1.An analysis of 238 neonates with long duration fever

Objective To study the etiologies of long duration fever in neonates and to provide clinical evidence for early diagnosis and intervention. Methods From January 1st, 2004 to December 31st, 2013, data from neonates admitted to the Department of Neonatology, Childrenˊ s Hospital of Chongqing Medical University were retrospectively analyzed. Results Among 238 neonates with long duration fever, 165 cases were male and 73 female. Pneumonia(n = 196), skin / mucosa infection (n =67), sepsis (n = 46), meningitis/ encephalitis (n = 25) and neonatal necrotizing enterocolitis (n = 24) were the most common etiologies. Diabetes insipidus was the only noninfectious cause of fever in this study. The incidence of high fever in cases diagnosed with meningitis/ encephalitis, sepsis, diabetes insipidus, immunodeficiency or urinary tract infection were significantly higher than other causes (P <0. 05) . Continuous fever or other special fever types were more common in cases diagnosed with pneumonia ( χ2 = 4. 581, P = 0. 032 ) . Incidences of sepsis, infectious diarrhea or meningitis/encephalitis in late neonates were significantly higher than early neonates ( P < 0. 05 ) . Conclusions Long duration fever in neonates is most likely caused by infectious diseases and diabetes insipidus is the only noninfectious cause of neonatal fever in our study.
2.Influence of two kinds of infusion methods on nosocomial infection of children with leukemia during treatment progress

Objective To investigate the incidence of nosocomial infection of placing peripherally inserted central catheter (PICC) and venous port acess(VPA) in children with leukemia. Methods 60 cases with leukemia were treated in our hospital from January 2005 to September 2009.35 cases were mail, 25 cases were femail.Their ages ranged from 2.0 to 16.5 years old.42 cases fell to the PICC group, hospital-ization for 202 times, 18 cases belonged to the VPA group, hospitalization for 109 times. The incidence of infection in hospital, infection site, common pathogens and prognosis of two groups were analyzed retro-spectively.Results The incidence of fever and increasing of C-reactive protein of the PICC group were significantly lower than those of the VPA group. There was no difference of the incidence of white blood cells decreasing between two groups. The incidence of infection in hospital in the PICC group was 38.61%, higher than 28.44% of the VPA group. Their difference was significant. There was significant difference of the distribution of infection between two groups. There was no significant difference of the major pathogens of sputum culture and blood culture between two groups. Conclusions Using venous port acess(VPA) could reduce the incidence of infection in hospital compared using peripherally insertied central catheters in children with leukemia, so venous port acess( VPA ) may be safer.
3.Preparation and Release Characteristics of Venlafaxine Hydrochloride Hydrophilic and Lipidic Matrix Sus-tained-release Tablets

OBJECTIVE:To prepare Venlafaxine hydrochloride sustained-release tablets,and to investigate the characteristics of drug release. METHODS:Using glyceryl behenate as lipidic matrix material and HPMC as hydrophilic matrix material,the kind and dosage of excipients were screened by single factor experiment. Using 4,8,24 h accumulative release rate and the deviation summation of ideal values as index,the viscosity of HPMC,the amounts of HPMC K15M and glyceryl behenate were optimized by orthogonal test. The dissolution curves were fitted by different equations. RESULTS:The optimal formulation was as follows as venlafaxine hydrochloride 8.5 kg,HPMC K15M 15 kg,glyceryl behenate 26 kg,magnesium stearate 0.5 kg. 4,8,24 h accumula-tive release rates of prepared matrix sustained-release tablets were 34.3%,63.9% and 99.2%,respectively. The releases profiles of hydrophilic and lipidic matrix sustained release tablets followed first-order equation in vitro mainly through matrix erosion. CON-CLUSIONS:Venlafaxine hydrochloride hydrophilic and lipidic matrix sustained-release tablets with good sustained-release effect have been prepared successfully.
4.Analysis of the effect of diversified nursing rehabilitation exercise on patients with Schatzker Ⅱ tibial plateau fracture

