1.Feasibility of ultrasonography in diagnosis of carpal tunnel syndrome by measuring the thickness of transverse carpal ligaments

[Objective]To evaluate the diagnostic value of ultrasonography in patients with carpaltunnel syndrome (CTS) by measuring the thickness of transverse carpal ligament.[Method]Forty patients who had clinically and electrophysiologically confirmed as CTS underwent surgery and ultrasonographic examination by the same operator for the thickness of transverse carpal ligaments at the level of the hook of the hamate bone in transverse plane preoperatively.Twenty-four asymptomatic normal wrists served as control.Data of the ultrasonography and surgey from patients and the control groups were compared to determine the diagnostic parameters.[Result]The ultrasonographic thickness of transverse carpal ligaments in CTS patients was (0.42?0.08)cm at hamate hook leve and the data from surgey was (0.40?0.10).The data in control group was (0.29?0.07)cm.There was significant difference in the mean value of thickness of transverse carpal ligaments between the two groups at the same level.There was no significant difference between the data of CTS from surgey and ultrasonography.[Conclusion]Ultrasonography examination of the thickness of transverse carpal ligamentsis is a new and feasibile method in the diagnosis of carpal tunnel syndrome.
2.Recent research progress of abscopal effect in tumor radiotherapy

If loosely defined,an abscopal effect occurs when localized irradiation affects the whole organism.In recent years,distant tumor regression and normal tissue damage after localized irradiation have been concerned by immuno-oncologists.However,the mechanisms of this effect are still far from clear.Mounting evidences suggest that the generation of abscopal effects is closely related to immune regulation.Radiotherapy might become a systemic tumor-treatment modality by enhancing immune function and played an important role in every stage of tumor development and prognosis.For the moment,targeting the immune checkpoint has become an attractive approach for malignant tumor therapy.Preclinical data have revealed that a strong abscopal effect could be effectively induced by the co-treatement of immune checkpoint inhibitors and irradiation,which could recruit antitumor T cells and achieve a powerful antitumor effect.This review discusses current progress and perspectives of abscopal effects in the combination of radiotherapy and immunotherapy.
3.Chitosan/PVA nerve conduits repair sciatic nerve defect in rats

ObjectiveTo investigate the effects of chitosan/PVA nerve conduits which used for repairing sciatics nerve defect in rats.MethodsTwenty-seven rats were divided into 3 groups randomly,with 9 rats in each group. Firstly, the 15mm defects in the left sciatic nerves were made in the rats and were respectively repaired with chitosan/PVA conduits graft (group A), the silicon conduits graft (group B),and autografts (group C). At 12 weeks after the operations, the left sciatic nerves were taken out, and the comparative evaluation was made on the repairing effects by wet weight of gastrocnemius and soleus muscles, histological examination,computerized imaging analysis and True Blue retrograde tracing. ResultsThe wet weight of gastrocnemius and soleus muscles showed no significant difference between the chitosan/PVA graft and autograft groups (P > 0.05). The wet weight of gastrocnemius and soleus muscles in significant difference between the chitosan/PVA graft and the silicon group at 12 weeks after the operation(P < 0.05). The nerve fiber density showed no statistically significant differences between the chitosan/PVA and autograft groups(P> 0.05).The regenerative nerve fiber in group B had normal morphological and structural characters under transmission electron microscope.True Blue-labeled neuron cell bodies were found within both anterior horn of gray matter in the spinal cord and dorsal root ganglions (DRGs) ipsilateral to the operated side of the tested rats on illumination with ultra-violet light 1 week after the injection of True Blue.Conclusion Chitosan/PVA nerve conduit can effectively promote the nerve regeneration and myelinization of rat sciatic nerve, which is expected to substitute for autograft to repair nerve defects succesfully.
4.The expression of IgG4 in salivary glands and the clinical analysis of 58 cases of Sj(o)gren's syndrome

