1.Exercise in the treatment of osteoporosis

BACKGROUND:Osteoporosis is a condition resulting in an increased risk of skeletal fractures due to a reduction in bone mass and degradation in bone microstructure. Treatment of osteoporosis usual y involves the application of pharmacological agents as the first choice. Pharmacotherapy alone is not sufficient for the comprehensive management of osteoporosis, as pharmacotherapy has no effect on increasing muscle strength, improving balance ability, and preventing fal s. Exercise, considered as an important non-pharmacologic therapy, plays a considerable role in the prevention of osteoporosis. The importance of therapeutic exercises in the treatment of osteoporosis has been recognized gradual y.
OBJECTIVE:To discuss the effect of exercise in the treatment of osteoporosis combined with some of the latest research literatures.
METHODS:Clinical reports and mechanism researches about effects of different types of exercise on bone strength especial y the treatment of osteoporosis were retrieved from CNKI database and Medline database with the key words of“osteoporosis, bone mineral density, bone strength, treatment, exercise, stress, tai chi, muscle, bone architecture”in both Chinese and English from January 2001 to February 2013. The repetitive researches and atypical reports were eliminated.
RESULTS AND CONCLUSLON:Therapeutic exercises including aerobic exercise, resistance, impact movement, and vibration, which can safely increase bone strength and muscle strength, improve equilibrium function, and prevent fal s and fractures. For spinal deformity patients, appropriate orthotics can improve the security and promote exercise therapy. As with drug therapy, therapeutic exercises are also individualized. Exercise program should be selected under the insurance of good compliance and security. Exercises that involve high strain rates seem to be more effective than others. However, al these benefits are comparatively smal , and should be maintained by continuous exercises.
3.Effect of cisplatin on toxicity and apoptosis of human luteinized granulose cells

Aim To investigate the effect of cisplatin(CDDP) on toxicity and apoptosis of human luteinized granulose cells.Method Human luteinized granulose cells were prepared from oocyte retrieval of fertilization procedure in vitro and treated with various concentrations of cisplatin(0,0.5,1.0,2.5,5.0 mg?L-1).The inhibitory rate of cells was examined by MTT assay.The change of nucleolus stained with Hochest 33258 was observed under fluorescent microscope,and bcl-2 and bax protein expressions were detected by flow cytometry(FCM).Results The results showed that CDDP inhibited the growth of ovarian granulose cells at the concentration of no less than 1 mg?L-1,in which Hochest 33258 staining demonstrated cell apoptosis.The expression of bax protein increased as compared with that of the control(P
4.Prognostic value of serum CA125 level change during chemotherapy post-surgery in patients with advanced epithelial ovarian carcinoma

Objective To investigate the prognostic value of the changes in serum CA125 level during chemotherapy post-surgery in patients with advaneed epithelial ovarian carcinoma.Methods A retrospective analysis was conducted on 142 patients with stage Ⅲ-Ⅳ epithelial ovarian carcinoma who had primary treatment in the Cancer Center of the Sun Yat-sen University during January 1998 to December 2003.The changes in CA125 levels during chemotherapy post-surgery in patients were analyzed.The survival outcomes of patients with various levels of CA125 were studied using Kaplan-Meier method.Multivariate Cox regression model Wag used to aggess the correlations between survival and the change in CA125 level during chemotherapy and other prognostic factors.Results The 3-year overall survival(OS)was 64%,71%,and 64%respectively in patients with different pretreatment CA125 levels(≤500,>500-1500 and>1500 kU/L;P>0.05).The CA125 level was normalized(0-35 kU/L)in 77(54.2%)patients after three cycles of postoperative chemotherapy.It revealed significant differences in 3-year OS(84%vs.42%)and 5-year OS(56%vs.15%)between the patients with normalized and elevated CA125 levels(n=48)after three cycles of chemothempy(P<0.01).Multivariate analysis showed that residual tumor size>1 cm(P<0.01)and elevated CA125 after three-cycle postoperative chemotherapies(P<0.01)were two independent factors related to survival.In the subgroup of optimal cytoreduction(residual tumor size≤1 cm),the 3-year and 5-year OS rate were 88%and 64%for patients with normalized CA125 level after three cycles of chemotherapy respectively,while only 52%and 18%for patients with elevated CA125 level(P<0.01).Similarly,even in the suboptimal cytoreduction group,the 3-year and 5-year OS were also significantly increased for patients with normalized CA125 level after three cycles of chemotherapy post-surgery,as compared with patients with elevated CA125 level(74%vs.33%in 3-year OS,32%vs.13%in 5-year OS;P<0.01).Conclusions CA125 level after three cycles of chemotherapy post-surgery is an independent predictor of survival for advanced ovarian carcinoma.Whatever the patients undergo,optimal or suboptimal cytoreduction,ifthe CA125 becomes normalized after three cycles of chemotherapy,they would have more favorable prognosis than those with elevated CA125 after three cycles of chemotherapy.
5.Study on the Expression of HER2 and TOPOⅡ? in Human Breast Cancer and Relationship between them

