The comprehensive and precise functional evaluation is imperative in finding the disabilities of patients with quadriplegia,and it is the basement of providing appropriate rehabilitation training.The predominant instruments which have been used to evaluate the functional capacity of the quadriplegics include Barthel Index,Functional Independence Measure,Quadriplegia Index of Function,a short-form of Quadriplegia Index of Function Scale and Spinal Cord Independence Measure.The purpose of this article is to introduce the characters of each instrument above in functional evaluation for quadriplegics and the comparison among them.
4.Skin adnexal tumor with multipote ntial differentiation complicated by nevus spilus: a case report

A 41-year-old female patient developed round,bright yellow patches on the left calvarial region without obvious precipitating factors 40 years prior to the presentation,which gradually grew to form plaques with age.Two years prior to the presentation,nipple-like lesions appeared in the calvarial and temporal region with an erythematous and wet surface; concurrently,black masses developed in the left temporal region and gradually enlarged with central ulceration but no subjective symptoms.At about 1 year of age,pitchy macules developed on the light tan patches located on the left jaw,posterior and anterior neck,trunk and upper limbs,and gradually increased in quantity and size with the involvement of the homolateral dorsal hand and gradual appearance of papules.Skin examination revealed two well-marginated,indurated,bright red neoplasms sized 3 cm × 2 cm and 2 cm × 1 cm respectively,with erosive and cauliflower-like surface; black or pink papules were scattered between these neoplasms.There was a ring-shaped black mass sized 1.5 cm × 1.5 cm in the left temporal region with central ulceration.Pitchy tough macules and papules were observed on the light tan patches located in the left cheek,lower mandible,posterior and anterior neck,protothorax,shoulder and back,upper limbs and dorsal hand.Based on the histopathology of multiple lesions,the cauliflower-like lesions on the head were diagnosed as syringocystadenoma papilliferum,the yellow plaques as syringocystadenoma papilliferum complicated by sebaceous adenoma,the black proliferative lesions in the temporal region as trichoblastoma accompanied by basal cell epithelioma,the black papuloid lesions and brown maculopapuloid lesions on the lower mandible as nevus spilus.The patient was diagnosed with skin adnexal tumor with multipotential differentiation (syringocystadenoma papilliferum,sebaceous adenoma,trichoblastoma and basal cell epithelioma)accompanied by nevus spilus.
5.Serum osteocalcin in patients with spinal cord injury

Objective To explore the correlation between the time course of disease and bone metabolism changes in patients with spinal cord injury. Methods Data on serum osteocalcin(SO)measured by radioimmunoassay(RIA)from a database on 167 patients with spinal cord injury(SCI)were analyzed retrospectively.The patients were divided into four groups according to the time post-injury:group A(<3 months post-injury,n=44),group B(3-6 months post-injury,n=42),group C(7-12 months post-injury,n=37)and group D(>12 months post-injury,n=44). Results The mean SO value in all 4 groups of patients was significantly higher than normal.The mean SO value of group B was significantly higher than that of group A,but not significantly different from that of group C.The mean SO value of group D was significantly lower than that of group C,but not significantly different from that of group A.Conclusion Serum osteocalcin is elevated in the early stages of SCI,and its subsequent de-cline is closely correlated with the time post-injury.
6.An experimental study of neuronal activation through body-weight-supposed treadmill training after spinal cord injury using laser confocal microscopy

Objective To investigate the effects of locomotor training on improving locomotor function after spinal cord injury(SCI)and the mechanism of spinal cord plasticity.Methods A model of complete thoracic cord transection was established using 84 adult female rats divided into sham,SCI and treadmill training(BWSTT) groups.The hind limb locomotor function of all the rats was evaluated.The fluorescence intensities due to (EphA4),vesicular glutamate transporter 2(VGluT2)and EphA4/VGluT2 double-positive neurons in the ventromedial area of the anterior horn of the lumbar COrd were detected using immunofluorescence double labeling and laser confocal microscopy.Results The rats in the BWSTT group showed better functional recovery in their hind limbs than those in the SCI group.BWSTT was correlated with markedly increased EphA4.VGIuT2 and EphA4/VGluT2 intensities in the ventromedial area.Conclusion BWSTT improves hind limb locomotor function in rats with thoracic cord transections by elevating the expression of EphA4/VGluT2,promoting neuronal plasticity in the lumbar anterior horn.
7.Study of abdominal visceral arteries atherosclerotic plaque distribution detected by MSCTA

