1.Quantitative metabolomics based on NMR.

Nuclear magnetic resonance (NMR) spectroscopy can be used to both identify and quantify chemicals from complex mixtures. Over the last several decades, significant technical and experimental advances have made quantitative nuclear magnetic resonance (qNMR) a valuable analytical tool for quantitative measurements of a wide variety of samples. This particular approach is now being exploited to characterize the metabolomes of many different biological samples and is called quantitative metabolomics or targeted metabolic profiling. In this review, some of the strengths, limitations of NMR-based quantitative metabolomics will be discussed as well as the practical considerations necessary for acquisition with an emphasis on their use for bioanalysis. Recent examples of the application of this particular approach to metabolomics studies will be also presented.
2.Anal pad preserving respective resection of the internal and external hemorrhoids for the treatment of circumferential mixed hemorrhoids

3.Distribution of nesfatin-1/NUCB2 in the digestive system of humans and rodents

Objective To investigate the regional distribution and morphological features of nesfatin-1/NUCB2 in digestive system of the humans, Sprague-Dawley (SD) rats and the institute of cancer research (ICR) mice, so as to lay the foundation for further study of its functions in the digestive system. Methods The specimens were obtained from SD rats and ICR mice as well as 20 patients with digestive disease, who were admitted to the First hospital affiliated to Nanjing Medical University and receired surgercal treatment. The specimens from patients with malignant tumors were obtained 5 cm apart from cancerous tissues and from patients with benign tumors were obtained near the focus. The resected tissues included pancreas, stomach, duodenum, esophagus, liver, small intestine or colon. The distribution of nesfatin-1/NUCB2 was examined with immunohistochemical (IHC) staining and its protein level in each organ was measured using Western blotting. Results The immuinohistocemical study revealed the similar distribution pattern of nesfatin-1/NUCB2 in the digestive system of the patients, SD rats and ICR mice. Nesfatin-1/NUCB2 was found to localize in the center of the pancreatic islets, the lower 1/3 to 1/2 of the gastric mucosal glands, as well as the submucosa of the duodenum. Western blotting examination showed the expression of NUCB2 in all tissues from patients, SD rats and ICR mice, whereas the protein level of the nesfatin-1/NUCB2 was higher in pancreas (0.84±0.03, 0. 84±0.05 and 0. 84±0.04, respectively), stomach (0.86±0.06,0.81±0.02 and 0. 78±0.02, respectively) or duodenum (0.79±0.09,0. 79±0.04 and 0.78±0.05)than that in esophagus (0.43±0.04,0.44 ± 0.02 and 0.47 ± 0.06, respectively), liver (0.42±0.01,0.44±0.04 and 0.43 ± 0.01, objectively), small intestine (0.32±0.04,0. 32 ± 0. 04 and 0.34 ±0.04, respectively) or colon (0. 29±0.01,0.32±0.03 and 0. 28±0.03, respectively)(all P values=0. 000). Conclusion Nesfatin-1/NUCB2 is widely expressed in the pancreatic islets, gastric mucosal glands and duodenum of the patients, SD rats and ICR mice, which indicates that nesfatin-1/NUCB2 may be involved in the regulation of food intake, carbohydrate metabolism and gastrointestinal motility.
4.Evaluation of dose-related effects of 2', 3', 5'-tri-O-acetyl-N6-(3-hydroxylaniline)adenosine using NMR-based metabolomics.

2', 3', 5'-Tri-O-acetyl-N6-(3-hydroxylaniline)adenosine (WS070117) is a derivative compound of natural product cordycepin. It has significant lipids regulating activity and low toxicity which has been proved by in vitro and in vivo experiments. In this study, 1H NMR-based metabolomics was used to investigate the dose-related effects of WS070117 on hyperlipidemia of high-fat-fed hamsters. The hyperlipidemic hamsters were administrated with six different doses of WS070117, including 3, 12, 50, 100, 200 and 400 mg x kg(-1) x d(-1). 1H NMR spectra of hamster serum were visually and statistically analyzed using two multivariate analyses: principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA). As a result, WS070117-treated groups showed dose-related regulation of metabolites associated with lipid metabolism, choline metabolism and glucose metabolism. The dose of 3 mg x kg(-1) x d(-1) of WS070117 only exhibited a little lipids regulating activity. However, the doses of 12 and 50 mg x kg(-1) x d(-1) of WS070117 both regulated the contents of metabolites to reverse significantly toward normal levels. When the dose of WS070117 reached 100 mg x kg(-1) x d(-1), it was more effective than positive control drugs. The work suggested that NMR-based metabolomics might be a valuable approach to evaluate dose-related effects of lipids regulating compounds.
5.Bacterial Contamination of Dialysate: A Surveillance and Investigation

OBJECTIVE To evaluate the cause of unqualified level of bacteria in dialysis fluid and to discover(available) resolution measure to select a batter aseptic sampling manipulation.METHODS Spot sampling in dialysis fluid with two methods A and B was used in a hemodialysis unit,1ml was taken to direct culture,and count the(colony).(RESULTS) The finding showed that the cause of unqualified level of bacteria in dialysis fluid focused in types Ⅲ and Ⅳ of(dialyzable) machine,and the average number of bacteria exceeding standard accounted for 18.48% with method B.CONCLUSIONS The contamination in dialysis fluid is related with dialyzable machine and manned(manipulation).

