1.Manifestation of the Ethical and Moral Principles in Humanistic Caring for the Perioperative Patients

As an essential part of all kinds of medical relations,the physician-patient relationship should be developed following the unceasing progress of medical modes.Patients were provided with the respect for their legal rights,dignity,freedom and personality with humanistic treatment and nursing care.With ethical and moral principles,such as patient first principle,body and mind unified principle,confidentiality principle,impartial treatment principle,optimization principle,medical informed consent principle,coordination principle and the principle of striving for perfection,patients' psychological state was taken into full consideration during perioperative period.Humanistic care is embodied by humanistic treatment and nursing care during perioperative period,and patients are provided with sympathy,encouragement and psychotherapy.The physician-patient relationship is improved and a harmonious medical atmosphere enhanced through humanistic caring.
2.Drug Resistance and Pathogens in General Surgery Department:Analysis of 158 Cases

OBJECTIVE To understand the pathogens and their drugs resistance in general surgery department and provide rational suggestion of antibiotics use for clinic treatment.METHODS A total of 158 cases with nosocomial infection among the general surgery department inpatients from Jun 2006 to Oct 2008 were retrospectively analyzed and summarized.RESULTS The common nosocomial infection sites were the lower respiratory tract,gastrointestinal tract,urinary tract and surgical sites.The G-bacilli of nosocomial infections in turn were Escherichia coli(18.02%),Acinetobacter baumannii(11.26%),Pseudomonas aeruginosa(7.66%),and Klebsiella pneumoniae(4.50%).The main G+cocci were Staphylococcus aureus(21.62%),Enterococcus faecium(5.86%)and E.faecalis(3.15%)in turn.In G-bacilli,the sensitivity to imipenem was the highest from 58.82% to 100.00%.The sensitivity to amikacin were more than 70.00% except A.baumannii,and to sulbactam/cefoperazone were more than 50.00% except Pseudomonas aeruginosa.In G+ cocci,the sensitivity to vancomycin of S.aureus and E.faecium was 100.00% and 84.62%.CONCLUSIONS Investigating the pathogens and their drug resistance in general surgery department is very important to prevent and control nosocomial infections.
3.Advances of mechanical conditions in engineering cartilage tissue

There are many physical factors affecting the development of cartilage tissue,the mechanical con-dition is the main important one that particularly act.The mechanical conditions used in engineering cartilage tissue,such as compressive and shear force,fluid flow,hydrostatic pressure and tissue deformation or with some of them combined,were reviewed.From the standpoint of bionics,the mechanical environments ap-plied on tissue engineering should work in three aspects:providing adequately mechanical stimuli to the cells seeded in 3-D scaffold;ensuring the efficient mass-transport of the nutrients and waste products in the cells:promoting the development of functionally extracellular matrix in 3-D scaffold.The mechanical environments currently used only represented the part of mechanical conditions of in vive articular cartilage will be reviewed.In our view that rolling depression load may achieve the fit mechanical environment for cultivation of functional cartilage constructs in vitro.
4.Study on Traditional Chinese Drug Colon Targeting System

Objective:To study possibility of traditional Chinese drug preparation of controlled drug delivered to the colonic region. Methods: After the herbs was extracted and refined, micropills was produced with pectin selected as coating material and thallium monochloride was as internal standard substance. Results: The preparation was testified to attain schedule colon target. Conclusions: This research provided a way for traditional Chinese drug preparation of colon targeting.
5.Effect of lipoxins on proliferation and secretion of peritoneal macrophages from patients with precclampsia in vitro

