1.Changes of nuclear factor-?B and inducible nitric oxide synthase in hypoxic pulmonary hypertension

AIM:To observe the changes of nuclear factor-?B (NF-?B) activity and inducible nitric oxide synthase (iNOS) expression in hypoxic pulmonary hypertension(HPH). METHODS:The rat model of HPH was used. The NF-?B activity and iNOS expression in lung tissue were determined by immunohistochemistry(IHC), in situ hybridization (ISH), RT-PCR and Western blot.RESULTS:ISH showed that iNOS mRNA expression in intraacinar pulmonary arteriole (IAPA) in H28 d group(hypoxic treatment for 28 days) was stronger than that in normal group, H5d group and H14 d group. RT-PCR showed that the iNOS mRNA in H28 group was 2.1 times, 1.9 times and 1.8 times higher than that in normal group, H5d group and H14 d group, respectively. The nucleic anti-NF-?B stain was observed in H28 d group, which was significantly stronger, but the I-?B amount was 2.8 times, 2.7 times and 2.5 times lower than that in normal group, H5d group and H14 d group, respectively. CONCLUSION: The activity of NF-?B was correlated with the hypoxic pulmonary vessel structural remodeling and iNOS expression.
2.Research on the effects of compound matrine injection combined with chemotherapy on elderly patients with colorectal cancer

Objective To study the effect of compound matrine injection combined with chemotherapy on the immune function and life quality of elderly patients with colorectal cancer. Methods Seventy patients with colorectal cancer treated in Shanghai Jiao Tong University Affiliated Sixth People′s Hospital South Campus from January 2010 to December 2013 were involved in this survey. They were divided into the control group and the observation group randomly,each group had 35 cases,the control group was treated with chemotherapy only, the observation group was treated with compound matrine injection combine with chemotherapy. The curative effect,life quality and immune function in the two groups were compared. Results The number of complete remission patients in the observation group was 8 cases ( 22. 86%) ,the number of partial remission cases was 16 cases ( 45. 71%) , the effective rate was 68. 57%, while the control group had 4 cases of complete remission (11. 43%),11 cases of partial remission (31. 43%),the effective rate was 42. 86% (Z=-2. 259,P=0. 024) . In the observation group,life quality was significantly improved in 10 cases (28. 57%),improved in 19 cases ( 54. 29%) ,the effective rate was 82. 86%,while in the control group,life quality was significantly improved in 4 cases ( 11. 43%) ,and improved in 10 cases ( 28. 57%) ,the effective rate was only 40%. The improvement rate of the two groups was statistically significant ( Z=-3. 497,P=0. 000) . Before treatment,the immune function indexes of patients in the two groups were close ( P>0. 05) ,after treatment,the immune function in both groups were significantly improved,CD3+,CD4+,CD4+/CD8+ levels of patients in the observation group were higher than those in the control group,while the CD8+ level in the observation group was lower than that of the control group ( t=-3. 968,P=0. 000;t=-5. 351,P=0. 000;t=-5. 474,P=0. 000;t=6. 407,P=0. 000) . The follow?up time of the two groups was 36 months. After 36 months,the survival rates of the observation group and the control group were 85. 7% (30/35) and 80. 0% (28/35) respectively,and the difference was not statistically significant (χ2=0. 402,P=0. 526) . Conclusion Compound matrine injection combined with chemotherapy can significantly improve the clinical efficacy of elderly patients with colorectal cancer,improve the immune function and quality of life,it is worthy of clinical application.
3.Observation study of clinical efficacy of Bevacizumab combined with Chemotherapy for advanced and metastatic Cancer

Objective To study the therapeutic efficacy and toxicity of Bevacizumab combined with Chemotherapy in patients with advanced and metastatic cancer.Methods Fifty-nine patients of advanced metastatic cancer (Forty-two patients of refractory metastatic colorectal cancer and 17 advanced adenocarcinoma) were treated with normalized chemotherapy combined with Bevacizumab.Patients with refractory metastatic colorectal cancer were treated with Bevacizumab combined with FOLFIRI or FOLFOX4,and patients with advanced adenocarcinoma of NSCLC were treated with Bevacizumab in combination with docetaxel and cisplatin for 4-6 cycles,Bevacizumab was used until progressive disease (PD).During treatment,adverse effect were assessed with Common Toxicity Criteria V3.0 developed by National Cancer Institute.Results Among the 42 patients of metastatic colorectal cancer,27 were treated with First-line 1 with complete remission (CR),13 partial response(PR),4 stable disease(SD) and 9 PD,the effective rate (ER) was 51.9% ( 5/15 ),disease control rate(DCR) was 66.7% (18/27);Fifteen patients were treated with second-line PR,of which 5 PRD,4 SD,6 PD.ER was 33.3% (5/15),and DCR was 60.0% (9/15).The effective rate were slightly higher in the First-line treatment group than that in the second-line treatment group,however,the difference was not statistically significant (x2=1.335,P=0.248 ).Among the 17 patients of NSCLC,of which 1 CR,7 PR,5 SD,4 PD.RR was 47.1% (8/17),and DCR was 76.5% (13/17).The major toxicities were one grade 3 hemoptysis observed in one patient,grade 3 thrombosis was observed in one patient.Other common adverse effects,were epistaxis,hemoptysis,hypertension and proteinuria,which were not severe and could be well tolerated.Conclusion Bevacizumab Combined with Chemotherapy was effective in short term for the patients with advanced and metastatic cancerMost patients could tolerate the side effects.Further studies should be done to prove the long-term effects.
4.Expression of hypoxia-inducible factor-1 alpha and endothelin-1 gene in hypoxic pulmonary hypertension.