Objective Mikulicz's disease (MD) was considered to be a subtype of Sj(o)gren's syndrome (SS) due to the clinical and histological similarities between them.Evidence had shown that there were differences between MD and typical SS.The purpose of this study was to investigate the correlation between MD and SS,by means of analyzing the expression of IgG4 in salivary glands and the clinical characteristics of patients who were previously considered as SS.Methods The paraffin sections of salivary glands from SS patients were stained with monoclonal antibodies to IgG4 and CD38.Patients were divided into two groups based on the pathological results.Analysis of the symptoms,the signs and the laboratory results were carried out in these patients.The difference in laboratory parameters and histopathological gradings in the two groups was analyzed.Normal and abnormal distributed data comparison was conducted using random independent samples t-test and rank sum test respectively.Two-sample rates were compared with Chi-square test.Results Based on immunohistochemistry of IgG4 distribution,the 58 patients with SS were divided into two groups:IgG4 related (9 cases) and non-IgG4 related (49 cases).Histopat-hologically,IgG4 related cases showed IgG4+ plasma cells/IgG+ plasma cells infiltration and there were more IgG4 related monoclonal antibody expressed when compared to IgG4 unrelated cases.In addition,there were also significant differences in clinical features between the two groups.IgG4 related disease was associated with male gender,higher level of plateletconnt,lymphocytes [(2.4±0.8)×109/L vs (1.4±0.7) ×109/L] count and CRP [(52±60) mg/L vs (15±17) mg/L] levels and lower titer of IgM [(1.2±0.7) g/L vs (1.8±0.8) g/L],antinuclear antibody (56% vs 87%) and anti-SSB antibodies (13% vs 54%) (P<0.05),when compared with IgG4-unrelated cases.There was no significant difference in other indicators (P>0.05).Conclusion The present study has demonstrated that some of the MD patients misdiagnosed as SS.Some of the laboratory tests such as the level of platelet and lymphocyte count,serum level of CRP,IgM,antinuclear antibody,anti-SSB antibodies,the serum levels of IgG4 and the histopathological presentations in the salivary gland are different between these two disorders.Because of good response to steroid in MD,so laboratory tests and pathological examinations for IgG4 can help to avoid misdiagnosis.
5.Expression of IgG4 in thyroid tissues of Hashimoto's thyroiditis and its clinical and histological characteristics

Immunohistochemistry was carried out for assessment of the expression of IgG4 and IgG in 29 patients with Hashimoto's thyroiditis (HT) and 26 other patients with subacute thyroiditis,thyroid adenoma,and thyroid carcinoma.HT group were divided into IgG4 HT (10/29) and non-IgG4 HT (19/29) groups.Statistical analyses of clinical and histological characteristics were also conducted in both groups.2 cases of subacute thyroiditis were IgG4 positive (2/26).The IgG4 positive plasma cells,IgG positive plasma cells,and ratio of IgG4 +/IgG + plasma cells in IgG4 HT cases were significantly higher than non-IgG4 HT ones(P<0.05).The serum Fl4 in IgG4 HT cases were lower than non-IgG4 HT ones(P<0.05).After operation,IgG4 HT had a greater proportion for accepting thyroxin treatment(P<0.05).IgG4 HT is a new-found disease which has particular histological and immunological features and would have good response for cortical hormone therapy.Serum and tissue examination of IgG4 seems to be helpful to the diagnosis of IgG4 HT.
6.Rat models of cardiac arrest induced by transcutaneous electrical stimulation on the epicardium

