1.Effect of processing on the contents of nuciferine and quercetin in Nelumbo nucifera Gaertn

AIM: To study the effect of the processing on the contents of nuciferine and quercetin existed in lotus leaf(Nelumbo nucifera Gaertn).METHODS: Nuciferine and quercetin were allowed for the markers and its contents were assayed by HPLC.Marker constituents were compared among raw herb,carbonizing by stir-frying and carbonizing by calcining.RESULTS: By means of carbonizing processing,nucifering content of lotus leaf reduced to 99.25% and 99.23% compared with the unprocessed lotus leaf,quercetin content of lotus leaf increased to 608.56% and 643.85% compared with the unprocessed lotus leaf.CONCLUSION: The heating processing has remarkable effect on the contents of nuciferine and quercetin in Nelumbo nucifera Gaertn.
3.Levels of vitamin D and the effect of added treatment on Multiple Sclerosis

Objective To evaluate the serum 25?hydroxy vitamin D3 level in patients with multiple sclerosis(MS)and normal healthy population,as well as the correlation between addition of oral 1,25?hydroxy vitamin D3 and the prevention of MS relapse and progression. Methods There were 60 cases in the relapsing?remitting MS(RRMS)group and 68 cases in the healthy group,respectively;and the differences in the sex,age, serum 25?hydroxy vitamin D3 level of the two groups were counted and evaluated. In addition ,the 60 cases of patients were divided randomly into the hormone therapy group and the addition treatment group ,with 30 cases in each group;the addition treatment group was added oral calcitriol soft capsules on the basis of the hormone therapy group;EDSS score evaluation was conducted on the two groups 6,12 and 24 months after treatment,the relapse frequency was counted after 24 months,and the relapse interval was calculated. Results The serum 25?hydroxy vitamin D3 levels in the patient group and the healthy group were(18.75 ± 8.35)nmol/L and(23.28 ± 9.31)nmol/L, respectively. There were statistically significant differences in the relapse frequency (P < 0.01),the relapse interval(P < 0.05),and EDSS score(after 24 months)(P < 0.05)between the hormone therapy group and the addition treatment group after treatment;while the differences in the EDSS score (after 6 months)(P = 0.457) and the EDSS score(after 12 months)(P = 0.118)between the two groups showed no statistically significance. Conclusion The serum 25?hydroxy vitamin D3 level in MS patients was markedly lower than that in normal healthy population. Addition of 1,25?hydroxy vitamin D3 contributes to preventing the relapse rate of MS and extending the relapse interval;in addition,maintaining long?term of oral 1,25?hydroxy vitamin D3 facilitates to delaying the progression of disabled disease.
4.Significance of serum CD62p level in patients with severe pulmonary contusion

Objective To determine the changes of patients with severe pulmonary contusion and the relationship among the serum level of alpha-granular membrane protein (CD62p), the severity of injury and prognosis. Methods The serum level of CD62p, was measured in 45 patients with severe pulmonary contusion on hour 2, hour 12,day 1,3,7 after injured. Forty-five patients were divided into two groups: the survive group and the death group. The levels of CD62p between two groups were compared. Results The serum levels of CD62p in patients with severe pulmonary contusion on 12 hours, 1,3 days after injury were higher than those on 2 hours (P<0.05). The serum levels of CD62p in the survive group and death group on 2 h, 12 hours after injury were higher than those in the control group(P<0.05), and the serum levels of CD62p in the death group were higher than those in the survive group. Conclusion It was helpful in judging the severity of head in jury and prognosis to determine the serum levels of CD62p in patients with severe pulmonary contusion.
5.Optimization of scanning parameters in children CT examination

Objective To reduce the radiation dose to children from CT scanning through proper adjustment to milliamps (mAs) and scan lengths with a view to learning the relationship between scanning condition and radiation dose.Methods To compare the differences in main scanning parameters used for head,chest and abdomen at multi-detector CT examination of paediatric patients (< 1 year old,1-5 years old,6-10 years old,11-15 years old) at seven hospitals in Jiangsu province.CT dose index (CTDI) and dose-length-product (DLP) were gained by using standard children dose model (diameter 16 cm) under the same scanning conditions.Effective doses (E) at different parts of the body from children CT scanning were estimated after modification by empirical weighting factor.Statistical analyses of mAs,scan lengths and DLP were performed with SPSS 16.0 software.The differences in radiation dose due to the choice of condition of scanning were compared between two typical hospitals.Results The mean values of effective doses to paediatric patients during head,chest and abdomen CT scanning were 2.46,5.69,11.86 mSv,respectively.DLP was correlated positively with mAs and scan length (head,chest and abdomen examination,r =0.81,0.81,0.92,P <0.05).Due to higher mAs used,the effective dose from chest and abdomen CT examination among all age groups was higher than that in Germany Galanski research.Due to larger scanning length in abdominal examination among all age groups,effective doses in hospital were the highest.Conclusions Reasonablely reducing the scan length and mAs during CT scanning could lower children's CT radiation risk,while clinical diagnosis is not affected.
6. Clinical application of covered metal stents in treating central venous occlusion of different branches in hemodialysis patients