OBJECTIVETo investigate the expression of hypoxia-inducible factor-1 alpha (HIF-1alpha) and endothelin-1 (ET-1) gene in hypoxic pulmonary hypertension (HPH).
METHODSThe animal model of HPH was replicated. The elastic fiber staining was applied to show the intraacinar pulmonary artery (IAPA). Radioimmunoassay (RIA) and in situ hybridization (ISH) were used for detection of HIF-1a and. ET-1.
RESULTSISH showed that HIF-1alpha mRNA was expressed in the IAPA of all hypoxic rat. The expression was stronger in the H14 d (0.256 9 +/- 0.046 8) and H28 d (0.225 8 +/- 0.045 3) groups than in the H5 d (0.1455 +/- 0.072 2) and control (0.110 9 +/- 0.022 4) groups (P < 0.05), the expression of ET-1 mRNA in the H14 d (0.412 2 +/- 0.078 3) and H28 d (0.368 4 +/- 0.072 9) groups was also stronger than that in the H5 d (0.201 7 +/- 0.034 9) and control (0.185 5 +/- 0.036 1) groups (P < 0.05). The amount of ET-1 in pulmonary arteial blood in the H14 d [(158.78 +/- 25.14) pg/ml] and H28 d [(142.93 +/- 23.38) pg/ml] groups was significantly higher than that in the H5 d [(79.68 +/- 12.54) pg/ml] and control [(65.37 +/- 10.82) pg/ml] groups (P < 0.05). The mean pulmonary arterial pressure (mPAP) in the H14 d [(34.0 +/- 5.8) mm Hg] and H 28 d [(29.0 +/- 4.7) mm Hg] groups was markedly higher than that in the H5 d [(19.0 +/- 3.5) mm Hg] and control [(17.0 +/- 2.8) mm Hg] groups (P < 0.05). A positive rank correlation existed between the mPAP and the amount of ET-1 (rs = 0.747, P < 0.05).
CONCLUSIONSExpression of HIF-1alpha and ET-1 mRNA in IAPA increase under long-term hypoxic condition and both show consistent expression, indicating that the expression of HIF-1a and ET-1 gene contribute to pathogenesis of HPH.

Objectives:
To explore the association between the polymorphism of persistent obesity and genetic variations in the LEP (human leptin gene, LEP) and LEPR (leptin receptor gene, LEPR) genes and different molecular subtypes of breast cancer.
Methods:
All 703 female patients of breast cancer diagnosed by histopathology in the Sichuan Cancer Hospital or the West China Hospital, excluding patients with metastatic breast cancer or mental disease, were selected as cases from April 2014 to May 2015. At the same time, 805 healthy women received physical examination in medical examination center of Sichuan People Hospital or Shuangliu maternal and child health care hospital, excluding those with therioma, breast disease, and mental disease, were enrolled in control group. A uniform questionnaire was used to collect general information including demographic characteristic, reproductive history height, weight, and so on. And the obesity status in recent 10 years was judged. Time of Flight Mass Spectrometer was used to determine the genotypes of LEP rs7799039, LEPR rs1137100 and LEPR rs1137101, while the multinomial logistic regression analysis was conducted to estimate the effect of risk factors related to breast cancer in different molecular subtypes; and then, the association between polymorphism of persistent obesity, the LEP, LEPR genes and breast cancer of different molecular subtypes was analyzed by binary logistic regression models.
Results:
The average age of controls was (48.98±8.83) years old, while the age of cases of TNBC, Luminal A, Luminal B, and HER-2+ were (51.43±11.33), (49.94±10.10), (49.73±9.38), (50.50±9.04) years old, respectively. The frequency of genotype LEP rs7799039, LEPR rs1137100 and LEPR rs1137101 in control group was separately 74.8%(1 157/1 546), 83.6%(1 339/1 602) and 88.4%(1 416/1 602); while 77.6% (1 074/1 384), 82.4% (1 155/1 402) and 87.9% (1 232/1 402) respectively in case group. Retrospective analysis of 258 patients with AML in CR (186 cases in CR1, 72 cases in CR2) who underwent allogeneic HSCT in our hospital between April 2012 and March 2015 according to NCCN (2015) risk stratification. 9.Application of self-made multifunctional accessory stent in endoscopic mucosal dissection

