1.Effect of socioenvironmental therapy on improving emotional distress for heroin addicts accepting compulsory isolation and labour camp rehabilitation

Objective To evaluate the psychological intervention ettectiveness ot socioenvironmental therapyfor heroin addicts accepting compulsory isolation.Methods Totally 100 male heroin addicts accepting compulsory isolation and Labour Camp Rehabilitation were selected and randomly assigned to socioenvironmental therapeutic group (ST group,50 cases) and control group (CT group,50 cases).A two-month socioenvironmental therapy was selectively proceeding in ST group by a same psychological team,while CT group still received conventional rehabilitation.During the 6-month follow-up,self-rating anxiety scale(SAS),self-rating depression scal (SDS) and self-made questionnaires were used every 3 months,to statistically estimate psychological status of subjects.Results Before treatment,no statistically significant difference between the two groups;after treatment,SDS score of CT group was 56.16 ± 9.12,and was lower than that in ST group(48.28-± 9.55) (P < 0.01) ; SAS score of ST group was 46.28 ± 7.78,and was lower than that in CT group (50.72 ± 8.32) (P < 0.01).In the 3-month follow-up evaluation,depression,anxiety scores in CT and ST group were significantly lower,the difference was statistically significant (P < 0.01) ; 6 months after treatment,depression,anxiety score showed was significant difference between two groups(P< 0.01).Conclusion Group psychotherapy especially socioenvironmental therapy is quick and long-acting in improving the psychological status of heroin addicts in aspects of depression and anxiety levels,which is distinctly evidenced by the short-term and long-term curative results.
2.Bidirectional Mendelian randomization study on gut microbiota and constipation

Objective:
To investigate the causal relationship between gut microbiota and constipation using Mendelian randomization (MR) method.
Methods:
Genetic variation data of gut microbiota were obtained from the MiBioGen Consortium database. The genetic variation data of constipation were sourced from the IEU Open GWAS database. A forward MR analysis was performed using the inverse-variance weighted (IVW) method with 2 511 SNPs associated with gut microbiota as instrumental variables, and constipation as study outcome, and a reverse MR analysis was performed with 13 microbiota-associated SNPs as instrumental variables and gut microbiota as study outcome. The heterogeneity was assessed using the Cochran test, reverse causation of SNP were examined using MR Steiger test, and the horizontal pleiotropy was assessed using the MR-PRESSO test and MR-Egger regression. In addition, the robustness of the results was verified with the leave-one-out.
Results:
Forward MR analysis results showed that an increased abundance of genus Coprococcus1 driven by host genetics was associated with a decreased risk of constipation (OR=0.791, 95%CI: 0.709-0.884), and increased abundance of phylum Bacteroidetes driven by host genetics was associated with an increased risk of constipation (OR=1.240, 95%CI: 1.102-1.394). Cochran test detected no heterogeneity (both P>0.05), MR Steiger test was not revealed reverse causation of SNP, and neither the MR-PRESSO test nor the MR-Egger regression revealed horizontal pleiotropy of instrumental variables (all P>0.05), and the leave-one-out method confirmed the robustness of results. Reverse MR analysis showed no association between gut microbiota and constipation (both P>0.05).
Conclusion
Genus Coprococcus1 and phylum Bacteroidetes in the gut microbiota are associated with constipation.

5.Role of nucleotide oligomerization domain 2-related intestinal barrier injury in ileum pouch inflammation

