Objective To investigate the role of double stapling device in anus-preserving operation of low rectal cancer. Methods Retrospective study was used to analyze the experience of double stapling device in anus-preserving operation of low rectal cancer for 79 patients between March 1999 to March 2003. Results 79 cases in the group all had the radical resection, no patient died. Complications included anastomosis fistula in 2 patients(2.5%),anastomosis stenosis in 3 patients(3.8%). The defecation function after surgery,the patients who received colonic Jpouch or transverse coloplasty pouch were much better than those not did. Five years survival rate 63.3 %, anastomosis recurrence rate 10.1% and recurrence rate in the pelvic soft tissue was 5.1%. Conclusion Anus-preserving operation of low rectal cancer can accomplished with double stapling device,and five years survival rate and local recurrence rate did not effect. Colonic J-pouch or transverse coloplasty pouch is good for improving the defecation function after anus-preserving operation.

Objective To compare therapeutic effect of single hemoperfusion and hemoperfusion combined with continuous veno-venous hemofiltration ( sequential continuous blood purification) on patients with severe acute organophosphate poisoning combined with myocardial injury.Methods A total of 80 patients with severe acute organophosphate poisoning combined with myocardial injury admitted to EICU of Beijing Chaoyang Hospital from Jan.2004 to Dec.2013 were respectively analyzed and divided into two groups: hemoperfusion group (patients collected from Jan.2004 to Dec.2008, n=41) and sequential continuous blood purification (patients collected from Jan.2009 to Dec.2013, n =39).The average cholinesterase recovery time, the average consciousness recovery time, the average doses of atropine, the average hospital time, the change of cardiac troponin I levels on days 1, 2, 3, 5 and 7 after admission, and the change of left ventricular ejection fraction within 24 h after admission and after 24 h following blood purification treatment were compared between the two groups.Results Patients in the sequential continuous blood purification group had less average cholinesterase recovery time, average consciousness recovery time, average doses of atropine and average hospital time compared with patients in the hemoperfusion group [ ( 87.6 ±8.8 ) h vs.( 113.8 ±13.3 ) h;(57.3 ±11.7) h vs.(100.5 ±10.8) h;(284.5 ±61.4) mg vs.(476.6 ±93.1) mg and (10.2 ±2.3) vs. (14.6 ±3.9) , all P<0.01].The levels of cardiac troponin I on days 2, 3, 5 and 7, and ejection fraction of left ventricule after 24 h following blood purification treatment in the sequential continuous blood purification group were significantly decreased compared with patients in the hemoperfusion group ( all P <0.05 ) . Conclusion Therapeutic effect of sequential continuous blood purification on patients with severe acute organophosphate poisoning combined with myocardial injury is superior to single hemoperfusion.It suggests that sequential continuous blood purification could be used as a preferred choice in treatment of patients with severe acute organophosphate poisoning combined with myocardial injury.

One hundred and five rats were equally divided into three groups and inflicted with radiation (550 rads of (?)Co ? ray), burn (15% full thickness burn) and radiation-burn combined injury respectively. Another ten were used as controls. After injuries, the outstanding changes in adrenal cortex included the following: (a) Transformation between clear cells and compact cells, on the basis of ultrastructural and cytochemical changes. (b)Acidophilic masses appeared in the cytoplasm of cortex cells, as a result of the formation of mitochondria-lipid droplets-lysosomes-hyperplastic SER complex, (c) The cortex cells or their cytoplasm entered the blood sinuses.This phenomenon is considered as an unusual pattern of discharge of endocrine secretion. Similar dynamic changes and distributing characteristics of cortical lesions were observed in all three groups of injured animals, and signifying hyperfunction of this gland. The degree of effect on the adrenal cortex in combined injury group was more marked than any of either single injurious factor. That is considered as an important feature of combined effect on the adrenal cortex.

Objective To observe the change and clinical significance of the heart type fatty acid binding protein (H-FABP) in ST Segment Elevation Myocardial Infarction (STEMI) after percutaneous coronary intervention (PCI).Methods Of 178 STEMI patients were recruited and were separated as ST segment drop group (137) and ST segment non-drop group (41).All the patients received H-FABP and cTnI tests,and re-tested H-FABP on 1 h,6 h,12 h and 24 h after the PCI,and re-tested cTnI on 12 h and 24 h after the PCI.Also,the ST segment drop amplitude (ΣSTR) was measured 2 h after the PCI.Results The demographics of both groups were similar.In both groups,the H-FABP on 6 h after PCI was significantly higher than the value measured prior to PCI.In the ST segment drop group,the H-FABP was reduced to normal value on 24 h after PCI,while it was not in the ST segment non-drop group.Conclusions The H-FABP elevation is associated with the severity of coronary lesions in ST segment elevation myocardial infarction,and the PCI may be involved in the H-FABP changes.ΣSTR≥50％ and normal HFABP may be used as index for judgment of myocardial injury and myocardial reperfusion.