Objective:To explore the effect of diversified nursing rehabilitation exercise in patients with SchatzkerⅡtibial plateau fracture.Methods:From January 2018 to December 2018, a total of 72 patients with Schatzker Ⅱtibial plateau fractures in Wuzhou traditional Chinese medicine hospital were selected by cluster sampling method for study. All patients were treated surgically. 36 of them were included in the control group (routine functional exercise after operation) by random number table method, and the remaining 36 patients were included in the observation group (on the basis of the control group, multiple functional rehabilitation exercise mode was given).The length of hospital stay, knee joint swelling degree and movement angle at different time after operation, preoperative and postoperative Harris Hip Score (HSS) and satisfaction were analyzed.Results:The time of fracture healing and hospitalization in the observation group were (92.58±20.65) d, (8.40±2.32) d, shorter than (108.56±25.38) d, (15.86±2.80) d in the control group ( tvalues were 2.930, 12.309, P<0.05). At 2 weeks and 6 weeks after operation, the swelling degree of knee joint in the observation group was (1.42±0.58) cm and (0.52±0.25) cm, which was less than (1.86±0.62) cm and (0.78±0.34) cm of the control group, respectively, the difference between the two groups was statistically significant ( tvalues were 3.110, 3.697, P<0.05); there was no significant difference in HSS between the two groups ( P>0.05); the score of observation group was (62.36±6.85), (70.32±5.69) points, which was significantly higher than (58.42±6.02), (63.45±6.28) points of control group ( tvalues were 2.592, 4.864, P<0.05); there was no significant difference in HSS score between the two groups 6 months after operation ( P>0.05). The satisfaction of the observation group was 97.22% (35/36), which was higher than 83.33% (30/36) of the control group, and the difference between the two groups was statistically significant ( χ 2value was 3.956, P<0.05). Conclusion:For Schatzker type Ⅱ tibial plateau fracture patients, taking diversified nursing rehabilitation exercise can quickly relieve their limb swelling, promote the recovery of joint function and fracture healing, make them recover as soon as possible, and improve their satisfaction.
5.Behavioral intervention on quality of life in chronic heart congestive failure patients with urinary incontinence: a pilot randomized controlled trial

Objective To explore the effect of behavioral intervention on the quality of life of chronic heart congestive failure patients with urinary incontinence, to further provide basis for more effective clinical nursing measures. Methods 62 patients with chronic heart congestive failure combining urinary incontinence were divided into two groups by random digital table method, the control group and the intervention group, with 31 cases in each group. Patients in the control group received routine nursing care, while patients in the intervention group received behavioral intervention on the basis of routine care. The behavioral intervention included pelvic floor muscle training, pectineus exercise and reconstruction of micturition habits. The effect of the intervention on incontinence, quality of life and depression were observed in two groups. Results 61 patients completed the study, including 30 cases in the intervention group and 31 cases in the control group. There were no significant differences in the scores of the International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form (ICI-Q-SF), the MOS item Short Form Health Survey (SF-36) and Zung Self-rating Depression Scale ( SDS) between two groups before the intervention (P>0.05). After 6 weeks of continuous intervention, the scores of ICI-Q-SF and SDS in the intervention group were(8.69±1.94)points and (55.91±4.57) points, which were significantly lower than (11.07±2.14) points and (61.44±5.98) points of the control group (t=5.04, 3.18, P<0.01). About the SF-36, scores of physical functioning and social functioning in the intervention group were (76.77 ±10.34) points and (77.69±6.17) points, which were significantly higher than (60.39±10.07) points and (59.38±8.25) points in the control group (t=43.31, 120.36, P<0.01). Conclusions For chronic heart congestive failure patients with urinary incontinence, behavioral intervention can effectively ameliorate symptoms of urinary incontinence, improve the quality of life and relieve patients' depression, which thereby potentially promote patients′physical and mental health.
6.Clinical Research of Electrocardiogram Monitoring Assisted Ultrasound Guidance for Modified Seldinger Technique in Peripherally Inserted Central Catheters

