1.APPLICATION OF ANTI REFLUX INDWELLING NEEDLE IN THE NEURAL DEPARTMENT OF INTERNAL MEDICINE NURSING

Objective To explore and study the anti reflux type indwelling needle in patients with neurologi-cal department of internal medicine .Methods 100 cases of hospitalized patients with neurological department of in-ternal medicine between 2014.1~2014.12 in our hospital, were randomly divided into two groups according to the admission number .50 cases of control group with general retaining needle , and group 50 cases by anti reflux type in-dwelling needle .Retention time and related complications were compared in the two groups of patients .Results The retention time with indwelling needle time was 3.7 ±1.7d, which was significantly higher than the control group (2.8 ±1.3d), with significant difference (p<0.05);The incidence rate of removal in the two groups had no significant difference (p>0.05), but in the needle blood study group of reflux , plugging rate of Guan Fasheng were significantly lower than that of the control group , with significant difference ( p<0.05) .Conclusion Anti reflux type can effec-tively prolong the indwelling time , reduce part of indwelling needle related complications , and improve the treatment of security, which is worthy of clinical popularization and application .
2.Correlation between Fatigue and Depression in Stroke Patients

Objective To explore the correlation between fatigue and depression after cerebral infarction. Methods 112 cases were assessed with Zung Self-rating Depression Scale (SDS), depression dimension of Symptom Checklist 90 (SCL-90d) and Checklist Individual Strength (CIS). The data were analyzed with Spearman correlation analysis. Results The correlation coefficient between SDS and SCL-90d was 0.822 (P<0.01), and were 0.671 (P<0.01) and 0.640 (P<0.01) between CIS and SDS, CIS and SCL-90d, respectively. Conclusion The fatigue and depression after cerebral infarction is correlated.
3.Etiology of Poststroke Fatigue:Review of Literature and Hypothesis Involving Inflammatory Cytokines

Poststroke fatigue is not a single etiology disorder.As a kind of central fatigue,the basal ganglia,thalamencephalon,limbic system and the high grade cortical area man be involved,but none of them can be identified as the fatigue area.Some literature suggest the interrelationship between the proinflammatory cytokines,such as IL-1,TNF-α,IL-6,and fatigue.Since the pathologic process of stroke involving cytokines,the researchers proposed that poststroke fatigue may be related to sickness behavior promoted by proinflammatory cytokines ultimately.The physical fatigue may be associated with the proinflammatory cytokines's activation to hypothalamic-pituitary-adrenal axis,and the psychological fatigue may involve that proinflammatory cytokines decreased the clearance of extracellular glutamate by astroglia.

AIM:To observe the therapeutic effect of dihydromyricetin on experimental schistosoma japonicum hepatic fibrosis in mice. 5.The clinic significant of determination of T-lymphocyte subsets in patients with pulmonary tuberculosis combined with diabetes

Objective To investigate the clinical diagnostic significance of detecting T in peripheral blood lymphocyte subsets (CD4+,CD8+ and the ratio of pulmonary tuberculosis with diabetes) and explore drug therapy of the five thymus peptide combined anti-tuberculosis on the diabetes complicated with pulmonary tuberculosis.Methods Eighty cases of pulmonary tuberculosis complicated with diabetes mellitus were selected as our treatment subject,who were treated with anti-tuberculosis drug treatment and five thymus peptide regulating immunity treatment.T lymphocyte subsets (CD4 +,CD8 + and their ratio) were detected.Meanwhile acid-fast bacilli and image detection were performed.Another 80 cases were as our control group that treated with anti-tuberculosis drug.Results Following the thymus peptide five treatment for 1,2,and 6 months,T lymphocyte subsets were (CD4+:(34.34 ± 7.32) %,(39.23 ± 6.62) %,(41.06 ± 7.13) %),(CD8+:(31.42 ±8.91)%,(32.52 ±8.58)%,(32.12 ±8.59)%),(CD4+/CD8+:(1.23 ±0.31),(1.34 ±0.30),(1.36 ± 0.32),higher than that of before treatment (30.26 ± 8.25) %,(30.36 ± 9.86) %,(1.13 ± 0.42) CD4 +.Through 2 months treatment,CD4+,CD4+/CD8+ of patient in treatment groups were different with that of control group (P < 0.01 or P < 0.05).80 patients after anti-TB plus application thymopentin regulating immunity for 2 months,normal sputum negative conversion rate of CD4+ was 97.2% and improvement rate was 98.0%.However cases with high CD4+ T lymphocyte subsets showed 78.6% negative rate of acid-fast bacilli and 88.0 % cured rate,and there was significant difference (x2 =4.764,7.680,respectively,P < 0.05).Conclusion Thymic peptide five might have the ability of improving the cellular immune function in patients with pulmonary tuberculosis and diabetes.The prognosis is better when T lymphocyte subsets CD4+ of patient was back to normal through treatment.
6. Giant ovarian mature cystic teratoma combined with contralateral ovarian endometriosis cyst: A case report and literature review

