1.Percutaneous vertebroplasty for the treatment of osteoporotic vertebral fracture in the elderly

Objective To investigate the effects of percutaneous vertebroplasty in the treatment of old symp -tomatic osteoporotic vertebral fractures in elderly .Methods 56 patients with osteoporotic vertebral fractures in elder-ly were selected and treated with percutaneous vertebroplasty .The age of the patients were from 67 to 83 years old, average of 72.5 years old.Among them,there were 15 male cases and 41 female cases.The Cobb angles,Visual Ana-logue Scale score ,and postoperative complications were measured .Results All the operations were successfully com-pleted which took the time from 30 min to 55min.2~5ml bone cement was used in the operation .Pains of 56 patients were significantly reduced or disappeared .VAS:(7.9 ±1.5) of pre-operation,VAS:(2.3 ±1.2) of post operation, Cobb angles:(25.8 ±4.9) of pre-operation,Cobb angles:(12.6 ±3.6) of post operation.The patients could move after operation with the protection of waist in 3 to 9 days.After the follow-up visit of 2 to 14 months,the pains of patients were disappear , no complications of nerves injury , pulmonary embolism and bone cement leakage of spinal canal.Conclusion It suggests that percutaneous vertebroplasty can relieve the pain and improve the life quality of the patients with old symptomatic osteoporotic vertebral fractures .It is worthy of clinical promotion .
2.SUPRAGLOTTIC HORIZONTAL PARTIAL LARYNGECTOMY FOR LARYNGEAL CARCINOMA: A Report of 41 Cases

From 1982 to 1988, we have performed supraglottic horizontal partial laryngectomy for 41 cases of laryngeal carcinoma. 19 of them were males and 22 females. 40 cases of squamous cell carcinoma accounted for 97.6%,and 75.6% of cases were in second or third stage. All the cases have been followed up. The three-year survival rate is 93.7%, the five-year survival rate is 86.5%. 95% of the patients retained their phonation ability. Decannulation rate of tracheostomy tube is 65.9%. Normal deglutition ability is found in 90.2% of cases. Indications for operation, postoperative complications, late results, and failure of removal of tracheostomy tube are discussed.

The related curriculum knowledge system was constructed according to the orientation and goal of the three oriented medical personnel training. In order to provide important foundation for further formulating curriculum standards, revising basic requirements of curriculum, compiling relevant teaching materials and innovation of curriculum reform. This study constructs three oriented curriculum system mainly from the following six aspects: medicine and humanity, patient and doctor, innovation and practice, medical foundation and clinical application, teachers and learning promotion, quality control and teaching evaluation. The reference to teaching innovation of integrated curriculum will be provided.
4.The treatment of periprosthetic femur fracture after hip arthroplasty

Objective To explore the treatment methods of periprosthetic femur fracture after hip arthroplasty.Methods 9 patiens with periprosthetic femur fracture after hip arthroplasty were selected.According to Vancouver classification,there were 1 case in A type,5 cases in B1 type,2 cases in B2 type,1 case in C type.One case were treated by nonoperative method and the other 8 cases were treated by operative method,including 5 cases treated by memory alloy embracing fixator and internal fixation,3 cases treated by long stem prosthetic replacement and iliac bone graft.Results 8 cases were followed up for 8 to 21 months,average 14.2 months.All fractures were united well with good alignment and internal fixation failure except one prosthesis loosing was observed.Conclusion For periprosthetic femur fracture after hip arthroplasty,Vancouver classification methods include the location and stability of the fracture,prosthesis loosening,and the femur in bone mass is importance to the clinical treatment.As to A type fracture,prosthesis is stable,and the conservative treatment can be choosed.As to B1 and B2 type fracture,the aggressive surgical treatment can be choosed based on patients' general condition.If prosthesis loosening after artificial hip arthroplasty,the patients should treated with long stem prosthetic replacement.
5.Comparison between PTVE of TH glue embolization through gastroesophageal and percutanenous and treatment of esophageal varices by variceal ligation

Objective To study the possibility and effects of fixed TH glue embolization through gastroesophageal and percutanenous. Methods Forty-six patients of bleeding esophageal varices were divided randomly into two groups: in treatment group:twenty-five patients treated with TH glue (?-cyanoacrylate) embolization PTVE+PSE. Absolute alcohol-cilia steel ring-TH glue ,but in control group, 21 patients of esophageal varices by variceal ligation. Analysis the percentage of rebleeding, death rate and improvement rate of liver functions. Results In the treatment group,the rebleeding of gastroesophageal varices was 8.7%, the percentage of death was 4.3%, the function of liver improved by 87.0% . In the control group, the percentage of rebleeding was 28.6%, death rate was in 19.1% and liver function improved by 48.0%. Conclusion PTVE+PSE fixed TH glue embolization is safe, less destroyed. Compared with dense edoscopic ligation, DEVL, the advantage is lessre bleeding, less death rate and higher liver function improvement rate. It is an effective method for gastroesophageal varices.
7.Effect of Low Frequency Ultrasound on Carotid Plaque

