1.Effect observation of intellectual training on cognitive function of patients with Alzheimer's disease

Objective To discuss effect of intellectual training on cognitive function, daily life ac-tivity and life quality of patients with Alzheimer's disease. Methods We used clinical dementia rating (CDR) scale to selecf 38 patients with mild to median degree of Alzheimer's disease.Intellectual training was carried out for 1 year and at the same time their family members took part in it and dietary instruction was given. Mini-mental state examination (MMSE), information-memory-concentration test (IMCT) and activity of daily living scale (ADL) were used as evaluation tools before and after training. Results The patients' degree of defect before training was not different from that of within the first three months after training (P>0.05),but greatly alleviated 6 months and one year later (P< 0.05,P< 0.01 ). Conclusions Beneficial intellectual training could improve the cognitive function of patients with Alzheimer's disease.It alleviated their symptom,prolonged the process of dementia and increase the living and life quality of them.
2.Research about using comfortable nurisng for patients with malignant tumor to promote their qual-ity of life

Objective To know the influence of comfortable nursing for paitnets with malignant tumor on promoting their quality of life. Methods Selected 995 patients with malignant tumor who had diagnosed by pathology and coured by chemotherapy or radioehemotherapy. Divided them into the experimental group (498 cases) and the control group (497 cases) randomly. Routine nursing cares were used in the control group,while the comfortable nursing cares were used in the experimental group in addition. QLQ-C30 was used to evaluate the level of patients'quality of life at the time of beginning, ending and the midpoint of the treatment. Results There was no significant difference between the two groups about the QLQ-C30 values at the beginning of the treatment, while the differences were signifi-cant at the other two time points, the level of quality of life in the experimental group was better than in the control group. Conclusions Comfortable nursing can promote quality of life of patients with malignant tumor, which should be developed in the clinical field.
3.The effect of propofol or/and procaine on human neutrophil function

Objective The study consisted of two parts: in part Ⅰ the effects of propofol or/and procaine on CD18, CD62L expression and superoxide anion (SOA) release of phorbol 12-myristal 13-acetate(PMA) stimulated human neutrophils (PMNs) were studied in vitro; in part Ⅱ the effects of propofol or/and procaine on IL-6 and TNF-?production in endotoxin-stimulated human whole blood were studied. Methods PMNs were separated from the whole blood obtained from healthy 20-40yr old subjects. Part Ⅰ consisted of 9 groups: in group 1 (control) phosphate buffer solution was added to PMNs; in group 2 PMNs were stimulated by PMA 100mg?ml-1 ; in group 3-5 different concentrations of propofol (0.4, 4.0,40?g?ml-1) were added to PMA stimulated PMNs; in group 6-8 different concentrations of procaine (1.5,15,150?g?ml-1 ) were added to PMA stimulated PMNs; in group 9 propofol 2?g?ml-1 and procaine 8?g?ml-1 were added to PMA stimulated PMNs. Part II also consisted of 9 groups: in groupl whole blood was mixed with normal saline; in group2 lipopolysaccharide (LPS) l?g?ml-1 was added to whole blood; in group 3-5 different concentrations of propofol (0.4,4.0,40?g?ml-1) were added to LPS stimulated whole blood; in group 6-8 different concentrations of procaine (1.5, 15, 150?g?ml-1 ) were added to LPS stimulated whole blood; in group 9 propofol 2?g?ml-1 and procaine 8?g?ml-1 were added to LPS stimulated whole blood. CD18, CD62L expressions were measured by flow cytometry. SOA release was determined by cytochrome C reduction in the presence or absence of superoxide dismutase (SOD) . IL-6 and TNF-? production were measured by using radioimmunoassay. Results Propofol or/and procaine depressed CD18 up-regulation, CD62L shedding and SOA release of PMA-stimulated PMNs and propofol was more effective than procaine. Propofol enhanced but procaine inhibited the increased production of TNF-? and IL-6 in the LPS stimulated whole blood but when propofol and procaine were used in combination. The effectof procaine predominated. Conclusions Propofol or/and procaine can attenuate tissue damage induced by neutrophils by inhibiting the function of neutrophils.
4.Study on the Expression of HER2 and TOPOⅡ? in Human Breast Cancer and Relationship between them

Objective To detect the expression of HER2 and TOPOⅡ? in human breast cancer to explore the relationship between them and the potential value in the chemotherapy of breast cancer. Method 24 breast cancer tissues and paired normal tissues were investigated. RT-PCR and imumohistochemical staining were used to detect the expressions of HER2 and TOPOⅡ? at the transcription and protein level respectively. Results HER2 mRNA expressed positively in 29% (7/24) breast cancer tissues vs 4% (1/24) in paired normal tissues. The positive expression of TOPOⅡ? mRNA in breast cancer tissues and paired normal tissues were 92% (22/24) and 21%(5/24) respectively. The result of the immunohistochemical staining showed that the positive rates of HER2 and TOPOⅡ? in breast cancer tissues were 30%(6/20) and 94%(16/17) respectively, which was consistent with RT-PCR. The 6 cancer samples with HER2 positive expression coexpressed TOPOⅡ?. The expression of these two proteins was correlated(R=0.524,P
5.Clinical study on the efficacy of tranexamic acid in reducing postpartum blood lose: a randomized, comparative, multicenter trial

