1.Assessment of Exposure to Polycyclic Aromatic Hydrocarbons in Traffic Police and Suburban Inhabitants in Beijing

Objective To assess the exposure to polycyclic aromatic hydrocarbons(PAHs) in traffic police and suburban inhabitants in Beijing and to explore the factors that can influence urinary 1-hydroxypyrene(1-OHP) which will be taken as PAHs exposure biomarker.Methods From Jun.to Aug.2007,Sixty-two traffic policemen and 35 male suburban inhabitants were selected.The ambient PM2.5 samples were collected within two consecutive days in the work place of traffic policeman and in the residential area of suburban inhabitants respectively.The levels of urinary 1-OHP and 10 PAHs species in the collected PM2.5 were determined by high performance liquid chromatography(HPLC),while the information on smoking,drinking alcohol,exercising habit and so on was investigated by questionnaire.Results The average levels of benzo[a]pyrene,pyrene and total PAHs in the traffic policeman group were 3.20,6.48 and 38.32 ng/m3 respectively,while the corresponding values in the suburban inhabitant group were 1.54,4.05 and 25.43 ng/m3 respectively.The concentration of urinary 1-OHP in the policeman group was higher than that of the suburban inhabitant group.In addition,the effect of smoking on the 1-OHP concentrations were also detected in the smoking suburban inhabitants and serious smoking traffic policemen(≥20 cigarettes per day).There was no effect of other factors(such as drinking alcohol,exercising and cooking habit) on 1-OHP.Conclusion Both traffic policemen and suburban inhabitants in Beijing were exposed to high level of PAHs.The ambient air exposure and smoking are the most important factors influencing the concentration of urinary 1-OHP.
2.Polycyclic Aromatic Hydrocarbons Exposure and DNA Oxidative Damage in Traffic Policemen of Beijing

Objective To explore polycyclic aromatic hydrocarbons(PAHs) exposure of traffic policemen in Beijing and its impact on DNA oxidative damage. Methods From Jun. to Aug. 2007,41 male traffic policemen and 34 male suburban inhabitants in Beijing were selected. The ambient fine particulate matter(PM2.5) was sampled within two consecutive days in the work places of traffic policeman and in the residential area of suburban inhabitants respectively . The levels of urinary 1-hydroxypyrene(1-OHP) and 10 PAHs species were both analyzed by high performance liquid chromatography(HPLC) . The levels of urinary 8-hydroxy-2’-deoxyguanosine(8-OHdG) were detected by ELISA kit,while the questionnaires for health information on smoking,drinking alcohol,exercise habit and so on were also inquired. Results The average levels of PM2.5,benzo[a]pyrene [B(a) P] and total PAHs in the traffic policeman group were 0.096 mg/m3,3.20 ng/m3,and 38.32 ng/m3 respectively,while the corresponding values in the suburban inhabitant group were 0.045 mg/m3,1.54 ng/m3 and 25.43 ng/m3 respectively. The concentration of urinary 1-OHP in the policeman group [(0.50 ?0.38 ) ?mol/mol Cr] was higher than that of the suburban inhabitant group([0.34?0.28) ?mol/mol Cr],P0.05. Multiple factors analysis indicated that urinary 1-OHP and smoking habit had influence on urinary 8-OHdG level,while there was no relationship with other factors(such as drinking alcohol,exercise and cooking habit) . Conclusion Traffic pollution may increase the level of DNA oxidative damage in policemen in Beijing.Traffic and smoking habit-related PAHs exposure is the important influencing factor.
3.Effect of Air Pollution on Serum Oxidative Stress Indices in Traffic Policemen

Objective To explore the effect of air pollution on oxidative stress indices(SOD,GSH-Px,MDA) in serum of traffic policemen. Methods From Jun to Aug,2007,sixty-two traffic policemen and thirty-five male suburban inhabitants were selected. The ambient concentrations of PM2.5,polycyclic aromatic hydrocarbons(PAHs) ,nitrogen dioxide,formaldehyde,benzene and toluene were monitored within two consecutive days in the work places of traffic policemen and in the residential areas of suburban inhabitants to assess their air pollution exposure levels. The activity of SOD,GSH-Px and the concentration of MDA in serum were determined with regent kits,while the information on smoking,drinking alcohol,exercising habit and so on was investigated by questionnaire. Results There were significant differences in the concentrations of nitrogen dioxide,formaldehyde,benzene and toluene between the two study groups. The average levels of PM2.5 and PAHs in the traffic policeman group were also higher than those in the control group,but this difference was not significant for small sample size. The activities of SOD and GSH-Px in the traffic policeman group were significantly higher than those in the control group,while the difference of MDA concentration between the two groups was not significant. The relationship between air pollution and the activity of SOD and GSH-Px were also significant after controlling the influence of other factors. Conclusion Air pollution can decrease the activity of SOD and GSH-Px in serum of traffic policemen and induce oxidative stress.
4.Introduce of the Benchmarking Framework of Unit Dose Dispensing

