1.Evaluation of different doses dezocine combined with propofol intravenous anesthesia for artificial abortion

Objective To evaluate clinical results of different doses of dezocine combined with propofol in-travenous anesthesia in abortion.Methods 150 patients with routinely abortion were randomly divided into group A, B,C,at induction the patients were given anesthesia dezocine intravenous doses of 0.025mg/kg,0.05mg/kg, 0.1mg/kg,10min after slowly intravenous injection of propofol,the clinical results were compared in three groups of artificial abortion.Results Each time during surgery,mean arterial pressure(MAP),heart rate(HR)and blood pres-sure pulse saturation(SpO2 )indicators in group B were more stable than groups A,C the operations security was higher (tA =6.06,tB =0.88,tC =1.73,PA <0.05,PB >0.05,PC >0.05).Dose of propofol,postoperative recovery time of group B [(113.56 ±18.12)mg,(6.72 ±1.69)min]were significantly better than those in group A [(150.01 ±21.45)mg,(10.43 ±3.38)min],and the differences were statistically significant(t =9.17,6.94,all P <0.05 ).The propofol dose,postoperative recovery time in group B were slightly higher those in group C [(110.21 ±19.32)mg,(7.40 ±1.87)min],and the differences were not statistically significant(t =0.89,1.91,all P >0.05).The incidence rate of adverse reactions of group B (8.00%)was significantly lower than that in A group (30.00%),and the difference was statistically significant(χ2 =2.21,P <0.05).The incidence rate of adverse reac-tions in group B,C had no significant difference(P >0.05).Conclusion Abortion application dose of 0.05mg/kg of dezocine combined with propofol intravenous anesthesia has the best clinical effect,each relatively stable vital signs af-ter surgery,can effectively reduce the dose of propofol,shorten the operation time and postoperative recovery time,and has fewer adverse reactions.
2.Clone and expression of nuclear factor ?B p65 TAD

Objective To acquire NF-?B p65 TAD(transcription activation domain) and construct its eukaryotic expression vector and express it in endothelial cells.Methods Human umbilical vein endothelial cells(HUVECs) were cultured and total RNA was extracted.The p65 TAD gene was amplified by RT-PCR.After sequencing,the p65 TAD gene was inserted into the eukaryotic expression vector pEGFP-N1 with the green fluorescence protein,named pEGFP-N1-p65 TAD.pEGFP-N1-p65 TAD was transfected into HUVECs and its expression was observed under fluorescence microscope and analyzed by Western blotting.Results p65 TAD(288-548) was cloned successfully.The constructed plasmid including p65 TAD gene was identical to the designed.p65 TAD gene was expressed successfully in HUVECs.Conclusion The construction of eukaryotic expression vector including p65 TAD gene and its expression in HUVECs are very successful.

Objective To investigate the role of the cough reflex sensitivity and airway neurogenic inflammation in the pathogenesis of cough post infectious(CPI).Methods Capsaicin provocation test and sputum inducing were performed in 10 patients with CPI(CPI group)and 10 healthy subjects(normal control group)to detect the cough reflex sensitivity and neurokinin of sputum.Total and differential cells count of induced sputum was performed.Substance P(SP),neurokinin A(NKA),neurokinin B(NKB)and calcitonin gene-related peptide(CGRP)in the supernatant of induced sputum were measured.The expression of SP,neurokinin 1 receptor and NKA in sputum cells was detected.Results The cough threshold(lg C5)of CPI was significantly lower than normal(1.16?0.61 vs 2.64?0.28,P
4.The effects of ureteropelvic junction obstruction on contralateral kidney

