1.Epidemiological survey of hepatitis B virus serologic markers among university freshmen in Beijing

To analyze the test results of five serum markers of hepatitis B virus (HBV) among university freshmen from 2007 to 2012.Among a total of 10 827 freshmen,there were 5 021 freshmen from 2007 to 2009 with all five serum markers.The overall ratio of HBsAg positive was 4.5%,4.8%,4.8% and 4.0% for each year.And it had no statistically significant difference.The overall ratio of single-HBsAb positive was 34.8%,37.4%,29.0% and 37.9% for each year and the figure of 2008 was lower than those of 2007 and 2009 (P < 0.01).The overall ratio of five markers all negative were 58.3%,55.4%,63.9% and 55.8% for each year and the figure of 2008 was higher than those of 2007 and 2009 (P < 0.01).And 34.5% of HBsAg positive freshmen were HBsAg(+),HBeAg (+),HBcAb (+) and 44.2% HBsAg(+),HBeAb(+),HBcAb(+).The prevalence ratio of HBsAg for males was higher than that for females (5.1% vs.3.2%,P <0.01).Among 5 806 freshmen tested during 2010-2012,522(9.0%) had a serum level of alanine transaminase (ALT) > 40 U/L,including HBsAg positive (n =12),single anti-HBs positive (n =213) and five negatives (n =288).Approximate 50% university freshmen have no anti-HBs indicating a low level of immunity.And the students should acquire the knowledge of hepatitis B,improve the ratio of vaccination,strengthen the monitoring and prevent its spread through concerted measures.
2.Relationship betwent surum gastrin and the expression of gastrin in the cancer cell with the clinical behavior of patients with colorectal carcinoma cancer

Objective To determine the serum gastrin level, the expression of gastrin in colorectal carcinoma cells and observe the ultrastructure of gastrin secretory granule in colorectal carcinoma cells,in order to explore the relationship between gastrin and clinical behavior of colorectal carcinoma. Methods The serum gastrin and gastrin expression in colorectal carcinoma tissues of the 53 cases were examined by using radio-immunity analysis(RIA), immunohistochemistry and immunoelectron microscopic technique. Results Compared with control group,the preoperative level of serum gastrin in colorectal group was significantly increased, especially in well-differentiation adenocarcinoma . In the tissue of colorectal carcinoma, the gastrin expression rate was 56.6%. The expression rate of well-differentiated adenocaroinoma was higher than that in moderate and poor differentiation adenocarcinoma. Immunoelectron microscopy showed that the granules of protein A-gold (PAG) could be seen in different electro- density secretion granules in carcinoma cells, in intercellular space and on the surface of membrane of microvillus.Conclusions The level of serum gastrin and the expression of gastrin in cancer tissues in colorectal carcinoma patients are increased. The colorectal carcinoma cells may synthesize and secrete gastrin themselves, which may be correlated with clinical behavior of colorectal carcinoma.
3.Efficacy and safety of Chushizhiyang ointment for the treatment of mild atopic dermatitis in infants: a multicenter clinical study

Objective To evaluate the efficacy and safety of Chushizhiyang ointment for the treatment of mild atopic dermatitis in infants.Methods A multicenter,randomized,open,active-controlled clinical trial was conducted.A total of 204 infants with atopic dermatitis were enrolled and randomly divided into 2 groups to be topically treated with Chushizhiyang ointment (test group,n =103) and hydrocortisone butyrate cream (control group,n =101),respectively,for 2 weeks.The improvement of eczema area and severity index (EASI) scores and quality of life was evaluated at 7 days and 14 days after the treatment,so was the incidence of adverse events and adverse reactions.Results Ninety-eight infants in the test group and 101 in the control group were included in the full analysis set,which revealed that the disease severity significantly decreased after the treatment in both groups.The EASI scores at the baseline and on days 7 and 14 were 2.47 ± 4.04,0.92 ± 1.25 and 0.39 ± 1.04 respectively in the test group,as well as 2.13 ± 2.01,0.85 ± 1.58 and 0.45 ± 1.65 respectively in the control group.Furthermore,the test group and control group both showed that EASI scores on days 7 and 14 significantly decreased compared with those at the baseline (the test group:T =-1 666,-1 793,respectively,both P < 0.001;the control group:T =-1 951,-1 941,respectively,both P < 0.001).No significant differences in EASI scores at the baseline or on days 7 and 14 were observed between the test group and control group (all P > 0.05).The response rates in the test group and control group were 47.96% (47/98) and 55.44% (56/101) respectively on day 7,as well as 79.59% (78/98) and 84.16% (85/101) respectively on day 14,and there were also no significant differences between the two groups (both P > 0.05).The adverse reactions mainly manifested as erythema,itching and scaling in the test group,as well as hypopigmentation,telangiectasia,scaling and hyperpigmentation in the control group.No significant difference in the incidence of adverse events was found between the test group (2.9%,3/103) and control group (6.9%,7/101).Conclusion Chushizhiyang ointment shows definite efficacy for mild atopic dermatitis in infants with good safety and tolerability,and can be a teatment option for mild atopic dermatitis in infants.
4.Pre-hospital analysis of death factors and management on 62 cases treatment of severe traffic accident trauma

