1.Two cases of acupuncture accidents at intra-orbital points.

Although small in size, the eyeballs are extremely important in function, and are delicate and complex in structure. With the wide application of acupuncture for eye diseases and the high frequency of use of intra-orbital points, the incidence of acupuncture accidents increases accordingly in treatment. We reported two recent accident cases, retinal tear and ciliochoroidal detachment that occurred during the regular operation of acupuncture and acupoint injection at intra-orbital points, in order to strengthen people's awareness of acupuncture accidents caused by acupuncture at the points in the orbital region, and enhance the sound development and comprehensive promotion of acupuncture in treatment of eye diseases.

2.Effect of Roy adaptation model-based nursing in patients with Guillain-Barre syndrome

Objective:To explore the effectiveness of nursing based on the Roy model in patients with Guillain-Barre syndrome (GBS) .Methods:Convenience sampling was used to select 55 patients with GBS at the First People's Hospital of Shangqiu between January 2020 and May 2024 as study subjects. Patients were divided into an intervention group ( n=28) and a control group ( n=27). Control group received conventional nursing, while intervention group received nursing based on Roy adaptation model. The recovery time, psychological state, and social support of the two groups of patients were compared. Results:Intervention group demonstrated shorter recovery times, lower anxiety and depression scores, and higher Social Support Rating Scale scores compared to control group, with all differences being statistically significant ( P<0.05) . Conclusions:The Roy model nursing in this study of GBS patients, effectively improves recovery outcomes, psychological state, and social support among GBS patients through comprehensive assessment, personalized nursing intervention, and ongoing evaluation.
3.Analysis of the acceptance and influencing factors of self-collection urine HPV testing in cervical cancer screening

Objective The aim of this study was to explore the acceptance and influencing factors of self-collected urine samples for human papillomavirus(HPV)testing in cervical cancer screening among eligible women,and to provide scientific evidence for promoting this testing in low resource areas.Methods A population-based cross-sectional study was conducted from 2022 to 2023 at the Maternal and Child Health Hospital in Shuangliu district,Chengdu City Sichuan Province.The study subjects were women aged 21 to 69 years old,and a customized questionnaire was used to conduct general information and acceptance surveys on the partic-ipants.Results A total of 2,062 women were included,with an average age of 51.58±9.34 years.Among them,1,501(72.79%)women believed that self-sampling urine was very easy.However,although 1,333(64.65%)women were still willing to accept doctor sampling as a cervical cancer screening method,only 729(35.35%)were more willing to accept self-sampling urine HPV testing.Age,educational level,annual household income,awareness of HPV,HPV vaccination status,and a sense of shame about the doctor's sampling process were all associated with the acceptance of self-collected urine HPV testing among women undergoing cervical cancer screening(P<0.001).The results of multivariate logistic regression analysis indicated that older women(OR=0.965,95%CI:0.951-0.979)and those who were not familiar with HPV(OR=0.760,95%CI:0.602-0.961)were more likely to undergo self sampling urine HPV testing,while those with junior high school education(OR=1.330,95%CI:1.053-1.682),high school education or a-bove(OR=1.990,95%CI:1.401-2.827),and a sense of shame towards the doctor's sampling process(OR=2.314,95%CI:1.706-3.142)were more likely to undergo self sampling urine for HPV testing.Conclusions Most women believe that self sampling urine for HPV testing is very easy,but compared to doctor sampling,only some women choose to self sample urine for HPV testing.Key health education interventions should be carried out for older and lower educated populations to promote acceptance of urine HPV testing.
4.Clinical characteristics of hyperbilirubinemia caused by ABO hemolytic disease of the newborns and influencing factors for phototherapy duration

