1.Effect of different methods of intestinal preparation on intestinal cleaning for elderly patients undergoing gynecological surgery

Objective To investigate the effect of different methods of intestinal preparation on intestinal cleaning for elderly gynecological patients undergoing surgery. Methods Sixty-four elderly patients undergoing gynecological surgery were divided into two groups according to admission seguence. The control group was treated with enema using 800~1000 mL 0.2%soapy water, while the experiment group was treated with enema using sodium phosphate salt oral solution mixed with 800 ~ 1000 mL 0.9% sodium chloride solution. The intestinal cleanness and enema times of the two groups were observed. Results The effect of intestinal cleanliness in the experiment group was better than that in the control group. There was significant differences between the two groups. (P<0.01). Conclusions For the elderly patients undergoing gynecological surgery, sodium phosphate salt oral solution can not only reduce the frequency of enema but also achieve the purpose of cleaning intestinal tract, to ensure the operation effect. It is the ideal method for cleaning bowels of the elderly patients in preoperative prepartion of the surgery.
2.Right Atrial Myxoma Diagnosed by Echocardiography

Objective To investigate the echocardiographic features in patients with right atrial myxoma and thus evaluate the value of echocardiog?raphy in the diagnosis of right atrial myxoma. Methods The echocardiographic findings of 20 patients with right atrial myxomas were retrospectively analyzed,and the echocardiographic features of these patients were summarized. Results The main clinical symptoms of right atrial myxoma includ?ed chest tightness,shortness of breath,lower limb edema,and syncope. Of these 20 patients,the echocardiography showed that the average size of the right atrial myxomas was 3.11 cm × 2.90 cm to 7.44 cm × 4.52 cm. The myxomas were round or oval shape in 15 patients(75%)and lobulated shape in 5(25%). The myxomas were attached to the atrial septum in 15 patients(75%)and to right atrial free wall in 25%of the patients. The aver?age width of the basement was 1.62±0.30 cm,and the width was greater than 1 cm in 80%of the patients had the width over 1 cm. Some myxomas had internal areas of calcification and anechoic cystic areas. The percentages of patients with the blocking of right atrium,tricuspid and pulmonary hy?pertension were 70%,50%,and 10%,respectively. Conclusion The echocardiographic features of right atrial myxoma were specific. Echocardiog?raphy can accurately assess the tumor and identify the secondary changes in cardiac structure and hemodynamics and thus provide evidence for time?ly and accurate diagnosis of right atrial myxoma.
3.Establishment of leukocyte-associated immunoglobulin like-receptor 2 (CD306) eukaryotic expression vectors and purification and identification of fusion protein

BACKGROUND: Biological function of leukocyte-associated immunoglobulin like-receptor 1 (LAIR 1) has clearly researched in China and abroad, but the in vivo biological function of LAIR is poorly understood. OBJECTIVE: To establish LAIR-2 (CD306) eukaryotic expression vectors and to purify and identify the fusion protein. DESIGN, TIME AND SETTING: A single sample observation experiment was performed at the Fourth Military Medical University of Chinese PLA between June 2007 and June 2008.MATERIALS: plg/3c vector was offered by Oxford University, pcDNA3.1 vector was provided by Meyaard doctor. Chinese hamster ovary (CHO) cell lines were preserved by the Department of Immunology, Fourth Military Medical University of Chinese PLA.METHODS: Two eukaryotic expression vectors plg/3c-LAIR-2 and pcDNA3.1-LAIR-2 were constructed and were transfected into CHO cells. The binding activities of LAIR-2 fusion protein to LAIR-2 mAbs were identified by Western blot, immunocytochemistry and flow cytometry assay.MAIN OUTCOME MEASURES: The construction of stably transfected cell lines, and the purification and identification of fusion protein. The activity of LAIR 2 protein combined to corresponding monoclonal antibody.RESULTS: Eukaryotic expression vectors were constructed and trasnsfected into CHO cells successfully. Two cells lines CHO/LAIR-2-Fc and CHO/LAIR-2 that steadily expressed LAIR-2-Fc fusion protein and LAIR-2 protein were established. Western blot assay showed that LAIR-2 protein could bind specially to LAIR-2 mAb 1A7, 3H12 and 4A9. Immunocytochemistry and flow cytometry assay demonstrated that 3H12 and 4A9 could bind to LAIR-2 expressed in the transfected CHO cells. CONCLUSION: Two ceils lines CHO/LAIR-2-Fc and CHO/LAIR-2 were successfully constructed, which can transfected to CHO cells. The eukaryotic expressed LAIR-2 protein has good binding activity to LAIR-2 mAbs.
4.Efficacy of ultrasound and microbubbles on augmentation phlebothrombosis inrabbit: an in vivo study