Objective To explore the method and management of pre-hospital care and raise the level of traffic injuries in pre-hospital care by summarizing the clinical features of death patients with severe tragic accident trauma Methods The basic data of 62 death patients with severe traffic accident trauma was analyzed according to death report statistics of severe traffic accident trauma from January 1st,2005 to December 31th,2008 Results Brain injury death accounted for most of traffic accident trauma death.The mortality rate of brain injury in the wounded wag 8.28%(13/157),but of asphyxia and hemorrhagic shock was 2.55%(4/157),3.18%(5/157)respectively in 2005.With the development of treatment and rescued in time, the mortality rate reduced to 6.11%(11/180),0,0.56%(1/180)in 2008.Conclesions It should be trying to shorten the time of pre-hospital care for pafients with trsffic accident trauma,especially in patients combined with hemorrhagic shock,asphyxia,severe brain injury.It is concluded that rapid and effective pre-hospital care can significantly reduce death rate and self-help or each other rescue training would also be effective to reduce mortality.
5.A multi-center clinical research of diagnostic value of serum gastrin-17 combined with pepsinogen for gastric cancer

Objective To evaluate the diagnostic value of gastrin?17( G?17) and pepsinogen( PG) for gastric cancer. Methods A multicenter cross?sectional study of patients with continuous stomach discomfort from four centers including Changhai Hospital Affiliated to Second Military Medical University, the First Hospital Affiliated to Anhui Medical University, Qinghai Provincial People′s Hospital and the First Hospital Affiliated to Zhejiang University of Chinese Medicine from May 2014 to September 2015 was conducted. Before gastroscopy, fasting serum gatrin?17 and pepsinogen were analyzed by enzyme?linked immunosorbent assay(ELISA). The efficacy of G?17 and PG were evaluated according to endoscopic and pathological results. Results Based on the results of the pathological diagnosis, 1 122 cases were enrolled and divided into chronic atrophic gastritis group ( 548 cases ) , chronic non?atrophic gastritis group ( 370 cases), and gastric cancer group(204 cases). Serum G?17 and PGⅡ levels significantly increased(P<0?05) and PGR significantly decreased( P<0?05) in gastric cancer group compared with other groups. There was no significant difference in PGⅠlevel among three groups. The cut?off value of G?17 to diagnose gastric cancer was 7 pmol/L. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of G?17 for gastric cancer were 59?31%, 70?59%, 68?54%, 30?95% and 88?65% respectively. The cut?off value of PG Ⅰ/PG Ⅱ( PGR ) to diagnose gastric cancer was 7. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of PGR for gastric cancer were 41?18%, 83?01%, 75?40%, 35?00% and 86?39% respectively. The cut?off value of PGⅡto diagnose gastric cancer was 10 μg/L. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of PGⅡfor gastric cancer were 73?53%, 53?05%, 56?77%, 25?82% and 90?02% respectively. If G?17>7 pmol/L and PGR<7 was regarded as the cut?off value of diagnosis of gastric cancer, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 25?00%, 91?29%, 79?23%, 38?93%and 84?56%respectively. If G?17>7 pmol/L and PGⅡ>10μg/L was regarded as the cut?off value, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 48?04%, 79?74%, 73?98%, 34?51% and 87?35% respectively. If PGR<7 and PGⅡ>10 μg/L was regarded as the cut?off value, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 33?82%, 84?86%, 75?58%, 33?17% and 85?23% respectively. Based on logistic regression analysis of the independent variables of high serum G?17 value(>7 pmol/L), low serum PGR value(<7) and high serum PGⅡvalue(>10 μg/L), their OR value were 2?592, 2?237 and 1?864 respectively, and high serum G?17 value showed the highest risk of gastric cancer. Conclusion High serum G?17 and PGⅡ, low PGR are indicators of gastric cancer. Combination of G?17 and PGR has the best diagnostic value for gastric cacer. Gastric cancer can be screened in large scale by combining G?17 and PGR in order to improve the early diagnostic rate of gastric cancer and reduce the mortality of gastric cancer in our country.
6.Relationship between complement-3 and progression of early cerebral infarction