OBJECTIVE:To supply the reference for benchmarking framework of unit dose dispensing(UDD).METHODS:To introduce the running module and the training mode for UDD pharmacists of the UDD management mode,and the working quality was controlled using 6? management pattern.RESULTS & CONCLUSIONS:The benchmarking framework of UDD is feasible in practice and it can help guarantee dispensing accuracy,improve the quality of pharmaceutical care.
5.Practice and Development of the New Round Talent Cultivation held by Qingpu Health System

In order to further deepen the reform of public hospitals, to improve the qualified personnel training in Qingpu, and to enhance the overall level of medical health development, Qingpu Municipal Commission of Health and Family Planning In October 2011, a new round of disciplinary construction and personnel training project was also started.Moreover, financial support has been provided to the project.By assessing the whole process, not only has the level of discipline personnel training and medical technology shown significant improvement, the tutors who provided the training also express academic interest and have new a vision for talent cultivation.Qingpu Municipal Commission of Health and Family Planning launched a new round of comprehensive program on researcher training and disciplinary construction in 2011, aimed to further deepen the reform of public hospitals,improve the capacity of health research in Qingpu, Shanghai.We have assessed this program and found that this program has significantly improved the quality of training, and medical technology level in those hospitals;in addition, trainees has shown the increase in the interesting in conducting research and raised vision and prospects.
6.Determination of five components in Muxiang Fenqi Pill by HPLC

AIM: To establish a method of simultaneously determining 5 components in Muxiang Fenqi Pill(Flos Caryophylli,Radix Aucklandiae,Cortex Magnoliae Officinalis). METHODS: Five components :eugenol,(costunolide),dehydrocostuslactone,magnolol and honokiol in Muxiang Fenqi Pill were determined simultaneously by HPLC,using a Kromasil C_(18) column(250 mm?4.6 mm,5.0 ?m),acetonitrile-menthanol-water(50∶8∶42) as a mobile phase.The detection wavelength was at 210 nm. RESULTS: The relationship between the concentrations and the peak areas of eugenol,costunolide,dehydrocostuslactone,magnolol and honokiol were linear respectively.The RSD of precision,repeatability and recovery were all less than 1.5%. CONCLUSION: The method is simultaneous determination for five components in Muxiang Fenqi Pill,and can be applied to the quality control of Muxiang Fenqi Pill.
7.Clinical study on transurethral plasmakinetic enucleation of prostate in the treatment of benign prostate hyperplasia diagnosed by transrectal biopsy of prostate

Objective To evaluate the efficiency and safety of transurethral plasmakinetic enucleation of prostate(PKEP) in the treatment of patients with benign prostate hyperplasia(BPH) after transrectal prostate biopsy(TRPB).Methods A total of 88 BPH patients who underwent PKEP in our hospital during Jan.2012 to May 2015 were retrospectively analyzed and followedup.38 patients underwent TRPB before PKEP were defined as TRPB group,and 50 patients underwent PKEP with no TRPB were defined as control group.The baseline data,perioperative data and postoperative follow-up outcomes were recorded and compared between the two groups.Results The mean age of the 88 patients were 69.7 years.Compared with control group,TRPB group showed that age was younger(t=2.62,P< 0.05)and prostate specific antigen(PSA) level was higher(t=13.64,P<0.01).Operation duration was longer in TRPB group than in control group (93.6 ± 31.0) min vs.(77.9 ± 17.3) min (t =2.6 4,P < 0.05).There were no significant differences in the preoperative data,blood loss,continuous bladder irrigation duration after operation and period of catheterization between two groups (all P > 0.05).Trans-operative time was significantly shortened when the time interval between TRPB and PKEP is more than 4 weeks(P< 0.01),while blood loss was similar in the two groups(P> 0.0 5).There were no adverse events of blood transfusion,transurethral resection syndrome and injury of bladder or rectal in both two groups.There were no significant differences in maximum flow rate(Qmax),international prostate symptom score(IPSS) or quality of life(QOL) scores between the two groups at 3,6 and 12 months of follow up(all P> 0.05).Conclusions PKEP after TRPB is a safe and effective treatment for BPH patient.When the time interval between TRPB and PKEP is more than 4 weeks,the performing of PKEP operation would reduce the difficulty of operative procedure and increase the safety.
8.Clinical study of whole course accelerated fractionation radiotherapy for 126 patients with esophageal carcinoma