Objective: To investigate the clinical characteristics of patient with giant ovarian mature cystic teratoma with contralateral ovarian endometriosis cysts, and to analyze the diagnostic experience and treatment process. Methods: The chinical materials of one case of giant hepatic mature cystic teratoma with contralateral ovarian endometriosis cyst was analyzed retrospectively, and the related literatures were reviewed. Results: The patient was admitted to the hospital for peritoneal giant mass in a physical examination. A large abdominal cysts was seen by gynecologic ultrasound. In the left ovary, a 61 mm×48 mm cystic light mass was seen, and the viscous liquid was found in it. The abdominal CT showed a huge lump of mixed density in the right abdomen. The areas of density of fat, calcification density and density of soft tissue were found in the abdomen. The liver was pressured by the mass. The right ovarian tumor size was about 40 cm×35 cm×30 cm, up to the diaphragm, down to the pelvic tumor; the posterior wall of tumor adhered the left lobe of liver and the liver was squeezed to the upper right. The size of the left ovarian endometriosis cyst was about 7 cm × 6 cm × 6 cm. The operation was to remove the right ovarian tumor and left ovarian endometriosis cyst, and to reconstruct bilateral ovaries. The pathological diagnosis was right ovarian mature cystic teratoma and left ovarian endometriosis cyst. After 2 years of follow-up, no recurrence of the tumor was found. The patient was conceived with in vitro fertili zation frozen embryo transfer (IVF-ET). Conclusion: The huge mature teratoma combined with contralateral ovarian endometrial cyst, which adhereds to the liver, is rare. Surgical treatment will gain good prognosis. IVF-ET should be early done when the patient is going to be infertile after surgery.
7.Development of Stereo Magnetic Intravenous Infusion Dressing

Objective To develop medical equipment suited for medical care.Methods The secondary auxiliary wings were added inside the original intravenous infusion dressing clips on the left and right,and then a new stereo magnetic infusion dressing came into being with the medical 3M sticking membrane and glutinous magnetic woundplast.Results The Stereo Magnetic Intravenous Infusion Dressing was gifted with the functions of observing,supporting,and fixing the sting area.The model successfully passed the animal experiments.Conclusion The new stereo magnetic infusion dressing,easy to use,can satisfy the requirements of medical care.
8.Clinical Application and Assessment of Chinese Version of Checklist Individual Strength Questionnaire in Stroke Patients

Objective To introduce Checklist Individual Strength(CIS)into China,and evaluate its reliability and validity.Methods CIS was translated into Chinese language and the reverse translation was done by several experts.Validity and reliability were implemented in 214 cases with cerebral infarction.Results 4 components were extracted in factor analysis,and the total cumulative contribution was 78.984%.By correlation analysis,the twenty items were divided into 4 domains:subjective feeling of fatigue,concentration,motivation and physical activity,it was same as in the English version.Cronbach α of four domains were 0.9256,0.9072,0.7598,and 0.9157,respectively.Conclusion Psychometric properties(reliabilities and validities)of CIS Chinese version is met with satisfaction and seems to be adaptable to Chinese cerebral infarction patients.
9.Evaluation of Chinese Version of 3 Fatigue Questionnaires for Stroke Patients

Objective To analyze the instruments for post stroke fatigue, Fatigue Severity Scale (FSS),Checklist Individual Strength (CIS) and Fatigue Impact Scale (FIS). Methods The FSS,CIS and FIS were applied in 214 cases with cerebral infarction. They were combined together at the (sub)scale level, and evaluated with Mokken Scale Analysis for Polytomous Items, for validity and reliability. Results and Conclusion Three scales were and The combined scale proved to be acceptable (H>0.5). 3 components were extracted in factor analysis, and the their total cumulative contribution was 77.504%, with reliability (α) of 0.8097,0.7094 and 0.8019 respectively. FSS put emphasis on the fatigue experiences of stroke patients, CIS on the influence of fatigue on, and FIS on the social function.
10.Prokaryotic expression, purification and immunoreactivity of Mycobacterium tuberculosis Ag85A protein

Objective To obtain M. Tuberculosis Ag85A protein by prokaryotic expression. Methods The fbpA gene encoding M. Tuberculosis Ag85A protein was amplified by polymerase chain reaction ( PCR) from M. Tuberculosis H37R_V strain. The PCR product was cloned into prokaryotic expression vector pProEXHTb to generate the recombi-nant plasmid pProfbpA, which was then transformed into the competence Escherichia coli BL21 cells. The recombi-nant Ag85A protein was successfully expressed by isopropyl thio-β-D-galactoside(IPTG) induction and purified by the Ni-purification system. The distribution of fbpA gene in different nonpathogenic mycobacterial strains was screened by PCR and ELISA was performed to determine the immunoreactivity of the recombinant Ag85A protein with serum from mice with mycobacterial infections. Results 32 ku Ag85A protein was successfully expressed and purified. It was confirmed by PCR and ELISA that fbpA gene presented in the genomes of M. Tuberculosis H37Rv, H37Ra, BCG, M. Smegmatis, M. Terra, M. Trivial and M. Phlei, but being absent in the genomes of M. Vaccae. The highest Ag85 A antibody titer was found in serum of TB patients and mice infected by M . Tuberculosis H37Rv .Conclusion The recombinant Ag85A protein was successfully expressed and purified.
