1.Effect of natural drying method on umbilical care of neonates

OBJECTIVE To compare the effect on umbilical care of the neonates between the natural dying method and the traditional ethanol disinfection method so as to provide a better method of umbilical nursing for the neo-nates.METHODS A total of 212 healthy neonates who were given birth in the First Medical Center of Chinese PLA General Hospital from Aug.2024 to Nov.2024 were recruited as the research subjects and were randomly divided into the natural drying method with 103 cases and the traditional ethanol disinfection method with 109 cases ac-cording to the method of umbilical care.The time of umbilical cord separation,rate of umbilical bleeding and inci-dence of umbilical secretions were observed and compared between the two groups of neonates.RESULTS There were 16 neonates with the healing time of umbilical cord separation no more than 7 days under the treatment of natural drying method,with 5 cases more than the neonates under the treatment of traditional ethanol disinfection method.The average healing time of umbilical cord separation was 11.69 days under the natural drying method,1.43 days shorter than 13.12 days under the traditional ethanol disinfection method,and there was significant difference(P<0.05).The rate of umbilical bleeding was 5.82%under the natural drying method,a reduction of 0.60%as compared with 6.42%under the traditional ethanol disinfection method;the incidence of umbilical se-cretions was 0.97%under the national drying method,a reduction of 1.78%as compared with 2.75%under the traditional ethanol disinfection method,but there were no significant differences.CONCLUSIONS As compared with the traditional ethanol disinfection method,the natural drying method can shorten the time of umbilical cord separation,reduce the risk of umbilical infection,and reduce the stress from the neonatal nursing.It is worthy to be promoted.
2.Mechanism study of benzyl isothiocyanate combined with sorafenib in the treatment of anaplastic thyroid cancer

Objective To investigate the mechanism of benzyl isothiocyanate(BITC)combined with sorafenib(Sor)in the treatment of anaplastic thyroid cancer(ATC).Methods Two ATC cell lines,8505C and CAL-62,were treated with Sor at concentrations of 0,20,30,40,and 50 μmol/L.The cell survival rate was assessed using CCK-8 assay.The combined dose of BITC and Sor was determined by calculating combination index(CI).CAL-62 and 8505C cells were exposed to 10 μmol/L BITC(BITC group),10 μmol/L Sor(Sor group),or a combination of 10 μmol/L BITC and 10 μmol/L Sor(BITC+Sor group)for 24 hours.The control group was not treated.The effects of Sor and BITC on ATC cell viability were evaluated using the CCK-8 method.Apoptosis was analyzed via flow cytometry.Western blot assay was employed to detect the protein expression levels of LC3B Ⅱ,Beclin-1 and nuclear factor(NF)-κB.Real-time fluorescence quantitative PCR was used to quantify the mRNA levels of LC3B.Additionally,CAL-62 cells were subcutaneously injected into mice to establish tumor xenograft model.Mice were treated with BITC(100 mg/kg,intraperitoneal injection),Sor(30 mg/kg,intragastric administration)or a combination of BITC and Sor every other day for 21 days.Finally,the expression levels of LC3B Ⅱ,Beclin-1 and NF-κB in tumor tissue were analyzed by Western blot assay.Results Sor significantly inhibited the viability of CAL-62 and 8505C cells in a concentration-dependent manner.The combination index(CI)was 0.710 at BITC 10 μmol/L and Sor 10 μmol/L,indicating a moderate synergistic effect between the two drugs.In both 8505C and CAL-62 cells,compared with the control group,treatment with BITC or Sor resulted in the decreased cell viability,as well as reduced expression levels of Beclin-1 and NF-κB proteins(P<0.05),and the apoptosis rate,LC3B mRNA and LC3B Ⅱ protein expression levels were significantly increased(P<0.05).When BITC and Sor were combined,the cell viability,Beclin-1 and NF-κB protein expressions were further reduced compared to either drug alone,while the apoptosis rate,LC3B mRNA and LC3B Ⅱ protein expression levels were significantly elevated(P<0.05).In the mouse xenograft tumor model,the BITC+Sor group exhibited increased LC3B Ⅱ expression,along with decreased Beclin-1 and NF-κB expression levels,tumor volume and tumor mass compared to the BITC or Sor groups(P<0.05).Conclusion The combination of BITC and Sor can inhibit ATC cells through NF-κB pathway,induce autophagy and promote apoptosis in vitro and in vivo.
3.Ultrasonic-assisted Deep Eutectic Solvent Extraction of Ziziphusjujube Polysaccharide and Its Hypoglycemic Activity