Objective: To retrospectively analyze the clinical data of 474 newborns with hyperbilirubinemia, and to investigate the clinical characteristics of hyperbilirubinemia caused by ABO hemolytic disease of the fetus and newborn (ABO-HDFN) and factors influencing the phototherapy duration. Methods: A total of 474 neonates with hyperbilirubinemia treated in the First Hospital of Lanzhou University from January 2019 to January 2023 were enrolled. Blood type identification and the standard serological tests (direct antiglobulin test, serum free antibody test, and antibody elution test) were performed for all neonates. Baseline clinical data were collected and analyzed. According to the results of the hemolysis tests, neonates were divided into hemolytic jaundice group and non-hemolytic jaundice group. Clinical indicators, including hemoglobin levels, length of hospital stay, and phototherapy duration, were compared between the two groups. A multiple linear regression model was used to explore clinical factors influencing the duration of phototherapy. Results: Among the 474 neonates with hyperbilirubinemia, 354 were diagnosed with ABO-HDFN (hemolytic group), while 120 were without ABO-HDFN (non-hemolytic group). The incidence of ABO-HDFN in neonates with blood type A (55.93%, 198/354) was significantly higher than those with blood type B (44.07%, 156/354) (P<0.05). Furthermore, neonates born to multiparous women had a significantly higher ABO-HDFN incidence (81.56%, 146/179) than first-born neonates (70.51%, 208/295) (P<0.05). Neonates in the hemolytic group had significantly lower hemoglobin levels (170.67±21.86 g/L vs 178.99±22.05 g/L, P<0.001), lower red blood cell counts (4.66±0.63×10
/L vs 4.89±0.59×10
/L, P<0.05), and lower hematocrit (50.05±6.56% vs 52.61±6.75%, P<0.05) compared to the non-hemolytic group. Additionally, the hemolytic group had significantly longer hospital stays (6 [5, 9] days vs 6 [4, 8] days), longer phototherapy duration (62 [38, 84.25] h vs 53 [34.25, 64.77] h), and higher frequency of jaundice episodes (9 [7, 13] times vs 8 [6, 12] times] compared to the non-hemolytic group (all P<0.05). Regression analysis indicated that a positive indirect Coombs test and multiparity were independent risk factors associated with prolonged phototherapy duration (P<0.05). Conclusion: ABO incompatibility is the leading cause of hemolytic disease in neonates, particularly in cases where the mother has blood type O and the neonate has blood type A. In such cases, close monitoring of bilirubin levels is strongly recommended. Multiparous pregnancies increase the risk of alloimmune hemolysis. Therefore, neonates born to multiparous women may require more frequent bilirubin monitoring and appropriate prenatal interventions when necessary. Additionally, changes in indicators such as hemoglobin level and red blood cell count should be closely monitored as early warning indicators for hemolytic anemia and bilirubin elevation.

7.Construction and application of a matching model between difficulty grading of peripheral venipuncture in infants and nurse's venipuncture competence

Objective:To construct a matching model between difficulty grading of peripheral venipuncture in infants and nurse's venipuncture competence, and to evaluate its effectiveness.Methods:Convenience sampling was used to select pediatric patients who were treated with peripheral venous infusion in the infusion room of the Affiliated Xuzhou Children's Hospital of Xuzhou Medical University from March to June 2024, and to establish a model for matching the difficulty grading of peripheral venipuncture in infants and the nurse's venipuncture competence. Pediatric patients before the implementation of the matching model (March to April 2024) were set up as control group, and pediatric patients after the implementation of the matching model (May to June 2024) were set up as observation group. The success rate of first peripheral venipuncture was counted and compared between the two groups.Results:The per capita first puncture success rate of the nurses in control group was (93.94±4.67) %, and the per capita first puncture success rate of the nurses in observation group was (96.10±2.65) %, and the difference was statistically significant ( P<0.05) . Conclusions:The establishment of a matching model between difficulty grading of peripheral venipuncture in infants and nurse's venipuncture competence can improve the success rate of first pediatric peripheral venipuncture and promote a steady improvement in the technical competence of pediatric venipuncture nurses.