Objective To establish a new model of cardiac arrest (CA) in rats by transcutaneous electrical epicardium stimulation. Method Two acupuncture needles connected to the anode and cathode of a stimulator were transcutaneously inserted into the epicardium as electrodes. The stimulating current was steered to the epicardium and the stimulation was maintained for 3 minutes to induce CA. Cardiopulmonary resuscitation (CPR) was performed at 6 minutes after a period of nonintervention. Results The success rate of induction was 12/20 at the current intensity of 1 mA; and reached 20/20 when the current intensity was increased to 2 mA. The average time from the electrical stimulation to CA induction was (5. 10 ± 2. 81) seconds. When the electrical stimulation stopped, 18/20 rats had ventricular fibrillation and 2/20 rats had pulseless electrical activity. CPR was performed for averagely 207.4 ( ± 148.8) seconds. The restoration of spontaneous circulation was 20/20. The death rate within 4 hours after CA was 5/20, and the 72-hour survival rate was 10/20. There were only two cases of complications, a minor muscle contraction and a minor lung lobe injury. Conclusions The model of CA in rats induced by transcutaneous electrical epicardium stimulation is a stable model that requires low-intensity current and has fewer complications.
7.Relationship between microRNA-126 and curative effect of chemoradiotherapy in patients with gastric cancer, and enhancement effect of microRNA-126 on radiosensitivity of SGC-7901 cells

AIM: To investigate the influences of microRNA-126 on the curative effect of chemoradiotherapy and radiosensitivity of SGC-7901 cells.METHODS: The patients of gastric cancer (n=60) were selected in this study including 32 males and 28 females with the average age of (51±7) years.All patients received similar chemoradiotherapy strategy.The tissue and blood samples were collected during treatment.The short-term curative effect was evaluated by the Response Evaluation Criteria in Solid Tumors (RECIST), and the patients were divided into sensitive group and insensitive group.The microRNA-126 levels were detected by RT-qPCR.The SGC-7901 cells were maintained in vitro and transfected with microRNA-126 mimic.The plate colony formation assay was used to determine the enhancement effect of microRNA-126 on radiosensitivity of the SGC-7901 cells.The apoptotic rate of the SGC-7901 cells induced by microRNA-126 was analyzed by flow cytometry.RESULTS: According to the RECIST, 28 cases were defined as sensitive patients and 32 cases were the insensitive patients.Compared with the sensitive patients, the microRNA-126 levels both in blood and tissue samples were lowered in the insensitive patients, and the relative fold changes were 0.72±0.04 and 0.48±0.03, respectively (P<0.05).After transfection with microRNA-126 minic, the SF2 and D0 in the SGC-7901 cells were decreased with the SER of 1.74.Furthermore, microRNA-126 induced apoptosis of SGC-7901 cells and enhanced their radiosensitivity.CONCLUSION: The patients with low microRNA-126 level may suffer a poor curative effect of chemoradiotherapy on the gastric cancer.MicroRNA-126 has an enhancement effect on the radiosensitivity to the SGC-7901 cells.
8.Comparison of the diagnostic value of hepatocellular carcinoma ≤ 3 cm in diameter in patients with chronic liver disease between contrast-enhanced ultrasound and gadolinium diethylene-triamine-pentaacetic acid-enhanced magnetic resonance imaging