Objective To evaluate the therapeutic effects of whole course hyperfractionation radiotherapy for esophageal carcinoma. Methods Form January 2000 to December 2001,126 patients with esophageal carcinoma were treated by whole course hyperfractionation radiotherapy(120~140 cGy per fraction, 2 times per day, to a total dose of 6400~7400 cGy) in our department. Results The short-term effective rate was 99.21%. The 1, 2 and 5 year survival rate was 61.11%, 41.27%, 23.02% and the median survival time was 1.29 years. The 1, 2 and 5 year disease-free survival rates was 57.94%, 38.10%, 19.84% and the median disease-free survival time was 1.16 years. Conclusion It is promising of whole course hyperfractionation radiotherapy for esophageal carcinoma. But it must be validated by the large-scale clinical trials.
9.Correlation of 5-hydroxytryptamine synthesis and secretion to endocrinal secretion of atrial natriuretic polypeptide in enterochromaffin cells of rat gastric mucosa

BACKGROUND: Previous researches have proved that both atrial natriuretic polypeptide (ANP) and 5-hydroxytryptamine (5-HT) exist in the same endocrine granule of enterochromaffin cell (EC). However, whether ANP may promote or inhibit synthesis and secretion of 5-HT needs to be further studied. OBJECTIVE: To investigate the effect of ANP on synthesis and secretion of 5-HT in EC of rat gastric mucosa.DESIGN: Randomized controlled animals study.SETTING: Immunology Laboratory, Chengde Medical College. MATERIALS: This study was performed at the Immunology Laboratory, Chengde Medical College from October 2004 to July 2007. Forty adult male Wistar rats were provided by the Experimental Animal Center of Chengde Medical College. The experiment was in accordance with animal ethics standards. ANP, 5-HT antibody and serum were provided by Santa Cruz Biotechnology Company, USA.METHODS: Forty rats were randomly into endocrine and exocrine groups, and rats in the two groups were sub-grouped into control and experimental groups with 10 in each group. ANP (28 μg, 14 mg/L) was directly injected into the stomach to mimic ANP luminal secretion and ANP (14 μg, 14 mg/L) was directly injected into the sublingual vein to mimic ANP endocrinal secretion. In above condition, 5-HT immunoreactive positive cell was displayed by using immunohistochemistry technique, numerical density (Nv) of endocrine granule (SG) was counted by using electron microscopic morphometry, and 5-HT level in the serum was measured by using HPLC-ECD technique. And then, the results were compared to the control group. MAIN OUTCOME MEASURES: Effects of ANP on density of 5-HT immunoreactive positive cell, numerical density (Nv) of SG and 5-HT level in the serum. RESULTS: Effect of luminal and endocrine ANP on the 5-HT secretion: The density of immunoreactive positive cell and the numerical density (Nv) of SG were significantly increased by luminal and endocrine ANP (P<0.05), while 5-HT level in serum was significantly reduced by luminal and endocrine ANP (P<0.05). CONCLUSION: Luminal and endocrinal ANP can inhibit 5-HT release of gastric mucosa but may not change or enhancement its synthesis in rat.
10.Perioperative management of procedure for prolapse and hemorrhoids in elderly patients with serious hemorrhoids

Objective To evaluate the perioperative management and therapeutic efficacy of procedure for prolapse and hemorrhoids (PPH) in elderly patients with serious hemorrhoids. Methods The clinical data of 52 elderly patients (aged 60 years or over) with serious hemorrhoids treated by PPH from March 2005 to November 2007 were analyzed restrospectively. Results All the operations were successfully completed. The average operation time was (15.3±3.2) min, the average blood loss was (15±1.7) ml and the average hospitalization time was (4.5±2.6) days.Patients were followed for 1-20 months with (14.2±2.4) months for average. The follow-up rate was 92.3%(48/52). Complications included urinary retention in 18 cases(37.5%), anastomotic bleedings in 2 cases (4.2%), postoperative pain of anus in 4 cases (8.4%), recurrence in 1 case (2.1%),hypoglycemia in 1 case(2.1%), and 1 case (2.1%)was found stricture in rectum at 2 months after the procedure and no case developed rectovaginal fistula. Conclusions Old age is not the contraindication for PPH treatment of the serious hemorrhoids. Proper perioperative management may decrease the postoperative complications and improve the life quality of the patients.
