Objective To analyze incidence,pathogenesis and treatment of the complications of transvaginal hysterectomy(TVH)collected during past six years,and to discuss the possible methods of prevention.Methods The patients involved in present study were admitted and underwent the transvaginal hysterectomy in the General Hospital of PLA from Jan.2000 to Jun.2006.The data of complications occurred at intra-or post-operation were retrospectively analyzed.Results Among all of the 1990 cases with TVH,complications occurred in 75 cases,the incidence of complication was 3.77%,including 21 cases of bladder injury(1.06%),5 cases of ureter trauma(0.25%),6 cases of rectal injury(0.3%),30 cases of vaginal stump inflammation(1.51%),8 cases of vaginal stump haematoma(0.4%),2 cases of intra-abdominal hemorrhage(0.1%),2 cases of vaginal narrowness(0.1%)and 1 case of intestinal obstruction(0.05%).The bladder injury and vaginal stump inflammation almost toke the two third of total cases.Some complications were got correction by surgery appropriately during or after the operation when injuries were found.Some cases were managed by conservative treatment.All of the patients with complications were healed without any sequelae.The organ injuries occurred more easily in the cases with pelvic adhesion or less movable large uteri,such as the cases with the history of pelvic operation,the endometriosis or the lower segment myoma.Conclusions The organs adjoined to uterus were easily damaged by TVH during the performance of operation,and the corresponding complications are common after the operations.The operator should familiar with the pelvic anatomy,master the skilled surgical techniques and suitably control the indications of the TVH.With the improvement of techniques in TVH and the research of new instrument applied in this procedure,the incidence of complications in TVH would be further reduced.

Objective To detect the expression of lysyl oxidase (LOX) in pelvic floor tissues and the serum copper ions level of the women with stress urinary incontinence (SUI) for exploring the role of LOX in SUI pathogenesis. Methods The sera of 25 patients with SUI were collected before the operation. The content of serum copper was measured by the technique of atomic absorption spectrum. Meanwhile, 25 patients with similar age but suffering from benign gynecological diseases and undergone total hysterectomy were enrolled as control. Biopsy specimens were obtained from the uterine cardinal sacral ligament of the patients with and without SUI during the operation. The histological structure of pelvic collagen fiber was examined with routine HE staining and Van Gieson (VG) staining. Immunohistochemistry was used to determine the location of LOX. Protein expression of the LOX in pelvic floor tissue was analyzed by Western blotting. Results There was no statistical difference in serum copper content between SUI group and the control group. In the HE and VG staining sections, collagen fiber density of pelvic tissue in SUI group was decreased and fibers were arranged disorderly. Immunohistochemistry showed that LOX was mainly expressed in the cytoplasm of fibroblast. The number of positive cells in pelvic tissues of the patients with SUI was smaller than that in the control group, and the positive signal in SUI group was weaker than that in the control group. Objective To compare the success rate, the one-time success rate, the average required time and the incidence of complications of two methods in oronasal conversion of nose biliary duct in naso-biliary drainage. Methods 60 cases of patients with ENBD in the ERCP in our department from March,2008 to March, 2010 were divided into the study group and the control group by random. The patients with sleeving the gastric tube by the guidewire during oronasal conversion of nose biliary duct were considered as the study group, and the patients using the traditional method were considered as the control group. The success rate of sleeving gastric tube, the one-time success rate of grasping, the average required time and the adverse reactions of the two groups were compared. Results The success rate of the study group was 100%, and the control group was 90%, there was no statistical significance. The one-time success rate of the study group was 76.7%, higher than 46.7% of the control group. The average required time of the study group were (1.3±0.3) min, shorter than (5.1±0.7) min of the control group. The incidence of complications in the study group were 13.3%, lower than 93.3% of the control group. Conclusions To sleeve the gastric tube by the guidewire can increase the one-time success rate of grasping the gastric tube, shorten time of grasping the gastric tube and significantly reduce discomfort of patients.

