1.The clinical observation of Sarcandra glabra extracts against oxidative damage induced by radiation

Objective To observe the clinical effect of Sarcandra glabra extracts against oxida-tive damage. Methods 46 patients diagnosed as nasopharyngeal careinoma stage Ⅲ and Ⅳ a were ran-domly divided into control group (n=25) and study group (n=21). All patients underwent combined chemoradiotherapy treatment. Besides, the patients in study group were given Sarcandra glabra 20 g/d orally. The total activity of superoxide dismutase (SOD) and plasma eontent of malonaldehyde (MDA) were measured before and after treatment. The tumor regression and aeute chemoradiotherapy side reaction were observed after treatment. Results All patients fulfilled the whole range treatment, After treatment, the total plasma SOD activity decreased and plasma MDA level was elevated in con-trol group, while both SOD aetivity and MDA level were reduced in study group (P<0. 05). When the therapy ended, the SOD of study group was higher and MDA was lower than that of control group (P<0.05). There was no difference in tumor local control rate after radiotherapy between two groups (P>0.05). The incidence rate and severity of acute chemoradiotherapy side reaction were slighter in study group than those in control group (P<0. 05). Conclusion Sarcandra glabra extracts exerts obvious inhibitory effect on oxidative damage. Combined with chemoradiotherapy, Sarcandra glabra contributes to the tumor regression of patients with stage Ⅲ and Ⅳa nasopharyngeal carcinoma and at-tenuation of chemoradiotherapy side reaction.
2.Clinical Research on the Relationship between Type 2 Diabetes Mellitus and Colorectal Cancer

Objective: To investigate the relationship between type 2 diabetes mellitus (T2DM) and the pathogenesis and metastasis of colorectal cancer. Methods: A case-control study was performed to compare 852 colorectal cancer patients with 940 controls (patients without cancer) recruited from 2001 to 2006, with respect to their sex, cancer subsite, the course of T2DM, hepatic metastasis, smoking and drinking. Correlated risk factors were analyzed. Results: The risk of colorectal cancer was increased in patients with T2DM and the relative risk (OR) was 2.466. The OR of male patients was higher than that of female patients, but with no significant difference (2.775 vs 2.070, P=0.394). The incidence of T2DM in patients with left hemicolon cancer was higher than that in those with right hemicolon cancer and rectal cancer, but with no significant difference between them. The colorectal cancer risk in T2DM patients with a DM course of 10 ~ 20 years was the highest, and the OR was 4.696. The rate of hepatic metastasis was higher in T2DM patients with colorectal cancer than that in celorectal cancer patients without T2DM and the OR was 2.888. Conclusion: T2DM may be one of the important pathogenic risk factors for colorectal cancer. The OR is increased with the extension of DM course within 20 years. Colorectal cancer patients with T2DM may be more prone to hepatic metastasis.
3.Value of early detection of cardiac troponin I and lactate in evaluating prognosis of emergency elderly patients with severe sepsis and septic shock

Objective To explore value of early detection of cardiac troponin I(cTnI) and lactate in evaluating prognosis of emergency elderly patients with severe sepsis and septic shock.Methods A total of 98 elderly patients with severe sepsis oRseptic shock were selected.The patients were divided into survival group and death group according to the prognosis with follow-up of 28 d.The levels of cTnI,lactate,APACHE Ⅱ score and SOFA score were compared between different groups.The correlation analysis was performed between cTnI,lactate and APACHE Ⅱ score,SOFA score.Results The level of lactate and cTnI,APACHE Ⅱ score and SOFA score in septic shock group were significantly higheRthan those in severe sepsis patients,and the level of lactate,cTnI,APACHE Ⅱ score and SOFA score in death group were significantly greateRthan those in survival group (P<0.05).The level of lactate and cTnI showed a certain correlation with APACHE Ⅱ score and SOFA score (P<0.05).The areas undeRthe ROC curve of cTnI and arterial blood lactate were 0.846 and 0.809 respectively.Conclusion The cTnI and arterial blood lactate can evaluate condition and prognosisof emergency elderly patients with severe sepsis and sepsis shock.

5.Current status and influencing factors of reproductive concerns among male thyroid cancer patients of childbearing age

Objective:To explore the reproductive concerns of male thyroid cancer patients of childbearing age and analyze their influencing factors.Methods:From May 2022 to August 2023, convenience sampling was used to select 298 male thyroid cancer patients of childbearing age who visited two ClassⅢ Grade A hospitals in Wuhan as research subjects. General Information Questionnaire, Reproductive Concerns after Cancer Scale-Male, Distress Disclosure Index, and 14-Item Resilience Scale were used for the survey. Single factor analysis and multiple linear hierarchical regression were used to analyze the influencing factors of reproductive concerns.Results:A total of 275 male thyroid cancer patients of childbearing age were included. The total scores of reproductive concerns, self-disclosure, and psychological resilience were (47.66±11.44), (37.22±5.80), and (62.45±8.73), respectively. The score of reproductive concerns was negatively correlated with the scores of self-disclosure and psychological resilience, and the difference was statistically significant ( r=-0.636, -0.656; P<0.01). Hierarchical regression analysis showed that educational level, monthly family income, number of children, fertility intention, 131I treatment, self-disclosure, and psychological resilience were the influencing factors of reproductive concerns in male thyroid cancer patients of childbearing age ( P<0.05). After controlling for demographic and sociological data, self-disclosure and psychological resilience explained 8.6% of reproductive concerns among male thyroid cancer patients of childbearing age. Conclusions:Male thyroid cancer patients of childbearing age have a moderate to high level of reproductive concerns. Medical and nursing staff should develop systematic and effective intervention strategies based on influencing factors to reduce reproductive concerns among male thyroid cancer patients of childbearing age.
6.Value of England National Early Warning Score in evaluating prognosis of elderly patients with severe sepsis and septic shock in emergency department

Objective To study the value of England National Early Warning Score(NEWS) for the prognosis of elderly patients with severe sepsis and septic shock in emergency department.Methods A total of 116 elderly patients with severe sepsis and septic shock in Xuanwu Hospital of Capital Medical University were selected.Routine physiological and biochemical indicators were collected and NEWS score, APACHEⅡscores and SOFA scores were given to them.The follow-up was lasted for 28 d, and afterwards the patients were divided into survival group and death group according to the prognosis.The NEWS scores, APACHEⅡ scores and SOFA scores of two groups were compared.And above indicators were also compared between the severe sepsis and septic shock patients.Meanwhile, the correlation between NEWS scores, and APACHEⅡ scores, SOFA scores were analyzed.The value of NEWS scores for the prognosis of elderly patients with severe sepsis and septic shock was predicted by the area under the ROC curve(AUC).Results The NEWS scores, and APACHEⅡ scores, and SOFA scores in septic shock group were higher than that in severe sepsis group, and the death group was higher than survival group(P<0.05).NEWS scores showed a correlation with APACHEII scores and SOFA scores(P<0.05).The areas under the ROC curve of NEWS scores, APACHEII scores and SOFA scores were 0.870, 0.880, 0.865, respectively.Conclusion NEWS scores could evaluate the condition and prognosis of elderly patients with severe sepsis and sepsis shock, and the consistently higher scores of NEWS is associated with poorer prognosis.
