1.Risk factors of contrast-induced nephropathy in patients with coronary diseases treated with interventional therapy

Objective To investigate the risk factors of contrast-induced renal nephropathy(CIN)in patients with coronary disease treated with interventional therapy and summarize the nursing strategies. Methods One hundred and twenly CHD patients treated with PCI were selected.Logistic regression analysis was done to screen out the risk factors of CIN. Result The main risk factors included renal inadequacy,advanced age,diabetes and contrast dosage(all P<0.05). Conclusion Risk assessment before PCI,rational use of contrast and preventive nursing measures are essential for the prevention and reduction of CIN.
2.Evaluation on drill method to prepare for healthcare-associated infection outbreak

Objective To evaluate the drill method to prepare for healthcare-associated infection (HAI)outbreak, and strengthen the control of HAI outbreak.Methods According to two cases of surgical site infection (SSI)re-ported by a neurosurgical department on January 16,2014,HAI outbreak emergency drill was started,SSI occurred from January 2012 to January 2014 were analyzed,specimens of air,object surface and hands of health care workers (HCWs)in the SSI-related operating room and neurosurgical intensive care unit(NSICU)were taken and analyzed. Results In July 2013,3 cases of SSI occurred in neurosurgical department (1 case was suspected of SSI,and didn’t per-form cerebrospinal fluid culture),SSI didn’t exceed 2 cases in the other months,and SSI outbreak couldn’t be confirmed. 16 specimens of air,object surface and hands of HCWs in operating room were taken,microbial detection results were all qualified.13 specimens of air,object surface and hands of HCWs in NSICU were taken,qualified rate was 61.54%;and mould was detected from 2 air specimens.24 disposable objects of neurosurgical department were performed bacterial cul-ture,3 were positive,and all were qualified after repeated detection.Conclusion HAI outbreak drill is helpful for HAI management professional personnel to grasp the method of HAI outbreak investigation method,improve the correct hand washing of surgeons and nurses,strengthen the standard operating procedure,and ensure the safety of pa-tients.
3.Effect of sleep on brain clearing metabolites: thinking based on lymphoid system

Disruption of the structure of regular sleep is a common cause of neurodegenerative diseases such as Alzheimer′s disease and Parkinson′s disease, and its pathogenesis may be related to the deposition of waste products in the central nervous system. The glymphatic pathway, which is essentially a periarterial cerebrospinal fluid inflow pathway and peripheral venous clearance pathway, is functionally dependent on interstitial bulk flow coupling supported by aquaporin-4 on the astrocyte end-foot, also known as the lymphoid glial system. The glymphatic pathway, which removes waste proteins from the brain, is active primarily during sleep, and sleep quality declines with age, while the glymphatic pathway system also deteriorates with age, suggesting a relationship between sleep disturbances and symptom progression in neurodegeneration, and glymphatic system as a link closely links the two. The interaction of sleep, aging, metabolic waste and glymphatic pathway reticulation provides new clues to the pathogenesis of central nervous system degenerative diseases, and the glymphatic pathway may constitute a new target on treatment. The recent research progress on the effects of sleep and sleep disorders on the circulation of the glymphatic system, and proposes the possibility of sleep intervention to slow down the impairment of the lymphoid system function or even restore the function of the lymphoid system and thus improve the disease development process were reviewed in this paper.
4.Integral classification method of thoracic deformity

Objective To introduce an integral classification method of thoracic deformity. Methods From March 2015 to March 2017, 869 patients with thoracic deformity were observed. The patients were classified according to the traditional classification method at first, and then were classified according to the integral classification method. According to the standard plane of the normal chest wall, the chest deformities were classified into types Ⅰ and Ⅱ: those below the plane belonged to type Ⅰ, and those above the plane belonged to type Ⅱ. Then two types were divided into subtypes Ⅰ-a, Ⅰ-b, Ⅰ-c, Ⅰ-d, Ⅰ-e, Ⅰ-f, Ⅰ-g and Ⅱ-a, Ⅱ-b, Ⅱ-c, Ⅱ-d according to the specific characteristics of malformation. Results There were 840 patients included in the classification system, accounting for 96.7% of the total patients, and the remaining 29 (3.3%) were complicated malformations, which could not be accurately described by using a single classification type. Compared with the traditional classification methods, patients with pectus excavatum and pectus carinatum were divided into different subtypes. Poland syndrome did not exist any longer, and Dove chest alone became the type Ⅱ-c. Conclusion Integral classification method is a simple and practical classification method. Since the method directly depends on the characteristics of malformation which is related to the operation, it plays a guiding role in the operation. But its superiority needs to be further confirmed.
5.SDP three dimensional classification method for pectus excavatum