Objective To optimize the extraction process of Ziziphusjujube polysaccharides and study their hypoglycemic effect in vitro.Methods Using deep eutectic solvent as the extraction solvent,the optimal extraction condition for Ziziphusjujube polysaccharides was selected using single factor experiments and Box Behnken experiments.The selected factors were water content,solid-liquid ratio,and ultrasound time.At the same time,the content of Ziziphusjujube polysaccharideswas determined,and the in vitro hypoglycemic effect of polysaccharides and the hypoglycemic ability of different parts of Ziziphusjujube were investigated.Results After Box-Behnken response surface optimization,the optimal extraction condition for Ziziphusjujube polysaccharides was as follows:after ultrasonic treatment for 41.68 minutes,solid-to liquid ratio of 1∶20,and the water content in the deep eutectic solvent was 19.96%,under these conditions,the Ziziphusjujube polysaccharide content reach(571.31±3.84)mg·g-1 α-amylase inhibition rate of Ziziphusjujube polysaccharides was 76.08%,the content of polysaccharide in whole fruit of Ziziphusjujube was the highest,while the pulp exhibited the strongest hypoglycemic activity.Conclusion The optimized process can providea areference for the development in industry and clinical application of Ziziphusjujube.

5.Development of a diagnostic model for severe coronary artery stenosis using resting echocardiography

Objective:To evaluate the diagnostic performance of resting echocardiography in detecting severe coronary artery stenosis.Methods:A total of 136 patients with suspected coronary artery disease(CAD)who presented to Sichuan Provincial People's Hospital between January 2021 and December 2024 were prospectively enrolled. All patients underwent both coronary computed tomography angiography(CCTA)and transthoracic echocardiography within one week. Based on CCTA results,the patients were divided into non-severe stenosis group( n=78)and severe stenosis group( n=58). Echocardiographic parameters including left atrial maximum volume(LAVmax),left ventricular global longitudinal strain(GLS),left ventricular longitudinal strain of endo-myocardium,mid-myocardium,epi-myocardium(LSendo,LSmid,LSepi),early diastolic mitral inflow velocity(E),early diastolic mitral annular velocity of the lateral and septal walls(e'),and E/e' were measured. Predictive factors for severe coronary stenosis were identified using LASSO regression,and a nomogram model was developed via multivariate Logistic regression. Model performance was evaluated using ROC curves,calibration curves,and decision curve analysis. Results:Multivariate Logistic regression analysis revealed LSendo,LAVmax,and E/e' as independent predictors of severe coronary artery stenosis. The nomogram constructed based on these predictors achieved an area under the curve of 0.798(95% CI=0.723-0.873),with sensitivity and specificity of 0.756 and 0.759,respectively. Conclusions:The resting echocardiography-based nomogram model demonstrates good diagnostic efficacy for severe coronary artery stenosis. It may serve as a noninvasive tool to assist in risk stratification and clinical decision-making in patients with suspected CAD.
6.Develop and assessment of a predictive model for the first-course efficacy of acute myeloid leukemia

Objective:To identify the relevant factors for the first-course remission of acute myeloid leukemia (AML) and to develop a predictive model as well as assess its predictive capability.Methods:Clinical data of 749 patients newly diagnosed with AML admitted to the Department of Hematology, the First Affiliated Hospital, Zhejiang University, School of Medicine from January 1, 2019, to April 30, 2023, were collected and randomly divided into training and validation sets. Multivariate logistic regression analysis was conducted to determine variables associated with complete remission in the first course of induction therapy, and a predictive model was established based on these variables. The receiver operating characteristic (ROC) curve of the predictive model was plotted, and the area under the curve (AUC) was calculated.Results:The indicators predicting the first remission course included peripheral blood white blood cell count during onset, CBF::MYH11 fusion gene, CEBPA bZIP region mutation, myelodysplastic syndrome-related gene mutation, and induction chemotherapy regimen selection as independent factors for the first remission course. The model’s area under the training and validation curves was 0.738 (95% CI: 0.696-0.780) and 0.726 (95% CI: 0.650-0.801), respectively. The Hosmer-Lemeshow test results yielded P-values of 0.993 and 0.335, respectively. Conclusion:In this study, the developed model demonstrates a strong predictive capability for the efficacy of the first course of patients with AML, providing valuable guidance to clinicians in assessing patient prognosis and selecting appropriate treatment strategies.
