1.Clinical research of tension-free herniorrhaphy using Kugel patch in 355 inguinal hernias

Objective To explore the clinical effects of Kugel surgical treatment for inguinal hernia.Methods Operative time,chornic pain,complications and charges of 355 patients with inguinal hernias were analyzed retrospectively.Results Recurrences occurred in 1 patient,during a mean followl-up of 36 months (range:7-85).The average operation time was 35.4 min (range:20-130),average surgical charges were 6 253 yuan.Postoperaive chronic pain appeared in 6 patients.There are 3 cases of sermoa,1 case of haematoma and 2 case of wound infection.Conclusion The Kugel inguinal hernia repair is safe and effective,with the advantage of mild pain and prompt recovery after operation,and a low risk of complications and recurrence.
2.A new steroidal glycoside from Lygodium japonicum

To investigate the chemical constituents from the aerial parts of Lygodium japonicum. Methods:Various chromatographic techniques were employed for isolation and purification of the constituents. The structures were elucidated by chemical evidence and spectral methods. Results:A new stigmasterol glycoside,(24R)-stigmastan-3β,5α,6β-triol 3-O-β-D-glucopyranoside (1),together with three known phenolic glycosides:6-O-p-coumaroyl-D-glucopyranose (2),6-O-caffeoyl-D-glucopyranose (3),1-O-(E)-caffeoyl-β-D-gentiobiose (4) were obtained and identified by spectroscopic methods. Conclusion:All compounds were isolated from Lygodiaceae for the first time.
3.Biological property and clinical application of tissue engineered skin

BACKGROUND:Tissue engineered skin has been used to repair defect skin, but it is not as good as to completely replace permanent skin. OBJECTIVE: To explore the biological characteristics and clinical application of various types of tissue engineered skin. METHODS: Pubmed data (1990-01/2005-12) was retrieved by computer with the key words of "tissue-engineered skin". CNKI database (2002-01/2007-12) was retrieved with the same key words. RESULTS AND CONCLUSION: Totally 226 literatures were primarily selected. After reading titles and Abstracts, studies addressing irrelative objective or repetitive content were excluded, and 11 literatures were included. The aim of skin tissue engineering is to create skin substitute that close to human.skin in histology and functional characteristics. There were many mature tissue engineered skin in clinic, and various artificial skins were developed. Their structure was similar to human, but only had skin barrier function and lack of skin appendages, so they did not have complete skin function, and did not reach skin reestablishment. Ideal skin substitute should process cuticular layer and dermial layer. The dermial layer can reach rapid vascularization and nerve redistribution, can promote physiological wound repair. The cuticular layer can rapidly obtain full barrier and protection function, which can be fully integration with wound surface. A good dermial carrier has good histocompatibility, and a certain physical and chemical properties, such as strength, scratchability, elasticity, gas permeation, water vapor permeability. These can provide a good adherence between dermis and wound surface, offer an ideal fiber stent for cell growth, and finally promote autologous skin growth. Tissue engineered skin has good prospect, but there are many problems that should be solved in biocompatibility and skin function reestablishment.
4.The Effects of Tibia Lengthening on the Histomorphology of Tibial Nerve in Rabbits

The effects of gradual tibia lengthening at different speeds on the histomorphology of tibial nerve were studied in 30 rabbits by self-made external distraction device. It was showed that the leg was tracted at the speed of 1 mm/d, the tibial nerve was lengthened correspondingly. Gradual stress could stimulate the growth of nervous tissue and no nervous functional and constructural injuries were found When the leg was distracted at the speed of 2 mm/d, limb elongation within 40% was safe, otherwise, severe nervous damage would occur.
5.Studies on origin, morphology and histology of Galla Cinnamomi Camphorae specially produced in Jiangxi Province

Object To study the origin, morphology and histology of Galla Cinnamomi Camphorae(GCC). Methods By the literature investigation and the survey in the production area, GCC was identified by microscopic technique. Results The origin and the medicinal part of GCC were defined. Conclusion GCC is the abnormal fruit of Cinnamomum camphora (L.) Presl which has been infected by Exobasidium sawadae Yamada and formed with hymenium and fruit. The characters in morphology and histology of GCC provide the scientific reference of identification of the crude drug.
6.Novel deletion mutation of type-Ⅳcollagen in a Chinese family with Alport syndrome