Objective:To observe and analyze the function of intestinal barrier in patients with pouchitis, and to explore the role of nucleotide oligomerization domain 2 (NOD2) in ileal pouchitis, so as to provide new ideas for the pathogenesis of pouchitis.Methods:From January 2011 to December 2016, the clinical pathological data of patients with ileal pouch-anal anastomosis who underwent pouch mucosa biopsy at the Endoscopy Center of General Hospital of Tianjin Medical University were retrospectively analyzed. According to the disease activity index of pouchitis, patients were divided into pouchitis group (20 cases) and non-pouchitis group (30 cases). In addition, UC patients who did not undergo surgery were selected as the control group (10 cases). The intestinal structure of patients of the pouchitis group and non-pouchitis group was observed under transmission electron microscope. The positive expression rates of occludin, alpha human defensin and NOD2 in the control group, non-pouchitis group and pouchitis group were detected and calculated by immunohistochemistry. Levene test, independent sample t test and Spearman correlation analysis were used for statistical analysis. Results:Under the transmission electron microscope, the tight junction epithelial structure and microvilli of intestinal mucosal of patients in the pouchitis group were severely injured. The results of immunohistochemistry showed that the positive expression rates of occludin, alpha human defensin and NOD2 in the intestinal mucosa of patients in the pouchitis group were all lower than those of the control group and non-pouchitis groups ((19.3±0.4)% vs. (84.0±0.3)% and (77.9±0.5)%; (60.0±1.3)% vs. (85.0±0.1)% and (77.3±0.4)%; (46.1±1.6)% vs. (72.0±0.7)% and (60.7±0.5)%), and the differences were statistically significant ( t=-8.451, -7.514, -3.943, -2.970, -5.115 and -2.982, all P<0.05). The correlation analysis showed that the expression level of NOD2 was positively correlated with occludin and alpha human defensin ( r=0.671 and 0.628, both P<0.01). Conclusions:Intestinal barrier function is impaired in patients with ileal pouchitis, and NOD2-related intestinal barrier injury may play an important role in the pathogenesis of ileal pouchitis.
6.Clinical analysis of emergency rescue in whole-course integrated of abdominal injury with multitrauma

Objective To explore the effect of emergency rescue in whole-course integrated of abdominal injury with multitrauma. Methods Two hundred and eleven cases of abdominal injury with multitrauma from January 2004 to December 2006,before the emergency rescue in whole-course integrated, treating severe trauma 102 cases (group A),from January 2007 to December 2009,after the emergency rescue in whole-course integrated, treating severe trauma 109 cases (group B) .retrospectively analyzing and comparing the clinical effect between the two groups. Results In group A,injury severity score (ISS) was (24.7 ± 6.2) scores, 14 cases died, the mortality was 13.7% ,the time in emergency room and the time to operation room averaged (1.8 ± 0.2), (2.3 ± 0.3) h. While in group B, ISS was (26.6 ± 7.4) scores,6 cases died, the mortality was 5.5%, the time in emergency room and the time to operation room averaged (1.1 ± 0.1), (1.5 ± 0.2 ) h. ISS indicated no significant difference between the two groups ( P > 0.05 ), the mortality, the time in emergency room and the time to operation room indicated significant difference between the two groups (P < 0.05 ).Conclusion Emergency rescue in whole-course integrated of abdominal injury with multitrauma is an effective way to improve the curative rate.
7.Changes of arginase Ⅰ expression in rat deep venous thrombosis models

BACKGROUND:Studies in recent years have demonstrated that arginase Ⅰ contribute to the process of numerous cardiovascular diseases,however,most of studies focus on arteries,few regarding venous diseases.OBJECTIVE:To explore the changes of arginase Ⅰ expression in rat traumatic deep venous thrombosis models,and to analyze the possible function of arginase Ⅰ in deep venous thrombosis formation.METHODS:Totally 100 Sprague Dawley rats were randomly divided into the control and model groups.Traumatic deep venous thrombosis models were established by clamping the femoral vein and immobilizing the bilateral hind limbs (hip spica cast fixation),and assigned into initial thrombosis,peak thrombosis and non-thrombosis groups according to different observing time points and pathophysiological situations of thrombosis.Whole blood RNA of each group was extracted,and the change of arginase I expression in blood cells of each group was detected by real-time PCR.RESULTS AND CONCLUSUON:Expression of arginase Ⅰ in the peak thrombosis group was significantly increased compared with other 3 groups (P < 0.01).There were no significances among control,initial thrombosis and non-thrombosis groups (P > 0.05).The finding demonstrated that arginase Ⅰ is closely related to deep vein thrombosis formation.
8.Value of lymph node regional sorting in radical colorectal cancer surgery