Tumor necrosis factor-related apoptosis-inducing ligand(TRAIL) is a recently identified cytokine of apoptosis that selectively induces the apoptosis of various tumor cells but not normal cells.The members of this family known to date are TRAIL and its five related receptors.TRAIL and its receptors are expressed in ovarian malignant tumor cells and can induce their apoptosis.Many chemotherapeutic agents can enhance TRAIL-induced apoptosis of malignant tumor cells,so TRAIL-induced cytotoxicity holds a powerful promise in the fight against ovarian malignant tumor.

Objective To review advance of gabapentin in treatment of refractory chronic cough, and to provide evidence for its clinical usage and further study.The original articles referring to gabapentin’ s effect on sensory neuropathy such as refractory chronic cough, which were retrieved from CNKI, Wanfang Medical Network, as well as PubMed over the last 15 years, were reviewed.The safety, efficacy and its mechanism of gabapentin were sorted, generalized and analyzed.Gabapentin appears to be effective and safe in the treatment of sensory neuropathic disorder such as refractory chronic cough, and its effective treatment results may come ture through improving central sensitization, which indicates the drug has new clinical application value.Relevant clinical trials investigating its efficacy and safety profile in the treatment of cough are limited and further research are needed.

Objective To investigate the effect of cerebrospinal lfuid replacement combined with vancomycin and dexamethasone intrathecal therapy on biochemical indicators of postoperative intracranial infection, in order to improve the clinical diagnosis and treatment. Methods 70 cases with intracranial infection collected in Third Hospital of Beijing Armed Police Corps from February 2010 to April 2013 were as subject, and randomly divided into two groups. Control group(n=35) were given cerebrospinal lfuid replacement and ceftriaxone intravenously, observation group(n=35) were given cerebrospinal lfuid replacement combined with vancomycin and dexamethasone intrathecal injection. The clinical effects and biochemical indicators were observed after treatment in two groups. Results In control group, the cure rate was 22.86%and total efifciency was 77.14%. In observation group, the cure rate was 37.14% and total efficiency was 91.43%. The differences between two groups were statistically significant (P<0.05). The differences of leukocytes, glucose, protein, intracranial pressure in two groups after treatment were also statistically signiifcant(P<0.05). Conclusion Cerebrospinal lfuid replacement combined with vancomycin and dexamethasone intrathecal injection therapy can increase intracranial infection.

Objective To investigate the cleaning effects of two different methods (modified ultrasound method VS traditional cleaning method) on endoscopy buttons,including suction button and water/gas injection button,and to provide effective clinical evidence for seeking better methods in cleaning endoscopy buttons.Methods A total of 200 endoscopy buttons were randomly divided into two groups:modified ultrasound cleaning group (experimental group) and traditional cleaning group (control group).The combination of multienzyme abluent and ultrasound vibration was applied to the experimental group and multienzyme abluent was used in the control group.ATP bioluminescence detection technology was applied to detect the residual status of organic substance and this parameter was used to evaluate the disinfection status of two different cleaning methods.Results The average organic substance residual was (217.0 ± 29.8) RLU and (42.74 ±8.6)RLU in control group and experimental group,respectively(P ＜0.01).The pass rates were 26％ (26/100) and 87％ (87/100) in in control group and experimental group respectively (P ＜ 0.01).Conclusion Modified ultrasound cleaning method combined with multienzyme abluent and ultrasound vibration has great cleaning effects on endoscopy buttons before disinfection.It can be regarded as a new method for cleaning endoscopy buttons.

Objective:To compare the smile characteristics of the subjects with different types of malocclusion.Methods:1 80 sub-jects with classⅠ(n =60),classⅡ(n =60)and class Ⅲ(n =60)malocclusion at the age of 1 8 to 25 years were included.The smile vidioes and posed smile images of the subjects were acquired.Smile quantitative evaluation indexes were measured and analysed.Re-sults:There were statistical differences in smile indexes and the amount of the gingival exposure in classⅡ subjects between the male and female(P <0.05).There were statistical differences in the quantitative evaluation indexes except the smile symmetry rate among the three groups(P <0.05).The buccal corridor and the gingival exposure ordered from larger to smaller:class Ⅱ >classⅠ >classⅢ,smile index of class Ⅲ was greater than that of class Ⅰ and class Ⅱ,the teeth exposure of class Ⅱ was less than that of class Ⅰand class Ⅲ.Conclusion:Malocclusion types may affect smile characteristics.
