1.CT Diagnosis and Differential Diagnosis of Spherical Pneumonia

Objective To analyze CT characteristics of spherical pneumonia in order to differentiate it from other lesions especially lung cancer. Methods The CT and X-ray film of 25 cases diagnosed clinically as spherical pneumonia were analysed retrospectively.Results CT features of 25 patients were summarized as follow : ⑴Lesions were usually located in the posterior lung fields.(2)The central part of the lesions showed high density, while periphery part of lesion were low in density and blurred. (3)The bilateral margin of lesions were straight, and the lesions appeared square or triangular in shape. ⑷Margins of the lesions may be irregular and blurred. ⑸There were marked pleural reaction around the lesions and some times pleura diffusely thickening occurred. ⑹Vascular marking around the lesions were exaggerated increase and thickened, nevertheless they were not stiff. Conclusion CT is valuable in diagnosing and differential diagnosing spherical pneumonia.
2.Relationship of TS mRNA Expression with Therapeutic Efficacy of Pemetrexed in the Treatment of Advanced Breast Cancer

OBJECTIVE:To explore the potential correlation between clinical efficacy of pemetrexed in the treatment of ad-vanced breast cancer and TS mRNA expression. METHODS:Patients with advanced breast cancer were recruited in our study. Three cycles of pemetrexed was given to these patients,TS mRNA expression of their tumor tissues were detected. The relationship of ther-apeutic efficacies of different chemotherapy with TS mRNA expression were observed and compared during the study period. RE-SULTS:58 patients received pemetrexed chemotherapy and therapeutic efficacies of them were evaluated. One patient (1.7%) had complete response,19 patients(32.8%)had partial response,31 patients(53.4%)had stable response and 7 patients(12.1%)has progressive disease. Objective response rate was 34.5%,and disease control rate was 87.9%. TS mRNA expression level of 55 pa-tients with completed TS mRNA expression analysis varied from 0.04 to 5.11,with a median of 0.62. Mann-Whitney rank test showed that,responders had lower TS mRNA expression than nonsponders at all visits,there was statistical significance (P<0.05). Chi-square test showed that patients with low expression level of baseline(before chemotherapy)TS mRNA had significantly higher objective response rate(63.3%)than those with high expression(36.7%),there was statistical significance(P<0.05). CON-CLUSIONS:TS mRNA expression level is related to therapeutic efficacy of pemetrexed,which may be useful for predicting thera-peutic efficacy of pemetrexed therapy in patients with advanced breast cancer.
3.Expression of miR-206 in peripheral blood of hemangioma children and its correlation with the efficacy of propranolol

Objective:To investigate the expression of microRNA(miR)-206 in the peripheral serum of hemangioma children and its correlation with curative effect of propranolol treatment.Methods:The expression of miR-206 in serum of 45 hemangiomas children treated with propranolol treatment was detected.The children were divided into the high expression group( n=19) and the low expression group( n=26) according to the mean value of miR-206, and the clinical characteristics of the two groups were compared; The children were divided into the good efficacy group( n=29) and general efficacy group( n=16) according to the efficacy after 6 months of treatment.Logistic regression was used to analyze whether miR-206 was an influencing factor of efficacy, and ROC curve was used to evaluate the predictive value of miR-206. Results:The proportion of children <6 months, in hyperplasia stage and with tumor area ≥10 cm 2 in the high expression group was significantly lower than that in the low expression group( χ2=4.664, 7.207, 8.927, P=0.031, 0.007, 0.012, respectively). The miR-206 level after treatment(3.25±0.64) was significantly higher than that before treatment(2.12±0.41, t=9.973, P<0.05). After six months of propranolol treatment, the proportion of high expression of miR-206 before treatment in the good efficacy group(24.1%) was significantly lower than that in the general efficacy group(75.0%, χ2=10.934, P=0.001). High expression of miR-206 before treatment was a risk factor for poor curative effect of propranolol in hemangioma children[ CI(95% CI)=6.423(1.436~28.731), P=0.015]; ROC curve analysis showed that the area under ROC curve of miR-206 for predicting the efficacy of propranolol was 0.798, the sensitivity was 0.83, and the specificity was 0.75. Conclusion:The expression of serum miR-206 was correlated with age, proliferative stage and tumor area and may be one of the important indicators for predicting propranolol effect in children with hemangioma.
4. Death of a neonate born to a critically ill mother with COVID-19: a case report

We report a critically ill pregnant woman in the third trimester with severe pneumonia due to COVID-19 who presented to Xiaolan People's Hospital of Zhongshan in February 2020. The 32-year-old patient was admitted at 35 +2 gestational weeks with a 4-day history of a sore throat and a fever for three hours. The patient had been to Xiaogan City, Hubei Province, and the symptoms occurred during a period of self-isolation after back home. The condition of the patient deteriorated rapidly, with left-sided chest and back pain, shortness of breath, dizziness, progressing to respiratory failure and septic shock 7 hours after her admission. In view of her critical condition and a history of two previous cesarean sections, an emergency cesarean section was performed. Blood gas analysis of the mother before the operation suggested respiratory failure, respiratory acidosis, and metabolic acidosis. During the operation, a baby boy was born. The Apgar score of the boy, birth weight of 2 700 g, was one at 1, 5 and 10 minutes despite the resuscitation efforts. The neonate died after withdrawing treatment. The patient was treated with tracheal intubation ventilator and other supportive treatments after the operation. The result of the new coronavirus nucleic acid test, taken on admission, but which was reported after delivery, was positive. The patient was transferred to the designated hospital for further treatment and was recovering with the withdrawal of extracorporeal membrane oxygenation and ventilation support at 26 and 36 days after surgery, respectively.
