1.OBSERVATION BY SCANNING ELECTRON MICROSCOPY ON RABBIT KIDNEY AFTER THE RELEASE OF CHRONIC URETERAL OBSTRUCTION
Acta Anatomica Sinica 2002;0(06):-
Objective To explore the mechanism of imperfect recovery of the renal function after release of chronic ureteral obstruction. Methods Adult New Zealand rabbits(n=36) were randomly divided into normal control group(n=3),obstructive control group(n=3) and experimental group(n=30).The latter two groups were subjected to chronic unilateral ureteral obstruction. Two weeks later,3 rabbits were sacrificed and the others underwent ureterostomy, which were sacrificed separately following 12 hours,24 hours,3 days,7 days,14 days. The renal papilla and cortex were examined by scanning electron microscopy. Results Shortly after the release of ureteral obstruction, massive loss of brush border and ballooning of epithelial cells were noted in proximal tubules.Distal tubules and collecting tubules also showed severe ballooning of epithelial cells and normal struction were not seen. Two weeks later, many tubular components became patent with normal epithelial lining. Some of them, however, were deteriorated further in spite of the release of obstruction. Thus there was a marked tubular heterogeneity in recovery. Conclusion Further deterioration of some tubules might be one of the important pathologic basis for imperfect recovery of renal function after the release of chronic ureteral obstruction.

Objective Selected the characteristic index of cervical CT tablets, and established stepwise regression equation, to explore the value of its application in forensic medicine identification. Method Each one cervical CT images of 130 cases were collected, 70 subjects were examined for 2 cervical CT images of each. Selected and measured 13 indicators of cervical CT iflms, the difference between the groups was calculated by the same measurement indicators after the random grouping of the inspectors, and the difference between the different measurement indexes of the same person. Using two classification logistic Stepwise regression analysis, establish the regression equation of the index and multiple regression equation of multinomial index, and the test of the equation was carried on blind test. Results Accuracy of 13 equations of the established element is 69.0%~90.4%,accuracy of 4 multiple regression equation is 90.4%~96.7%, blind accuracy rate is 100%. Conclusion 13 feature indicators selected from CT tablets can be used for the same identiifcation, in practice researchers should use multivariate index to get more accurate result.

Objective To evaluate the value of retroperitoneal laparoscopic adrenalectomy for benign adrenal tumors. Methods From March 2003 to May 2008,78 patients with adrenal tumors received retroperitoneal laparoscopic adrenalectomy in our hospital. During the operation, three trocars were introduced into the lumbar region to establish retroperitoneal space by blunt dissection with the camera. Afterwards, the tumor was removed by using a harmonic scalpel. Results The procedures were successfully completed in all the 78 cases without blood transfusion, the operation time ranged from 45 to 180 min (mean,90 min). Drainage tubes were removed 24 hours after the operation. And the patients were discharged from hospital in 3 to 5 days postoperation. One patient showed regional subcutaneous emphysema after the surgery and was cured spontaneously. The patients were followed up for 3 to 24 months with a mean of 10 months, during which 36 patients achieved normal blood pressure in 3 months, while in the other 7 patients, oral antihypertension drugs were still needed. No patients had recurrent tumor during the follow-up. Conclusions Retroperitoneal laparoscopic can be the first choice for adrenal tumors,as it is safe and effective with minimal invasion, quick recovery, and short postoperative hospital stay.

ObjectiveTo observe the protective effect of high-dosage methylprednisolone (MP) after lumbar spinal stenosis operation. Methods20 Patients with lumbar spinal stenosis accepted laminectomy. The treatment group (11 cases) received MP 30 mg/kg when the skin incised, and followed with MP 40 mg twice a day for 3 d after operation. The control group (9 cases) received MP 40 mg twice a day for 3 d after operation. They were assessed with JOA scale before and after operation. ResultsThe JOA score was (9.25±2.12) and (9.53±2.10) in treatment group and control group before operation (P>0.05), and was (13.43±2.01) and (11.21±2.13) 1 week after operation (P<0.05), (14.62±2.15) and (13.04±2.11) 3 months after operation (P<0.05). The neurological symptom deteriorated in 1 cases of control group. No peptic ulcer, poor wound healing or other associated complications has been found. ConclusionHigh-dosage methylprednisolone may protect the spine cord intraoperative the lumbar spinal stenosis.

Objective To investigate the effect of continuous positive airway pressure (CPAP) on seizure frequency in epilepsy patients complicated with obstructive sleep apnea (OSA). Methods Subjects were divided into CPAP group (20 subjects) and medication group (22 subjects) according to whether they can tolerate CPAP. CPAP group were treated with CPAP combined with antiepileptic drugs. Medication group were treated with antiepileptic drugs alone. Seizure frequency and apnea-hypopnea index (AHI) were compared between groups before and after 4 weeks and 24 weeks of treatment. Results Baseline seizure frequency and apnea-hypopnea index (AHI) were compared between groups(P>0.05). A sig?nificant reduction of seizure frequency was observed in CPAP group after 4 weeks and 24 weeks of treatment compared to that before treatment (P<0.01). In medication group, no significant difference in seizure frequency was noted betweem 4 weeks and 24 weeks after treatment compared to that before treatment(P>0.05). A significant reduction of AHI was ob?served in the CPAP group after 24 weeks of treatment compared to that before treatment (P<0.01), but no change of AHI by treatment was observed in medication group (P>0.05). Conclusion Treatment of OSA in patients complicated with epilep?sy may improve seizure control in short and long term, but longer observation time and more samples are needed for further research.
