1.Study of the individualized health management of senile mental disorders

Objective To evaluate the effect of the individualized health management on senile mental disorders.Methods 421 patients with senile mental disorders were randomized into two groups:the intervention group (n =213) received routine antipsychotic drugs treatment,health education and individual health management,and the control group(n=208) received only population health education and routine drug antipsychiatric treatment.Results The Brief Psychiatric Rating Scale (BPRS) scores in two group were significantly higher after treatment than before treatment (all P<0.01).The BPRS score in each period after treatment were significantly higher in intervention group than in control group (all P<0.01).The mini mental state examination (MMSE) score in two treatment groups were significantly higher after treatment than before treatment (all P<0.01).The MMSE score in each period after treatment were significantly higher in intervention group than in control group (all P<0.05).Conclusions The individualized health management in senile mental disorders is not only feasible,but also effective.
2.Study on systolic and diastolic function by myocardial strain rate imaging in patients with hypertrophic cardiomyopathy

Objective To evaluate the systolic and diastolic function in hypertrophic segments by myocardial strain rate imaging in the patients with hypertrophic cardiomyopathy (HCM).Methods Ten patients with HCM in the mid segment of the ventricular septum and 20 age-matched healthy controls were included.The myocardial strain rate (SR) curves in bas,mid and apical segments were respectively acquired on the left ventricular septum longitudinal axis position for all the participants,and the SR curves of endocardial,mid-myocardial,epicardial layers in mid segment were also acquired.The SR values including the peak systolic SR,the peak early-diastolic SR,the peak late-diastolic SR were respectively measured,the left ventricular eject fraction (EF) and the Doppler transmitral filling velocities (E and A values) were also measured.Results SR values measured from the hypertrophic segments were significantly lower than those from the corresponding segments of the septum in control group (P

METHODS:Seventy-five male rats were randomly divided into sham operation group (n=25),ARF group (n=25) and pyrrolidine dithiocarbamat (PDTC) group (n=25),five subgroups at time points of 3 h,6 h,12 h,24 h and 48 h after reperfusion were set up in each group. 4.The Therapeutic Effects of Intratumoral Injection with MHC Class Ⅱ Gene on Tumor-Bearing Mice

Many gene therapy protocols can induce antitumor immunity, however, the ex vivo approach restricts their uses. This sutydy intended to induce antitumor immunity by direct transfer of MHC class Ⅱ gene in vivo. Methods: MHC class Ⅱ gene cDNA was introduced directly into two tumors: P815, (a murine weak immunogenic mas-tocytoma) and B16 (a murine nonirnmunogenic melanoma) to observe the survival rate of the mice. Results: Tumori-genicity of P815 was reduced when MHC class Ⅱ gene was introduced directly into tumors in vivo. Further more, most vaccinated mice could survive after second challenge of P815. Co-injection of MHC class Ⅱ and B7 genes in the B16 also resulted in the tumor grow slowly, while the injection of MHC class Ⅱ gene was not enough to induce effective antitumor responses. Conclusion: The results showed the potential applications of direct transfer of MHC class Ⅱ gene in the treatment of tumor.
5.Antitumor Responses Induced by Recombinant Vaccinia Viruses Expressing p53 Antigenic Peptide and B7

Objective: To explore antitumor responses induced by recombinant vaccinia viruses expressing a p53 antigenic peptide (rVV p53 M) and enhanced effect of recombinant vaccinia viruses expressing costimulatory molecule B7 (rVV B7). Methods: A 135 Cys to Tyr point mutation p53 transduced P815 mastocytoma (P815 mp53) was used as an experimental tumor and the p53 protein as the model of tumor associated antigen. rVV p53 M and rVV B7 were used as vaccine to test their induction of CTL and antitumor immunity. Results: Immunization of BABL/c mice with rVV p53 M could elicit specific CTLs, which could specifically lyse P815 mp53 cells. Immunization of mice with rVV p53 M could survive a part of mice challenged with 1?10 6 P815 mp53. Treatment with rVV p53 M could significantly prolong the survival of tumor bearing mice. Admixture at 1∶1 ratio of rVV p53 M and rVV B7 could enhance antitumor responses induced by rVV p53 M. Conclusion: Immunization with recombinant vaccinia virus expressing antigenic peptide is a useful alternative to peptide based vaccine. Costimulatory molecule B7 can enhance antigenic peptide to induce antitumor responses.

7.Effect of NF - κB inhibitor on renal expression of apolipoprotein M in rats with acute renal failure

The expressions of apoM in cytoplasm and NF - κB p65 in nucleus of renal cortex were detected at the indicated time points. RESULTS: The expression of apoM in ARF group was obviously higher than that in sham operation group ( P <0.01 ) , and two peaks were detected, the first peak was at 6 h after reperfusion, while the second one was from 24 h to 48 h. The tendency of apoM expression in PDTC group was similar to that in ARF group, while the expression in every subgroup was prevalently lower than that in ARF group (P < 0.01). Otherwise, a significant correlation ( r = 0.852, P < 0.01) was found between the expression of apoM and NF -κB p65.CONCLUSION: The results indicate that apoM feasibly take part in the pathogenesis of ARF through the inflammatory reaction mediated by NF - κB.
8.Study of structural and functional change of the left ventricle after transcatheter closure of atrial septal defect using strain rate imaging

