1.Relationship between benign prostatic hyperplasia and urine albumin excretion

Objective To investigate the relationship between benign prostatic hyperplasia (BPH) and urine albumin excretion.Methods A total of 258 male inpatients aged 60 to 70 years were enrolled in this study.According to prostate volume (PV) measured by transrectal ultrasonography,patients were divided into BPH group(PV≥20 ml) and NBPH(non benign prostatic hyperplasia) group(PV<20 ml).Urine albumin and urine creatinine concentrations were measured and urine albumin/creatinine ratio(UACR) was calculated.Fasting blood glucose(FBG) and fasting insulin(FINS) also were measured,insulin resistance index(HOMA-IR) was calculated based on the formula HOMA-IR=ln(FBG × FINS/22.5).The correlation between PV and other indicators were analyzed by Pearson correlation analysis.The risk factors for BPH were analyzed by logistic regression analysis.Results UACR was significantly higher in BPH group than in NBPH group[(163.6± 69.8) mg/g vs.(73.3 ± 55.6) mg/g,t =4.81,P< 0.01],and hypertension duration was longer in BPH group than in NBPH group [(9.2±6.6)years vs.(6.0±3.6)years,t=2.27,P<0.05].The prevalence rate of BPH and PV were increased along with increasing of urine albumin excretion(x2=4.58,F=4.85,respectively,P<0.05 or 0.01).Prostate volume was positively correlated with UACR,HOMA-IR,hypertension duration,body mass index(r=0.248,0.229,0.183,0.157,respectively,all P < 0.05).Logistic regression analysis indicated that UACR,HOMA-IR and hypertension duration were the risk factors for BPH(OR=16.348,1.040,0.166,respectively,all P <0.05).Conclusions BPH is correlated with UACR,which indicates that there is a close relationship between atherosclerosis and BPH.Insulin resistance probably plays an important role in the progression of arteriosclerosis and BPH.
2.Analysis of the diagnostic and therapeutic status of elderly benign prostatic hyperplasia

Objective To evaluate the diagnostic and therapeutic status of elderly benign prostatic hyperplasia (BPH).Methods Totally 188 male elderly in outpatient department from January 2010 to March 2012 were enrolled in this study.Medical history was inquired in detail,and prostatic volumes (PV)were measured by abdominal ultrasound,PV≥ 20ml was diagnosed as BPH.Lower urinary tract symptoms (LUTS) was evaluated by international prostate symptom score (IPSS),prostate specific antigen (PSA) was measured.Age≥62 years,PSA≥1.6 μg/L and PV≥31 ml were considered as higher risk of clinical progression of BPH.We investigated the rate of prevalence,awareness and treatment of BPH.PV,IPSS and PSA were compared among different age groups by ANOVA.Logistic regression analysis was made to show the risk factors of BPH.Results Among 188 male elderly,the prevalence rate of BPH was 48.4%,and the total rate of awareness was 41.5%.The rate of awareness resulting from seeing doctor due to LUTS was 10.6%.PV,IPSS and PSA were increased with ageing.Risk factors contributing to BPH were age,hypertension and diabetes.The treatment rate with drugs of 5a-reductase inhibitor was 47.3 %,however 10 subjects had not been given drugs treatment among 46 patients with higher risk of clinical progression of BPH.Conclusions The prevalence rate of BPH is increased with ageing,the proportion of seeing a doctor with passion is lower because the male elderly have not paid enough attention to LUTS.And doctors have not given drugs intervention to patients with clinical progression BPH in time.We should launch propaganda and education among the elderly patients with hypertension and diabetes and take comprehensive treatment.
3.Effect of Shenmai injection on cytokines and ESAT-6/CFP-10 levels in patients with COPD with pulmonary tuberculosis