Objective To observe the effects of low frequency ultrasound on carotid artery plaque and artery stenosis. Methods 156 patients with carotid atherosclerosis were divided into treatment group (n=80) and control group (n=76). The control group was administered routine medicine, while the treatment group accepted low frequency ultrasound therapy in addition. The size and shape of carotid artery plaque, severity of stenosis and the level of lipid were observed before and after treatment, and the side-effects were recorded. Results The intima-media thickness (IMT), diameter of plaque, plaque score decreased after treatment in both groups, and decreased more in the treatment group than in the control group (P<0.05); while the frequence of moderate stenosis and severe stenosis was less (P<0.05). The levels of low density lipoprotein- cholesterol and total cholesterol decreased in both groups after treatment (P<0.05), and decreased more in the treatment group than in the control group (P<0.05). No serious side-effect was observed. Conclusion Low frequency ultrasound can reduce the atherosclerotic plaques in carotid artery and relieve the stenosis.
8.Mitochondria-mediated Apoptosis in Human Lung Type Ⅱ Alveolar Epithelial-like A549 Cells by Paraquat

Objective To investigate the apoptosis mechanism induced by paraquat (PQ) in human type Ⅱ alveolar epithelial-like A549 cells.Methods A549 cells were cultured in vitro.The cells in the experimental group were exposed to various concentrations of PQ (50,100,150,and 200 μmol/L),while those in the control group were cultured in RPMI1640 medium.After treatment for 24 and 48 h,the cell survival rate was assessed by MTT assay.Morphological changes in the nuclei were observed by Hoechst 33258 fluorescence staining.Cellular apoptosis and mitochondrial transmembrane potential were assayed by flow cytometry.The activities of caspase-3 and caspase-9 were assayed by spectrophotometry.Western blotting was used to analyze the expression of proteins in the Bcl-2 family,such as Bcl-2,Bcl-xL,Bax,and Bak.Results PQ exhibited significant anti-proliferative activity in A549 cells.PQ-treated A549 cells were subjected to Hoechst 33258 staining.The hallmarks of apoptosis were detected,and the degree of apoptosis increased.Mitochondrial membrane potential was decreased,the levels of active caspase-3 and caspase-9 increased,the expression of Bcl-2 and Bcl-xL was decreased,and the expression of Bax and Bak was increased.These effects occurred in concentration-and time-dependent manners.Conclusion PQ efficiently induced intracellular apoptosis through the mitochondrial pathway in A549 cells.
9.Cryopreserved allogenic osteochondral pillars transplantation for cartilage defects

CONCLUSION: Cryopreserved allogenic osteochondral pillars transplantation can repair small full-thickness articular cartilage defects. The chondrocytes are alive in short time, and they can secret cartilage matrix without obvious rejection. It has similar efficacy in histology with autogenic osteochondral pillar transplantation.
10.MnTMPyP Reduces Paraquat-Induced Lung Epithelial-Like Cell Injury by Inhibiting Oxidative Stress and Endoplasmic Reticulum Stress

Objective To investigate the protective effect and underlying mechanism of the superoxide dismutase mimic, manganese (Ⅲ) tetrakis (1-methyl-4-pyridyl) porphyrin pentachloride (MnTMPyP), on paraquat (PQ) -induced lung epithelial-like cell injury. Methods Alveolar epithelial-like cells (A549) were pretreated with 10 μmol/L of MnTMPyP for 1.5 h and then cultured with or without PQ (750 μmol/L) for 24 h. Cell survival was determined using the MTT assay. Reactive oxygen species (ROS) production and Ca2+ levels were measured using flow cytometry. Glutathione reductase (GR) activity was determined using spectrophotometry. Expressions of the endoplasmic reticulum (ER) stress proteins, glucose regulatory protein 78 (Grp78) and C/EBP homologous protein (CHOP), were measured using Western blotting. Results Cell viability and GR activity were decreased, but ROS production, cytoplasmic Ca2+ levels, and expressions of Grp78 and CHOP were all increased in the PQ group compared to those in the control group. There were no statistically significant changes in the MnTMPyP group. Cell viability and GR activity were increased, while ROS production, cytoplasmic Ca2+ levels, and expressions of Grp78 and CHOP were all significantly reduced in the MnTMPyP group compared to those in the PQ group. Conclusion MnTMPyP effectively reduced PQ-induced lung epithelial-like cell injury, and the underlying mechanism is related to antagonism of PQ-induced ER stress and oxidative stress.
