1.Application of dynamic vascular patterns in differential diagnosis of focal liver lesions in contrast-enhanced ultrasound

Objective To explore the application value of dynamic vascular patterns(DVP) curve and parameter figure in the focal liver lesions by contrast-enhanced ultrasound(CEUS).Methods 83 cases of focal liver lesions diagnosed by pathologically or clinically that had undergone CEUS were analyzed by SonoLiver software.Then DVP parametric images were constructed and the time-intensity curve (TIC) parameters were calculated by TIC and DVP curve.Results Rise time(RT),time to peak(TTP),maximum intensity(IMAX),mean transit (mTT),slope of rise,and the absolute value for slope of half down between malignant and benign lesions groups indicated statistically significant difference (P <0.05).94.9% of type Ⅰ was malignant lesions and 100% Ⅳ type was benign lesions.The four types of DVP curve(Ⅰ,Ⅱ,Ⅲ,Ⅳ)in malignant and benign lesions were 72.5%,13.7%,13.7%,0 and 6.2%,43.8%,31.2%,18.8%,respectively.The proportion and characteristic of DVP parametric images and DVP curves in malignant and benign lesions groups were consistent.Conclusions DVP curve and parametric images of CEUS can demonstrate the difference of flow perfusion static between lesions area and surrounding liver parenchyma dynamically and directly.These are new qualitative indexes of the differential diagnosis in focal liver lesions.
2.Evaluation of an exercise and dietary intervention for obesity and overweight migrant children

Objective To describe body mass index (BMI) changes in migrant children with overweight and obesity in Beijing treated by the exercise and dietary intervention programme.Methods BMI was assessed at before and after the intervention in 30 migrant obesity and overweight children aged 9 ~ 12 in two schools from March,2010.Results BMI reduced significantly one week after the intervention ( P < 0.05 ),but from two months to six months after the intervention BMI increased again to the pretreatment level( P < 0.05 ).Conclusion The intervention was successful in decreasing BMI,however maintenance of BMI after treatment was not easy.The controlling of obesity and overweight is a long term process which needs the cooperation and supervision of their parents.

4.Clinical comparison of four treatment methods for cesarean scar pregnancy

Objective To explore the best approach to treatment of cesarean scar pregnancy (CSP). Methods A total of 138 patients with CSP treated between January and December, 2013 were retrospectively analyzed. The patients were treated with conservative drug therapy, direct curettage, uterine curettage after embolization, or open or transvaginal surgery. The amount of blood loss, proportion of patients with blooding loss greater than 50 mL, hospitalization days, and hospitalization expenses were compared among the groups. Results The median volume of blood loss was 370 mL in the conservative treatment group, 59 mL in direct curettage group, 67 mL in interventional therapy group, and 1425 mL in the surgical group, and the proportion of patients with blood loss over 50 mL was 76.9%, 38.8%, 27.5%, and 100%in the 4 groups, respectively. The midian hospital stay of the 4 groups was 9.0, 4.0, 6.0 and 10.0 days, with median hospitalization expenses of 12281.0, 3843.5, 14805.0, and 17202.2 RMB Yuan, respectively. All these data were significantly different among the 4 groups (P<0.05). Conclusions Direct curettage surgery should be encouraged for treatment of CSP. Embolization therapy can reduce the risk of bleeding but is associated with potential complications and more costly, and should be performed with caution. Open or trasnvaginal surgery can be considered in difficult cases of CSP, and its combination with interventional therapy is an option to better preserving the uterus.

7.Current status and influencing factors of patient privacy protection behavior of clinical nurses

Objective:To investigate the status quo of patient privacy protection behavior of clinical nurses and analyze its influencing factors.Methods:Using the convenient sampling method, clinical nurses from tertiary hospitals in China were selected to conduct a questionnaire survey on from March to April 2018. The Chinese version of Patient Privacy Scale (PPS) were used to conduct an investigation through Questionnaire Star electronic questionnaire. A total of 1, 043 valid questionnaires were collected, and the effective response rate was 100%.Results:The PPS total score of 1 043 clinical nurses was (124.86±12.16) . The item with the highest score was "You would use screens, curtains and sheets to protect patients' privacy when necessary" (4.89±0.39) , while the item with the lowest score was "You would knock on the door before entering the patient's room and enter after asking for the patient's consent" (4.11±1.14) . The results of multiple regression analysis showed that education level and department had effects on patients' privacy protection behavior ( P<0.05) . Conclusions:The patient privacy protection behavior of clinical nurses is generally at a good level, but there is still room for improvement in individual behaviors. Teaching hospitals should strengthen education related to patient privacy protection for nurses with higher education degree who undertake teaching tasks and nurses in surgical monitoring departments and formulate specific relevant teaching procedures and detailed rules.
8.Changes of Expression of Stretch-activated Potassium Channel TREK-1 mRNA and Protein in Hypertrophic Myocardium

