1.Expression of miRNAs in peripheral blood plasma of the residents surrounding hot springs with radon

Objective To investigate the expressions of miR-16,miR-106b,miR-449a,miR-34a and let-7g in peripheral blood plasma of the residents surrounding hot springs with radon in Hebei province.Methods A total of 41 randomly selected residents surrounding hot springs with radon were considered as the radon group,and 46 residents with same living habit but without contact with hot springs were considered as control.The miRNAs in the peripheral blood plasma of these two groups were detected with qRT-PCR.Results The levels of miR-16,miR-106b,miR-449a and let-7g in the radon group were significantly higher than those in control group (Z=-2.278,-3.835,-2.719,-2.721,P<0.05).Alterations of these miRNAs were associated with radon exposure (t =2.154,3.711,2.319,2.015,P < 0.05) but had no relationship with age,sex,smoking and drinking factors.No significant difference was observed in the plasma levels of miR-34a between the two groups.Conclusions miR-16,miR-106b,miR-449a and let-7g could be applied as potential biomarkers for radon exposure.
2.The digital artery series and parallel of island flap for repairing degloving injury of the fingertip

Objective To investigate the method and effectiveness of the digital artery series and parallel of island flap for repairing degloved injuries of the fingertip of thumb or adjacent finger Methods Between September 2008 and July 2012,the finger artery series retrograde island flaps tiled in the repair of finger degloving injury in 13 cases,repair of adjacent finger tip degloving injury in 11 cases; 8 cases of degloved injuries of the fingertip were tiled with the digital artery parallel island flap from ulnaris middle finger and radialis ring finger of arteria digitalis communis pedicled which were from the same palm side.The size of skin and soft tissue defect ranged from 2.0 cm × 1.8 cm-7.2 cm ×5.5 cm,gutted flap minimum ranged from 1.1 cm × 1.0cm-1.5 cm × 1.3cm,and the maximum ranged from 3.0 cm × 2.2 cm-5.5 cm × 4.5 cm.The donor sites were repaired with the intermediate split thickness free skin grafts and performed with pressure dressing.Results The group of 32 cases,in 1 case the flap vasospasm occurred in operation of free process,the symptoms disappeared after local application of papaverine and hot compress ; 1 case of flap occurred disturbance of blood circulation after operation because of tight suturing for pedicle,the symptoms relieved after removing the stitches at intervals; 2 cases blisters appeared after operation,disappeared after a week.All skin flaps were survived,incision and skin graft donor sites healed by first intention.Twenty patients were followed-up from 6 to 12 months after operation.All flaps presented the satisfactory appearance and texture,recovered protective feeling.At last follow-up,the two-point discrimination was 7 to 10mm,and the flexion and extension function of wounded fingers recovered to normal.According to the functional assessment criteria of upper limb formulated by the Hand Surgery Branch of Chinese Medical Association,there were excellent in 19 cases,good in 10 cases,and moderate in 3 cases.No significant loss to the donor shape and function.Conclusion The surgery by adopted the digital artery series or parallel of island flap for repairing degloved injuries of the fingertip of thumb or adjacent finger,not only has the advantages of simpleness,safety and reliability,but also can satisfy the patients who aren't willing to accept or because of physical reasons can't accept the treatment of abdominal skin tube or nail flap from hallux toe,which performs in both high-end and low level hospitals,and deserves of general application.
3.Investigation on Active Components inTong-MaiGranules Reflecting Its Effect of Activating Blood Circulation Based on Serum Pharmacochemistry/Serum Pharmacology

Tong-Mai(TM) granules were composed ofRadix Salviae Miltiorrhizae(Danshen),Rhizoma Chuanxiong(Chuanxiong) andRadix Puerariae(Gegen). It had the effect of activating blood circulation. It had been used to treat ischemic cardio-cerebrovascular diseases in the clinical practice. This research combined serum pharmacochemistry and serum pharmaocology to study the material basis of active components in TM granules. After single or multiple intragastric administrations of TM granules, serum blood samples of rats were collected at different time points. LC-MS/MS method was developed to analyze chemical components of TM in blood serum samples. The cardiomyocyte hypoxia / reoxygenation (H/R) model was used in the evaluation of cardiomyocyte protection by TM. The correlation analysis was also conducted between serum concentration of TM and cardiomyocyte activity. The results showed that 8 components of pueraria flavonoid, 5 components of salvianolic acids and 2 components fromGegen were promptly absorbed and reached their highest concentrations at 5 or 30 min after administration. After 3 times of medication, the serum concentration was obviously higher compared to single medication. The drug-serum of TM showed significant protective effect on the cardiomyocyte H/R injury with dose-effect relationship. Daidzein, lithospermic acid, salvianolic acid A, salvianic acid A and rosmarinic acid presented as the most correlated components linked to the effect of activating blood circulation by TM. The serum pharmacochemistry / serum pharmacology related studies provided references for the verification of material basis of active components in compound Chinese medicine.
4.Laparoscopic versus open radical resection for hilar cholangiocarcinoma