ObjectiveTo investigate the relationship between complement-3(C3) and progression of early cerebral infarction.Methods168 consecutive patients with acute cerebral infarction within the first 24 hours from onset were recruited.Fasting venous blood sample was drawn on the morning of the second or the third day after admission.C3 and other clinical chemistry variables were measured.The relationship of C3 and progressing stroke(PS) was analyzed by Logistic regression.Results68 cases(40.48%) were considered as PS.Level of serum C3 in PS patients was 1.28±0.20 g/L higher than in those with non-PS(1.16±0.25 g/L,P=(0.000)).A higher C3 accompanied a higher tendency of prevalence of PS(P=0.001).Logistic regression analysis showed that C3 was significantly correlated with PS(OR for each 1.0 g/L=7.27;95% CI=1.52~36.39; P=0.013).ConclusionElevated serum C3 may play an important role in the prevalence of PS.
7.Comparison of desonide 0.05% cream versus hydrocortisone butyrate cream in the treatment of infantile atopic dermatitis: a multicenter, randomized, parallel-group, controlled clinical study

Objective To evaluate the efficacy and safety of desonide 0.05% cream in the treatment of mild to moderate infantile atopic dermatitis (AD).Methods A multicenter,randomized,openlabeled,active-controlled clinical trial was conducted.A total of 120 infants with AD were enrolled from Department of Dermatology,Beijing Children's Hospital,Capital Medical University,Children's Hospital of Chongqing Medical University and Children's Hospital of Fudan University between December 2016 and November 2017.These patients were randomly divided into 2 groups to be topically treated with desonide 0.05% cream (test group,n =61) and hydrocortisone butyrate 0.1% cream (control group,n =59),respectively.After the treatment for 1,2 and 3 weeks,the response rate,improvement of eczema area and severity index (EASI) and infants' dermatitis quality of life (IDQOL) were evaluated,and adverse events and reactions were recorded.Statistical analysis was carried out with SAS 9.4 software using confidence interval method and non-inferiority test for the comparison of response rates between the test group and control group,two-sample t test for the comparison of quantitative data between two groups,paired t test for comparing pre-and post-treatment EASI scores and IDQOL scores.If the data were not normally distributed or there was heterogeneity of variance,Wilcoxon rank sum test was used for the comparisons of pre-and posttreatment EASI scores and IDQOL scores before and after the treatment,and chi-square test was used for the comparison of unordered categorical data between the test group and control group.Results At weeks 1,2and 3 after the initial treatment,analysis of the full analysis set (FAS) showed that 46 (86.79%),49(92.45%) and 51 (96.23%) patients in the test group received improvement respectively,and 36 (83.72%),35 (81.40%) and 41 (95.35%) in the control group received improvement respectively.The non-inferiority test showed that the response rate in the test group was non-inferior to that in the control group (non-inferiority boundary value was-0.15).At the baseline,1,2 and 3 weeks,the EASI scores in the test group were 4.57 ± 3.19,0.72 ± 0.89,0.45 ± 0.87 and 0.18 ± 0.40 respectively,and the EASI scores in control group were 4.50 ± 3.29,1.03 ± 1.81,0.62 ± 0.85 and 0.28 ± 0.82 respectively.Paired Wilcoxon rank sum test showed that the EASI scores in the test group and control group significantly decreased after the treatment for 1,2 and 3 weeks compared with those at the baseline (the test group:T =-715.5,-9.4,-715.5,respectively,all P < 0.001;the control group:T =-437.5,-473.0,-472.0,respectively,all P <0.001).However,Wilcoxon rank sum test revealed that there was no significant difference in the EASI scores between the two groups at any of the above time points (P > 0.05).Paired Wilcoxon rank sum test showed that IDQOL scores in the test group significantly decreased at the above time points after the treatment compared with those at the baseline (all P < 0.001),while Wilcoxon rank sum test revealed that there was no significant difference in the IDQOL score between the test group and control group (all P >0.05).Adverse reactions were observed in 12 (19.7%) patients in the test group,and 10 (16.9%) in the control group,and there was no significant difference in the adverse reactions between the two groups (x2 =0.029,P > 0.05).Conclusion The efficacy of desonide 0.05% cream in the treatment of mild to moderate infantile AD is equivalent to that of hydrocortisone butyrate 0.1% cream,so desonide 0.05% cream can be a treatment option for mild to moderate infantile AD.
8.Influencing factors of depression in patients with orthopedic trauma surgery