For the average blood loss at 2 hour postpartum, it was 129 7 ml, 133 9 ml, 168 5 ml and 178 2 ml for group Ⅰ, Ⅱ, Ⅲ and Ⅳ respectively, while the total blood loss for the 4 groups was 243 3 ml, 242 9 ml, 308 1 ml, and 314 8 ml respectively. The average blood loss of group Ⅰ and Ⅱ was significantly less than group Ⅲ and Ⅳ ( P 0 05). The occurrences of postpartum hemorrhage (blood lose ≥400 ml) were 6 4%, 13 3%, 20 7% and 25 3% for group Ⅰ, Ⅱ, Ⅲ and Ⅳ respectively. There was no major adverse effects appeared. Conclusions Transamin is efficient and safe in reducing the postpartum blood loss. 1 0 g of Transamin has the best efficacy, and 0.5g of Transamin followed.
7.The effect of tripterygium wilfordii on proteinuria in patients with diabetic nephropathy

8.Superior labrum anterior-posterior lesions on shoulder MR arthrography

Objective To evaluate the diagnostic value of shoulder MR arthrography for superior labral anterior-posterior (SLAP) lesions.Methods A retrospective study was conducted in 137 cases of shoulder MR arthrography confirmed by subsequent shoulder arthroscopy.Two radiologists analyzed all MR examinations independently and the results were compared with those of arthrescopy.The superior labrum was described as normal or torn.In addition, each iabral tear was classified as type Ⅰ-Ⅳ.Type Ⅰ lesions were defined as marked fraying of the articulating surface of the superior labrum; type Ⅱ, avulsion of the labral-bicipital complex from the ghnoid; type Ⅲ, displaced bucket handle tear of the superior labrum; and type Ⅳ, bucket handle tear of the superior labrum with extension into the fibers of the biceps tendon.Sensitivity, specificity, and accuracy were calculated.Kappa values were calculated to quantify the level, of inter-observer agreement.Results SLAP lesions were arthroscopically diagnosed in 59 of the 137 patients.Six of the 59 lesions ( 10.2% ) were classified as type Ⅰ , 50 (84.7% ) as type Ⅱ, and 3 (5.1% ) as type Ⅲ.The overall sensitivity, specificity, and accuracy of MR arthrographic detection of SLAP lesions were 86.4% (51/59), 78.2% (61/78), and 81.8% ( 112/137), respectively, for observer A, and 88.1% (52/59), 84.6% (66/78), and 86.1% (118/137), respectively, for observer B.At inter-observer comparison, agreement was very good (Kappa values = 0.796 ).The MR arthrographic classification showed correlation with the arthroscopic classification of SLAP lesions were 83.1% (49/59)and 79.7% (47/59) for two observers, respectively.Conclusion Shoulder MR arthrography is a reliable method for evaluating SLAP lesions.
9.Tear of the long head of the biceps brachii tendon: evaluation with shoulder MRI

Objective To assess the diagnostic value of MRI for the long head of the biceps brachii tendon tear,and to compare the diagnostic efficiency between routing MRI and MR arthrography. Methods A retrospective study was conducted in 215 cases with shoulder MR examination (107 with MR arthrography, and 108 with routing MRI) and subsequent shoulder arthrescopy and surgery. Two radiologists analyzed all MR examinations independently, and the results were compared with those of arthroscopy and surgery. Sensitivity, specificity, and accuracy were calculated. Kappa values were used to quantify the interobserver agreement. Results Based on the results of arthroacopy and surgery, 215 patients comprised 7 cases of complete tear of the long head of the biceps brachii tendon, 29 cases of partial tear, and 179 cases without tear. The overall sensitivity, specificity, and accuracy for the diagnosis of tears (complete and partial tear) of the long head of the biceps brachii tendon were 72.2% (26/36), 91.6% (164/179), and 88.4% (190/215) respectively for observer 1, 80.6% (29/36), 93.8% (168/179), and 91.6% (197/215) respectively for observer 2. The interobserver agreement was good (Kappa value=0.681). For the complete tear of the biceps brachii tendon, the sensitivity, specificity, and accuracy were all 100% (7/7), 100% (208/208), and 100% (215/215) for both observers. For the tear of the long head of the biceps brachii tendon, the accuracy of MR arthrography were 93.4% (100/107) for observer 1 and 96.3% (103/107) for observer 2. They were higher than the Accuracy of routing MRI, which were 83.3% (90/108)and 87.0% (94/108) respectively for two observers (P<0.05). Conclusion Shoulder MRI is a moderate reliable method for evaluating the tear of the long head of the biceps brachii tendon, and the accuracy of MR arthrography is found to be superior to that of routine MRI.
10.Current status and training requirements for retraining program of general practitioner

The current status and training requirements for general practitioner (GP) were surveyed with questionnaires in 84 trainees from the first Beijing GP retraining program.Results showed that three most demanded skills of health service in community were electrocardiogram (98.8%),clinical lab results reading (96.4%),dressing change (90.5%).The services not provided but should be provided in community were aerosol rebreathing method (23.8%),resection of small superficial lumps (22.6%) and reading X-ray films (20.2%).The reasons for services not provided were insufficient resources (41.7%),lack of skilled personnel (35.7%) and staffing shortage (20.2%).The most required training skills were reading X-ray films (64.3%),electrocardiogram (54.8%) and laboratory results reading (51.2%).There were some skills inadequately trained including cricothyroid membrane puncture,suprapubic bladder puncture and resection of superficial small lumps.The results indicate that the required skills in community health service and individual demands of general practitioners should be the priority of the future training programs.
