1.50 cases of clinical diagnosis and treatment of solid-pseudopapillary tumor of pancreas

Objective:This study explored the clinical characteristics, imaging features, biological characteristics, treatment, and prog-nosis of solid-pseudopapillary tumor of the pancreas (SPT). Methods:We collected clinical data of 50 cases of SPT in Tianjin Medical University Cancer Istitute and Hospital from January 2011 to October 2016. We then retrospectively reviewed and analyzed clinical and pathological features of these patients. We conducted follow-up consultations and summarized data on patient characteristics, pathological features, diagnosis, treatment, and prognosis. Results:Among 50 SPT patients, mean age was 33.0±12.00 years, and male-to-female ratio was 1:5.25. Clinical presentation was mostly an abdominal placeholder diagnosed by physical examination, and tumor was usually located in the head or body and tail of the pancreas. All patients received surgery;procedures included pancreaticoduode-nectomy, and distal pancreatectomy plus spleen resection. SPT was clearly diagnosed with postoperative histopathological examina-tion. Hospital stay lasted for 13.9±5.16 days. All 50 cases were followed up, with consultation period lasting for 3-70 months. No recur-rence or metastasis appeared in 49 cases, and perioperative death was not noted in our patients. Conclusion:SPT is a rare, potential low-grade malignant tumor, which mostly affects young females. There is no obvious specificity in the clinical manifestation and labora-tory examination. Tumor marker levels are almost within normal range. Surgery for SPT provides good prognosis and long survival dura-tion.
2.Steroids and Anthraquinones from Astragalus hoantchy

AIM:To study the chemical constituents of the roots of Astragalus hoantchy Franch.. METHOD:Isolation and elucidation of the chemical constituents,were conducted by chromatography and spectral evidences. RESULTS and CONCLUSION:Six steroids and four anthraquinones were isolated from the roots of A. hoantchy. Their structures were identified to be stigmastane-3,6-dione (1),5α,8α-epidioxy- (22E,24R)-ergosta-6,22-dien-3β-ol (2),stigmastane-3β,6α-diol (3),daucosterol (4),β-sitosterol (5),stigmasterol (6),chrysophanol (7),emodin (8),physion (9) and aloe-emodin (10) on the basis of spectral data and physical constants. Among them,compounds 1,2,3,7,8,9,10 were isolated from the genus Astragalus for the first time.
3.CYCLOARTANE TRITERPENES AND GLYCOSIDES FROM CIMICIFUGA ACERINA

AIM To investigate the chemical constituents from the rhizomes of Cimicifuga acerina (Sieb. et Zucc.) Tanaka. METHODS Column chromatgraphy (including silica gel and ODS) together with HPLC was used to separate the chemical constituents whose structures were determined by FAB-MS, NMR (1D and 2D) and hydrolysis methods. RESULTS Five cycloartane triterpenoids were isolated and identified as: (22R)-22-hydroxycimigenol (I), (22R)-22-hydroxy-24-O-acetylhydroshengmanol 3-O-β-D-xylopyranoside (II), dahurinol (III), 24-epi-24-O-acetyl-7,8-didehydroshengmanol 3-O-β-D-xylopyranoside (IV), 25-O-acetyl-7,8-didehydrocimigenol 3-O-β-D-xylopyranoside (V). CONCLUSION Compound I is a new natural product, compound II is a new compound and compounds IV and V were isolated from this plant for the first time.
4. Meta-analysis of total knee arthroplasty guided by kinematic alignment and mechanical alignment

BACKGROUND: Although mechanical alignment technique ensures the long-term survival rate of knee arthrosis, some patients are still not satisfied with the effect of total knee arthroplasty and the function of knee joint after operation. OBJECTIVE: Meta-analysis was used to evaluate the effect of kinematic alignment technique and mechanical alignment technique in the guidance of total knee arthroplasty. METHODS: The literature of clinical controlled study of kinematic alignment and mechanical alignment in the guidance of total knee arthroplasty published from the date of establishment to July 2019 was searched in Chinese Biomedical Literature Database, Wanfang data, CNKI, VIP data, PubMed, Cochrane Library, and Embase. The literature was screened, and evaluated; data were extracted. Review Manager 5.3 software was used for statistical analysis. RESULTS AND CONCLUSION: (1) A total of 945 cases were included in 12 articles, including 470 cases in kinematic alignment group and 475 cases in mechanical alignment group. (2) The results of meta-analysis showed that the operation time was shorter in the kinematic alignment group than in the mechanical alignment group [MD=-15.44, 95%C/(-27.16, -3.71)]. Western Ontario and McMaster University Osteoarthritis Index was better in the kinematic alignment group than in the mechanical alignment group [MD=-8.40, 95%C/(-15.39, -1.40)]. Oxford knee score was better in the kinematic alignment group than in the mechanical alignment group [M7=4.72, 95%C/(0.24, 9.21)]. Distal angle of mechanical lateral femur and proximal angle of mechanical medial tibia were significantly larger in the mechanical alignment group than in the kinematic alignment group (all P < 0.05). (3) There was no difference in average hospitalization day, maximum walking distance before discharge, knee society score, knee joint straightening angle and postoperative complications between kinematic alignment group and mechanical alignment group (all P > 0.05). (4) The function of knee joint after kinematic alignment for guiding total knee arthroplasty was better than that in mechanical alignment group. However, the imaging and perioperative results were similar, and the incidence of complications was not increased after operation. It is suggested that kinematic alignment may be a lower extremity force alignment method to guide total knee arthroplasty.
5.Evaluation of cellular immune responses in mice elicited by Chinese AIDS candidate vaccines