Objective:To compare the treatment outcomes between laparoscopic versus open radical resection for hilar cholangiocarcinoma (HCCA).Methods:From January 2017 to January 2020, the clinical data of 34 patients who underwent radical resection for HCCA were retrospectively collected and analyzed. These patients were divided into the laparotomy group ( n=17) and the laparoscopic group ( n=17) based on the operation they received. Clinical data including perioperative outcomes, oral re-intake time, first out-of-bed activity time, drainage tube removal time, postoperative hospital stay, 30-day and 90-day mortality rates were compared between groups. Results:Of 34 patients who underwent radical for HCCA in the study, there were 16 males and 18 females, aged (64.3±1.7) years. The mean operation time of the laparotomy group was significantly less than those in the laparoscopic group [(436.2±33.4) vs (522.1±24.0) min, P<0.05]. The 2 groups showed comparable results in extent of operation, intraoperative bleeding, incidences of portal vein reconstruction, yields of lymph nodes, and tumor diameter. The laparoscopic group showed advantage trends over the laparotomy group in incidences oral re-intake time [(4.7±0.3) vs (4.6±0.3) days], first out-of-bed activity time [(2.9±0.4) vs (2.2±0.3) days], drainage tube removal time [(12.7±1.3) vs (11.1±1.0) days] and postoperative hospital stay [(18.3±1.7) vs (15.8±1.3) days], but the differences failed to reach statistical significance ( P>0.05). Conclusion:Compared with open surgery, laparoscopic radical resection of HCCA in properly selected patients, was safe and feasible. There were comparable clinical outcomes.
5.Determination of Ethyl Pyruvate by GC

Objective:To establish a method for the determination of ethyl pyruvate by GC. Methods:A Varian CP7502 capillary column(25 m × 0. 25 mm,0. 25 μm)was used. The carrier gas was nitrogen with the flow rate of 30 ml·min-1 ,the gas was hydrogen with the flow rate of 40 ml·min -1 and the oxidant gas was air with the flow rate of 400 ml·min-1 . The detector was an FID and the inlet temperature was 210℃ . The temperature program was as follows:the initial column temperature was 40℃, and then risen to 200℃ with a rate of 15 ℃·min -1 . The split ratio was 100 ∶1 and the injection volume was 1μl. Results:Ethyl pyruvate had a good linear relationship within the range of 0. 025 6- 8. 192 8 mg·ml -1 ,and r was 0. 999 8. The average recovery was99. 99% and RSD was 1. 38%(n = 9). Conclusion:The method is accurate,simple and rapid with good stability and high sensitivity,which can be used to determine the content of ethyl pyruvate.
6.The effect of hypoxia on uPA activity in human tongue carcinoma cell line Tca8113

Objective:To observe the activity of urokinase-type plasminogen activator (uPA) in human tongue squamous cell carcinoma cell line Tca8113 under hypoxia in vitro. Methods:According to the different time of hypoxia, the test was divided into four groups as following: (1) 0 h; (2) 6 h;(3)12 h;(4)24 h. The expression of uPA was examined using immunochemistry. The expression of uPA mRNA was examined using in situ hybridization (ISH). The results of ISH staining of uPA mRNA were analysed using Spot and IPP image analysis system.Results:The uPA expression was found in cytoplasm and cytomembrane of Tca8113 cells.uPA mRNA expression was found in cytoplasm of the cells.The staining intensity of uPA mRNA in the groups of (1),(2),(3) and (4) was 0.175 251?(0.013) 863,0.263 191?(0.000 680),0.406 745?0.014 016 and 0.478 919?0.018 998 respectively(P
7. An analysis of medical X-ray examination frequency in ten hospitals in Tianjin, China

Objective:
To investigate the usage frequency of radiological diagnosis devices in ten hospitals in Tianjin, China, and to provide a basis for the survey of medical radiation frequency in Tianjin.
Methods:
Ten hospitals from the ten districts in Tianjin were enrolled as subjects by a convenient sampling method. A survey was conducted to assess the general information and radiological diagnosis device information and usage frequency in those hospitals in 2016. The radiological examination frequency in Tianjin was estimated.
Results:
A total of 610 458 patients received radiological examinations in the sampled hospitals in 2016. In those patients, 371 882 received X-ray examinations for imaging and 238 576 for computed tomography (CT) ; there were slightly more female patients than male patients, suggesting a relatively equal gender distribution; patients older than 40 years accounted for 65.53%, which was the highest among all the age groups. Different types of radiological diagnosis devices were mostly used in tertiary and secondary hospitals. In Tianjin, the estimated frequency of X-ray examination for imaging and CT scanning was 451 and 188 per thousand people, respectively, in 2016.
Conclusion
The frequency of radiological diagnosis is relatively high in the ten hospitals in Tianjin. The investigation of medical radiation in Tianjin needs to be improved.
8.Investigation on the frequency of medical exposure of X-ray diagnostic equipment in Tianjin