Objective Alport syndrome is one of the diseases that may lead to the end-stage renal disease ( ESRD) in chil-dren, and the methods for its diagnosis and treatment remain quite limited.This study aimed to investigate the clinical and genetic di-agnosis of a Chinese family with hematuria companied by genetic nephritis. Methods We analyzed the renal pathology of 7 patients in a family, performed immunofluorescence staining of type-Ⅳcollagen in the nephridial and skin tissue, conducted gene sequencing i-dentification using the exon sequence method, and examined the blood and urine samples from the patients. Results Renal patholo-gy manifested mesenterium hyperplasia in the index patient, with IgM+under the light microscope, no thickening or thinning under the electromicroscope, and no absence of type-Ⅳcollagen on immunofluorescence analysis.Mutation of c.1365_1373del TCCAGGCCC (p.Pro456_Pro458del3) was observed in exon 21 of the COL4A5 gene.Only 1682 amino acids were found in the mutated protein as compared with 1685 in the wild type. Conclusion This is the first case of Alport syndrome induced by gene deletion mutation ever reported in China and abroad.There are many female patients in this family, all with a high risk of reproduction failure.Antepartal gene diagnosis or genetic diagnosis before embryo transfer may contribute to the prevention of the disease.
7.Rapid Determination of 25 Kinds of Fungicides in Cereals, Fruits and Vegetables by Solid Phase Extraction and Liquid Chromatography Coupled with Quadrupole-Time of Flight Mass Spectrometry

A method for simultaneous determination of 25 kinds of fungicides in cereals, vegetables and fruits using SPE-LC-Q-TOF/MS technique was developed.The samples were extracted with acetonitrile containing 1% (V/V) acetic acid, purified by solid phase extraction (SPE) with a Crabon/NH2 cartridge, eluted with acetonitrile-toluene(3∶1, V/V), separated by a reversed phase C18 column, gradiently eluted with acetonitrile and 0.1% formic acid solution (Containing 5 mmol/L ammonium acetate), determined by LC-Q-TOF/MS, and quantified by external standard method.A data base of the accurate mass numbers and a library which contains 25 kinds of fungicides were established.The automatic retrieval of detection results was carried on according to the characteristics of the compound, such as accurate mass, retention time, isotope peak distribution, isotopic ratios, and so on.Based on the above results, the qualitative identifications of the 25 new fungicides were accomplished without the contrast of standard substances.The results indicated that 25 fungicides showed good linearity in the range of 0.02-200 μg/L, and the limits of detection (LOD) and the limits of quantification (LOQ) were 0.01-5.00 μg/kg and 0.02-20.00 μg/kg, respectively.The linear relative coefficients were greater than 0.995.The recoveries were in the range 71.8%-114.0% and the relative standard deviations (RSD) were ranged from 0.1% to 21.3% (n=3).The method has some advantages such as simplicity, rapidity and high sensitivity, and is suitable for the rapid determination of the common fungicides in cereals, vegetables and fruits.
8.A new phenolic glycoside from fruits of Citrus aurantium

Objective To study the bioactive constituents of the immature fruits of Citrus aurantium. Methods Silica gel and Sephadex LH-20 were used to isolate the constituents of this plant. The spectroscopic methods were applied for their structural elucidation. Results Four compounds were isolated from the ethanol extract of the immature fruits of C. aurantium. These compounds were elucidated as 3, 5-dihydroxyphenyl 1-O-(6′-O-trans-feruloyl)-?-D-glucopyranoside (Ⅰ), hesperetin (Ⅱ), 4′, 5, 7, 8-tetra- methoxyflavone (Ⅲ), and limonin (Ⅳ). Conclusion Compound Ⅰ is a new phenolic glycoside.
9.Phalanx-Lengthening Method in the Treatment of Amputated Fingers

79 cases (180 amputated fingers) underwent gradual distraction through phanlanges. The bone elongation obtained varied from 1 to 3.2 cm, with an average of 2.2 cm. Finger stumps were examined by measurement of 2 point discrimination (2PD) and sensative nerve conduction velocity as well as SO, both before and post lengthening. We confirmed that it was safe to distract the finger stump at a daily rate of 1 mm. Several fingers can be lengthened simultaneously using our device. The adjacent joint was also stretched other than compressed. Therefore, the lateral accessory ligament contraction and joint crush injury were avoided. The long-term length increase was maintained by subperiosteal osteotomy. Gradual finger lengthening is a simple, safe and effective method.
10.Alkaloids from the leaves of Nauclea officinalis

To study chemical constituents of the leaves of Nauclea officinalis, eight alkaloids were isolated from 95% ethanol extract by various chromatographic methods. The structures were elucidated on the basis of spectroscopic data (IR, UV, ESI-MS, 1D and 2D NMR) and identified as naucleactonin C (1), strictosamide (2), vincosamide (3), pumiloside (4), angustoline (5), angustine (6), 18, 19-dihydroangustine (7) and naucleofficine D (8). Compound 1 is a new indole alkaloid. Compounds 6 and 7 were isolated from this plant for the first time.