Objective To compare cellular immune responses in mice elicited by Chinese different AIDS candidate vaccines.Methods According to their different immunization procedures,BALB/c mice were immunized with 6 AIDS candidate vaccines,separately.Spleen cells were isolated for the detection of cellular immune response to HIV-specific peptides using enzyme-linked immunosorbent spot(ELISPOT)assay and intracellular cytokine staining(ICS)method.Results AIDS vaccines were evaluated by using potential T-cell epitopes(PTE)Gag,Env and Pol peptides pool and ELISPOT.The positive conversion rates for cellular immune response of 1#-6# vaccines fluctuated from 70% to 100%.The vaccine-induced cellular immune responses to specific peptides pool are different not only in magnitude but also in breadth.The Th1type cytokines,IFN-γand IL-2,were detected with ELISPOT in 1# and 2# vaccines.The productions of IFN-γand IL-2 induced by both of the two vaccines showed a moderate correlation(r1 =0.62,P1 <0.01 ;r2=0.79,P2 < 0.01).The positive conversion rate of IFN-γ secreting cells of 1 # vaccine was 66.7%(10/15)mice detected with both ELISPOT and ICS.And the results tested by ELISPOT and ICS showed moderate correlation(r = 0.55,P < 0.05).Conclusion The magnitude and breadth of cellular immune responses induced by different AIDS candidate vaccines are different.Being induced by different AIDS candidate vaccines,the IFN-γand other Th1 type cytokines detected by ELISPOT or ICS could be used to evaluate the cellular immune responses in mice.
6.The impact of site-directed deglycosylation of the HIV-1 envelope (Env) on its immunogenicity and assembly of functional pseudovirus

Objective To study the influence of site-directed deglycosylation of the HIV-1 envelope (Env) on its immunogenicity and assembly of functional pseudovirus. Methods Site-directed deglycosylation were performed using cycling mutagenesis and selection of mutants with DpnⅠ. Single-cycle infection assay was employed to analyze the effect of the mutations on the ability of functional pseudovirus assembly. The influence of deglycosylations on the immunodeficiency of Env was evaluated using pseudovirusbased neutralization assay and ELISPOT assay. Results Mutant N197Q induced higher neutralization activities for both pseudoviruses, but lower Env-specific T-cell response. And N197Q rendered the Env to lose the ability of functional pseudovirus assembly. Mutant G2 induced higher neutralization activities for pseudovirus 74-2 but lower for pseudovirus Wt, and had almost no influence on Env-specific T-cell response and functional pseudovirus forming. Conclusion The site-directed deglycosylation of the HIV-1 Env affects the pseudovirus forming and its immunogenicity.
7.Analysis of epidemiological survey results of hydatid disease in Gannan Tibetan Autonomous Prefecture Gansu Province

Objective To understand the epidemic situation of hydatid disease in Gansu Tibetan Autonomous Prefecture(Gannan state) Gannan Province,and prevalence of the disease in population,livestock and final host dogs,and to evaluate the effectiveness of prevention and control measures.Methods In 2012:①An investigation was carried out according to the requirements of Technical Solutions of Hydatid Disease Prevalence,Gansu Province; in every county(city) of Gannan Prefecture,administrative villages were selected as survey units,by stratified cluster sampling based on the semi agricultural semi pastoral areas,pastoral areas,agricultural areas and towns.According to the proportion of the population of each layer to the population of each county,the number of people and the number of villages to be surveyed were determined.Sixteen villages were selected,and 200 people were selected in each village(from a neighboring village to make up the insufficient number in case of need),and B ultrasound method was used to carry out the census.②Children serum survey:stratification was done according to pastoral areas,semi agricultural semi pastoral areas,agricultural areas and towns population in each county(city),1 primary school was selected,children less than 12 years old were examined by B Ultrasound,and serum antibodies of Echinococcus granulosus were tested by enzyme-linked immunosorbent assay(ELISA).③ Surveillance of source of infection:in the selected villages,20 kennel households were selected in each village,dog feces was collected,and canine Echinococcus antigen was detected by double antibody sandwich ELISA method.④)Monitoring of intermediate host:1 000 sheep (or 500 cattle) were selected in each county(city),and hydatid disease was examined by anatomical method.⑤Investigation of health education was done in the form of a questionnaire survey of hydatid disease prevention knowledge and behavior survey on the awareness rate.Results The prevalence rate of population was 0.10% (29/28 960); prevalence rate of herdsmen was 0.13% (17/13 015); farmers prevalence rate was 0.06%(8/12 780); in children under 12 years of age,the infection rate was 2.64% (336/12 728) ; dog infection rate were 3.90% (117/3 001) ; livestock infection rate was 1.72% (121/7 027); and the infection rates of cattle and sheep were 2.41%(38/1 574) and 1.52%(83/5 453),respectively.The rate increased from 40%(720/1 800) to 70%(1 260/1 800) of hydatid disease prevention knowledge awareness in farmers and herdsmen after health education.Conclusions The scope of hydatid disease in Gannan Prefecture is broad; the patient population mainly is herdsmen,and farmer is second.Epidemic factors include contact canine and vices; the farmers and herdsmen have low awareness of hydatid disease prevention knowledge.Poor production and lifestyle are difficult to change in the short term,we need to further strengthen the prevention measures.
8.Effects of scalp acupuncture plus acupuncture exercise therapy on walking ability in children with spastic cerebral palsy