Objective To introduce a novel classification method for pectus excavatum. Methods A retrospective review of 569 operative patients with pectus excavatum between January 2015 and September 2017 in our hospital was performed. The malformations were described and classified from three dimensions: symmetry (abbreviated as S), depth (abbreviated as D) and position (abbreviated as P). The final integral was calculated according to the deformity index, so that the severity of the deformity could be evaluated. All patients underwent Nuss or modified Nuss surgery. Results All pectus excavatum deformities were classified into 24 types. The most common type was S1D2P1, accounting for 27.2% of the total patients followed by S1D2P2, S1D1P2 and S2D2P2, accounting for 12.0%, 11.6% and 11.4% of all patients respectively. S1D3P3, S1D3P4 and S2D3P3 were not found in our patients. According to the integral, 321 patients were mild, 23 severe, and the rest were moderate. The appearance of all patients improved after the operation. Among them, 405 patients were implanted with a plate, 159 with two plates and 5 with three plates. The length of hospital stay was 5-12 d, with an average of 7.3±1.5 d. Conclusion This classification method is a more comprehensive classification, but its scientific and practical characteristics need to be further confirmed.
6.Experience with Wang procedure for treatment of pectus excavatum in young children.

OBJECTIVE:
To review the experience with Wang procedure for treatment of pectus excavatum in young children.
METHODS:
The clinical data of 21 children with a mean age of 3.3 ± 1.1 years (ranging from 1.5-6 years) undergoing Wang procedure for pectus excavatum were analyzed. A longitudinal incision (1 to 2 cm) was made in the front of the xiphoid, and two tunnels were created using steel bars beneath the muscles on two sides of the chest wall. The fibrous tissue between the diaphragm and the sternum was dissociated, and the steel wires were sutured through the deformed chest wall. After the steel bar was placed in the tunnels, the wires were pulled and fixed in the middle of the bar, and the incision was sutured.
RESULTS:
All the operations were performed using 3 wires and 1 steel bar. The operation time was 25 to 51 (38.1 ± 9.6) min with an intraoperative bleeding volume of 5 to 10 (7.1±1.5) mL. The time of hospitalization of the patients ranged from 6 to 10 days (mean 8.1±1.3 days). In all the patients, the incision healed smoothly without serious pain or obvious complications. All the patients were followed up for 1 to 13 months after the operation. During the follow- up, no recess recurred and no such complications as bar displacement or transposition occurred. According to the evaluation criteria after pectus excavatum operation, 13 cases had a total score of 9, and 8 had a total score of 8. The overall effect was satisfactory, and there were no cases rated as basically satisfactory or unsatisfactory.
CONCLUSIONS
Wang procedure is a good option for treatment of pectus excavatum in young children.

7. Relevance analysis of nurses' willingness to stay in operation room with job stressor, job burnout and work-family conflict

Objective:
To explore and analyze the correlation between nurses' willingness to stay in the operating room and job stressor, job burnout and work-family conflict.
Methods:
A total of 128 nurses from Mianyang Central Hospital, Third People′s Hospital of Mianyang, Mianyang People′s Hospital were selected as the research objects. The questionnaire of nurses' retention intention, the scale of nurses' job stressor, the scale of nurses' occupational fatigue and the scale of work stress conflict were used to investigate and analyze the correlation between nurses' retention intention and work stressor, occupational fatigue and family conflict.
Results:
The average retention intention of the nurses surveyed in this study was (3.16±0.71), and the retention intention of the nurses was in the middle level. Operating room nurses' total score of work stress was (95.27±15.28). Among them, the scores of time allocation and workload, nursing specialty and work were the highest (2.86 ± 0.73) and (2.75 ± 0.64), respectively. The highest score of personal achievement was (33.26 ± 8.39). The total score of work family conflict of nurses in operating room was (58.32 ± 11.29), and the total score was (3.09 ± 0.58). The average score of work family conflict based on time was the highest, which was (3.91 ± 0.64), which was the main source of work family conflict of nurses. Pearson correlation analysis showed that nurses' intention to stay was negatively correlated with work pressure, job burnout and work family conflict (