Objective: To study electrocardiogram (ECG) monitoring assisted ultrasound guidance for modiifed seldinger technique (MST) in peripherally inserted central catheters (PICC) and to estimate the accuracy of ECG for presenting catheter tip location. Methods: The patients were connected with ECG monitoring system, the guide wire was connected to RA electrode in ECG monitor for leading out P wave of intracavitary ECG. The location of catheter tip was estimated by P wave characteristics and then, estimated ECG result was further compared with chest X-ray presented catheter tip position. The coherence of catheter tip position between ECG and chest X-ray was compared, the sensitivity and speciifcity of ECG estimation were calculated. Results: There were 100/104 patients showed speciifc P wave in ECG, the coherence to chest X-ray was 96.2%; 4 patients had no speciifc P wave and their chest X-ray presented that the catheter tip position at upper superior vena cava in 2 patients, at lower superior vena cava in 1 patient, at contralateral brachiocephalic vein in 1 patient. The accuracy of 2 methods had no statistic meaning. ECG estimation for PICC catheter tip position had the sensitivity at 99% and speciifcity at 100%. Conclusion: ECG monitoring may determine the catheter tip position for MST in PICC guided by ultrasound. It doesn’t need chest X-ray, without additional trauma and no radioactive contamination, which make PICC catheters get more extensive application in clinical practice.

8.Value of CT imaging in detection of mesenteric injury in closed abdominal trauma

Objective To investigate the CT manifestations in mesenteric injury.Methods CT imaging data were analyzed retrospectively in 28 patients with mesenteric injury caused by abdominal trauma,of which 26 patients were confirmed by surgery and 2 patients received conservative treatment.Results CT showed simple mesenteric injury in 5 cases,and multiple organ injury in other 23 cases.Only 18 cases were dignosed by preoperative abdominal CT,and the rate of missed diagnosis was 36%.Conclusion Multi-slice CT is an important evaluation method for the detection of acute mesenteric injury.

10.Protective effect of schisandrin B on cerebral ischemia reperfusion injury of rats and its mechanisms

Objective:To investigate the protective effect of schisandrin B (SchB)on the cerebral ischemia reperfusion injury of the rats and the influence in HSPA12B/PI3K/Akt signaling pathway,and to explore the mechanisms.Methods:130 SD rats were divided into sham group,cerebral ischemia reperfusion injury model group (model group),low dose of SchB group (SchB 3 mg· kg-1 ,SchB1 group),middle dose of SchB group (SchB 10 mg·kg-1 ,SchB2 group)and high dose of SchB group (SchB 30 mg·kg-1 ,SchB3 group)(n=26).The rats in sham group didn’t plug lines;the rats in model were used to establish ischemia reperfusion models;the rats in SchB1 ,SchB2 and SchB3 groups were firstly pretreated with different doses of SchB for 7 d,and then they were used to build cerebral ischemia reperfusion injury models.The nerve dysfunction of rats was evaluated by neurologic deficit score.The cerebral edema was detected by measuring the content of water in brain tissue.The morphological changes of brain tissue were observed by toluidine blue staining.The levels of nuclear factor kappa B (NF-κB), tumor necrosis factor-α(TNF-α),interleukin-1 (IL-1)and interleukin-6 (IL-6)in the brain tissue were detected by ELISA.Western blotting method was used to detect the protein expression levels of heat shock protein A12B (HSPA12B ), serine-threonine kinase (Akt ) and phosphorylation serine-threonine kinase (p-AKT ). Results:Compared with sham group,the neurologic deficit score of rats in model group was significantly increased (P <0.01),and the content of water in brain tissue was increased (P < 0.01 );the brain tissue structure was loosened,and the mesenchyme appeared edema;the NF-κB,TNF-α,IL-1,and IL-6 levels were increased (P <0.01),and the expression levels of HSPA12B and p-Akt proteins were decreased (P <0.01).Compared with model group,the neurologic deficit scores of the rats in SchB1 ,SchB2 ,and SchB3 groups were decreased (P <0.01),and the contents of water in brain tissue of the rats in SchB2 and SchB3 groups were decreased (P <0.05);the edema of nerve cells was alleviated,and the cavities were reduced;the NF-κB,TNF-α,IL-1,and IL-6 levels were decreased (P <0.05 or P <0.01),the expression levels of the HSPA12B protein in SchB2 ,and SchB3 groups were increased (P <0.05),and the p-Akt protein expression levels of the rats in SchB1 ,SchB2 ,and SchB3 groups were increased (P <0.01).Conclusion:SchB could protect the cerebral ischemia reperfusion injury of rats,its mechanism may be related to regulating the HSPA12B/PI3K/Akt signaling pathway and inhibiting the inflammatory reaction damage to the nerve cells of reperfusion.