Objective To study the changes of left ventricular structure and function after transcatheter closure of atrial septal defect(ASD)by strain rate(SR)imaging.Methods Twenty-eight patients with secundum type ASD were undergone transcatheter closure.Left and right ventricular end-diastolic diameter(LVED and RVED),left ventricular ejection fraction(LVEF),the Doppler transmitral filling velocities(E values and A values)and tricuspid regurgitation pressure were measured respectively at pre-operation,3 days,1 month and 3 months of post-operation.According to tricuspid regurgitation pressure,systolic pulmonary artery pressure(SPAP)was estimated.SR curves in all segments of left ventricular walls were acquired for all patients.The SR values including the peak of systolic SR,early-diastolic SR,late-diastolic SR were measured respectively.ResultsAfter operation,LVED,LVEF,E values and A values increased significantly while RVED and SPAP decreased.Almost all left ventricular walls' SR increased at post-operation except that SR of interventricular septeum(IVS)and left ventricular post wall(LVPW)decreased at 3 days,1 month of post-operation.Conclusions LV systolic and diastolic function are improved after operation,but SR characteristic changes of IVS and LVPW is the one of the most unique characters.
9.Ultrasound-mediated destruction of microbubbles directs SERCA2a gene delivery to rats with myocardial infarction by two-dimensional ultrasound speckle tracking imaging

Objective To evaluate the therapeutic effects of rAAV2-SERCA2a gene targeted to myocardium by the ultrasound mediated microbubbles in rats with myocardial infarction(MI) using speckle tracking echocardiography (STE). Methods Thirty-five male SD rats were divided into Sham group, MI group and gene therapy group. rAAV2-SERCA2a was attached to the surface of SonoVue, sulphur hexafluoride-filled microbubbles. Therapy group animals were infused with those mixed fluids through tail vein. The other two groups were infused with SonoVue microbubbles. These bubbles were infused with simultaneous echocardiography. Four weeks after ultrasound microbubbles destructed,Circumferential strain rate(SRcir), radial strain rate(SRrad) and left ventricular torsion (LVtor) of regional myocardium were measured in short-axis view using speckle tracking imaging software for each animal. Results Compared with the M1 group,there were significant increase in SRcir-S,SRcir-E, SRrad-S,SRrad-E and LVtor in rats of gene therapy group( P <0. 01 ). There was no significant difference between gene therapy group and sham group. Conclusions rAAV2-SERCA2a gene targeted to myocardium by the ultrasound mediated microbubbles could enhance cardiac function of rats with MI.
10.Evaluation value of the levels of peripheral blood CD20+ CD24hi CD38hi regulatory B cells on the prognosis of elderly patients with sepsis

Objective To explore the predicting value of peripheral blood CD20+ CD24hi CD38hi regulatory B cells (Bregs) on the prognosis of elderly patients with sepsis.Methods A prospective study was conducted. Septic patients aged > 65 years old, compliance with diagnostic criteria for Sepsis-3, admitted to emergency and emergency intensive care unit (ICU) of Shanghai East Hospital of Tongji University from April 2016 to February 2017 were enrolled. Procalcitonin (PCT), C-reaction protein (CRP) and lactate (Lac) were routinely measured. According to the worst clinical index value within 24 hours, acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score and sequential organ failure assessment (SOFA) score were recorded. The concentrations of peripheral blood CD20+ CD24hi CD38hi Bregs were measured by flow cytometry at 1, 3 and 7 days after diagnosed in elderly patients. All patients with sepsis were followed up for 28 days and then divided into death group and survival group according to 28-day outcome. The difference of clinic data and Bregs were compared between the two groups. The significant different factors of elderly sepsis patients were analyzed by binary logistic regression analysis. The correlation between Bregs level and other indicators was analyzed by Spearman correlation. The receiver operating characteristic curve (ROC) was used to evaluate the prognosis value of Bregs in elderly patients with sepsis.Results Fifty-eight patients were enrolled in the study, with 38 male and 20 female; age of (79.91±7.97) years; 32 in sepsis group, 26 in septic shock group; 35 deaths, 28-day mortality rate was 60.3%. APACHE Ⅱ score and SOFA score in death group exhibited much higher than that in survival group (APACHE Ⅱ: 18.14±4.52 vs. 14.91±3.56, SOFA: 8.80±4.56 vs. 6.35±3.00, bothP < 0.05), the Bregs was significantly decreased at 1, 3 and 7 days in death group [cells/μL: 0.70 (0.20, 1.40) vs. 1.50 (0.70, 2.20), 0.54 (0.20, 1.00) vs. 1.42 (1.10, 2.12), 0.25 (0.10, 0.50) vs. 0.80 (0.50, 1.00), allP < 0.05]. Correlation analysis showed that the concentrations of peripheral blood Bregs at 1 day in elderly patients with sepsis was negatively correlated with APACHE Ⅱ score (r = -0.351,P = 0.007), and it was not correlated with PCT, CRP, Lac or SOFA score. It was shown by binary logistic regression that Bregs [odds ratio (OR) = 1.865,P = 0.028] and APACHE Ⅱ score (OR = 0.853,P = 0.026) were independent risk factors for elderly sepsis outcome. It was shown by ROC curve analysis that the prognostic value of the levels of Bregs at 1, 3, 7 days and APACHE Ⅱ score were higher in the elderly patients with sepsis, and the area under ROC curve (AUC) and 95% confidence interval (95%CI) were 0.842 (0.647-0.954), 0.770 (0.564-0.911), 0.888 (0.703-0.977), 0.855 (0.661-0.961), respectively, allP < 0.01. The 7-day Bregs was most powerful to predict outcome, when the cut-off value was 0.50 cells/μL, the sensitivity was 72.73% and specificity was 86.67%. Conclusions The level of peripheral blood CD20+ CD24hi CD38hi Bregs could predict the clinical outcome of elderly patients with sepsis.
