WIFI wireless technology, including special new WIFI module, wearable multi parameter telemetry monitor, wireless relay box network and medical monitoring software, testing and data transmission of ECG, respiration, blood oxygen using a wireless network, can the patient monitoring in normal life condition. Results:Whenever and wherever possible the electrophysiological data sent to the hospital, the doctor can according to the electrophysiological data of the patient for remote diagnosis and further treatment. The accuracy of the stability, the reliability of the system and the parameters of the tested strictly, data transmission accurate rate reaches more than 98%. Conclusion: The patients and doctors have been received good feedback, clinical trials, has very practical clinical significance.
3.ELECTRON MICROSCOPIC LOCALIZATION OF SUCCINIC DEHYDROGENASE ACTIVITY OF THE TUMOR-LIKE GROWTH TISSUE IN GOLDFISH CARASSIUS AURTUS

In the present study, electron microscopic and cytochemical localization of succinic dehydrogenase (SDH) of the tumor-like growth tissue in the Tiger-head goldfish (Carassius aurtus) were attempted. The SDH was demonstrated according to the modified method described by Kerpel-Fronius and Hajos (1968). The main results are as follows:When the goldfish were twenty-months old, there was not only a highly active positive reaction of SDH on the periphery of jejune vesicle, but also on the mitochondria, cristae and cytoplasm membrane in the cells of the hyperplasitic tissue. At the same time, there were much crystal formation because cell disintegration. When the Tiger-head goldfish were ten and thirty-months old, the active reaction of SDH was not so strong as in the corresponding positions of cytodeme as When it was twenty-months old. The crystal formation were lesser than the others. But the control groups displayed the negative reaction of SDH in the corresponding positions.
4.Expression of TGF-beta1 and collagen fibers in chronic nasal-sinusitis nasal mucosa of patients.

OBJECTIVE:
Explore the TGF-beta1 and collagen fibers in chronic nasal sinusitis each type and degree of expression of the normal control group and in the nasal mucosa epithelial tissue remodeling and the role of TGF-beta1 and collagen deposition relationship.
METHOD:
Sixty-two patients experimental group (CRSwNP of 21 cases. CRSs NP group of 15 cases. 11 cases of recurrent nasal polyps; control group 15 cases specimens for immunohistochemistry and masson collagen staining. Manual counting the number of positive cells by the Mann-Whitney U test to analyze the expression in experimental group and the control group. Experimental group and control group between the single-factor analysis of variance with a One-Way ANOVA analysis.
RESULT:
Experimental group and control group were expressed TGF-beta1 with collagen deposition. Which, TGF-beta1 in CRSsNP group was significantly lower than the control group (P < 0.05), CRSwNP group was significantly increased compared with CRSsNP group (P < 0.05); masson collagen staining, CRSsNP group was significantly lower than the control group (P < 0.01), recurrent nasal polyps group than in the control group was significantly increased (P < 0.05). TGF-beta1 and collagen staining masson positive correlation between (P < 0.01 ).
CONCLUSION
TGF-beta1 and collagen deposi tion and chronic nasal-sinusitis tissue remodeling has its relevance. Furthermore. TGF-beta1 expression increased with excessive deposition of collagen fibers also positively correlated.

5.The Effects of Inhalation of Aerosolized Low Molecular Weight Heparin on the Clotting Time in Guinea Pigs and Bleeding Time in Mice

AIM:To study the effects of inhalation of aerosolized low molecular weight heparin(LMWH)and standard heparin(SH) on the clotting time and bleeding time in guinea pigs and mice.METHODS:Guinea-pigs and mice inhaled aersolized LMWH and SH for 7 days,and the clotting time in guinea-pigs and tail bleeding time in mice were observed.RESULTS:Compared with vehicle,LMWH 800IU/L,400IU/L and SH 800IU/L,400IU/L had no significant effects on the clotting time in guinea-pigs and tail bleeding time in mice(P>0.05);There were no significant differences between LMWH 800IU/L,400IU/L and SH 800IU/L,400IU/L(P>0.05).CONCLUSION:Inhalation of aerosolized LMWH and SH had no significant effects on clotting time and bleeding time in animals.
6.Effect of calcium dobesilate capsules in the treatment of diabetic retinopathy

Objective To investigate the effect of Calcium Dobesilate Capsules in the treatment of diabetic retinopathy ( DR ) and its effect on hemorheology, vascular endothelial function and thrombin.Methods According to the random number table the 80 cases of DR patients admitted in our hospital from September 2014 to August 2016 were divided into observation group and control group, 40 cases in each group, two groups of patients were treated with conventional therapy, the observation group was treated with Calcium Dobesilate capsules.Two groups of treatment were three months.Compared of the therapeutic effects, the before and after treatment changes of visual acuity, hemorheology, vascular endothelial function and thrombin index.Results The total efficiency of the observation group 92.50% was higher than the control group 72.50% (P<0.05);After treatment, two groups visual acuity increased, and the observation group after treatment higher than the control group (P<0.05); Plasma viscosity, whole blood viscosity at high shear and low shear whole blood viscosity, erythrocyte aggregation index in two groups were decreased, and observation group lower than the control group (P<0.05);Plasma NO and ET-1 levels in two groups were reduce, and observation lower than the control group after treatment (P<0.05); After treatment, two groups PT, TT and APTT increased, and observation group PT, TT and APTT higher than the control group (P<0.05) .There were no obvious adverse reactions in the two groups during the treatment period.Conclusion The effect of calcium dobesilate capsules in the treatment of patients with DR is obvious, and it can improve hemorheology.
7.Expression of Toll-like receptors signaling pathway in chronic rhinosinusitis with nasal polyps