Objective To study the effects of diagnostic ultrasound combined with microbubble contrast agent on augmentation thrombolysis in vivo and to discuss the synergism and mechanism of combination of urokinase. Methods Thirty-two rabbits of femoral vein thrombus were divided into four groups: simple urokinase group, ultrasound and contrast microbubble group, ultrasound and contrast microbubble combined with urokinase group, control group. Then according to the best parameter combination which was definited in previous experiment, the experiments combination of urokinase were performed. Color Doppler was used to evaluate the recanalization of different group in 15 rain,30 min and 60 min. Results After the treatment of 15 rain and 30 min,the recanalization rate in ultrasound and contrast microbubble combined with urokinase group improved significantly compared with other groups (P<0.05);after 60 min, the recanalization grade of ultrasound and contrast microbubble and urokinase group was evidently better than other two group. The result of immunohistochemistry showed that disaggregation of clot and fibrous in this group at 60 min was better than the results at 15 min, 30 min. Conclusions Diagnostic ultrasound with microbubble contrast agent showed superior effects of thrombolysis and the synergism of combination of urokinase in vivo.
5.Investigation of attentional bias in high obsessive compulsive symptom individuals

Objective To explore the attentional biases in individuals with different types of obsessivecompulsive symptom and the correlations between the attentional bias of high obsessive compulsive symptom (HOC) and symptoms. Methods 22 individuals with HOC and 38 individuals with low obsessive compulsive (LOC) symptom completed the Chinese Emotional Stroop task that assessed the attentional bias. Comparisons were made between HOC and LOC and in different types of HOC, and the correlations between the attentional bias of HOC and symptom severity scores were searched for. Results The comparison between HOC and LOC on the reaction time of neutral, negative and disgust-related words were not significant. The contamination/washing subtype in HOC showed faster reaction time on negative and disgust-related words compared to that of neutral words,and its attentional bias exhibited significant correlation with symptom severity scores( r= -0. 648, P=0. 031 ). Conclusion Such information point out the contamination/washing subtype may have a different neural mechanism compared to the other subtypes of OCD.
6.Longitudinal Strain of Primary Mitral Regurgitation Predicts Postoperative Worsening of Left Ventricular Systolic Function

Objective To analyze subclinical changes of left ventricular deformation by using two-dimensional (2-D) speckle tracking imaging (STI) in primary mitral regurgitation (MR) and to identify sensitive parameters of left ventricular deformation that reflect reduction of left ventricu-lar function after mitral valve repair.Methods We evaluated patients with primary MR who underwent mitral valve repair between May 2013 and June 2015.According to the reduction in ejection fraction (EF),the MR patients were divided into MR1 (EF reduction of ≤10%) and MR2 groups (EF reduction of >10%).The parameters of left ventricular deformation were measured by using 2-D STI.Results Compared with the MR1 group,the MR2 group had increased global longitudinal strain (GLS;P < 0.05).A positive correlation was found between postoperative EF reduction and left ventricular intemal dimension at end-diastole,left ventricular internal dimension at end-systole,end-diastohc volume,left ventricular mass index,regurgitant volume,and GLS (P < 0.05).GLS showed a diagnostic value in predicting EF reductions of > 10%.Conclusion STI is useful for evaluating the postoperative reduction of left ventricular function in primary MR patients and thus is helpful for optimizing surgical timing.