Objective To compare the diagnostic ability of gadolinium diethylene-triamine-pentaacetic acid-enhanced magnetic resonance imaging (Gd-EOB-DTPA MRI) and contrast-enhanced ultrasound (CEUS) for the diagnosis of hepatocellular carcinoma (HCC) ≤ 3 cm in diameter in patients with hepatic cirrhosis or chronic hepatitis B.Methods Ninty-two cases with single focal liver lesion ≤ 3 cm who underwent CEUS and Gd-EOB-DTPA MRI in Southwest Hospital Affiliated to Third Military Medical University were enrolled in the study.And all cases were diagnosed with hepatic cirrhosis or chronic hepatitis B.Pathology was the golden standard and all cases were diagnosed pathologically by surgical operation.Sensitivity,specificity,positive predictive value,negative predictive value and accuracy rate of the diagnosis of small HCC by CEUS,Gd-EOB-DTPA MRI and CEUS combined with Gd-EOB-DTPA MRI were calculated and compared with x2 test or Fisher exact probability.Results Of the 92 cases,82 cases were diagnosed as HCC and the other 10 cases were diagnosed as non-HCC.The sensitivity and the specificity for the diagnosis of HCC was 79.2% (65/82) and 70.0% (7/10) of CEUS alone and was comparable with that of Gd-EOB-DTPA MRI [74.4% (61/82) vs 70.0% (7/10)] alone (x2=0.548,P=0.459;P=1.000).When both CEUS and Gd-EOB-DTPA MRI met the diagnostic criteria for HCC,the sensitivity was lower [61.0% (60/82),P=0.010] than that of CEUS alone,and the specificity [80.0% (8/10),P=1.000] was not statistically different from that of CEUS alone.When CEUS or Gd-EOB-DTPA MRI met the diagnostic criteria for HCC,the sensitivity [92.7% (76/82)] was higher than that of CEUS alone (x2=6.119,P=0.013) or Gd-EOB-DTPA MRI alone (x2=9.972,P=0.002),and the specificity [60.0% (6/10)] was not statistically different from that of CEUS alone (P=1.000) or by Gd-EOB-DTPA MRI alone (P=1.000).Conclusions In patients with cirrhosis or chronic hepatitis B,the diagnostic ability of HCC ≤ 3cm in diameter by CEUS alone was comparable to that by Gd-EOB-DTPA MRI alone.Combination of CEUS and Gd-EOB-DTPA MRI may increase the sensitivity for the diagnosis of HCC,but the specificity not statistically improved.
9.Experimental study of insulin receptor change from bladder after conus medullaris injury in rats

Objective To observe the change rules of insulin receptor from rat bladders after losing lower cen-tal innervations so as to explore the function of insulin receptor in denervated bladder. Methods From January, 2016 to June, 2017, 40 Sprague-Dawley rats were randomly divided into experimental group and control group,each group was 20. The conus medullaris of rats in experimental group were damaged, and rats in control group received sham opera-tion. The rats in 2 groups were sacrificed at different time after surgery (1 day, 1 week, 1 month, 3 months). Bladder specimens were harvested to perform wet weight measurement and immunohistochemical detection of insulin receptor. Results Corresponding to 1 day, 1 week, 1 month and 3 months after surgery, the bladder wet weight of control group were (0.089±0.022)g, (0.094±0.038)g, (0.106±0.112)g and(0.102±0.048)g, and of experimental group were (0.092± 0.026)g, (0.110 ±0.034)g, (0.538 ±0.098)g and (1.528 ±0.462)g. One month and 3 months after surgery, bladder wet weight of experiment group were significantly increased as compared with those of control group (P<0.05). One day, 1 week, 1 month and 3 months after the operation, the positive rate of insulin receptor expression was 60%, 100%, 100%and 80%. And strongly positive rate was 55%. In control group, the positive rate of insulin receptor expression was 20%, 40%, 40%, and 0%. The expression of insulin receptor in experimental group was significantly higher than that in con-trol group in every stage(P<0.05). Conclusion The expression of insulin receptor is significantly increased after den-ervation of bladder. The defections of insulin receptor might lead to apoptosis and muscle wasting after denervation. Re-store insulin receptor function might be key point to prevent bladder tissue from irreversible damage.
10.Identification of mentality facticity based on multi-channel event-related potentials.

This paper explores the use of multi-channel event-related potentials (ERP) to identify mentality facticity or detect lie. Some identifiably meaning information, such as subjects' name and birthday, were selected as concealed information to be identified, 15 subjects were tested by concealed information test (CIT) paradigm and their electroencephalographs (EEG) were recorded from 30 electrodes. In virtue of analysis on the statistically significant difference between multi-channel ERPs evoked by probe information and that evoked by irrelevant information, the P300 amplitudes of 15 electrodes were selected as F-test samples. The significant difference of feature sample values between probe and irrelevant information was applied to identify mentality facticity. The results indicate that P300 amplitudes evoked in many electrodes are significantly different between probe and irrelevant information (P < 0.01). According to the significant difference of space sample values between probe and irrelevant information, the detection correctness to probe information reaches to 93.3% and is better than that of methods based single-channel ERP. The method proposed has the advantages of non-invasion and better accuracy, which could be used to identify mentality facticity effectively.