Objegtlve To study the effect of lipexins on the proliferation and secretion of peritoneal macrophages from patients with preeclampsia in vitro.Methods Peritoneal macrophages were obtained from 24 patients with preeclampsia(preeclampsia group)and 24 normal pregnant women(normal pregnant group)who were treated in the First Affiliated Hospital of Wenzhou Medical Coilege from March to July 2007.Enzyme linked immunosorbent assay (ELISA) was used to detect the concentration of tumor necrosis factor-α(TNF-α)in the supernatant of macrophages which were pulsed with lipoxins at different concentrations(0,10,100 nmol/L)in both groups after 48 hours.Methyl thiazolyl tetrazolium (MTT)assay was used to detect the inhibition rate of cell proliferation of macrophages which were pulsed with lipoxins at different concentrations(0,10,100 nmol/L)in both groups after 24 hours.Results (1)The concentration of TNF-α:the levels of TNF-α were(1867.5±47.3),(1836.9±4.5) and (1800.5±2.7)ng/L after treatment with differed concentrations of lipoxins(0,10,100 nmol/L)in preeclampsia group vs normal pregnant group[(791.3±62.2),(789.4±2.3),(781.5±1.9)ng/L].The levels of TNF-α in preeclampsia group were significantly higher than that in normal pregnant group(P<0.05).Lipoxins significantly inhibited the concentration of TNF-α in a dose-dependent manner in preeclampsia group (P<0.05),while it had no significant effect in normal pregnant group(P>0.05).(2)Cell proliferation inhibition:Incubation with lipoxins produced a dose-dependent(0,10,100 nmol/L)inhibitory effect on proliferation in preeclampsia group,[(14.8±6.3)%,(32.9±3.6)%,(36.7±3.8)%],vs normal pregnant group[(16.8±6.9)%,(16.7±5.4)%,(15.9±2.1)%].The rate of cell proliferation in preeclampsia group was significantly hisher than that in normal pregnant group.Lipoxins significandy inhibited this growth(P<0.05),while it had no significant effect in normal pregnant group(P>0.05).Conclusion Lipoxins can inhibit the proliferation of macrophage and secretion of TNF-α in preeclampsia in a dose-dependent manner.Lipoxins may be potentially useful in prevention and treatment of preeclampsia.
6.Clinical application of semiconductor gene sequencing in screening thalassemia

Objective To investigate the feasibility of semiconductor gene sequencing technology for thalassemia clinical screening and evaluate its application as compared with the results of PCR technology.Methods 197 visiting patients were randomly selected as prospective samples and200 patients ever diagnosed with thalassemia as previous samples.All the samples were detected by semiconductor technology gene sequencing and PCR technology at the same time and then evaluation of the advantage of semiconductor gene sequencing technology.Results 22 cases of 197 prospective samples were detected as thalassemia mutations by PCR technology,including 18 cases of α-thalassemia,3 cases of β-thalassemia,1 case of oα merge β thalassemia mutations.Semiconductor technology gene sequencing detected another 6 cases of rare type of thalassemia.By semiconductor gene sequencing technology on previous samples,118 cases of α-thalassemia,65 cases of β-thalassemia,17 case of α merge β thalassemia mutations,1 case of thalassemia mutations (HBA 1:c.223G > C) were detected.By statistical analysis,the total coincidence rate of PCR technology and semiconductor gene sequencing was 98.5%,withthe Kappa =0.97(≥ 0.8).Conclusion Semiconductor gene sequencing technology for thalassemia clinical screening is feasible,for it can detect both thalassemia gene type,and new mutation.The results of semiconductor gene sequencing technology are accurate and the technology could be popularized in clinical application.
7.Early results of thoracic remodeling for patients with bar removal after pectus excavatum correction in minimal-procedure

Objective To study thoracic re-remodeling and therapeutic effect after the bar removal for pectus excavatum corrected by minimal-invasive technique.Methods 145 cases with pectus excavatum,male 115,femal 30;adults in 59,children 86;corrected by minimal-invasive technique improved and performed by the same group surgeon.Bar removed 12-82 months after the procedure,appraising index of curative effects include in chest appearance,thoracic index,thoracic computer tomography(CT) and the distance between the behind of sternum to the anterior border of thoracic spine in the sagittal view.Results The chest shape was good.Thoracic index:before bar removal 2.36 ± 0.32 in children,2.60 ± 0.45 in adults;after that,2.77 ± 0.44 in children,3.04 ± 0.56 in adults.There was all subsidence on the each point of the sternum,descent the maximum at the inferior end of the midsternum,(15.18 ±7.95)mm in children,(14.93 ± 8.81) mm in adults,comparing with bar removal before and after.There was statistical significance.Not the signs of compressing the heart on the CT view.The time interval of the bar removed 3-year in children,5-year in adults without affecting the development of the patients' thorax.Conclusion The sternum descended slightly after bar removal when pectus excavatum corrected to expecting effects.After that,thoracic remodeling again,the chest shape well.
8.Clinical characteristics of vestibular paroxysmia in a northern Chinese cohort of 70 patients