Objective: To compare the effect of scalp acupuncture and scalp acupuncture plus acupuncture exercise therapy (AET) on walking ability in children with spastic cerebral palsy (CP). Methods: A total of 60 spastic CP children with gross motor function classification system (GMFCS) grades Ⅰ-Ⅲ were divided into a control group and an observation group by the random number table method, with 30 cases in each group. Both groups were treated with the same conventional rehabilitation and scalp acupuncture therapy for CP. The control group received conventional rehabilitation first and then scalp acupuncture. The observation group received AET, which was to receive the conventional rehabilitation and scalp acupuncture simultaneously. Before and after treatment, the clinical efficacy was evaluated by the modified Ashworth scale (MAS) score, scores of dimensions D and E of the gross motor function measure (GMFM) scale, walking speed, and walking distance. Results: During treatment, there were 2 dropouts in the observation group. After 3 courses of treatment, the MAS scores in both the control group and observation group decreased compared with the same group before treatment (P<0.05), and the scores of dimensions D and E of the GMFM, walking speed, and walking distance were increased (P<0.05); the between-group comparison showed that the MAS score in the observation group was lower than that in the control group (P<0.05), and the scores of dimensions D and E of the GMFM, walking speed, and walking distance in the observation group were higher or longer than those in the control group (P<0.05). Conclusion: W ith the same treatments, scalp acupuncture combined with AET is superior to the conventional scalp acupuncture method in reducing lower-limb muscle tone, improving standing balance ability, and walking stability in children with spastic CP.
9.Preliminary Study on a Potential Panel for Quality Assurance of ELISPOT

The ELISPOT assay is increasingly used for assessing cellular immune responses in clinical trials of HIV-1 or cancer vaccines. However, to date, data from clinical trials do not consistently show that immune responses are correlated with clinical endpoints. This is due in part to the lack of assay standardization and validation across laboratories and therefore, a quality control panel is required to establish competency and comparability amongst different laboratories. In this study peripheral blood mononuclear cells (PBMCs) from healthy individuals were screened and frozen in liquid nitrogen. The recovery and viability of the PBMCs and the frequencies of interferon (IFN)-γ-secreting cells after CEF peptide pool stimulation were detected after various intervals in seven different laboratories. The recovery and viability did not differ significantly after different intervals. Although the frequencies of IFN (interferon)-γ-secreting cells among thawed PBMCs (peripheral blood mononuclear cells) fluctuated after CEF peptide pool stimulation at different intervals, they were not significantly decreased compared with those among fresh PBMCs. However, the viabilities, recoveries and frequencies of IFN-γ-secreting cells differed significantly among the seven laboratories. Our results indicate that cryopreserved PBMCs could be used as a quality control panel for ELISPOT. However, the procedures for ELISPOT need to be standardized amongst different laboratories.
10.Comparison of the Immunogenicities of HIV-1 Mutants Based on Structural Modification of env

Eleven env mutants were designed and generated by site-directed mutagenesis of the regions around Nab epitopes and deletions of variable regions in env.The immunogenicities of the generated mutants were evaluated using single-cycle infection neutralization assays with two pseudoviruses and IFN-γELISPOT.Overall,five mutants(dWt,M2,M5-2,M5-1 and dM7)induced highed neutralization activities for both pseudoviruses than plasmid Wt,while only two of the mutants(dWt and M5-2)showed significant differences(P<0.05).Two mutants(M2 and dM2)induced more Env-specific T cells than plasmid Wt.Statistically however,significance was only reached for mutant M2.Thus,properly modified HIV-1 Env may have the potential to induce potent cellular and humoral immune responses.
