1.Value of preoperative neutrophil-to-lymphocyte ratio as a prognostic predictor after radical resection of bladder cancer for muscle-invasive bladder cancer

Objective To investigate the value of preoperative neutrophil?to?lymphocyte ratio(NLR) as a prognostic predictor after radical resection of bladder cancer for muscle?bladder cancer. Methods Two hundred and fifty?two cases consecutive muscle?invasive bladder cancer patients treated with total excision radi?cal bladder at the Rology Surgery Department of Affiliated Hospital of Weifang Medical College from January 2006 to March 2010 were retrospectively analyzed,and all patients were diagnosed bladder cancer by pathological examination. Based on the cut?off value of 2. 7,all the patients were divided into two groups:a low NLR(<2. 7,n=142) group and a high NLR(≥2. 7,n=110) group. None accepted neoadjuvant therapy. Results The 5?year overall survival rate was 75. 2% in patients of the low NLR group and 53% in the high NLR group,the difference was significant(P=0. 005). Univariate analysis showed that NLR(P=0. 002),tumor size(P=0. 035),T stage (P=0. 0022),tumor grade(P=0. 04),hydronep?hrosis or no(P=0. 031),Lymph node metastasis or no(P=0. 003) were associated with survival. Cox multivariate analysis revealed that NLR was independent risk factors of prognosis(HR:2. 586;95%CI:1. 256-2. 851;P=0. 039). Conclusion Preoperative NLR≥2. 7 may be a convenient biomarker to predict patients with a poor prognosis after radical cystectomy for muscle?invasive blad?der cancer.
2.Role of adenosine A1 receptors in hippocampal neurons in cognitive dysfunction caused by isoflurane anesthesia in aged mice

Objective To evaluate the role of adenosine A1 receptors in hippocampal neurons in the cognitive dysfunction caused by isoflurane anesthesia in aged mice.Methods Sixteen male adenosine A1 receptor gene knockout homozygote mice (gene knockout mice) and 16 male wild-type mice,aged 18-22 months,weighing 27-32 g,were studied.Each type of mice was randomly divided into 2 groups (n=8 each) using a random number table:control group (group C) and isoflurane anesthesia group (group Ⅰ).Mice inhaled 1.4% isoflurane in 100% O2 for 2 h in group Ⅰ,and 100% O2 for 2 h in group C.All the mice underwent Morris water maze test at 24 h after isoflurane or O2 inhalation.After the test,the mice were sacrificed and the hippocampal tissues were harvested to determine the number of β-amyloid1-42 (Aβ1-42) plaques (using immunohistochemistry) and expression of phosphorylated tau (p-tau) protein,and 2B subunit-containing N-methyl-D-aspartate receptors (NR2B) (by Western blot analysis).Results Compared with group C of wild type mice,the escape latency was significantly prolonged,the number of Aβ1-42 plaques was enlarged,the expression of p-tau protein was up-regulated,and the expression of N R2B was down-regulated in group Ⅰ of wild type mice.Compared with group Ⅰ of wild type mice,the escape latency was significantly shortened,the number of Aβ1-42 plaques was decreased,the expression of p-tau protein was down-regulated,and the expression of NR2B was up-regulated in group Ⅰ of gene knockout mice.There was no significant difference in the parameters mentioned above between group Ⅰ and group C of gene knockout mice.Conclusion Adenosine A1 receptors in hippocampal neurons mediate isoflurane anesthesia-induced cognitive dysfunction in aged mice,and the mechanism may be related to promotion of deposition of Aβ,phosphorylation of tau protein and inhibition of activities of NR2B.
3.Curcumin stimulates proIiferation of rat neural stem cells by inhibiting glucocorticoid receptors

OBJECTIVE To investigate the effect of curcumin on proliferation of neural stem cells (NSCs) of rats and the mechanism. METHODS NSCs derived from the forebrain of rat E15 embryos were cultured in vitro and identified by neuroepithelial stem cell protein ( nestin and SOX2) staining. NSCs were treated with curcumin 0.1, 0.5, 2.5, 12.5 and 62.5 μmol.L-1 for 24 h, respectively. The cyto-toxicity was estimated by measuring the release of lactate dehydrogenase(LDH). Cell viability and prolif-eration were analyzed respectively by MTT and BrdU assay. The mRNA expression levels of glucocorti-coid receptor (GR), Stat3, Notch1 and p21 were detected by qRT-PCR. The protein expression levels of total GR, Stat3 and phosphorylated Stat3 were measured by Western blotting. RESULTS The primary neural stem cells were identified as NSCs. Curcumin 12.5 and 62.5 μmol.L-1 had cell cytotoxicity( P<0.05). Cell viability assay indicated that curcumin 0.5 and 2.5 μmol.L-1 enhanced NSCs viability( P <0.05), but in 62.5 μmol.L-1 group the cell cytotoxicity was inhibited(P<0.05). Curcumin 0.1, 0.5 and 2.5 μmol.L-1 increased NSCs proliferation ( P < 0. 05), whereas 12. 5 and 62. 5 μmol.L-1 caused a decrease in NSCs proliferation(P<0.05). The mRNA expression level of GR in 0.5 μmol.L-1 group was significantly reduced( P<0.05). Western blotting analysis revealed that the protein expression of GR, Stat3 and p-Stat3 was inhibited by curcumin in 0.5 μmol.L-1 group(P<0.05). CONCLUSION Curcumin stimulates NSCs proliferation, possibly by inhibiting GR mRNA and related protein expression.
4.Radical pancreaticoduodenectomy via artery approach combined with portal vein and splenic vein reconstruction

For patients with malignant pancreatic cancer combined with vascular invasion,radical pancreaticoduodenectomy with vascular resection and anastomosis is the treatment of choice.Because this procedure is difficult to manage and with high risks,it is a great challenge to surgeons.A 50-year old patient with pancreatic head cancer whose portal vein and superior mesenteric vein were involved received radical pancreaticoduodenectomy in the Second Affiliated Hospital of Harbin Medical University.In the surgery,the tumor and its surrounding tissues were dissected,and then the portal vein and splenic vein were reconstructed.The patient was discharged at the 10th day after the surgery with favorable prognosis.
5.Cluster analysis on hypocrellin A content in anamorphs of Shiraia bambusicola.