Objective:To investigate the expression of Toll-like receptors(TLRs)signaling pathway in chronic rhinosinusitis(CRS)with nasal polyps.Method:Gene microarray analysis was used to detect the expression of TLRs signaling pathway in CRS with nasal polyps.Result:Of 19 differentially expressed(two-fold changes),4genes were upregulated and 15 genes were downregulated.Conclusion:The differentially expressed genes in TLRs signaling pathway may exert its effect in the pathogenesis of CRS.In addition,the roles of TLR9 and its agonists need further study.
8.Research progresses on the pathogenesis of bacterial biofilm in chronic rhinosinusitis.

The role of bacterial biofilms in the chronic diseases has been recognized more and more. The experts in the centers for disease control and prevention of the United States estimate that 65% of human bacterial infection is involved with the biofilms. In the recent study. it has been proved that there is a high correlation between the bacterial biofilm and the pathogenesis and prognosis of chronic rhinosinusitis(CRS). In order to elucidate the role of bacteria films in the pathogenesis of CRS, we reviewed various method of testing the bacterial biofilms in CRS, and summarized the related researches. Based on the detection researches and summaries, we conclude that bacterial biofilm can contribute to the continuance and development (diffusion and repeated) of CRS. It can lead to the continuous inflammation by influencing the immune system of the mucous membrane. While there is little research on the bacterial biofilm. Further researches will be needed for the exact mechanism of the bacterial biofilm in CRS. in order to find more effective therapeutic method and targets.

9.The clinical observation of Sarcandra glabra extracts against oxidative damage induced by radiation

Objective To observe the clinical effect of Sarcandra glabra extracts against oxida-tive damage. Methods 46 patients diagnosed as nasopharyngeal careinoma stage Ⅲ and Ⅳ a were ran-domly divided into control group (n=25) and study group (n=21). All patients underwent combined chemoradiotherapy treatment. Besides, the patients in study group were given Sarcandra glabra 20 g/d orally. The total activity of superoxide dismutase (SOD) and plasma eontent of malonaldehyde (MDA) were measured before and after treatment. The tumor regression and aeute chemoradiotherapy side reaction were observed after treatment. Results All patients fulfilled the whole range treatment, After treatment, the total plasma SOD activity decreased and plasma MDA level was elevated in con-trol group, while both SOD aetivity and MDA level were reduced in study group (P<0. 05). When the therapy ended, the SOD of study group was higher and MDA was lower than that of control group (P<0.05). There was no difference in tumor local control rate after radiotherapy between two groups (P>0.05). The incidence rate and severity of acute chemoradiotherapy side reaction were slighter in study group than those in control group (P<0. 05). Conclusion Sarcandra glabra extracts exerts obvious inhibitory effect on oxidative damage. Combined with chemoradiotherapy, Sarcandra glabra contributes to the tumor regression of patients with stage Ⅲ and Ⅳa nasopharyngeal carcinoma and at-tenuation of chemoradiotherapy side reaction.
10.Serum S-100B concentration of umbilical cord blood in the early diagnosis of newborn hypoxic ischemic encephalopathy

Objective To investigate the role of serum S-100B of umbilical cord blood in the early diagnosis of newborn hypoxic ischemic encephalopathy(HIE).Methotis 307 full-term newborns with asphyxia were selected as subiects and 43 normal full-term newborns delivered during the corresponding period were selected as controls.The umbilical cord blood sample was acquired.46 cases of 307 subjects with asphyxia developed into HIE,31 cases with mild HIE and 15 cases with moderate and severe HIE.The serum S-100B concentration of umbilical cord blood was detected by ELISA analysis.Results ①There was no significant difference in the serum S-100B concentration of umbilical cord blood between the male sub-group and female sub-group in normal group;②The concentration in control group and HIE group were(1.03±0.32)μg/L and(2.53±1.1)μg/L respectively(t=8.848,P<0.05=;③The concentration in mild HIE group and moderate to severe HIE group were(1.03±0.32)μg/L nd(3.49±1.23)μg/L respectively,which were significantly different with control group(F=79.6909,P<0.01).Conclusion Sex has no significant relation to the serum S100B concentration.Serum S-100B concentration of umbilical cord blood can be biomarkers for early diagnosis of newborn HIE.