Objective To analyze the plaque distribution of abdominal aorta and its branches by multislice spiral CT angiography (MSCTA).Methods 145 patients recommended to abdominal enhanced CT were randomly selected in this investigation,and divided into three groups according to their age,i.e.,young,middle and old-aged group.CTA was performed with the use of multiple planar reconstruction(MPR),volume rendering (VR)and vessel probe(VP)technology.Results 124 patients with atherosclerotic plaques of abdominal aorta and all its branch-vessels,the incidences of the three groups were 9.7%(14 cases),30.3%(44 cases)and 45.5%(66 cases)respectively(P <0.005).It showed that calcified plaques were detected in a total of 1 50 in 302 plaques of all the branch-vessels.230 plaques(76.2%)were detected in abdominal branch-vessels of grade 1,of which,the incidences of superior mesenteric artery plaques was the highest(32.4%).Of the 54 plaques(1 7.9%)detected in abdominal branch-vessels of grade 2,the incidence of splenic artery plaques was the highest(13.8%).In abdominal branch-vessels of grade 3,the plaques were mainly distributed in splen-ic lobial artery.The splenic artery plaques mainly spreaded in the middle segment,while other plaques were mostly in peristome and proximal segment of vessels.The incidences of plaques in the three branches were 47.6%(69 cases),1 5.2%(22 cases),and 9.0%(13 cases)respectively(P <0.005).Conclusion The incidences of atherosclerotic plaques are higher in the middle and aged people. The plaques of the three abdominal branch-vessels mainly distribute in the peristome and proximal segment,and are much likely to be detected in abdominal branch-vessels of grade 1.
8.Beneficial effects of position on patent foramen ovale-induced right-to-left shunt by contrast transcranial Doppler ultrasonography

Objective To investigate the influence on right-to-left shunt (RLS) of patent foramen ovale with contrast transcranial Dopple (cTCD) in different positions.Methods 276 consecutive patients included ischemic cerebrovascular disease (ICVD),migraine,episodic vertigo and syncope.They underwent cTCD at rest and after Valsalva maneuver in 4 positions:upright sitting,supine,left lateral decubitus and right lateral decubitus,in random order.RLS was graded 0 (no microbubbles detected),1 (1-10microbubbles),2 (> 10 microbubbles but no curtain),and 3 (curtain,shower of microbubbles).Meanwhile,each patients was examined by contrast transthoracic echocardiography(cTTE),contrast transesophageal echocardiography (cTEE) or right heart catheterization.Results RLS detection rate was 48.2% (133/276)through cTCD.They were also confirmed patent foramen ovale by 82 cases in cTTE,28 cases in cTEE,and 23 patients in right heart catheterization.At rest,RLS detection rate and microbubbles of 4 positions had no significant difference (P >0.05).After the Valsalva maneuver,RLS detection rate and microbubbles of each position were significantly higher than at rest (P <0.001),and the RLS detection rate and microbubbles in upright sitting were higher than that at rest(P <0.05).Conclusions RLS is best detected in the upright sitting position with Valsalva maneuver with cTCD.Upright sitting detection helps to improve the detection rate and microbubbles of RLS.
9.Retrospective Analysis of Slight and Moderate Hydronephrosis in 58 Spinal Cord Injured Patients

ObjectiveTo investigate the clinical manifestation, mechanism and optimal treatment of hydronephrosis after spinal cord injury. Methods58 slight and moderate cases were retrospectively analyzed. ResultsThere were no differences in the remitted time among the therapies of indwelling catheter, intermittent catheterization and medicine, mixed therapy. 56 (96.55%) SCI hydronephrosis remitted after treatment, 28(48.28%) recurred within 3 months. The recurrence rate of the patients with detrusor sphincter dyssynergia (57.14%) was higher than that of the detrusor hyperreflexia (28.57%) and the detrusor hyporeflexia (14.29%). The recurrence was associated with the fact that many patients couldn't keep up with the systematic therapy.ConclusionThe onset of SCI hydronephrosis is indistinct and recurrent easily. Keeping up with intermittent catheterization and taking medicine can prevent SCI hydronephrosis recurrence effectively.
10.Effect of Baclofen on Spasticity in Spinal Cord Injury:136 Cases Report

Objective To study the antispasticity effect of Baclofen on spasticity in spinal cord injured patients.Methods 136 cases were treated with Baclofen administration for at least 2 months.They were assessed with revised Ashworth Scale.Results The spasticity was reduced 1 grade in average in 131 cases.Conclusion Baclofen shows a good antispasticity effect on spasticity in spinal cord injury with different age,sex,duration of postinjury,level or completeness of injury.No hepatic or renal side-effects have been found.