Objective To investigate the effect of Shenmai injection on cytokines and ESAT-6/CFP-10 levels in patients with Chronic obstructive pulmonary disease(COPD)with pulmonary tuberculosis.Methods158 patients with COPD with pulmonary tuberculosis from the Department of Respiratory in our hospital were selected and divided into 2 groups, 79 cases in the control group treated with routine treatment, 79 cases in the experiment group treated with Shenmai injection on the basis of the control group, levels of neurotransmitters in the brain, serum ESAT-6 protein, CFP-10 protein, IFN-γ levels, cytokine levels, peripheral blood immune cell levels, and the clinical effect and the focus absorption efficiency were compared after the treatment.ResultsThe clinical total effective rate in the control group(84.81%)was lower than that in the experiment group(94.94%), with significant difference (P<0.05);the focus absorption efficiency in the control group(87.33%)was lower than that in the experiment group(96.21%), with significant difference (P<0.05);compared with the control group, serum ESAT-6 protein、 CFP-10 protein、IFN-γ levels were lower after treatment in the experiment group, the serum levels of TNF-α、IL-6、sIL-2R were lower, peripheral blood levels of NK cells, CD4+T lymphocytes and CD4+/CD8+ were higher, peripheral blood level of CD8+T lymphocytes was lower, with significant difference (P<0.05).ConclusionShenmai injection can significantly reduce the serum levels of TNF-α、IL-6、sIL-2R and ESAT-6/CFP-10 in patients with COPD with pulmonary tuberculosis, improve cellular immune function, promote the absorption of lesions effectively,and improve clinical efficacy.
4.Effect of general nursing intervention on the mental state of patients with breast cancer undergoing chemotherapy

Objective To investigate the effect of general nursing intervention on the mental state of the patients with breast cancer undergoing chemotherapy.Methods Forty-eight breast cancer patients undergoing postoperative chemotherapy received nursing general interventions including healthy education,psychological nursing care,pantosomatous,relaxation therapy and rehabilitative exercise. All patients were evaluated by self-rating depression scale (SDS) and depression status inventory (DSI) before and after the general intervention.Result The scores of SDS and DSI after intervention were significantly lower than those before intervention (bothP<0.05).Conclusion The general nursing intervention could significantly improve patients’ psychological state and life quality,promote rehabilitation and improve their quality of life.
5.Efficacy Observation of Qianliexin Combined with Tamsulosin Hydrochloride and Finasteride in the Treatment of Elderly Benign Prostatic Hyperplasia

OBJECTIVE:To observe the clinical efficacy of qianliexin combined with tamsulosin hydrochloride and finasteride in the treatment of elderly benign prostatic hyperplasia.METHODS:Ninety-four patients diagnosed as benign prostatic hyperplasia in our hospital during May.2012-Oct.2014 were selected and divided into observation group (48 cases) and control group (46 cases) according to even and odd admission number.Both groups received Finasteride tablet 5 mg,po,qd.Basedon this,control group was additionally given Tamsulosin hydrochloride sustained-release capsules 0.2 mg,po,qd;observation group was additionally given Qianliexin capsules 2.5 g,po,tid,on the basis of control group.Clinical efficacies of 2 groups were observed as well as IPSS,BS,IIEF-5 score,ultrasonic measurement indexes and TCM syndrome score before and after treatment.RESULTS:Clinical total response rate of observation group was 93.75%,which was significantly higher than 76.09% of control group,with statistical significance (P<0.05).Before treatment,there was no statistical significance in IPSS,BS,IIEF-5 score,ultrasonic measurement indexes and TCM syndrome score between 2 groups (P>0.05).After treatment,IPSS and BS score,prostate volume (PV),residual urine(RU),each item score and total score of TCM syndrome were significantly decreased in 2 groups,while IIEF-5 score and Qmax were increased significantly;the observation group was significantly better than the control group,with statistical significance (P<0.05).CONCLUSIONS:Qianliexin combined with tamsulosin hydrochloride and finasteride shows significant therapeutic effi cacy for elderly benign prostatic hyperplasia,and is helpful to improve prostate symptoms and TCM syndromes,reduce PV and RU,and improve sexual function.
6.Efficacy Observation of Extracorporeal Shock Wave Lithotripsy Combined Use Paishi Decoction in the Treatment of Kidney Stone

OBJECTIVE:To observe clinical efficacy of extracorporeal shock wave lithotripsy combined with Paishi decoction in the treatment of kidney stone. METHODS:160 patients with kidney stone were selected and randomly divided into observation group and control group,with 80 cases in each group. Control group only received HB-ESWL-VG extracorporeal shock wave litho-tripsy;observation group additionally received Paishi decoction 300 ml,tid,for consecutive 1 week,on the basis of control group. Clinical efficacy was observed in 2 groups;the levels of serum inflammatory factor IL-2,IL-6,TNF-α,T lymphocyte subset CD3+,CD4+,CD8+ and CD4+/CD8+,erythrocyte immune function indicator C3b receptor rosette(C3bRR),immunity complex ro-sette(ICR)and tumor erythrocyte rosette(TER)level were detected in 2 groups before and after treatment. RESULTS:The effec-tive rate of observation group(98.75%)was significantly higher than that of control group(91.25%),with statistical significance (P<0.05). After treatment,the levels of IL-2,IL-6 and TNF-α in observation group was significantly lower than in control group, the levels of CD3+,CD4+,CD4+/CD8+,C3bRR,ICR and TER were significantly higher than control group,with statistical signifi-cance(P<0.05). Comparsion of CD8+between 2 groups was not statistically significant(P>0.05). CONCLUSIONS:The extracor-poreal shock wave lithotripsy combined with Paishi decoction can relieve inflammation reaction,enhance immune function and im-prove therapeutic efficacy.
7.Expression of Galectin-3 and Bcl-2 in Epithelial Ovarian Cancer and Their Clinical Significance