Objective To investigate the clinical characteristics of vestibular paroxysmia (VP): age distribution,triggers,accompanying symptoms,maximum frequency per day,nystagmus features during attacks,and neurovascular compression. Methods The clinical data of 70 patients with VP were collected to analyze their age distribution,triggers,accompanying symptoms,maximum frequency per day,nystagmus features during attacks,and neurovascular compression by using descriptive statistical methods. Results Among the 70 patients with VP,30 (42.86%) were elderly,23 (32.86%) were young,and 17 (24.29%) were middle-aged patients. The maximum frequency per day was 2-5 in 53 cases (75.71%),6-10 in 9 cases (12.86%),and >10 in 8 cases (11.43%). Thirty-six patients (51.43%) had triggers,including rapid walking,emotional excitement,driving,cycling,sexual intercourse,coughing or wheezing,speaking or chatting,bathing,defecation,drinking alcohol,and sound stimulation. Thirty-three patients (47.14%) had unilateral tinnitus,bilateral tinnitus,and tinnitus with facial spasms; nystagmus during VP episodes met the characteristics of the head coordinate system,which was horizontal-torsional towards the affected side. Among 57 patients examined for the relationship between the vestibulocochlear nerve and vessels using magnetic resonance angiography,37 patients (64.91%) had contact between the vestibulocochlear nerve and the right anterior inferior cerebellar artery,15 patients (26.32%) had contact between the vestibulocochlear nerve and the left anterior inferior cerebellar artery,and 5 patients (8.77%) had contact between the vestibulocochlear nerve and the basilar artery. Conclusion VP is most prevalent in the elderly,then young and middle-aged people. Most patients with VP have a maximum frequency of 2-5 per day. VP can be triggered by anger,excitement,exercise,and various stimuli. VP attacks can be accompanied by tinnitus and facial spasms. Its nystagmus is horizontal-torsional towards the affected side. The percentage of contact between the vestibulocochlear nerve and the right anterior inferior cerebellar artery is highest in patients with VP.
9.Bone remodelling model including mechanism of damage and repair.

From the visual angle of mechanics, bone remodelling plays an important role in adapting to load environment and repairing microcracks. In order to describe the process of bone remodelling explicitly, we introduced a bone remodelling model representing the biological features of damage/repair in bone. Based on Basic Multicellular Units (BMUs), the model embodies not only the mechano-transduction of osteocytes' response to mechanical stimuli, but also the biological process of the randomly ocurring microcracks that are resorbed by osteoclasts and then refilled by osteoblasts. A 2-D trabecular bone finite element model was used as an initial configuration in the research. The developments in bone microstructure and material properties have been monitored in the course of 10 simulated years. The conclusions are: (1) Initially, trabecular bone changes prominently, but it tends to be stable later on; (2) The material properties of trabecular bone in compression are much better than in tension.

10.Ocular opsoclonus associated with anti-N-methyl-D-aspartate receptor encephalitis: a case report

Ocular opsoclonus is a rare disorder of the saccadic system, in which fixation is continuously interrupted by involuntary, chaotic, rapid and multi-directional saccadic eye movements, interrupting the gaze-holding function, leading to“dancing eyes”. Ocular opsoclonus has rarely been reported in China. A case of ocular opsoclonus associated with anti-N-methyl-D-aspartate receptor encephalitis is reported to improve awareness about the ocular sign. The patient′s condition reached the peak at the 5th week. He developed ocular opsoclonus on the basis of nonconvulsive status epilepticus, accompanied by frequent involuntary movements of the mouth and limbs. The ocular opsoclonus gradually relieved on the 7th week with the active treatment.
