Objective To explore the beauty of mesalazine combined with Bifid Triple viable bacilli clinical efficacy in the treatment of ulcerative colitis.Methods 72 patients with ulcerative colitis ( UC) in zhoushan maternal and child health hospital were selected and divided into experimental group and control group,36 cases in each group.The control group was treated with oral administration of melamine and the experimental group was treated with Bifidobacterium triple live bacteria on the basis of oral control group.The levels of superoxide dismutase (SOD), malondialdehyde (MDA) and reactive protein ( CRP) concentration in serum of patients were detected.Follow up observation and record of the experimental group and the control group to improve the symptoms, clinical efficacy and adverse reactions after treatment.Results After treatment,serum MDA, CRP concentrations of the two groups were significantly lower than those before treatment concentration, SOD concentration was significantly increased, the differences were statistically significant (P<0.05) , serum MDA, CRP levels in the experimental group were lower than the control group, the concentration of SOD was significantly higher than control group,the differences were statistically significant (P<0.05) .After treatment, abdominal pain, diarrhea, mucus and blood stool symptoms of two groups were significantly improved compared with before treatment, abdominal pain, diarrhea and bloody mucus symptoms in the experimental group were lower than the control group(P<0.05).Incidence of adverse reactions in the experimental group was significantly lower than the control group, the differences were statistically significant (P<0.05).Conclusion The efficacy of oral administration of methadiazole in combination with Bifidobacterium triple live bacteriain the treatment of ulcerative colitis is significant, Can reduce MDA, CRP concentration, increased SOD activity.

AIM:To establish the endoplasmic reticulum stress ( ERS) cell model in vascular smooth muscle cells ( VSMCs) of Sprague-Dawley (SD) rats.METHODS:Under sterile condition, the coronary arteries were isolated from SD rats .The primary VSMCs were cultured by tissue-sticking method , and observed the basic morphological characteristics under optical microscope .The marker proteins of VSMCs including α-smooth muscle actin (α-SMA) and smooth muscle myosin heavy chain ( SM-MHC) were identified by immuno-fluorescence technique .VSMCs were treated with thapsigargin (0.5, 1 and 2 μmol/L) for 24 h, and the expression levels of binding immunoglobulin protein (BiP) and C/EBP homologus protein (CHOP), the marker molecules of ERS, were detected using Western blotting.RESULTS:VSMCs climbed out from coronary artery tissues after about six days , and the cells had a nice state and formed the VSMC-like typical peak valley.The results of immunofluorescence technique show that the marker proteins of VSMCs ,α-SMA and SM-MHC were expressed significantly .The results of Western blotting show that the protein expression levels of BiP and CHOP were increased by thapsigargin in a dose-dependent manner .CONCLUSION:VSMCs can be successfully cultured by tissue-sticking method and built the ERS model induced by thapsigargin .

Objective: To explore the effects of hydrogen sulfide (H 2S) on hypoxic pulmonary vascular smooth muscle cell (VSMC) apoptosis in rats. Methods: Twenty four Wistar rats were divided into 3 groups: control group ( n =8), hypoxia group ( n =8), and hypoxia +NaHS group ( n =8). The plasma level of H 2S was determined by methylene blue spectrophotometric method. VSMC apoptosis was measured by terminal deoxynucleotidyl transferase biotin nick end labeling (TUNEL). The protein expressions of Bcl 2, Fas and caspase 3 in pulmonary arteries were detected by immunohistochemical technique. Results: Compared with rats in the control group, the plasma level of H 2S decreased by 36% in rats of hypoxic group . The apoptotic rate per area in VSMCs detected with TUNEL was significantly decreased by 52.9% in rats of hypoxic group . The expressing integral score of Bcl 2 of VSMCs was increased by 123.9%,while Fas protein expression of VSMCs was decreased by 45% and caspase 3 protein expression of VSMCs was not significantly changed in rats of hypoxia group. But compared with rats in the hypoxia group, the plasma level of H 2S increased by 65% in rats of hypoxia+NaHS group. The apoptotic rate in VSMCs of TUNEL was significantly increased by 62.5% in rats of hypoxia+NaHS group. The Bcl 2 protein expression of VSMCs was decreased by 36.4% in rats of hypoxia+NaHS group. The expressing integral scores of Fas and caspase 3 were significantly higher in rats of hypoxia+NaHS group than in those of hypoxia group. Conclusion: Hypoxia decreased the pulmonary artery smooth muscle cell apoptosis. H 2S inhibited Bcl 2 protein expression of VSMCs and activated Fas and caspase-3 protein expressions of VSMCs, and therefore promoted the pulmonary artery smooth muscle cell apoptosis.