Objective To assess the frequency of medical exposure from X-ray diagnostic equipment in Tianjin,and provide recommendations for regulating medical radiation management and rational allocation and use of medical and health resources.Methods Using the census method,questionnaries were issued in the form of an administrative document to investigate the basic information on X-ray diagnostic equipment and medical procedure frequency in 687 hospitals (excluding military and armed police hospitals) in 2018.Mean values of frequencies were derived by dividing the number of procedures for each type of X-ray equipment by the total number of permanent residents.Results There were 1 562 pieces of X-ray diagnostic equipment (including interventional radiology),with an average of 1.00 unit per 10 000 population.There were 4 782 radiation workers engaged with X-ray diagnostic work (including interventional radiology),averaged on 3.06 workers per unit,with the highest of 5.25 workers per unit in tertiary hospitals and the lowest of 1.18 in ungraded hospitals.The city's X-ray diagnostic medical exposure came from 8 905 970 medical procedures,including 54.81% for tertiary hospitals,18.00% for secondary hospitals,16.21% for primary hospital,and 10.98% for ungraded hospitals.X-ray photography contributed 71.30% with annual frequency of 407.84 examinations per 1 000 population,CT scans 21.81% with annual frequency of 124.74 examinations per 1 000 population,interventional radiology 1.04%,with annual frequency of 5.98 examinations per 1 000 population,and other types of X-ray examinations 5.85% with annual frequency of 33.47 examinations per 1 000 population.Conclusions The configuration of X-ray diagnostic equipment in Tianjin is basically reasonable.The frequency of X-ray diagnostic medical exposure is increasing year by year.The rationality of the distribution of X-ray diagnostic examinations needs to be further improved.To reduce dose to population,for the public health and safety is important issues in the future of radiation protection.
9.Investigation and analysis of CT dose to paediatric patients in Tianjin

Objective:To study the level of CT dose to paediatric patients in Tianjin, and to provide basic data for establishing the diagnostic reference levels for paediatric patients from CT examinations in Tianjin.Methods:In 2022, a general survey was carried out of the CT doses to the head, chest, abdomen and pelvis of the scanned paediatric patients in two tertiary pediatric hospitals and seven tertiary general hospitals in Tianjin. The scanned pediatric patients were divided into four age groups in terms of the age of -1, -5, -10 and -15 years, and 20 patients were investigated in each age group in each hospital. The basic information were collected on the scanned patients, CT scanning parameters, volume CT dose index (CTDI vol), dose length product (DLP), and the differences in CTDI vol and DLP on the same site among different age groups and different types of hospitals were analyzed. Results:There were significant differences in CTDI vol and DLP between different age groups at the same site (head, chest, abdominal and pelvic, CTDI vol:χ2=296.51, 193.82, 291.72, P<0.001; DLP: χ2=291.22, 263.63, 344.97, P<0.001). There were significant differences in CTDI vol and DLP among different types of hospitals on the same site (head, chest, abdominal and pelvic, CTDI vol:Z=-13.13, -7.57, -15.27, P<0.001; DLP: Z=-9.07, -6.15, -11.57, P < 0.001). Conclusions:The 75 th percentile values of CTDI vol and DLP for pediatric head, chest, abdomen and pelvis CT scanning in tertiary hospitals in Tianjin are at a good level, and the dose on some examination sites are relatively high. Further optimization of pediatric CT scanning procedures is necessary.
10.Repair of digit soft tissue defect with dorsal branch of proper palmar digital artery island flap

Objective:To investigate the surgical technique and clinical effect of the island flap of dorsal branch of proper palmar digital artery in repair of the soft tissue defect of digits.Methods:From March 2013 to March 2021, 22 cases of digit soft tissue defects were repaired with dorsal branch of proper palmar digital artery island flap. The digit defects involved: 9 thumbs, 5 index fingers, 3 middle fingers, 3 ring fingers and 2 little fingers. The repair of defects covered 8 digit-tips, 7 pulps and 7 dorsal and nail beds. The defected area of soft tissue was 0.8 cm×0.5 cm-1.5 cm×8.0 cm, and the size of flap was 1.0 cm×0.7 cm-1.8 cm×1.0 cm. The donor site in 6 cases was closed directly. The other 16 cases were covered with medium thickness skin graft and pressurised bandaging. The follow-up reviews were carried out via the outpatient clinic visit, telephone or WeChat interview. Results:After operation, 1 flap had cyanosis due to a tight suture and it was relieved after the removal of intermittent suture; Tension blisters appeared in 2 cases and disappeared after 1 week; One case had necrosis at distal flap and healed after dressing change. Other flaps survived successfully and the incision and donor site healed in the first stage. All patients were entered to 6 to 18(mean 10) months of follow-up. At the final follow-up, the appearance and texture of the flaps were good and the protective sensation was restored. The flexion and extension function of the affected digit was normal with the TPD at 7-11 mm. The original shape and function of the digit body were basically reconstructed, except the failure in reconstruction of the special structure of digit body, such as nail, finger pulp thread and fine sensation. According to the Evaluation Standard of Upper Limb Function of Chinese Hand Surgery Society, 11 cases were in excellent, 9 in good and 2 in fair. The excellent and good rate was 91%. The function at donor sites was not affected.Conclusion:Repair of digit soft tissue defect with dorsal branch of proper palmar digital artery island flap is easy to operate, and with a low risk, high success rate and satisfactory curative effect.