Objective The aim of this study was to investigate the relationship between juxtapapillary duodenal diverticula (JDDs)and stenosing papillo odditis,and the diagnosis and treatment for this clinical entity.Methods Patients underwent upper gastrointestinal radiography,ERCP, and radionuclide scan. 18 cases were treated with operation,8 cases underwent sphincteroplasty,3 with combined diverticulo sphincteroplasty, 3 with choledochoduodenostomy or choledochojejunostomy,1 with gastrojejunostomy,1 case with diverticulectomy.Results Sphincteroplasty effects a good result for coexistant JDDs and stenosing papilloodditis. There were no recurrence and complication after the combined diverticulo sphincteroplasty. Conclusion There was a causal relation between JDDs and stenosing papillo odditis. Effective treatment was achieved by combining sphincteroplasty with diverticulo sphincteroplasty.

Objective To investigate the microstructure and ultrastructure changes of cardiac muscle, pallium, lung, liver and kidney tissue and hemodynamic effects after the success of cardiac arrest (CA)-cardiopulmonary resuscitation(CPR) of swine. Method A total of 16 Beijing swine(weight 29 ～ 35 kg)were randomly (random number) divided into normal-control group ( n = 8) and standard CPR group ( n = 8). The swine of the former group were only given anesthetized and intubated, without ventricular fibrillation and CPR. The swine of the latter group were given standard CPR after 4 min of untreated VF, from 10 min after restoration of spontaneous cirkg) and keep for 6 h. And cardiac output (CO), left ventricular maximal rate of systolic pressure ( + dp/dtmax),maximum reduction of left ventricular diastolic velocity ( - dp/dtmax), heart rate (HR) and mean arterial pressure (MAP) of these animals before ventricular fibrillation and 0.5 h, 2 h, 4 h, 6 h after ROSC have been monitored.All swine were put to death after 6 h,and got their cardiac apex, pallium, left lung, right lobe of liver and upper pole of left kidney quickly for microstructure and ultrastructure studies. Statistical analysis was performed using two paired samples t test. Results At different time points after restoration of spontaneous circulation, the cardiac output (CO),left ventricular maximal rate of systolic pressure ( + dp/dtmax), maximum reduction of left ventricular diastolic velocity (- dp/dtmax) were significantly lower than before ventricular fibrillation, with significant difference ( P ＜ 0.05). And HR of different time points were increased significantly ( P ＜ 0.05), with no significantly difference between MAP of each time points ( P ＞ 0.05). Compared with the normal-control group, the cardiac muscle, pallium, lung, liver and kidney tissue of the swine in standard CPR group were found different degree of damages in their microstructure and ultrastructure sections. The damages of the cardiac muscle, pallium, and lung tissue were more obviously. Conclusions Sudden cardiac arrest(CA)-cardiopulmonary resuscitation(CPR) could cause different degree of damages and hemodynamic effects on the body, through our ROSC 6 h model, the damages of the cardiac muscle, pallium and lung were more severity, the damages of liver and kidney were lighter.

Objective To evaluate the effectiveness of antibiotics prevention for the patients with coma in acute poisoning complicated by infection.Methods Clinical data for 47 cases with acute poisoning during May 1999 to May 2001 admitted to the Emergency Room of Beijing Chaoyang Hospital were collected and analyzed for complicated infection.Results Seventeen of the 47 cases of acute poisoning were complicated with infection (36.2%).Fourteen of those with infection were complicated with coma (14/17),and 26 of the 30 without infection complicated with coma (26/30).Average length appearing coma was (39.1?26.0) h in the group complicated with infection,as compared with (24.4 ?16.7) h in the group without infection,with a statistical significance.There were less cases who received antibiotics prophylactally in the group with coma and infection (6/14 or 42.9%) than in the controls (20/26 or 76.9%),with P

The associating rules have been widely used in traditional Chinese medical (TCM) prescription data mining research work,while Mongolian medical prescription data mining work hardly carried out.In this study,Apriori algorithm based on reduced binary matrix was adopted for the data mining of Mongolian medical prescriptions with the provision of the decision support for research and development of new drugs.Twenty-seven Mongolian medical prescriptions for He Yi disease were involved in the data mining with Apriori algorithm based on reduced binary matrix.It was found that Aquilaria agallocha,Myristica fragrans,Zhen He Yi,or Aquilaria agallocha combined with Myristica fragrans,or Zhen He Yi combined with Myristica fragrans were determined as the Mongolian drugs frequently used in He Yi disease,with 80％ confidence level and 60％ support,and two algorithms were output,[Myristica fragrans--＞Zhen He Yi,0.83] and [Aquilaria agallocha--＞Myristica fragrans,1.00].In conclusion,some potential associating rules and drugs with high frequency in the Mongolian medical prescriptions can be fast mined using Apriori algorithm based on reduced binary matrix,providing a new way for unveiling the medication rules of Mongolian medical prescriptions.