Objective To investigated the clinicopathological findings and prognosis of 60 patients with pleomorphic carcinoma.Methods 60 cases of pleomorphic pulmonary carcinoma were collected and studied clinicopathologically between 1999 and 2011.All patients underwent pulmonary resections.Results There was no in-hospital death.1 had haemothorax that required reoperation,1 patient had a late cicatricial bronchial stenosis after sleeve lobectomy of the upper right lobe,and treated successfully by the resection of the right two lobes.The 3,5-year survival rates of all patients were 47.2％ and 25.6％ respectively.For N0 and N1-N2 patients,3-,5-year survival rates were 57.3％,32.5 ％ and 25.4％,17.1％,respectively.Overall survivals were of significant difference between radical resection and uncomplete resection.Multivariate Cox regression analyses of overall survival of all 60 patients indicated that radical resection (P =0.041) and nodal status (P =0.029) were significant independent prognostic factors.Conclusion Pleomorphic carcinoma is a rare type of lung cancer.Pleomorphic carcinoma of the lung was often found as a large mass peripherally located and was associated with poor prognosis.Radical resection and nodal status were significant independent prognostic factors.

Objective To investigate the influence of sperm morphology, sperm DNA fragmentation index and seminal plasma zinc on pregnancy outcome of in vitro fertilization and embryo transfer (IVF-ET).MethodsA total of 341 infertile couples underwent IVF-ET were selected from January 2016 to June 2016 in our hospital.Sperm morphology, sperm DNA fragmentation index and seminal plasma zinc level were compared according to pregnancy.ResultsIn 341 cases, 204 cases pregnancy and 137 cases of no pregnancy, with the pregnancy rate of 59.8%(204/341).Compared with the pregnancy group, the percentage of normal sperm percentage was low, the abnormal sperm index was higher in the non-pregnant group (P<0.05), the sperm DNA index was higher (P<0.05).The percentage of normal sperm, sperm DNA fragmentation index and seminal plasma zinc and pregnancy rate in sperm morphology were linear (P<0.05), And there was a negative correlation between abnormal sperm index and sperm DNA fragment index (P<0.05), and other indicators were positively correlated (P<0.05).ConclusionSperm morphology, sperm DNA fragmentation index and seminal plasma zinc levels may influence the outcome of IVF-ET.The above parameters can be used to predict pregnancy outcome of IVF-ET.

Aim of the ex-ploration is provide references for the construction and application of national excellent course and its sus-tainable development.

Objective To analyze the clinical features of 385 postoperative thyroid nodules patients,and to compare the correlation affected by different factors between benign and malignant thyroid nodules.Methods Total 385 patients underwent thyroid operation in general surgery department of our hospital were enrolled.Their clinical data,ultrasonic scanning of thyroid,and pathological results were compared between benign and malignant thyroid nodules.Results (1)264 cases with benign nodules (68.57％) and 121 cases with malignant nodules (31.43 ％)were found among 385 cases.The average age of benign group was (48.1 ± 13.4) years old,while malignant nodules group was (45.6 ± 12.5) years old.(2) No gender difference was found between benign and malignant nodules group (P＞0.05).(3) According to body mass index (BMI),the highest prevalence of malignant nodules was in overweight group,followed by obesity group.(4) Fasting blood glucose was (5.61 ± 1.07) mmol/L in benign group and (5.86±1.21) mmol/L in malignant group(P＜0.05).(5) Thyroid-stimulating hormone (TSH) in benign nodules group was (1.62 ± 1.30) mIU/L,while in malignant nodules group,which was (2.02 ±2.59) mIU/L(P＜0.05).(6) Irregular shape,unclear boundary,calcification,and lymph node metastasis were more frequent in malignant group than those in benign group under ultrasonic scanning (P＜0.05).(7) Logistic regression analysis revealed that the existence of thyroid cancer was significantly associated with age,fasting blood glucose,ultrasonic images(lymph node metastasis,irregular shape,and calcification).Conclusion (1) Fasting blood glucose levels and TSH in the malignant nodules group are higher than those in the benign group.(2) A negative correlation exists between age and thyroid cancer prevalence.(3) Ultrasonic images,including lymph node metastasis,irregular nodule shape,and calcification,may suggest the high possibility of thyroid cancer.

