1.Analysis of results of technique competition for parasitic disease diagnosis in Class A tertiary hospitals in Jiangxi Province,2015

Objective To understand the capability of parasitic disease diagnosis among professionals from the clinic labora?tory of Class A tertiary hospitals in Jiangxi Province. Methods The teams that took part in the competition were formed from 20 Class A tertiary hospitals,with 2 contestants per team. The competition contents included written examination and skill opera?tion. The written examination involved the life cycle of parasites,immunological basis,detecting techniques,etiological diagno?sis etc.,and the skill operation involved making and dying thin and thick blood smears,making Kato?Katz’s fecal thick smears, as well as microscopic examinations of smears. Results A total of 40 participants took part in the competition. Their total aver?age score was 97.3±22.4 with the pass rate of 15.0%,in which the mean score of theoretical knowledge was 56.6±12.8 with the pass rate of 52.5%,and the mean score of skill operation was 40.8±12.4 with the pass rate of 5.0%. In the written examination, the scoring rate of the life cycle of soil?transmitted helminths was the highest(90.0%),and the rate of the basic knowledge of food?borne parasites was the lowest(31.5%). Both the pass rates of blood smear making and examination were higher than those of Kato?Katz’s thick smears,and the differences were statistically significant( both P<0.05). The detection rates of Plasmodi?um vivax,Plasmodium falciparum,Plasmodium ovale and negative slides of the contestants were 38.8%,45.0%,35.0%and 25.0% respectively. As to the microscopic examination of helminth eggs,the detection rate of Trichuris trichiura egg was the highest(87.5%),and the rate of the Sparganum mansoni egg was the lowest(2.5%). The scores of Kato?Katz’s thick smear making and examination of the contestants from the provincial level hospitals were higher than those from the city level hospitals (both P<0.05),and the score of those from teaching hospitals in Kato?Katz’s thick smear making was higher than that of those from non?teaching hospitals(P < 0.05). Conclusions The technique level of professionals from the clinic laboratory of the Class A tertiary hospitals cannot meet the needs of the diagnosis of parasitic diseases in Jiangxi Province ,which should arouse the attention of the health authorities and hospitals.
2.Effects of nurse-led discharge planning on discharge readiness and ostomy psychological adaptation in patients with enterostomy

Objective:To investigate the intervention effects of nurse-led discharge planning on discharge readiness and ostomy psychological adaptation in patients with enterostomy.Methods:A total of 70 patients with enterostomy admitted from January 2019 to October 2020 in Jiangsu Province Hospital of Chinese Medicine were enrolled in the present study. They were assigned to experimental group and control group according to the admission time, there were 35 cases in each group. There was one case lost in the control group in the end. The control group received the routine care, while the experimental group implemented nurse-led discharge planning. The Chinese version of Readiness for Hospital Discharge Scale (RHDS) and Ostomy Adjustment Inventory (OAI) were adopted as indicators, the intervention effects was compared between the two groups.Results:On discharge, disease knowledge, coping ability, personal status and expected support dimension scores and total scores in RHDS were (58.06 ± 12.54) , (23.57 ± 3.73), (24.29 ± 3.48) , (30.40 ± 5.25), (136.31 ± 14.32) points in the experimental group, which were significantly higher than those in the control group (49.57 ± 13.28), (18.63 ± 4.97), (22.37 ± 4.28), (26.11 ± 5.66), (101.68 ± 13.04) points, and the differences were statistically significant ( t values were 2.05-4.91, all P<0.05) . On discharge and 1 month after discharge, the persistent worry, acceptance, positive attitude towards life dimension scores and total scores in OAI were (21.34 ± 2.72) , (13.29 ± 2.36), (15.26 ± 3.24), (49.89 ± 5.70) points and (22.03 ± 3.45), (12.49 ± 3.10), (15.09 ± 3.06), (49.60 ± 5.55) points in the experimental group, which were significantly higher than those in the control group (19.35 ± 2.98) , (11.56 ± 2.79), (13.26 ± 3.15), (44.18 ± 5.63) points and (19.91 ± 3.34), (10.76 ± 2.80), (12.24 ± 3.25), (42.91 ± 4.76) points, the differences were statistically significant ( t values were 2.42-5.36, all P<0.05). Conclusions:Nurse-led discharge planning can promote discharge readiness and ostomy psychological adaptation in patients with enterostomy.
3.Application of fast-track surgery in perioperative nursing of patients with percutaneous nephrolithotomy lithotomy with upper-pole access