Objective To investigate the factors affecting the depression of patients with orthopedic trauma surgery and nursing countermeasures .Methods A total of 210 patients were chosen in our hospital from January 2012 to January 2013.The Self-rating Depression Scale (SDS), Social Support Rating Scale (SSRS) and general information questionnaires were used in the survey .Results A total of 190 cases of patients finished the investigation.83 patients (43.68%) did not had depression, 58 patients (30.53%) had mild depression, 29 patients (15.26%) had moderate depression, 20 (10.53%) patients had severe depression .The reason for depression in patients with univariate and multivariate Logistic regression analysis showed that self -financed medical care , diagnosis and treatment of worry , fear poor prognosis , bad attitude and other aspects of health care were the main factors in patients with depression , but a high degree of social support in patients with depression was the protective factors .Conclusions The incidence of depression in patients with orthopedic surgery was high.But intensive care from the medical staff , support from their families and intimate care from friends can help patients get rid of depression after orthopedic surgery .

10.Correlation analysis of survival time in patients with advanced breast cancer and the scores of Care Evaluation Scale in cancer patients

Objective:To explore the value of Care Evaluation Scale (CES) in cancer patients for predicting survival time in patients with advanced breast cancer.Methods:From November 2018 to June 2020, the convenient sampling was used to select 140 patients with advanced breast cancer who were treated in Qingdao Central Hospital Affiliated to Qingdao University and 140 home caregivers as the research objects. Researchers contacted home caregivers of patients with advanced breast cancer to set up a time and place to conduct one-on-one surveys of research objects using the Cancer Patient CES. Pearson correlation was used to analyze the correlation between survival time and CES score in patients with advanced breast cancer. Multiple stepwise regression analysis was used to analyze the influencing factors of CES scores in patients with advanced breast cancer. A total of 140 questionnaires were distributed, of which 133 were valid, and the effective recovery rate was 95.00% (133/140) .Results:Among the 133 patients with advanced breast cancer, 76 were Luminal A, 19 were Luminal B, 12 were ERBB 2+ and 26 were Basal-like. Luminal type A advanced breast cancer patients had the longest survival time, and the differences were statistically significant compared with those of ERBB 2+ and Basal-like advanced breast cancer patients ( P<0.05) . There was no statistically significant difference in survival time between Luminal B type advanced breast cancer and Luminal A, ERBB 2+ and Basal-like advanced breast cancer patients ( P>0.05) . Luminal type A advanced breast cancer patients had the highest CES score, and the differences were statistically significant compared with those of Luminal type B, ERBB 2+ and Basal-like advanced breast cancer patients ( P<0.05) . There was no statistically significant difference in the CES score between Luminal B advanced breast cancer and Luminal A, ERBB 2+ and Basal-like advanced breast cancer ( P>0.05) . Pearson correlation analysis found that the survival time of patients with advanced breast cancer was positively correlated with the CES score ( r=0.892, P<0.05) . The influencing factors of CES empowerment in patients with advanced breast cancer included physical care provided by doctors, mental health care, environment, explanation or instructions by doctors to family members, collaboration and persistence, consideration for family members, availability, cost, explanation or instructions by doctors to patients and physical care provided by nurses. Conclusions:CES in cancer patients can effectively predict the survival time of patients with advanced breast cancer. The molecular types of patients with advanced breast cancer are different and the effect of predicting survival time is also different.