Objective To detect the expression of HER2 and TOPOⅡ? in human breast cancer to explore the relationship between them and the potential value in the chemotherapy of breast cancer. Method 24 breast cancer tissues and paired normal tissues were investigated. RT-PCR and imumohistochemical staining were used to detect the expressions of HER2 and TOPOⅡ? at the transcription and protein level respectively. Results HER2 mRNA expressed positively in 29% (7/24) breast cancer tissues vs 4% (1/24) in paired normal tissues. The positive expression of TOPOⅡ? mRNA in breast cancer tissues and paired normal tissues were 92% (22/24) and 21%(5/24) respectively. The result of the immunohistochemical staining showed that the positive rates of HER2 and TOPOⅡ? in breast cancer tissues were 30%(6/20) and 94%(16/17) respectively, which was consistent with RT-PCR. 7.Correlation between TEG Parameters and PAgT and D-Dimer in Patients with ACIH

Objective To investigate the correlation between thrombelastogram (TEG)parameters and plasma D-dimer (D-D) and platelet aggregation rate (PAgT)in patients with acute cerebral infarction (ACI).Methods From January 2015 to Jan-uary 2016,120 patients with ACI (ACI group),and 60 healthy subjects (healthy group)were enrolled in the study.The TEG parameters were detected by thromboelastography,including blood coagulation time (R),blood clotting time (K),rate of blood clotting (αangle),coagulation comprehensive index (CI)and the maximum intensity of blood clotting (MA).Blood coagulation indexes also were detected,including prothrombin time (PT),fibrinogen (FIB,detected by automatic coagulation analyzer),plasma D-D (detected by colloidal gold method)and PAgT (detected by blood aggregation tester).Results R (5.36±0.95 min),K (2.34±0.98 min)and PT (11.88±1.67 s)in the ACI group were shorter than those in the healthy group (6.20±1.28 min,2.87±0.87 min,12.40±1.82 s),(t=4.962,3.547,1.911,all P<0.05).Theαangle (61.65± 7.84)degrees,CI (-0.21±1.11),MA (58.94±6.14 mm),D-D (0.34±0.08 mg/L),PAgT (66.9±6.8)% and FIB (4.41±0.96 g/L)in the ACI group were higher than those in the healthy group [(56.02 ± 6.94)degrees,(-1.50 ± 1.30),(53.86±7.85 mm),(0.24±0.06 mg/L),(55.4±7)%,(2.87±0.88 g/L)],(t=4.714,6.209,5.583,8.550, 10.592,7.039,all P<0.05).In ACI group,D-D,PAgt and the TEG parameters (αangle,CI,MA)were significantly nega-tively correlated (r=-0.481,-0.470,-0.504,-0.488,all P<0.05).D-D,PAgt and the TEG parameters (αangle,CI, MA)were significantly positively correlated (r=0.338,0.395,0.427,0.391,0.436,-0.482,all P<0.05).Conclusion TEG parameters can reflect the severity of ACI to some extent,and they were significantly correlated with plasma D-D and PAgt.
8.Development experiences of integrated healthcare in England:A case in Oxfordshire

An integration of British healthcare and social services has been underway in recent years, and a typical methodology has been formed in the process of regional explorations . Using qualitative research methods of on-site observation and in-depth interviews, the paper studies the Care Programme Approach ( CPA) system of mental health and emergency multidisciplinary teams of Oxfordshire, a typical case of the integrated healthcare system re-forms in the British healthcare system. England’s integrated healthcare reforms have placed primary care and commu-nity health services at the center of the healthcare system and have included social services within the system. The ex-periences of multidisciplinary teams, integrated care pathways, personalized care planning and patient involvement and independence from the reforms are worthy subjects of study for China’s own healthcare delivery system reforms.

This paper summarizes the connotation and value of humanistic education. According to the characteristics of the ethicality and social ability of medicine and comparing to the international medical humanistic education standard, it analyzes the present situation and difficulties we are facing and puts for-ward some strategies and measures such as renewing humanity educational ideas, building two dimensional curriculum system of General and Professional Course, cultivating and strengthening teachers team with both professional ability and humanity equality and confirming principles of humanistic education. These measures can increase the connotation of humanistic education and extend its extension. They have great practical significance towards cultivating outstanding medical talents with high comprehensive quality and healthy personality.
10.Influence of obstructive sleep apnea hypopnea syndrome on hyperglycemia

OBJECTIVE To investigate the relationship of obstructive sleep apnea hypopnea syndrome (OSAHS) and hyperglycemia by analyzing the level of blood-fasting sugar (BFS) in patients with OSAHS. METHODS The data of BFS and AHI in patients with OSAHS (n=300) and the data of BFS of controls (n=225) were collected. The difference of high blood glucose in the OSAHS and the controls.and the difference of AHI between OSAHS patients with high blood glucose and OSAHS patients with normal blood glucose was analyzed. RESULTS There were 97 patients (32.3 %) with high BFS in 300 OSAHS patients, but only 14 cases (6.2 %) with high BFS in 225 controls. There was a significant difference between two groups. The average of AHI in OSAHS patients with high BFS was 67.8/h, and average of AHI in OSAHS patients with normal BFS was 38.4/h. There was a significant difference between the two groups. CONCLUSION There is a close correlation of OSAHS and hyperglycemia.