Objective To explore the clinical diagnostic value of narrow-band imaging combined with magnification chromoendoscopy for suspicious neoplasia lesions of early gastric cancer.Methods A total of 115 patients which had been diagnosed as having suspicious lesions byconventional endoscopy were enrolled from Jan.2010 to Dec.2012.They were observed by magnifying endoscopy(C-WLI), magnifying endoscopy combined with narrow-band imaging (ME-NBI), magnification chromoendoscopy and magnification chromoendoscopy combined with narrow-band imaging, respectively.The lesion outline sharpness, opening of the gland sharpness and microvascular morphology sharpness were recorded and the subtypes of opening of the gland and microvascular morphology were compared.The histological examination was performed on the most significant changes in lesion site and the accuracy, sensitivity and specificity of the four procedures were calculated.Results The score of outline sharpness was 377 and the score of pit sharpness was 458 by magnification chromoendoscopy combined with narrow-band imaging, higher than those of ME-NBI (340 and 408 respectively) and magnification chromoendoscopy (354 and 386 respectively) (P ＜ 0.05), significantly higher than those of C-WLI (276 and 280 respectively) (P ＜ 0.01).The score of microvascular morphology sharpness was 380 by magnification chromoendoscopy combined with narrow-band imaging,higher than that of ME-NBI (348, P ＜ 0.05), C-WLI (267, P ＜ 0.01) and magnification chromoendoscopy (280, P ＜ 0.01).The detection rate of C type by magnification chromoendoscopy combined with narrow-band imaging was higher than that by magnification chromoendoscopy (93.0％ VS 79.7％, P ＜ 0.05).The accuracy, sensitivity and specificity of magnification chromoendoscopy combined with narrow-band imaging was 92.17％, 88.33％ ,96.36％ respectively.Conclusion Narrow-band imaging endoscopy combined with pigment amplification can yield more clear image of the microvascular morphology and opening of the gland, significantly improve the lesion detection rate, reduce the missing rate, and is worthy of further clinical observation and promotion.

Since the examination of doctor's qualifications came into effect, methods that are set by standard to judge whether one passes the examination or not were in dispute all the while. This article is going to introduce four kinds of methods (Angoff method、Modified Angoff method、Nedelsky method、Borderline group method) which are commonly used at home and abroad. Summarizing their advantages, disadvantages and the usage in the process of standards setting in various countries, we aim to provide reference for setting cutoff score of the examination of doctor's qualifications in China.

In order to further deepen the reform of public hospitals, to improve the qualified personnel training in Qingpu, and to enhance the overall level of medical health development, Qingpu Municipal Commission of Health and Family Planning In October 2011, a new round of disciplinary construction and personnel training project was also started.Moreover, financial support has been provided to the project.By assessing the whole process, not only has the level of discipline personnel training and medical technology shown significant improvement, the tutors who provided the training also express academic interest and have new a vision for talent cultivation.Qingpu Municipal Commission of Health and Family Planning launched a new round of comprehensive program on researcher training and disciplinary construction in 2011, aimed to further deepen the reform of public hospitals,improve the capacity of health research in Qingpu, Shanghai.We have assessed this program and found that this program has significantly improved the quality of training, and medical technology level in those hospitals;in addition, trainees has shown the increase in the interesting in conducting research and raised vision and prospects.