Objective:
To evaluate the efficacy of covered stent (CS) in the treatment of central venous occlusive disease (CVOD) of different branches in hemodialysis patients.
Methods:
Twenty-five cases of CVOD in the First Affiliated Hospital of Sun Yat-sen University from Oct 2015 to June 2018 were enrolled. All patients underwent percutaneous transluminal angioplasty (PTA)+stent graft (PTS) successfully. The stent grafts of different diameters were implanted according to intraoperative angiography to measure the diameter of normal blood vessels around the diseased vessels. The operation was successful and the follow-up data was complete. According to the different branches of central venous lesions, the patients were divided into three groups: subclavian vein group, brachiocephalic vein group and superior vena cava group. The stent diameter, primary patency and assisted primary patency time were analyzed and compared in the three groups.
Results:
The diameters of the subclavian vein group, the brachiocephalic vein group, and the superior vena cava group were (10.29±0.42) mm, (12.29±0.32) mm and 13.00 mm, respectively. 7.Dosimetry audits research for 31 medical accelerators by using radiophoto luminescent glass dosimeter in Jiangsu province

Objective:To perform dosimetric audits on medical accelerators in radiotherapy institutions in Jiangsu province by using a radiophoto luminescent glass dosimeter (RPLGD) and to evaluate both the accuracy of radiotherapy doses and the effectiveness of RPLGD as an auditing tool.Methods:Firstly, the purposive sampling method was used to select three prefecture-level cities in Jiangsu province with good cooperation between radiotherapy institutions and a large number of patients treated by medical accelerators. Secondly, a simple random sampling method was used to select 31 radiotherapy institutions from all radiotherapy institutions in the selected three cities. Finally, one medical accelerator was selected from each institution for dose audit. A dose audit was performed by using RPLGD combined with a 30 cm × 30 cm × 30 cm standard phantom. The dosimeter was placed 10 cm underwater in the center of a 10 cm × 10 cm field with a 100 cm source skin distance. The dosimeter was irradiated with absorbed dose of 2 Gy. After the completion of irradiation, the dosimeter was read by the reader and the deviation was calculated. The acceptance level of the present research was ±5%. A non-parametric test was used to test the significance of differences between different manufacturers, installation years, and geographic distribution.Results:The dose deviation of 31 accelerators was from -16.9% to 2.0%. Of these, 28 accelerators pass the audit with pass rate of 90.3%. A follow-up audit was conducted on the only accelerator whose dose deviation exceeds±10%, also with discrepancy within ±5%. Two accelerators with a dose deviation larger than ±5% were re-measured using ionization chambers, both with dose deviation within ±3%. The non-parametric test result showed that there were no significant statistical differences in dose deviation for accelerators with different manufacturers, installation years, and geographic distribution( P>0.05). Conclusions:The levels of dose deviation for accelerators in Jiangsu province are consistent with those in the previous studies. However, there is still a difference with developed countries. RPLGD is feasible as a dose audit tool. The coverage of the RPLGD audit should be expanded to promote the quality of the radiotherapy through dose aduit.
8.Impacts of stroke center construction upon diagnosis and treatment of patients with acute ischemic stroke

