1.Complications and prevention for the peri-operative period of 565 hepatic cancer arterial chemotherapy embolism

Objective To explore the complications and prevention for the peri-operative period of hepatic cancer arterial chemotherapy embolism (TACE). Methods 565 hepatic cancer sufferers' medical record through TACE therapy were analyzed retrospectively. Results The complications could be divided into three types according to complications genesic date:during the puncture and intubation,23 local hematomas, 14 arterial intima damnification; during the TACE operation period, 112 bellyache,242 spew,3 chough; after the TACE operation, 528 fever, 264 bellyache ,3 liver abscess,2 hepatorenal complication. The research results indicated that the most complications could be cured or lightened. Only 5 sufferers died within two weeks. Conclusion There were many kinds of complications during TACE operations, and the sufferers could get well and the complications could be effectively reduced if the nurse got along abortively during the whole operation.
2.Clinical study on continuous oral dosing of etopside combined with prednisone in the treatment of elderly patients with unspecific peripheral T-cell lymphoma

Objective To investigate the efficacy and toxicity of etoposide combined with prednisone (EtoP regimen) for elderly patients with unspecific peripheral T-cell lymphoma (PTCL-U) compared with the CHOP regimen.Methods 23 elderly PTCL-U patients were randomly assigned to EtoP group (n =12) or CHOP group (n =11).The curative effects and the adverse reactions were analyzed and compared between the two groups by Log-rank test and x2 test.Results The overall response (OR) rate was 66.67 % (8/12) in EtoP group and 63.64 % (7/11) in CHOP group.There was no statistically significant difference between the two groups(x2 =0.023,P =0.879).The progression-free survival (PFS) was 7.55 months in EtoP group and 4.38 months in CHOP group,statistically significant difference were observed between the two groups (x2 =23.000,P =0.011).The overall survival (OS) was 15.02 months in EtoP group and 12.26 months in CHOP group,no statistically significant difference was observed between the two groups (x 2 =14.985,P =0.597).The main side effects were bone marrow depression and digestive tract toxicity,no hematology toxicity and digestive tract toxicity of grade Ⅲ-Ⅳ occurred in EtoP group.Of those in CHOP group were 36.36 % and 27.27 %.Conclusion For elderly PTCL-U patients,EtoP regimen is an effective and well tolerated therapeutic schedule.

Objective To study the effects of Rukangping Capsules(RC) on trace elements in liver and breast of mammary gland hyperplasia rats.Methods SD rats were divided into blank group,model group,RC group,Xiaoyao pills group and tamoxifen group.The changes of content of copper(Cu),zinc(Zn),iron(Fe),manganese(Mn),selenium(Se) in liver and breast were observed.Results Compared with the model group,the hepatic and mammary trace element content were improved in the three medication groups and the difference being significant.Conclusion RC can regulate the trace elements content in liver and breast of mammary gland hyperplasia rats.

AIM: To study the effects of recombinant soluble MHC class Ⅰ chain-related protein A (sMICA) on the cytotoxicity, secretion of IFN-γ, proliferation and apoptosis of peripheral NK cells. METHODS: After NK cells were cocultured with recombinant soluble MICA proteins overnight, the cytotoxicity of NK cell on target cells was detected by flow cytometry. The supemant was collected to determine the concentration of IFN-γ by ELISA. The proliferation of NK cells to sMICA was detected by MTS/PMS. NK cells were labeled with annexin V and PI to analyze their apoptosis. RESULTS: Soluble MICA inhibited the cytotoxicity of NK cells and down-regulated the secretion of IFN-γ, but it showed no effects on the proliferation and apoptosis of freshly isolated peripheral NK cells. CONCLUSION: The soluble MICA shedding from tumor cells could be a pathway of cancer immune evasion by down-regulating the biologic activities of NK cells.
6.Feasibility, efficacy and cosmetic effect of three-dimensional conformal external beam partial breast irradiation for the selected early stage breast cancer patients after breast-conserving surgery

ObjectiveTo explore the feasibility,efficacy and cosmetic effect of three-dimensional conformal external beam partial breast irradiation (EB-PBI) after breast-conserving surgery for the selected Chinese early stage breast cancer patients.MethodsFrom June 2003 to December 2010,Forty-four early stage breast cancer patients underwent EB-PBI after breast-conserving surgery.Twenty patients had CT simulation scan in moderate deep inspiration breathing hold,and twenty-four patients in free breathing.EB-PBI was planned and delivered by three-dimensional conformal radiotherapy (3DCRT)with four noncoplanar beams.The prescribed dose was 3.40 Gy per fraction in thirty-nine patients and 3.85 Gy per fraction in five patients,twice per day at an interval of at least six hours,in five consecutive days.Results The number of patients with follow up time of 2,3 and 5 years were 39,31 and 16.Grade 1 acute radiationinduced dermatitis was observed in 17 patients (39%) at three months.Cosmesis was good or excellent in all cases at six months after radiotherapy and in 95% cases at two years after radiotherapy.The 2-,3-and 5-year local control rates were 100%,99% and 94%,respectively.The 2-,3-,and 5-year survival rates were all 100% and no metastases occurred.Conclusions EB-PBI delivered by 3DCRT is feasible for selected Chinese early stage breast cancer patients after breast-conserving surgery.The cosmetic effect,local control rate and long-term survival rate are satisfactory,and acute radiation toxicity is very low.