[Objective] Examine the expression of Galectin-3 (Gal-3),B cell lymphoma-2 protein (Bcl-2) in epithelial ovarian cancer tissues with regard to clinicopathological and survival variables,testing whether they can serve as prognostic markers and provide evidence for further research.[Methods] 98 EOC tissues and 10 normal ovarian tissues were collected,as well as their clinical documents.The expression of Gal-3 and Bcl-2 was tested via immunohistochemical staining analysis.Then the correlation between these two proteins and the clinicopathological and survival variables of these patients were evaluated.[Results] Of the 98 EOC,Gal-3 and Bcl-2 were all cytoplasmic staining.Gal-3 was observed in a majority of the EOC tumor cell proper (75/98,76.5%) but not in normal ovarian tissues.Bcl-2 was observed in a majority of the EOC tumor cell proper (50/98,51.0%) but not in normal ovarian tissue.Spearman bivariate correlation test show that there was no correlation between the expression Gal-3 and Bcl-2 (P =0.867).Gal-3 expression in EOC was associated with histology and platinum sensitivity (P =0.043,P =0.038),while Bcl-2 was associated with FIGO stage (P =0.029).Cox univariate analysis revealed that Gal-3,Bcl-2 expression,platinum sensitivity,stage were associated with overall survival (P =0.008,P =0.021,P < 0.001,P < 0.001).Further multivariate analysis showed that only Gal-3 expression,platinum sensitivity,stage affected the survival of these patients (P =0.014,P =0.021,P < 0.001).[Conclusions]The expression of Gal-3 and Bcl-2 in EOC was significantly higher than that in normal ovarian tissue,supposed that they may involve in the development and progression of EOC.Gal-3 expression was associated with histology,platinum sensitivity and survival,it may be an independent predictive factor for EOC prognosis.There was no correlation between the expression Gal-3 and Bcl-2 in EOC,the functional mechanism between these two protein need further research.
8.Factors affecting postpartum stress urinary incontinence among lying-in women at ages of 35 years and older

Objective:
To investigate the factors affecting postpartum stress urinary incontinence (PSUI) among lying-in women at ages of 35 years and older, so as to provide insights into PSUI prevention.
Methods:
Lying-in women at ages of 35 years and older receiving postpartum examinations were sampled using a convenient sampling method from Hangzhou Obstetrics and Gynecology Hospital during the period from January 2021 to April 2022. Participants' demographic data, type of delivery, birth weight, diastasis recti abdominis, pelvic floor muscle strength injury, pelvic floor muscle training were collected, and the development of PSUI was evaluated using the International Consultation on Incontinence modular questionnaire. The factors affecting the development of PSUI were identified among lying-in women at ages of 35 years and older using a multivariable logistic regression model.
Results:
A total of 230 questionnaires were allocated, and 226 valid questionnaires were recovered, with an effective recovery rate of 98.26%. The lying-in women had a mean age of (37.30±2.11) years, and 75.66% had a pre-pregnancy body mass index (BMI) of 18.5 to 24.0 kg/m2. There were 29 women with postpartum BMI of 24.0 kg/m2 and greater (12.83%), 201 women with gestational weeks of 37 weeks and greater at delivery (88.94%), 105 women with vaginal delivery (46.46%), 20 women with neonatal birth weights of 4 000 g and higher (8.85%), 149 women with diastasis recti abdominis (65.93%), 154 women with pelvic floor muscle strength injury (68.14%). The prevalence of PSUI was 25.22% among the study subjects. Multivariable logistic regression analysis showed that vaginal delivery (OR=4.061, 95%CI: 2.124-7.763), postpartum BMI of 24 kg/m2 and higher (OR=1.903, 95%CI: 1.275-3.288), neonatal birth weight of 4 000 g and higher (OR=2.108, 95%CI: 1.420-4.135), diastasis recti abdominis (OR=1.487, 95%CI: 1.110-2.169) and pelvic floor muscle strength injury (OR=2.924, 95%CI: 1.726-4.803) were risk factors for PSUI among lying-in women at ages of 35 years and older, and pelvic floor muscle training was a protective factor for PSUI among lying-in women at ages of 35 years and older (OR=0.410, 95%CI: 0.216-0.780).
Conclusions
The development of PSUI correlates with the type of delivery, postpartum BMI, neonatal birth weight, diastasis recti abdominis and pelvic floor muscle strength injury among lying-in women. Reasonable weight control and active pelvic floor muscle training may facilitate the prevention of PSUI.
9.White matter change in diffusion tensor imaging of amnestic mild cognitive impairment