Objective To explore the effects of ureteropelvic junction(UPJ) obstruction on contralateral kidney.Methods Twenty-four adult New Zealand rabbits were divided randomly into 3 groups,ie,complete obstruction group(group 1,n=8),partial obstruction group(group 2,n=8) and shamoperation group(group 3,n=8).In groups 1 and 2, the right ureter was ligated completely or partially.In group 3,the right ureter was only exposed but not ligated.The animals were killed 4 weeks after operation,and the contralateral kidneys were resected.Before killing,anterior urography showed that right hydronephrosis occurred in all the rabbits in groups 1 and 2.The contralateral renal weight was measured by electron balance.The apoptosis in the contralateral renal cortex and medulla was detected by FITC-AnnexinV/PI flow cytometric assay.The morphological changes of the samples were also studied.Results After 4 weeks,the contralateral renal weight of the rabbits in groups 1,2 and 3 was(3.96?0.20),(3.47?0.19) and(3.20?0.21)g/kgBW,respectively.The difference of contralateral renal weight between groups 1,2 and group 3 was significant(P0.05).In contrast,the apoptosis rates of contralateral renal medulla cells in groups 1,2 and 3 were(23.75?2.34)%,(11.82?1.40)% and(2.36?0.65)%,respectively.There were significant differences between these 3 groups(P

6.Changes of pulse wave velocity in young and middle -aged normal body mass patients with mild -moderate hypertensive

Objective To analyze the relationship between pulse wave velocity (PWV)in young and middle-aged normal body mass patients with mild -moderate hypertensive and risk factors of cardiovascular diseases,and to explore the main factors influencing large arterial elasticity.Methods PWV were measured by Complior (Colson, France).Systolic blood pressure (SBP)and diastolic blood pressure were measured and fasting blood glucose,blood lipid were tested at the same time.The results was analyzed by correlation and multiple regression method.Results (1)Patients with hypertension had statistically significant higher cfPWV than nonhypertensive cases(P <0.01).(2) Correlation analysis showed that the cfPWV were positively related to age,systolic pressure,diastolic pressure,pres-sure difference(r =0.401,0.467,0.289,0.278,P =0.001,0.000,0.020);Deleting the factors of age,abdomen circumference and BMI,the cfPWV was still positively related to systolic pressure,diastolic pressure and pressure differences(r =0.417,0.297,0.315,P =0.001,0.018,0.012).Multiple regression analysis showed that age,systol-ic pressure were mostly related to cfPWV(t =3.945,3.481,P =0.000,0.001).Conclusion Artery elastic function in young and middle aged people is obviously lower in patients with hypertension.cfPWV has strong positive relation with age and systolic pressure.
7.The study on dynamic changes of plasma D-dimer in elderly patients with acute exacerbation of COPD combined with type 2 diabetes mellitus

Objective To investigate dynamic changes of plasma D-dimer, C-reactive protein (CRP) and fasting plas?ma glucose (FBG) levels in elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) combined with type 2 diabetes mellitus. Methods AECOPD group (n=65) was used as A group, AECOPD combined with type 2 diabetes mellitus group (n=65) was used as B group. Levels of D-dimer, CRP and FBG were observed and compared between two groups in the first day, the 3rd day, the 7th day and the 14th day after hospital admission. The correlation be?tween D-dimer, CRP and FBG was analyzed. Logistic regression analysis was used to estimate the factors affecting increased plasma levels of D-dimer. Results Levels of D-dimer, CRP and FBG were significantly higher in B group than those of A group in the first day, the 3rd day, the 7th after hospital admission (P<0.01). Values of D-dimer and CRP were declined ob?viously in A group in the 7th day after hospital admission, but in B group they were declined obviously until the 14th day. There was no significant difference in FBG of A group. The level of FBG was significantly decreased in B group. There was a positive correlation between D-dimer, CRP and FBG (P<0.05). Logistic regression analysis showed that CRP, FBG, p(O2) and age were the influence factors for the increased levels of D-dimer. Conclusion In elderly patients with AECOPD com?bined with type 2 diabetes mellitus, we should pay attention to anti-inflammatory and controlling blood sugar, and the dy?namic monitoring levels of D-dimer, which helps to improve the prognosis for patients with AECOPD, and reduce mortality.
8.Clinical analysis of the curative effects in 64 patients with hypopharyngeal carcinoma