Objective:To evaluate the value of lymph node regional sorting in radical colorectal cancer surgery.Methods:Between Jan 2021 and Mar 2022, A total of 307 colorectal cancer patients undergoing radical surgery at the Tianjin Medical University General Hospital were retrospectively included and assigned to either the lymph node regional sorting group (123) or no-sorting group (184) according to the lymph node examination methods.Results:There was a significant difference between the two groups in the number of total lymph nodes and negative lymph nodes (26.74±13.08 vs. 16.80±4.87, t=9.381, P<0.001; 25.65±13.25 vs. 16.04±4.87, t=8.979, P<0.001). The number of positive lymph nodes of lymph node regional sorting group increased compared with no-sorting group (1.09±2.28 vs. 0.76±1.34, t=1.589, P=0.113). A total of 52 cases in the lymph node regional sorting group showed metastasis, of which 10 showed lymph node skip metastases. There was no significant difference in tumor location and TNM stage between skip and non-skip lymph node metastasis patients. Conclusions:The lymph node regional sorting increase the number of examined lymph nodes. The method can determine the positive lymph node distribution and ensure the accuracy of staging.
9.Impact of personalized psychological intervention based upon acceptance and commitment therapy on pediatric liver transplantation donors and spouses

Objective:To explore the effect of personalized psychological intervention based upon acceptance and commitment therapy on paternal donors and spouses after pediatric liver transplantation.Methods:From July 2017 to November 2019, a total of 160 parental donors and spouses undergoing pediatric liver transplantation were selected as research objects.Through a random number table, 40 donors and 40 liver donor spouses were assigned into control and experimental groups.The control group received routine psychological intervention while the experimental group had personalized psychological intervention with acceptance and commitment therapy based upon conventional psychological intervention.Symptom checklist-90, numeric rating scale, chronic pain acceptance questionnaire-8, length of stay, hospitalization cost, complication rate, initial postoperative ambulation time and hospital satisfaction were employed as evaluation tools.Results:After interventions, SCL-90 score, pain assessment score and initial ambulation time of donor group were significantly lower than those of control group( P<0.05). Hospitalization satisfaction and chronic pain acceptance were significantly higher than those of control group( P<0.05). SCL-90 score was significantly lower in liver spouse observation group than that in control group and hospitalization satisfaction higher than control group( P<0.05). Conclusions:Personalized psychological intervention based upon acceptance and commitment therapy can help parental donors and spouses to improve their mental health, help donors to recover sooner and boost hospitalization satisfaction.
10.Study of a fetus with confined placental mosaicism for trisomy 2 in conjunct with fetal uniparental disomy and a literature review.

OBJECTIVE:
To carry out genetic analysis for a fetus with confined placental mosaicism (CPM) for trisomy 2 (T2) in conjunct with fetal uniparental disomy (UPD).
METHODS:
Amniocentesis and chromosomal karyotyping was carried out for a pregnant woman with a high risk for chromosome 2 anomalies indicated by non-invasive prenatal testing (NIPT). Single nucleotide polymorphism array (SNP-array) and trio-whole exome sequencing (Trio-WES) were carried out. Ultrasonography was used to closely monitor the fetal growth. Multifocal sampling of the placenta was performed after delivery for copy number variation sequencing (CNV-seq).
RESULTS:
The fetus was found to have a normal chromosomal karyotype. SNP-array has revealed multiple regions with loss of heterozygosity (LOH) on chromosome 2. Trio-WES confirmed the presence of maternal UPD for chromosome 2. Ultrasonography has revealed intrauterine growth restriction and oligohydramnios. Intrauterine fetal demise had occurred at 23+4 weeks of gestation. Pathological examination had failed to find salient visceral abnormality. The placenta was proved to contain complete T2 by CNV-seq.
CONCLUSION
T2 CPM can cause false positive result for NIPT and may be complicated with fetal UPD, leading to adverse obstetric outcomes such as intrauterine growth restriction, oligohydramnios and intrauterine fetal demise.