Objective To investigate the features of white matter impairment and its relationship with cognition in patients with amnestic mild cognitive impairment (aMCI).Methods Eighty-three cases of aMCI and 85 normal aging volunteers were scanned with diffusion tensor imaging (DTI) using MR system.All subjects completed the neuropsychological battery.We analyzed the differences between two groups using tract-based spatial statistics and the association between regions in difference and cognition using correlation analysis.Results There were significant differences between aMCI and normal control in the neuropsychological battery including the Mini-Mental State Examination(26.2 ± 2.6 vs 28.3 ± 1.3,F =43.224,P =0.000),Mattis Dementia Rating Scale-2 (131.4 ± 6.9 vs 138.0 ± 3.5,F =62.308,P =0.000),Auditory Verbal Learning Test-delayed recall(2.4 ± 1.6 vs 7.5 ± 2.0,F =324.018,P =0.000),Boston Naming Test(8.7 ± 1.4 vs 9.2 ± 1.0,F =6.821,P =0.010),Rey-Osterrich Complex Figure Test (12.1 ± 7.3 vs 18.5 ± 6.1,F =40.674,P =0.000),Symbol Digit Modulation Test (30.0 ± 10.1 vs 38.6 ± 9.8,F =30.786,P =0.000),Trail-Making Test Part B ((256.8 ± 124.5) s vs (178.1 ± 59.0) s,F =27.601,P =0.000).Significantly higher diffusivity indexes and radial diffusivity were also found in aMCI subjects compared to healthy elders in the parahippocampal,superior longitudinal fasciculus,inferior longitudinal fasciculus,superior fronto-occipital fasciculus,inferior fronto-occipital fasciculus,unciform fasciculus,corticospinal tract,corpus callosum,cingulum,corona radiate.We also found that axial diffusivity was significantly increased in the parahippocampal,superior longitudinal fasciculus,inferior longitudinal fasciculus,superior fronto-occipital fasciculus,inferior fronto-occipital fasciculus,unciform fasciculus,corticospinal tract and corpus callosum,whereas fractional anisotropy changes were not observed in aMCI.Diffusivity indexes values in bilateral frontal lobe (left r =0.67 ; right r =0.70),left cingulum (r =0.63),parietal white matter (r =0.69) and radial diffusivity values in left parietal (r =0.68) were significantly related to Trail Making Test A among aMCI (all P < 0.05).Conclusions In aMCI patients,there was a wide range of white matter damage,with no brain region-specific.Executive function deficit was related to the white matter impairment in bilateral frontal lobe,left cingulate and parietal lobe.The specificity and sensitivity of four DTI parameters fordetecting white matter lesions are variant.Trial registration Clinical Research Center of Jiangsu Province (BL2013025)
10.Enhancement of tropane alkaloids production in transgenic hair roots of Atropa belladonna by overexpressing endogenous genes AbPMT and AbH6H.

Atropa belladonna L. is the officially medicinal plant species and the main commercial source of scopolamine and hyoscyamine in China. In this study, we reported the simultaneous overexpression of two functional genes involved in biosynthesis of scopolamine, which respectively encoded the upstream key enzyme putrescine N-methyltransferase (PMT; EC 2.1.1.53) and the downstream key enzyme hyoscyamine 6beta-hydroxylase (H6H; EC 1.14.11.11) in transgenic hair root cultures of Atropa belladonna L. HPLC results suggested that four transgenic hair root lines produced higher content of scopolamine at different levels compared with nontransgenic hair root cultures. And scopolamine content increased to 8.2 fold in transgenic line PH2 compared with that of control line; and the other four transgenic lines showed an increase of scopolamine compared with the control. Two of the transgenic hair root lines produced higher levels of tropane alkaloids, and the content increased to 2.7 fold in transgenic line PH2 compared with the control. The gene expression profile indicated that both PMT and H6H expressed at a different levels in different transgenic hair root lines, which would be helpful for biosynthesis of scopolamine. Our studies suggested that overexpression of A. belladonna endogenous genes PMT and H6H could enhance tropane alkaloid biosynthesis.