ObjectiveTo investigate the relationship between complement-3(C3) and progression of early cerebral infarction.Methods168 consecutive patients with acute cerebral infarction within the first 24 hours from onset were recruited.Fasting venous blood sample was drawn on the morning of the second or the third day after admission.C3 and other clinical chemistry variables were measured.The relationship of C3 and progressing stroke(PS) was analyzed by Logistic regression.Results68 cases(40.48%) were considered as PS.Level of serum C3 in PS patients was 1.28±0.20 g/L higher than in those with non-PS(1.16±0.25 g/L,P=(0.000)).A higher C3 accompanied a higher tendency of prevalence of PS(P=0.001).Logistic regression analysis showed that C3 was significantly correlated with PS(OR for each 1.0 g/L=7.27;95% CI=1.52~36.39; P=0.013).ConclusionElevated serum C3 may play an important role in the prevalence of PS.
8.Clinical and pathological analysis of 19 patients with superficial CD34-positive fibroblastic tumor

Objective:To investigate clinical, histopathological, molecular genetic characteristics, and immunophenotypes of superficial CD34-positive fibroblastic tumor (SCPFT) .Methods:The pathological data were collected from 19 cases of SCPFT from January 2015 to July 2022 in the First Affiliated Hospital of Nanjing Medical University, and analyzed retrospectively.Results:In total, 19 cases of SCPET were enrolled, including 8 males and 11 females. Their age at onset ranged from 16 to 67 years, and the disease duration ranged from 3 months to 3 years. Tumors occurred in the thigh (6 cases), back (4 cases), shoulder (3 cases), abdominal wall (2 cases), popliteal fossa (1 case), lower leg (1 case), foot (1 case) and forehead (1 case). The tumor lengths were 0.8 - 4 (2.36 ± 0.76) cm, with relatively clear borders. Surgical excision was performed in all the 19 cases, and 1 case experienced recurrence. Histopathological examination showed that tumors were located in the dermis and subcutis, and infiltrated the peripheral fat tissues or the dermis; tumors were composed of abundant spindle cells and polygonal cells arranged in interlacing fascicles; cells with nuclear pleomorphism were observed in all cases, and were characterized by large, hyperchromatic and irregular nuclei, intranuclear pseudoinclusions, and prominent nucleoli; the mitotic figure was rare, and tumor necrosis was not found; inflammatory cell infiltration to different extents was observed in the tumor stroma, and lymphocyte sheaths were observed around the tumors in 2 cases. Immunohistochemical staining showed strong diffuse expression of CD34 in the tumor cells in all the 19 cases, the Ki-67 proliferation index was < 10% in 18 cases, and CKpan (2/12), desmin (2/15), and smooth muscle actin (1/19) were focally expressed. Fluorescence in situ hybridization for the detection of PRDM10 gene rearrangements was performed in 3 cases, and identified PRDM10 rearrangements in 2 cases, including 1 case of SCPFT accompanied by lymphocyte sheaths without EWSR1 rearrangements. Conclusion:SCPFT is an intermediate fibroblastic tumor with no characteristic clinical manifestations, often accompanied by PRDM10 rearrangements, and its diagnosis depends on histopathological patterns.

OBJECTIVETo study the high risk factors of the third level of lymphatic metastasis in breast cancer patients to guide clinical practice.
METHODSThe clinical data of 746 breast cancer patients (all female, aged from 33 to 80 years with a median of 46 years) received radical or modified mastectomy between 2001 and 2011 was analyzed retrospectively. Eleven individual variables were selected to investigate high risk factors of the third level of lymphatic metastasis in different conditions.
RESULTSAxillary nodes metastasis status (OR = 4.541, 95%CI:3.569-5.776), tumor site (OR = 1.437, 95%CI:1.029-2.007), external nodes involved (OR = 3.809, 95%CI:1.683-8.618) and estrogen receptor (OR = 0.740, 95%CI:0.569-0.964) were high risk factors of the third level of lymphatic metastasis. Further analysis found that it is prone to happen a metastasis, especially when the tumor with a size over 5 cm and located at the lateral quadrant. Negative estrogen receptor was a risk factor of the third level lymphatic metastasis along with the tumor stage.
CONCLUSIONFor preoperative tumor biopsy shows Negative estrogen receptor of tumor stage T3 and over stage T3 when considering suspicious lymph node metastasis or external tissues metastasis intraoperatively should take in account into third level axillary lymph node dissection actively.