Objective To explore the ultrasonic characteristics of cystic adventitial disease (CAD) and to evaluate the value of color Doppler ultrasound in diagnosis of CAD. Methods We retrospectively analyzed the ultrasonograms and other imaging methods of eight patients with CAD conformed by surgery in The first Hospital of China Medical University. Results Five of the eight CAD cases were confirmed by color Doppler ultrasound before surgery, and three were misdiagnosed. All the cases were single, with five in popliteal artery, two in iliofemoral artery, and one in external iliac vein. We divided the cysts into concentric cysts and eccentric cysts according to the cystic shape and the lumen stenosis. Six cases were eccentric cysts, in which two had septations;two cases were concentric cysts, and both had septations. The affected vessel expanded, but its shape did not change and the lumen was compressed. There were three layers of wall between the cyst and the lumen. The peak systolic velocity of the affected lumen increased and the distal end systolic peak velocity decreased when lesion occurred in artery. Distal end lumen reflux were slow when lesion occurred in distal veins. Angiography showed the stenosis of the affected lumen, which was described asscimitar sign. CT showed uniorlocular or multilocular cyst, and the compressed lumen. All of the eight patients underwent the diseased adventitia resected along with excision of the cyst. Conclusions The color Doppler ultrasound can display the location, shape, and internal echo of CAD, and thus it can accurately evaluate luminal stenosis and flow pattern. This is of important clinical significance for the diagnosis, treatment, and prognosis of CAD.
9.Evaluation of left ventricular torsion in patients with apical hypertrophic cardiomyopathy with two-dimensional ultrasound speckle tracking imaging

Objective To explore the value of two-dimensional ultrasound speckle-tracking imaging (STI) in assessing the left ventricular torsion (LVtor) in patients with apical hypertrophic cardiomyopathy (AHCM). Methods Thirty-four patients with AHCM and 21 healthy volunteers underwent two-dimensional echocardiography. High frame rate two-dimensional images of left ventricular short-axis views at the levels of mitral annulus and apex were obtained. Peak rotation (Prot) and the time to Prot, peak rotation velocity on basal and apical plane in left ventricular short-axis views, the peak twist (Ptw) and time to peak twist, half time of untwisting (HTU) were measured using two-dimensional strain software while twisting rate (Untw R) was calculated. All the above indices were compared between patients and healthy volunteers. Correlation was analyzed between HTU and E, A, E/A ratio. Results Compare with healthy volunteers, Prot and time to Prot and peak rotation velocity on apical plane in AHCM significantly increased, so did Ptw and peak rotation velocity of left ventricular (all P<0.05). HTU delayed with the Untw R and peak untwisting velocity decreased in the AHCM group (all P<0.05). There was negative correlation between HTU and E/A ratio in patients with AHCM. Conclusion When the left ventricular systolic function maintained normal in patients with AHCM, the peak rotation on apical plane and the LVtor increased with Untw R decreased. STI can sensitively evaluate the function alteration of myocardium in patients with AHCM.
10.The expression and clinical significance of the serum Eotaxin, IL-13 and total IgE in children with asthma

Objective To study the nosogenesis and the relationship of different levels of serum Eotaxin,IL-13 and total IgE(TIgE)in asthmatic children. Methods Thirty children with asthma aging from 5 to 12 years were selected as study group (attack period and remission period), and 22 healthy children were selected as control group. Serum Eotaxin and IL-13 were measured with enzyme linked immunosorbent assay, and serum TIgE was determined with enzyme-linked fluoroimmune assay in the study group and control group, and pulmonary function was measured at the same time in the study group. Results (1) There were significant differences in pulmonary function between the attack period and the remission period (P < 0. 001). (2) There were significant differences in the level of serum Eotaxin, IL-13 and IgE between the study group and the control group,especially for the attack period and the control group(P <0. 001). (3) There were significant positive correlations between the level of serum Eotaxin and IL-13, Eotaxin and TIgE, IL-13 and TIgE respectively in the study group (P <0. 05). (4) There were negative correlcations between the level of serum Eotaxin and FEV1 ,PEF,and serum IL-13 and FEV1, PEF in asthmatic children (P < 0. 05), there were no correlations between the level of serum TIgE and FEV1, PEF in the study group (P > 0. 05). Conclusion Serum Eotaxin, IL-13 and TIgE were participated in the nosogenesis of asthma, and the level of serum Eotaxin and IL-13 can be a assessment of asthmatic patients' condition.