7.Effects of PCNA antisense oligonucleotide and VEGF gene transduction on restenosis after angioplasty

Objective To investigate the effect of antisense oligonucleofide(PCNA ASON)and vascuiar endothelial growth factor (VEGF) gene traneduction on restenosis after balloon angiopasty.Memods chinese rabbits were randomly divided into control group(I),PCNA ASON(II),VEGF only (Ⅲ),PCNA ASON+VEGF(Ⅳ)groups.Each group included 7 rabbits.Restenosis wasevaluated by pathology immunohistochemistry and Western-blotting for the expression of PCNA,and the depth and area oftunica media and tunicca intima were measured. Results All rabbits experienced restenosis on different severities,especially in control group.Lesions were less severe in PCNA ASON and VEGF groups than in controls.The proliferation of smooth muscle and intima significantly ameliorated in PCNA ASON and VEGF combination group that in PCNA ASON or VEGF groups(P<0.01).But the difference between the PCNA ASON and VEGF group was not significant(P>0.7). Conclusion PCNA ASON and VEGF gene transductionn are effective in preventing restenosis after balloon angioplasty caused vessel injury in rabbits.
8.A 1H NMR based metabonomics approach to progression of coronary atherosclerosis in a hamster model.

To obtain a better understanding of the progression of atherosclerosis and identify potential biomarkers, proton nuclear magnetic resonance spectroscopy (1H NMR)-based metabonomics was used to study the metabolic changes in the plasma of hamster fed with a high-fat/cholesterol diet. Plasma samples were collected at different time points during the progression of atherosclerosis and individual proton NMR spectra were visually and statistically assessed using multivariate analyses. NMR results for all samples showed a time-dependent development from physiological to pathophysiological status during atherosclerosis. Analysis of the identified biomarkers of atherosclerosis suggests that lipid and amino acid metabolisms are significantly disturbed, together with inflammation, oxidative stress, following cholesterol overloading. The results enriched our understanding of the mechanism of atherosclerosis and demonstrated the effectiveness of the NMR-based metabonomics approach to study such a complex disease.
9.Clinical significance of Wilm tumor gene(WT1) expression in breast cancer

Objective To investigate the clinical significance of Wilm tumor gene (WT1) expression in breast cancer. Methods Real-time quantitative reverse transcriptase polymerase chain reaction (RQ-RT-PCR) method was established for detecting WT1 and GAPDH mRNA expression levels in 110 cases of various breast tumor and the corresponding adjacent normal breast tissue. Normalized WT1 expression level (WT1_N)was determined as a ratio between WT1 and GAPDH for each case. The tumor tissue WT1_N over the normaltissue WT1_N of the same case was calculated as T/N_(WT1) ratio, and T/N_(WT1) value was analyzed with the clinicalpathological parameters. Results The WT1_N expression levels of the 102 breast cancer tissues were significantly higher than those of the adjacent normal breast tissues, with the median WT1_N of 2.38 (ranged from 0.12 to 112.3) and 0.81 (ranged from 0.03 to 11.65) for each (P <0.01), but there were no statistical differences between the WT1_N of 8 benign breast tumors and the nearing normal tissues, with the median WT1_N of 0.46 (ranged from 0.16 to 5.04) and 0.53 (ranged from 0.14 to 4.94) for each. Furthermore the WT1_Nas well as the T/N_(WT1) ratio of the malignant breast cancer tissues were significantly higher than those of the benign tumor tissues, with the median T/N_(WT1) value of 2.54 (ranged from 0.28 to 172.88) and 1.17 (ranged from 0.09 to 2.63) for each. Non-parameter correlation analysis showed that the T/N_(WT1) in breast cancers were of no relevance to lymph node metastasis, clinical-pathological types, estrogen receptor and progestone receptor status, but positively correlated with the expression level of IL-8 gene which calculated with T/N IL-8(r =0.723, P <0.01). Conclusion The WT1 gene is highly expressed in breast cancer which suggests that WT1 level assessed by RQ-RT-PCR could be a novel marker of disease progression and poor prognosis.
10.Investigation on the cognition and needs of integrating old-age care with medical service among different population groups

Objective To investigate the cognition and attitude toward integrating old-age care with medical services among the elderly,nurses from medical institutions and old-age care institution staffs,in order to provide references for developing implementation strategies of integrating old-age care with medical service.Methods The convenience sampling method was conducted,and 963 people.from 4 cities in Henan Province were enrolled in this survey from July to September 2016,including 599 elderly people,248 nurses from medical institutions and 166 old-age care institution staffs.All volunteers were investigated by using self-designed questionnaires.Results A total of 1 000 questionnaires were distributed,and 963 valid questionnaires were retrieved.The effective recovery rate was 96.3%.The cognition of integrating old-age care with medical service in all volunteers was low.There were differences in the needs of all nursing care services in the integrative old-age care and medical services,the needs of Chinese traditional medical care services were low.The highest demand of service capabilities was skills of coping urgent issues,while the lowest demand of service capabilities was education levels of care givers;the scores on evaluation of the two demands in the elderly were (4.803± 0.453) and (3.124± 1.246) points,those in the old-age care institution staffs were (4.819±0.468) and (2.698± 1.346) points,there was no statistically significant difference between the two groups (P>0.05).Conclusion In the progress of promoting the integration of old-age care and medical services,it is necessary pay more attention on the promotion among non-urban elderly people,and improving living facilities as well as the quality of care services.Integrated management and hierarchical training for nursing professionals and old-age care institutions staffs should be carried out gradually.