Objective:To investigate the safety and efficacy of self-made multifunctional endoscopic instrument accessory stent in endoscopic mucosal dissection (ESD).Methods:A total of 80 patients who received ESD in 924th Hospital of Joint Logistic Support Force of Chinese People′s Liberation Army from May 2019 to February 2021 were selected as research object. Random number table method was used to divide patients into control group and experimental group, 40 cases in each group. The experimental group used self-made endoscopic instrument accessory stent, and the control group did not use self-made endoscopic instrument accessory stent. The length of operation, the number of instruments taken by mistake, the number of instruments polluted during operation and the infection of postoperative 3-7 days under the same operation position, operating doctors and nurses were analyzed and compared between the two groups.Results:The incidence of instruments taken by mistake, the incidence of instruments polluted during operation and the infection rate of postoperative 3-7 days were 0.9% (8/856), 1.4% (12/856) and 2.5% (1/40) in the experimental group, which in the control group were 10.8% (96/887), 11.8% (105/887) and 15.0% (6/40) respectively, there were significant differences between the two groups ( χ2=75.92, 75.76, 3.91, all P<0.05). The length of operation in the experimental group was (51.56 ± 2.32) min, and that in the control group was (79.02 ± 2.83) min, the difference was statistically significant ( t=-8.72, P<0.05). Conclusions:When the patients underwent ESD surgery, the self-made multifunctional accessory stent was used to place the required instrument accessory. The length of operation was shorter, the incidence of instruments taken by mistake, the incidence of instruments polluted during operation and the infection rate of postoperative 3-7 days were reduced, the quality of minimally invasive endoscopic surgery was improved.
10.Metagenomics of Fecal Gut Microbiota in Common Traditional Chinese Medicine Syndrome Types of Irritable Bowel Syndrome with Diarrhea:A Cross-sectional Study

ObjectiveTo investigate the structural and functional characteristics of gut microbiota in common traditional Chinese medicine (TCM) syndromes of irritable bowel syndrome with diarrhea (IBS-D). MethodsIBS-D patients who visited the Hospital of Chengdu University of Traditional Chinese Medicine, and healthy participants from the Physical Examination Centre of the same hospital were recruited from 1st January 2020 to 31st March 2021.The IBS-D patients were classified into syndrome of liver constraint and spleen deficiency, and syndrome of spleen deficiency and dampness exuberance; together with the recruited healthy participants, there were liver-constraint group, dampness-exuberance group, and healthy group. General information, including age, gender and body mass index (BMI), were collected, and Irritable Bowel Syndrome Symptom Severity Scale (IBS-SSS) as well as Irritable Bowel Syndrome Quality of Life Scale (IBS-QOL) scores were additionally collected from IBS-D patients. Fresh fecal samples were also collected and tested by macro-genome sequencing technology for abundance statistical display, PCoA, Anosim, LEfSe bioinformatic analysis of the annotated gut microbiota structure and function. ResultsThere was no statistically significant difference in the general information of the participants in the three groups (P>0.05); the difference in the IBS-SSS and IBS-QOL scores between liver-constraint group and dampness-exuberance group were not statistically significant (P>0.05). The study included 28 cases each in liver-constraint group, dampness-exuberance group, and healthy group. The number of specific genes to patients in liver-constraint group was 269 135, with 216 156 in dampness-exuberance group and 249 759 in healthy group, accounting of total 1 784 036 in the three groups. There were differences in the relative abundance distribution of the top ten species of gut microbiota among the three groups, with smaller differences at the phylum, class and order levels, and larger differences at the family, genus and species levels. There were differences in the relative abundance of structure and function of the gut microbiota among the three groups. Species PCoA and Anosim analyses at the species level showed significant differences in the composition of the microbiota among the three groups. Further LEfSe analyses showed that patients in liver-constraint group were screened for 14 dominant strains, of which Clostridium sp. CAG 217, Lachnospira pectinoschiza, Anaerotruncus sp. CAG 528, Paeniclostridium sordellii, Eubecterium sp. CAG 76, Bacillus cereus were affected to a greater extent in abundance differences; dampness-exuberance group screened 24 species of dominant bacteria, of which Roseburia inulinivorans, Eubacterium sp. CAG 251, Roseburia hominis, Unclassified Eubacterium rectale, Roseburia intestinalis, and Megamonas funiformis were affected to a greater extent in abundance differences; no dominant functional genes were screened for patients in liver-constraint group, and dampness-exuberance group was screened for flagellum assembly (ko02040), porphyrin metabolism ( ko00860), salmonella infection (ko05132), and benzoic acid degradation (ko00362). The differentially dominant functional genes in liver-constraint group and dampness-exuberance group may mainly focus on metabolism (including biodegradation and metabolism of exogenous substances, energy metabolism, lipid metabolism, etc.). ConclusionIBS-D with syndrome of liver constraint and spleen deficiency is characterized by the enrichment of 14 gut microbiota, such as Clostridium sp. CAG 217, while IBS-D with syndrome of spleen deficiency and dampness exuberance is characterized by the enrichment of 24 gut microbiota, such as Roseburia inulinivorans, and 4 functional enrichments, such as flagellum assembly. Clostridium sp. CAG 217 and Roseburia inulinivorans are expected to be biomarkers for IBS-D patients in the two syndromes, respectively.