Objective To discuss the impacts of stroke center construction upon therapeutic indexes for di-agnosing and treating patients with acute ischemic stroke. Methods Patients were divided into a control group (180 patients)and a formal group(245 patients)based on the time of stroke center construction.Patients in both groups were recorded time points in the course of diagnosis and treatment,and compared in number of cases with intravenous thrombolysis,number of cases receiving intravascular interventional therapies,DTN(door-to-needle) time,number of deaths,National Institutes of Health Stroke Scale(NIHSS)scores upon grouping,NIHSS scores after four weeks,NIHSS scores after 3 months,days of hospital stay and hospital charges.Results After the con-struction of the stroke center,time spent in different links were decreased.The number of cases with pure intrave-nous thrombolysis and DTN time(shorter than 60 min)were increased,and the difference were statistically signifi-cant(P<0.01).The number of cases who only received intravascular interventional therapies is increased,mean-while,DTN time was decreased,NIHSS scores after 4 weeks was increased and NIHSS scores after 3 months were also increased(P < 0.05)in these cases. No statistically significant differences existed in number of cases who were treated by bridging with intravenous thrombolysis in combination with intravascular interventional therapies and death cases(P=0.153,P=0.247).There were no statistically significant differences in days of hospital stay and hospital charges(P=0.152,P=0.406).Conclusions After the stroke center construction,it is helpful for significantly improving medical institutions′diagnosis and treatment of stroke,reducing time of such diagnosis and treatment in different links,shortening DTN time,increasing thrombolysis rate,improving patients′prognosis and bringing more benefits to patients with acute ischemic stroke by optimizing procedures for diagnosing and treating stroke.
9.Comparison of clinical effects of bare stent and covered metal stent graft in the treatment of complete occlusion of central venography in hemodialysis patients

Objective To evaluate the efficacy of bare mental stent (BMS) and covered stent (CS) in the treatment of complete central venous occlusive disease (CVOD) in hemodialysis patients.Methods A total of 66 cases of CVOD who have been treated by endovascular methods successfully in the First Affiliated Hospital of Sun Yat-sen University from Jan 2015 to Jan 2017 were enrolled in this study.According to the type of stent,the patients were divided into two groups,BMS group (n=46)and CS group (n=20).The demographic data,clinical signs and symptoms,and pre-procedure and post-procedure imaging data were followed up and recorded.The primary patency rates were calculated at 1,3,6,9,and 12 months.Results The related symptoms were improved within 2 day post-procedure.The primary patency rates of BMS group in 1,3,6,9 and 12 months were 97.83%,95.65%,69.56%,41.3%,and 34.78% respectively.The rates of CS group were 100%,100%,95%,65%,and 60%respectively.They did not reached statistical significance for primary patency rates between two groups in 1,3,and 6 months (P > 0.05 respectively).However,from 9 months after procedure,it began to show the significant difference between two groups (P < 0.05).The median patency time of the CS group was (10.30±5.32) months,while BMS group was (8.52±0.49) months.The difference between the two groups was statistically significant (P=0.046).Conclusions Stent implantation for complete occlusion of central venous in hemodialysis patients can get credible effect.The use of CS for CVOD provides superior patency as well as patency time in long period after procedure as compared with BMS.
10.The use of multi-detector computed tomography venography and three dimensional reconstruction image in evaluating the central venous occlusive disease of hemodialysis patients

Objective To evaluate the feasibility and value of multi-detector computed tomography venography (MDCTV) and three dimensional reconstruction image in the assessment of central venous occlusive disease in hemodialysis patients,and in the value of guiding interventional treatments.Methods Sixty hemodialysis patients with swelling of upper limbs were scanned by Toshiba 128-multislice spiral computed tomography (128-MSCT) and totally 80-100 ml non-ionic contrast media was injected into each of the patients via the peripheral veins of the contralateral limb with the rate of 4 ml/s.MSCT scanning was taken by the technique of intelligent triggering after setting scanning triggering threshold,with the monitoring point set in the development of the lumen of inferior vena cava,to detect the position and degree of vascular stenosis.The images were reformed as maximum intensity projection (MIP),volume rendering (VR),curved planar reformation and threedimensional image reconstruction technique.Results MDCTV clearly demonstrated the lesion location in all cases enrolled.Seventy-five occlusive lesions were detected in the total of 60 hemodialysis patients with swelling of upper limbs by MDCTV,of which the lesions of brachiocephalic vein was 47,superior vena cava 15 and subclavian vein 13.Among the 75 stenosis lesions,the number of complete occlusive,severe,moderate and mild stenosis was 31,24,19 and only 1,respectively.MDCTV provided information coincident with that of digital subtraction angiography (DSA),which the correlation index was 0.401,while DSA showed that number of complete occlusive,severe,moderate and mild stenosis was 49,7,14 and 5,respectively.Pereutaneous transluminal angioplasty was performed in 53 patients,and stent placement was done in 40 patients.After interventional treatments,swelling of upper limbs were obviously relieved and vascular accesses got functional recovery to the extent that they could meet the requirement of hemodialysis.Conclusions MDCTV is the first choice to evaluate the condition of central venous occlusive diseases of hemodialysis patients with advantages of non-invasion,high definition and three-dimensional reconstruction.It can provide accurate evaluations of the conditions of occlusive lesions,which can be of great clinical significance to subsequent interventional therapy.