Objective:To investigate the normal range of exhaled nitric oxide (FeNO) in 6-18-year-old children in China, so as to provide a data base for the establishment of FeNO standards for Chinese children.Methods:A multi-center study was conducted on 5 949 children aged 6-18 (3 101 males and 2 848 females) in 16 pro-vinces of 7 administrative districts in China.According to the technical standard recommended by American Thoracic Society/European Respiratory Association, FeNO was measured, and the relationship of FeNO with the sex, age, height, weight, body mass index and region was discussed.Results:The geometric mean FeNO value of Chinese children aged 6-18 was 14.1 ppb, and its 95% confidence interval (skewness distribution) was 1.0-38.2 ppb.The geometric mean FeNO values of children aged 6-11 and 12-18 were 13.1 ppb and 15.7 ppb, respectively, and their 95% confidence intervals (skewness distribution) were 1.0-38.1 ppb and 2.0-38.2 ppb.For children at and under 11 years old, FeNO decreased with age, with a mean decline of 1 ppb per year.The multiple linear regression results suggested that there was a significant correlation between FeNO and age for children aged 6-11, and FeNO of children aged 12-18 was significantly correlated with the gender, height, and region(all P<0.01). Conclusions:FeNO values of Chinese children and adolescents in this study are higher than those obtained by the previous study conducted from 2010 to 2012.For children aged 12-18, 16 ppb is recommended as the clinical cut-off point.For children at or under 11 years old, the influence of age on FeNO should be considered, and the cut-off point of FeNO decreases by 1 ppb as the age is reduced by one year.
9.Study on the bioequivalence of Palbociclib capsules in human

OBJECTIVE To evaluate the bioequivalence of a single oral administration of two palbociclib preparations in healthy subjects under fasting and fed conditions. METHODS Twenty-four healthy subjects (fasting test) and twenty healthy subjects (fed test) were enrolled and divided into two groups. A single-center, open-label, single-dose, two-formulation, two- period, two-sequence and crossover trial was designed. The subjects in the two groups were given the test preparation (domestic Palbociclib capsules) or the reference preparation (original Palbociclib capsules) orally under fasting or fed conditions respectively followed by a 14-day washout period. The blood samples were collected at different time points before and after treatment. After pretreatment, the mass concentration of palbociclib in vivo was determined by high-performance liquid chromatography-tandem mass spectrometry with palbociclib-d8 as the internal standard. SAS V9.4 software was used to calculate the pharmacokinetic parameters and evaluate the bioequivalence. RESULTS Under fasting condition, the cmax of the test preparation and the reference preparation were (71.4±18.1) and (73.8±19.0) ng/mL; AUC0-t were (1 754±412) and (1 793±448) h·ng/mL; AUC0-∞ were (1 851±456) and (1 887±478) h·ng/mL, respectively. Under fed condition, the cmax of the test preparation and the reference preparation were (78.4±18.3) and (81.9±21.7) ng/mL; AUC0-t were (1 905±375) and (1 932±318) h·ng/mL; AUC0-∞ were (2 027±411) and (2 050±342) h·ng/mL, respectively. The 90%CI of the geometric mean ratio of the above parameters was within the acceptable range (80.00%-125.00%). Under fasting and fed conditions, there were 20 and 16 adverse events in 9 and 8 subjects, respectively, but no serious adverse event was observed. CONCLUSIONS Under the fasting and fed conditions, the test preparation and the reference preparation of Pibociclib capsules are bioequivalent and have comparable safety.
10.Trends in the prevalence of common chronic diseases among workers of automotive industry in 2019 - 2021

Objective To analyze the prevalence, annual trends, and co-morbidity trends of common chronic diseases among workers in a large automotive industry from 2019 to 2021, and to provide a scientific basis for the health management of workers in the automotive industry. Methods The health examination data of workers in a large automotive industry from 2019-2021 were analyzed. Trends in the prevalence of chronic diseases and co-morbidities were analyzed using Join Point software and trend χ2 test. Results The prevalence of metabolic syndrome, hyperuricemia, and fatty liver in the 2019 – 2021 health checkups of workers in this enterprise increased at an average rate of 9.27%, 11.35%, and 3.99% per year, respectively. The prevalence of metabolic syndrome, hyperuricemia, and fatty liver in male workers showed an increasing trend at an average rate of 7.05%, 9.25%, and 2.91% per year, respectively. The prevalence of metabolic syndrome in female workers showed an increasing trend at an average rate of 20.76% per year. The prevalence of metabolic syndrome, hyperuricemia and fatty liver was on the rise in the age groups ≤ 29 years old and 40 – 49 years old. The proportion of metabolic syndrome and its co-morbidity with one or two common chronic diseases showed an increasing trend. Conclusion The prevalence and co-morbidity of common chronic diseases in this enterprise are generally on the rise. The enterprise should focus on health education and preventive care for chronic diseases among workers aged ≤ 29 and 40 – 49 years old and male workers and control the annual increasing trend of metabolic syndrome among female workers and workers in the age group ≤ 29 years.