Objective To explore the effectiveness of fast-track surgery ( FTS) in perioperative nursing of patients with percutaneous nephrolithotomy lithotomy with upper-pole access (PCNL).Methods Seventy-six kidney stone ( KS) patients with PCNL in our hospital from November 2011 to December 2013 were chosen and randomly divided into the control group and the intervention group , each with 38 cases.The control group received the routine nursing , and the intervention group received the FTS nursing intervention .The hospital stays, the costs of treatment , the score of BI index of living scale and the incidence of complication were recorded and the patients ’ satisfaction were investigated at the end of nursing in two groups .Results The hospital stays, the costs of treatment, the score of BI index of living scale were respectively (8.8 ±2.3) d, (10 980.6 ±966.3)yuan and (84.6 ±9.4) in the intervention group, and were better than (12.4 ±3.0)d, (14 315.3 ±1 537.3) yuan and (68.9 ±7.3) in the control group, and the differences were statistically significant (t =4.682, 9.473, 6.741, respectively;P <0.05).The incidence rate of complication in the intervention group was 10.5% including 2 cases of postoperative fever and 2 cases of postoperative pain , and was better than 31.6% including 4 cases of severe bleeding , 6 cases of postoperative fever , and 2 cases of postoperative pain in the control group , and the difference was statistically significant (χ2 =5.067, P<0.05). The patients’ satisfaction was 94.7%in the intervention group, and was high than 78.9%in the control group, and the difference was statistically significant (χ2 =4.145,P<0.05).Conclusions FTS nursing intervention adopted in KS patients with PCNL during perioperative period can effectively reduce the incidence of complications , and be accepted by the patients , and which is worthy of further promotion in clinical practice .
4.Application of comprehensive nursing intervention in patients with non-muscle invasive bladder transitional cell carcinoma

Objective To find out the efficacy of postoperative care of patients with non-muscle invasive bladder transitional cell carcinoma and to provide effective interventions for the treatment and rehabilitation of patients after surgery .Methods 109 patients with non-muscle invasive bladder transitional cell carcinoma were randomly divided into the observation group (59 cases) and the control group (50 cases).The control group was cared with the measures included postoperative general nursing intervention , bladder irrigation, intravesical chemotherapy , postoperative complication nursing .Based on these nursing interventions , the observation group was treated with integrated care measures .Patients’ quality of life, mental state and depression were assessed by the Short Form 36 ( SF-36 ) , Self-rating Anxiety Scale ( SAS ) and Self-rating Depression Scale ( SDS), respectively.Nursing satisfaction was investigated by self-made questionnaire. Results Two groups of patients were successfully operated .The postoperative follow-up period was ( 26 .1 ±7.8) months.Recurrence rate of the observation group and the control group were 20.3%(12/59) and 18.0%(9/50), respectively, showed no significant difference (χ2 =1.237,P>0.05).Postoperative incidence of adverse events of observation group and the control group were 25.4% (15/59) and 30.0% (15/50), respectively, showed no significant difference (χ2 =2.301,P>0.05).The SF-36 scores of TH, RP, MH, VT, RE, SF for the observation group were significantly higher ( t =4.596,5.028,5.137,5.918,7.009, 6.402, respectively;P<0.05) than those in the control group .After integrated care measures , the SDS and SAS scores of both groups were significantly lower than before (t=5.891,4.013, respectively;P<0.05).And the SDS and SAS scores in the observation group were significantly lower than those in the control group ( t=3.815,4.202, respectively;P<0.05).Satisfaction with nursing for the observation group was 93.2%(55/59) and 72.0% (36/50) for the control group.There was a significant difference (U =4.716,P <0.05). Conclusions Integrated care measures can effectively improve the quality of life , mental status, and nursing satisfaction of patients with non-muscle invasive bladder transitional cell carcinoma after surgery .
5.Rapid Identification of Gastrodiae Rhizoma with Different Sulfur Fumigation Levels Based on Ultra-fast Gas Phase Electronic Nose

ObjectiveIn order to find a fast odor-based method for the identification of sulfur fumigated Gastrodiae Rhizoma, an ultra-fast gas phase electronic nose technology was used to identify the odors of different degrees of sulfur fumigated Gastrodiae Rhizoma decoction pieces. MethodHeracles NEO ultra-fast gas phase electronic nose was employed to collect gas chromatograms of unsulfured and sulfured with different degrees of Gastrodiae Rhizoma decoction pieces, gas chromatograms were performed under programmed temperature (initial temperature of 40 ℃, 0.2 ℃·s-1 to 60 ℃, and then 4 ℃·s-1 to 250 ℃), the sample volume was 5 mL, the incubation temperature was 65 ℃ and incubation time was 35 min. Kovats retention index and the AroChemBase database were used for qualitative analysis, and stoichiometric analysis was performed on this basis. Principal component analysis (PCA), discriminant factor analysis (DFA) and partial least squares-discriminant analysis (PLS-DA) models were established to identify the Gastrodiae Rhizoma decoction pieces with different degrees of sulfur fumigation. ResultAccording to the comparative analysis of AroChemBase database, there were significant differences in the odor characteristics of sulfur fumigated and non-sulfur fumigated Gastrodiae Rhizoma, cyclopentane, acetone and heptane might be the odor components to distinguish the degree of sulfur fumigation in Gastrodiae Rhizoma decoction pieces. The identification index of PCA model was 81, the accumulative discriminant index of the discriminating factors was 92.09% in DFA model, the supervisory model interpretation rate of PLS-DA model was 0.963 and the predictive ability parameter was 0.956, indicating that PCA, DFA and PLS-DA models could well distinguish Gastrodiae Rhizoma decoction pieces with different sulfur fumigation degrees. ConclusionHeracles NEO ultra-fast gas phase electronic nose can be used as a rapid method to identify and distinguish Gastrodiae Rhizoma decoction pieces with different levels of sulfur fumigation. Meanwhile, it can provide a rapid, simple and green method and technology for identification of traditional Chinese medicine decoction pieces by sulfur fumigation.