Objective:To study the curative effects of hypopharyngeal carcinoma by treatment of pharyngectomy and repairment surgery.Methods:Sixty-four patients with hypopharyngeal squamous cell carcinoma were treated by pharyngectomy and repaired the defect of pharynx from 1989 to 1995.There were 58 males and 6 females,ranging in age from 37 to 73 years old.There were stages T1 in 3(T1N0M0 1,T1N2M0 2),T2 in 12(T2N0M0 4,T2N1M0 5,T2N2M0 2,T2N3M0 1),T3 in 29(T3N0M0 7, T3N1M0 14,T3N2M0 6,T3N3M0 2),and T4 in 20 (T4N0M0 2, T4N1M0 3,T4N2M0 8,T4N3M0 7).Among 64 patients,26 cases were treated by surgery of laryngeal function preservation (group A,stages T1,2,3),38 cases did not laryngeal function preservation (group B,stage T4).Results:The swallow function was returned to normal in all 64 patients after treatments,and the complications were 26/64(40.6%)most of those was pharyngeal fistula.The rate of decannulation was 53.8%(14/26) in the group A,and the 3-year and 5-year survival rate were 65.4% (17/26) and 50.0% (13/26) respectively. In the group B, the 3\|year and 5\|year surviral rates were 52.6%(29/38)and 44.7%(17/38)respectively.There were no significant differences in the 3 or 5-year survival rate between two groups (P>0.05).Conclusion:The indications of those procedure of laryngeal function preservation were stages T1,2,3of patients with hypopharyngeal carcinoma,which were no affection on the 3 or 5-year survival rates,and the survival quality of patients were improved.

Objertive:To search for the measures of improving the effects on aesthetics of front teethmetal-poircelain crown. Method:The causes and mechanisms of different effects on aesthetics of front teethmetalprocelain crowns in 52 cases were analyzed. Resuts:The main causes that influenced aesthetics offront metalporcelain crown were out of color and bad imitation of teeth. Conclusion: Important factors thatimprove the effects on aesthetics of front teeth metal-porcelain crown include the design and the prepara-tion of cervical layer,accurate choice of the color and correct making process.
10.Chitosan-coated ophthalmic submicro emulsion for pilocarpine nitrate.

The study is to design chitosan-coated pilocarpine nitrate submicro emulsion (CS-PN/SE) for the development of a novel mucoadhesive submicro emulsion, aiming to prolong the precorneal retention time and improve the ocular absorption. CS-PN/SE was fabricated in two steps: firstly, pilocarpine nitrate submicro emulsion (PN/SE) was prepared by high-speed shear with medium chain triglycerides (MCT) as oil phase and Tween 80 as the main emulsifier, and then incubated with chitosan (CS) acetic solution. The preparation process was optimized by central composite design-response surface methodology. Besides the particle size, zeta potential, entrapment efficiency and micromorphology were investigated, CS-PN/SE's precorneal residence properties and miotic effect were especially studied using New Zealand rabbits as the animal model. When CS-PN/SE was administered topically to rabbit eyes, the ocular clearance and the mean resident time (MRT) of pilocarpine nitrate were found to be dramatically improved (P < 0.05) compared with PN/SE and pilocarpine nitrate solution (PNs), since the K(CS-PN/SE) was declined to 0.006 4 +/- 0.000 3 min(-1) while MRT was prolonged up to 155.4 min. Pharmacodynamics results showed that the maximum miosis of CS-PN/SE was as high as 46.3%, while the miotic response lasted 480 min which is 255 min and 105 min longer than that of PNs and PN/SE, respectively. A larger area under the miotic percentage vs time curve (AUC) of CS-PN/SE was exhibited which is 1.6 folds and 1.2 folds as much as that of PNs and PN/SE, respectively (P < 0.05). Therefore, CS-PN/SE could enhance the duration of action and ocular bioavailability by improving the precorneal residence and ocular absorption significantly.
