1.Causes and Treatment of Recurrent Laryngeal Nerve Injury During Thyroidectomy

Objective To investigate the causes and treatment of recurrent laryngeal nerve (RLN) injury during the operation of thyroidectomy. MethodsClinical data of 48 patients that RLN were injured during thyroidectomy in and out of our hospital from Jun. 2003 to Mar. 2007 were reviewed. ResultsNo patient died while operation and staying in hospital. There were 47 cases of unilateral RLN injury, 1 case of bilateral RLN injury; 21 cases (43.7%) were injured because of suture or scar adhesion, 13 cases (27.1%) were partly broken with formed scar, 14 cases (29.2%) were completely cut off; The locations of RLN injuries were closely adjacent to the crossing of the inferior thyroid artery and RLN in 13 cases (27.1%) and 35 cases (72.9%) were within 2 cm below the point of RLN entering into throat. The injured RLN were repaired surgically in 43 cases, among which 39 cases’ phonation and vocal cord movement were restored completely or had their vocal cord movement recovered partly; There were only 4 cases that the phonation and vocal cord movement were not recovered. Another 5 cases that did not take any repair did not recovered naturally. ConclusionThe location of most RLN injuries caused by mechanical injury during thyroid surgery is closely adjacent to the entrance of RLN into throat. Early nerve exploratory operation should be performed once the RLN is injured, and the method of repair should be decided according to concrete conditions of injury.
2.Reoperation after local excision of differentiated thyroid cancer

Objective To study the choice of initial operative approach for thyroid nodule and the necessity of reoperation after local excision of differentiated thyroid cancer.Methods Clinical data of 138 patients with differentiated thyroid cancer after local tumor excision had reoperation at our hospital during a period of four year were retrospectively reviewed.All the reoperations were bilateral thyroidectomy and cervical lymph node excision.Results There were 76.8% patients with residual cancer in thyroid and cervical lymph node.There were 23 patients(16.7%) with recurrent laryngeal nerve injury that included 22 cases with unilateral and 1 case with bilateral injury after the initial operation,and 19 of the 23 patients with recurrent laryngeal nerve injury recovered after nerve repair.After thyroid reoperation,3 cases(2.2%) had recurrent laryngeal nerve injury,2 cases(1.4%) had partial parathyroid gland injury,2 cases(1.4%) had superior laryngeal nerve injury,but there were no cases of esophageal injury or postoperative bleeding.Conclusions The incidence of residual cancer is high after local excision for differentiated thyroid cancer,therefore,reoperation with bilateral thyroidectomy and cervical lymph node excision is necessary.
3.Clinical application of resonant liposuction

Objective To study the method of operation and effect of the application of resonant liposuction in the body figure reconstruction. Methods Resonant liposuction was employed in 136 cases, including local fat deposit in some part of the body. Operation's safety, mircotrauma, speediness, and effect after operation were observed. Results The follow-up for 6 to 24 months showed that the reconstructed body figure was very satisfactory. Conclusion Resonant liposuction is an ideal method to reconstruct the body figure owing to its safety, miniinvasion, easy performance and efficacy.
4.Preliminary study of Geranium Strictipes R.Knuth anticarie activity and anticaries mechanism

Objeetive:To screen the different extracts from Geranium strictipes with anticaries activity,and to study the effect of the active extract on the production of acid and polysaccharides of oral cariogenic bacteria.Methods:Chlorhexidine and Galla Chinensis were used as the positive controls.Tube double dilution method was used to study the extracts of Geranium strictipes on the growth of Streptococcus mutans (S.mutans) and Actinomyce viscosus (A.viscosus).First,the activity of 5 extracts fom Geranium strictipes against S.mutans,A.viscosus was detected.Their minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) were obtained.The effects of 4 lower concentrations(< MIC) of the active extract on the production of oral bacterial acid polysaccharides were examined.Results:The effect of n-butanol extraction part of Geranium strictipes was the strongest among all the extracts against the two kinds of bacteria in the inhibition of bacteria growth,bacterial acid production and polysaccharides production,but weaker than that of Chlorhexidine and Galla Chinensis.Conclusion:N-butanol extraction part of Geranium strictipes may be a natural and effective anti-caries agent.
5.Optimizing the establishment of endometriosis model in rats by orthogonal experiment

Aim To probe into the best conditions for the establishment of endometriosis model in rats by the orthogonal experiment method.Methods Autotransplantation was used to establish the endometriosis model in rats by the orthogonal experiment method in which four main factors were considered to optimize the condition of model,according to the volume of heterotopic endometrium.Results The para ligament of uterus was the best place for transplanting.Both dissecting muscular layer and injecting estrin played an important role in the establishment of endometriosis model in rats. The value of four factors decreased progressively according to the injection of estrin,the dissection of muscular layer,the direction of endometrium implantation and oestrus.Conclusions By optimizing the condition for the establishment of endometriosis model in rats by orthogonal experiment,the volume of heterotopic endometrium was improved.A technology platform for pharmacodynamics research was thus offered.
6.The antibacterial activity of Shenning Suppository in vitro and vivo

AIM: The bacteriostatic effects of Shenning suppository in the treatment of vaginitis were investigated in vivo and in vitro. METHODS: The minimal inhibitory concentration(MIC)against ten clinical pathogens isolates was determined by tube dilution method,minimal bactericide concentration(MBC) was measured by the agar dilution method.The model of vaginitis from Candida albicans in the rabbit was investigated. RESULTS: Shenning suppository could inhibit and/or destroy Staphylococcus aureus,Candida albicans,ect in virto.It also improved the pathological change in the model of vaginitis from Candida albicans in the rabbit. CONCLUSION: Shenning Suppository could treat bacterial and mycotic vaginitis

AIM To investigate the effect of oral roxithromycin on serum concentrations at steady state and its pharmacokinetics of aminophylline in New Zealand rabbits. To investigate the role of ligustrazine on relaxation of the isolated rabbit corpus cavernosum tissue in vitro, the effects of ligustrazine on the corpus cavernosum were observed by using experimental method of smooth muscle strips. Concentration-responses to phenylephine (PE) and KCl were recorded. The results showed that ligustrazine concentration-dependently depressed the contraction response of smooth muscle strips induced by PE. The maximum percentage relaxation of cavernosal strips by ligustrazine was 74.1% +/- 6.2% (compared with control: 21.9% +/- 5.6%, P < 0.01). Ligustrazine concentration-dependently reduced the amplitude of the contraction induced by cumulative doses of PE or KCl, shifted the cumulative concentration response curves of PE and KCI to the right and depressed their maximal responses. It was concluded that ligustrazine could significantly relax the cavernosal muscle contraction induced by PE in vitro. The results suggested that ligustrazine inhibited calcium ion influx.