9.Expression and in vitro activity of a neutralizing antibody against West Nile virus that reduces antibody-dependent enhancement

Objective:To establish an antibody expression system to reduce the antibody-dependent enhancement (ADE) effect of target antibody.Methods:Site-directed mutagenesis was used to mutate the 234 and 235 sites of the Fc region of the mammalian cell antibody expression vector-L234A and L235A to establish the antibody expression vector pFRT-IgG1κ-FcM. An antibody Wt-WNV with significant ADE effect obtained in previous work was selected and expressed by the pFRT-IgG1κ-FcM system to obtain mutant antibody FcM-WNV. The binding ability of FcM-WNV to target antigen West Nile virus envelope protein-DⅢ (WNV E-DⅢ) was detected by ELISA, and the its binding ability to human high-affinity IgG Fc receptor hFcγRⅠ (hCD64 ) was analyzed by flow cytometry. The neutralizing activity of FcM-WNV in vitro was detected by pseudovirus infection of host cells (BHK21 and K562). Results:The expression levels of FcM-WNV and Wt-WNV were comparable, and FcM-WNV could recognize and bind to WNVE-DIII in a concentration-dependent manner. Compared with Wt-WNV, the binding ability of FcM-WNV to hCD64 was significantly weakened, showing a significant decrease in fluorescence intensity. Consistent with the previous experimental results, Wt-WNV at a concentration of 5 μg/ml significantly enhanced the infection of K562 by WNV pseudovirus, while FcM-WNV at a concentration of 5 μg/ml could effectively block pseudovirus infection in both K562 and BHK21 cells.Conclusions:The established antibody expression system can effectively reduce the ADE effect of the target antibody.
10.Current status and countermeasures of routine supervision by disciplinary inspection departments in Beijing municipal public hospitals

Objective:To analyze the current status of routine supervision by the discipline inspection departments of public hospitals and the existing problems, and discussion of countermeasures, in order to provide reference for improving the supervision and management of discipline inspection.Methods:In January 2023, 19 full-time discipline inspection cadres from Beijing municipal public hospitals were selected as the subjects of the investigation. A self-made questionnaire was used, and the survey was conducted through the Questionnaire Star platform to understand the current status of routine supervision by discipline inspection departments in various public hospitals, with descriptive analysis of the questionnaire data. From October to December 2023, 12 full-time discipline inspection cadres from 8 Beijing municipal public hospitals were selected as interview subjects. Face-to-face interviews were conducted on how to effectively carry out routine supervision and how to utilize the roles of discipline committee members and branch discipline inspection committee members, with Colaizzi′s seven-step method used to analyze the interview data.Results:41 valid questionnaires were collected. The results showed that the routine supervision matters mainly involved political supervision and supervision of key links in the operation of power, including the implementation of the Central Eight Regulations and their detailed rules (40 people, 97.6%), the selection and appointment of cadres (40 people, 97.6%), the execution of party discipline by party members and leading cadres (39 people, 95.1%), and the implementation of significant decisions and deployments by the state, Beijing municipal and higher-level departments (38 people, 92.7%), etc.; routine supervision was mainly carried out through meeting supervision (41 people, 100.0%), collaborative supervision (41 people, 100.0%), special supervision (40 people, 97.6%), and letter and visit supervision (40 people, 97.6%); the weakness in routine supervision mainly included insufficient supervision of the party committee at the same level (29 people, 70.7%), etc. The lack of information technology means such as " internet" and " big data" (26 people, 63.4%), insufficient professional knowledge training for discipline inspection cadres and lack of professional personnel (26 people, 63.4%), insufficient supervision capabilities of discipline inspection cadres (26 people, 63.4%), limited methods of routine supervision (26 people, 63.4%), and insufficient innovation in supervision methods (26 people, 63.4%) had affected the effectiveness of routine supervision. Analysis of the interview materials yielded five themes: building a supervision system, grasping the key content of routine supervision, optimizing the methods of routine supervision, enriching the paths of supervision, and strengthening the construction of integrity culture and discipline inspection cadres.Conclusions:The routine supervision efficiency of the discipline inspection departments in Beijing municipal public hospitals still needs to be further improved. It is recommended to further improve the relevant work from the aspects of strengthening the construction of the discipline inspection supervision system, enhancing the efficiency of discipline inspection supervision, reinforcing information technology supervision methods, and perfecting the construction of discipline inspection cadre teams.