The expression of stretch-activated potassium channel TREK-1 mRNA and protein of hypertrophic myocardium was measured. Using a model of hypertrophy induced by coarctation of abdominal aorta in male Wistar rats, the expression of TREK-1 mRNA and protein was detected by using semi quantitative RT PCR and Western blot respectively. At 4th and 8th week after constriction of the abdominal aorta, rats developed significant left ventricular hypertrophy. As compared to sham operated group, stretch activated potassium channel TREK-1 mRNA was strongly expressed and protein was up regulated in operation groups (P<0.05). It was concluded that the expression of TREK 1 was up regulated in hypertrophic myocardium induced by chronic pressure overload in Wistar rats.
9.Effect of metacognitive training on social cognition and insight of schizophrenic patients.

Objective To investigate the effects of olanzapine combined with metacognitive training on social cognition and insight of schizophrenic patients.Methods One hundred twenty patients with schizophrenia were randomly divided into two groups. All patients received four week treatment. The study gr oup received olanzapine combined metacognitive training treatment and the control group only used olanzapine treatment. The positive and negative symptoms scale (PANSS), the Insight and Treatment Attitude Questionnaire (ITAQ), the Beck Cognitive Insight Scale (BCIS) and Faux Pas Recognition Task (FPRT) were assessed at baseline and after four weeks of treatment. Result There was no statistically significant difference in the scores of all battery between two groups before treatment (P>0.05). the scores of total PANSS and subscales, ITAQ, BCIS and FPRT were statistically significant between study and control groups (P<0.05).After 4 weeks of treatment,the total PANSS scores and subscales scores were significantly lower in the study group than in the control group (P<0.05). The ITAQ score, self-reflection factor of BCIS, composite index and FPRT scores were significantly higher in the study group than in the control group (P<0.05). The difference in self-certainty of BCIS difference was not statistically significant(P>0.05).Differences of the remaining scales were statistically significant(P<0.05). Conclusion Olanzapine combined with metacognitive training has a significant effect on the social cognitive dysfunction in schizophrenic patients,improving the patient's insight and enhancing the social function.
10.Effectiveness of the Five Elements Music Therapy in Relieving Stress with Different Cultural Backgrounds

Objective To explore the effectiveness of the Five Elements Music Therapy(FEMT)in relieving stress in participants with different cultural backgrounds,and to compare the differences between the FEMT and the Western Art Music Therapy(WAMT)in stress relief.Methods This was a comparative pilot randomized pre-post repeated measures study,37 subjects were allocated with 2 dropped out,with an inclusion-ended sample of 35 subjects,23 from Canada and 12 from China.After informed consent,all subjects were randomly assigned to listening to either Five Elements Music or Western Art Music at home for 30 minutes,twice a week for four weeks.Participants were asked to use headphones,measure their pulse rate before and after each session,and fill out five questionnaires,including a background and demographic survey(reporting age,gender,education,cultural background,listening experience).Self-assessment of stress(pre-post after each session),General Hospital Anxiety/Depression Scale(HADS,weekly after 2nd session),Perceived Stress Survey(PSS,weekly after 2nd session),the Music Therapy Intervention Survey(MTIS,pre-post each session).Results ①There was a significant decrease in self-assessed stress scores after the second session in the FEMT compared with the WAMT group(t=-2.057,P=0.046).②In both groups,there was a significant decrease in stress scores pre-post treatment in each group(WAMT t=5.026;FEMT T=7.645,P=0.000).③There was no significant difference between the two groups in post-intervention HADS scores(P>0.05);In the Chinese sample,there was a significant difference in HADS scores after the eighth session in both FEMT and WAMT groups(t=-3.862,P=0.003),and a statistically significant difference in HADS pre-post intervention in the FEMT((t=5.117,P=0.004).There was a significant difference in MTIS pre-post treatment in the WAMT(t=-2.572,P=0.023),but in not the FEMT group(t= 1.331,P=0.207).Conclusion This pilot trial explores a safe and feasible self-administered music therapy approach for stress in two distinct cultural groups,and for the first time provides preliminary comparison and evidence of effectiveness of FEMT and WAMT in alleviating stress and anxiety.Further investigation with bigger randomized samples is needed to elucidate the effects of different kinds of music and cultural groups on stress and anxiety levels.