OBJECTIVEBy examining the colony growth rate, colony texture, substrate mycelium color and separation method of 40 anamorph strains of Shiraia bambusicola to investigate the relationship between these indexes and content of hypocrellin A.
METHODIn UV-spectrophotometry and microscopic observation were applied in the experiments. The results were analyzed by the clustering and variance analysis.
RESULTSPSS clustering analysis showed that the colony characters of S. bambusicola anamorph strains had significant differences with hypocrellin A content.
CONCLUSIONIn the tested conditions, substrate mycelium color of S. bambusicola anamophs has the most distinct correlation with the yield of hypocrellin A.

6.Diagnostic value of plasma miRNA in endometriosis

Objective To explore the diagnostic value of microRNA(miR)-451a,miR-3613-5p and miR-125b-5p in endometriosis(EMT).Methods A tatol of 35 patients with EMT in the Department of Gynecology,the Affiliated People's Hospital of Fujian University of Traditional Chinese Medicine were selected as the case group from December 2021 to February 2023,30 healthy women of childbearing age were selected as the control group over the same period.The expression levels of miR-451a,miR-3613-5p and miR-125b-5p in the plasma of two groups were measured using real-time quantitative reverse transcription polymerase chain reaction,the diagnostic value of these miRNAs in EMT was evaluated by using receiver operating characteristic(ROC)curve.Results ①The expression levels of miR-451a,miR-3613-5p and miR-125b-5p in case group were higher than those in control group,the difference was statistically significant(P<0.05).②In case group,there was no significant difference in miRNA expression levels between mild-moderate patients(stage Ⅱ-Ⅲ)and severe patients(stage Ⅳ)(P>0.05).The expression levels of carbohydrate antigen 125(CA125)in plasma was significantly different(P<0.05).③The diagnostic efficacy of miRNA for EMT was evaluated through ROC analysis,with a sensitivity of 85.7%and specificity of 40.0%for miR-451a,a sensitivity of 60.0%and specificity of 90.0%for miR-3613-5p,and a sensitivity of 60.0%and specificity of 76.7%for miR-125b-5p.Combined with a sensitivity of 48.6%and specificity of 96.7%.Conclusion Compared to healthy women,the expression levels of miR-451a,miR-3613-5p and miR-125b-5p in plasma of EMT are increased,which are not affected by clinical stage,and are expected to be candidate markers for the non-invasive diagnosis of EMT,and are not affected by clinical staging,making them promising candidate biomarkers for non-invasive diagnosis of EMT.
7.Establishment of a candidate reference method for angiotensin Ⅱ in human plasma

Objective:To establish a candidate reference method for the determination of angiotensin Ⅱ in human plasma by isotope dilution liquid chromatography tandem mass spectrometry (ID-LC-MS/MS) and to evaluate its performance.Methods:Using [ 13C 6- 15N]-angiotensin Ⅱ as the internal standard, the plasma was accurately weighed by gravimetric method and mixed with a certain amount of internal standard. At the same time, enzyme inhibitor was added. After zinc sulfate solution protein precipitation and reversed-phase solid-phase extraction plate treatment, it was analyzed by liquid chromatography tandem mass spectrometry. The multi reaction ion monitoring mode(MRM)was selected by mass spectrometry to detect specific ion fragments of angiotensin Ⅱ and internal standard. The linearity, sensitivity, precision, recovery rate and uncertainty of the performance of the established method were evaluated according to ISO15193. Results:Angiotensin Ⅱ had good linearity in the range of 10-1 000 pg/g ( r=0.999 5), the lower limit of quantification was 7.68 pg/g, the analytical recoveries were 97.14% to 102.85%, intra-batch imprecision≤3.21%, inter-batch imprecision≤2.96%, and total imprecision≤3.67%. Conclusion:A method for the determination of plasma angiotensin Ⅱ was established by ID-LC-MS/MS. The method is accurate and reliable, and is expected to be a reference method for the determination of plasma angiotensin Ⅱ.

Traumatic cerebrospinal fluid leakage commonly presents in traumatic brain injury patients, and it may lead to complications such as meningitis, ventriculitis, brain abscess, subdural hematoma or tension pneumocephalus. When misdiagnosed or inappropriately treated, traumatic cerebrospinal fluid leakage may result in severe complications and may be life-threatening. Some traumatic cerebrospinal fluid leakage has concealed manifestations and is prone to misdiagnosis. Due to different sites and mechanisms of trauma and degree of cerebrospinal fluid leak, treatments for traumatic cerebrospinal fluid leakage varies greatly. Hence, the Craniocerebral Trauma Professional Group of Neurosurgery Branch of Chinese Medical Association and the Neurological Injury Professional Group of Trauma Branch of Chinese Medical Association organized relevant experts to formulate the " Chinese expert consensus on the diagnosis and treatment of traumatic cerebrospinal fluid leakage in adults ( version 2023)" based on existing clinical evidence and experience. The consensus consisted of 16 recommendations, covering the leakage diagnosis, localization, treatments, and intracranial infection prevention, so as to standardize the diagnosis and treatment of traumatic cerebrospinal fluid leakage and improve the overall prognosis of the patients.
