1.Quality Analysis of ADR Reports in Our Hospital

OBJECTIVE: To provide reference for the monitoring of adverse drug reactions(ADR).METHODS: All ADR reports collected during 2005~2007 were statistically analyzed and evaluated in accordance with the requirements on the writing of ADR reports in China.RESULTS: Of the 576 ADR reports collected,the average missing items totaled 13.86,12.16 and 13.61 respectively over the 3 years from 2005 to 2007.The number of ADR reports increased year by year,but no significant improvement was noted for the quality of the reports.The poor quality of the ADR reports manifested as serious missing of items,nonstandard writing of the content and the failure to meet standards in the proportion of serious cases etc.CONCLUSION: It is advisable that the special training of ADR reports should be done and the publicity of ADR knowledge should be strengthened so as to improve ADR reporters' cognition level on ADR report quality.
2.Application of Digital Radiography with Dual-energy Subtraction in Coronary Artery Calcification

Objective To explore the clinical value of digital radiography with dual-energy subtraction in diagnosis of coronary artery and valve calcification. Methods 920 patients(older than 70) was examined by GE Revolution XRD with chest dual-energy subtraction. The positive rate of calcification of 146 patients who were highly or generally suspected for coronary artery calcification in bone window's and high Kilovotor digital graph of dual-energy subtraction were compared with the result of 16 slices MSCT. Results 135 patients with highly suspected coronary artery calcification and 11 patients with generally suspected coronary artery calcification in bone window's digital graph were examined by 16 slices MSCT. In those 135 patients with highly suspected coronary artery calcification, 128 cases were proved to be positive with 16 slices MSCT. In those 11 patients with generally suspected coronary artery calcification, 3 cases were proved to be positive with 16 slices MSCT. In those 12 patients who were positive with coronary artery calcification in high Kilovotor graph, 10 patients were confirmed to have coronary artery calcification in MSCT. In those 134 patients who were negative with coronary artery calcification in DDR, 126 patients were confirmed to have coronary artery calcification in MSCT. Conclusion Dual-energy subtraction digital radiography can offer potentially important new information in the evaluation of coronary artery calcium.
3.Study on LipofectinRM mediated murine SCF genetr ansduction into umbilical blood cells

Purpose The aim is to study the transduction of m urine stem cell factor(SCF) into umbilical blood cells by LipofectinRMmediated and expression in them.Methods Murine SCF cDNA encoding extra cellu lar domain was isolated using PCR from plasmid pRC/CMV(containing SCF gene), and then recombined into the expression vector pcDNA3,and transferred into enrichme nt cultural umbilical blood cells. Semi-quantitative RT-PCR was used to examin e the expression on mRNA level.And the effects of supernatant of transfected umb ilical blood cells were investigated alone or in coordinate with GM-CSF by colo ny formation test of human bone marrow cells in semisolid culture.Results SCF mRNA was expressed in transfected umbilical blood cells.The su pernat ant of transfected umbilical blood cells could increase the number of CFU-GM, s ynergizing with GM-CSF.Conclusion The supernatant of umbil ical b lood cells transfected with vector containing mSCF gene can stimulate the colony formation of human bone marrow cells in combination with GM-CSF.
4.Laparoscopic-assisted intubating application of interstitial brachytherapy for locally advanced carcinoma(a report of 6 cases)

Objective To access the clinical value of interstitial brachytherpay in patients with advanced carcinoma. Methods 6 patients(1 case of hepatic cancer,3 cases of prostate cancer,2 cases of local recurrence of rectal cancer)received interstitial high dose rate(Ir-192)brachytherpay with afterloading technique(6.0 Gy/day in 4 days) Tubes were inserted with laparoscopic guidance to minimize the risk of tube misplacement. Results The size of the tumors was reduced and the local control rate was encouraging(6/6). one patient with local recurrence of rectal cancer aquired CR,the ofher five cases acquired PR. Conclusions Interstitial tube insertion with laparoscopic guidance to treat advanced carcinoma in afterloading technique is a simple,minimally invasive,safe and economical method,especially fit for elder patients who intolerate or disagree to operation.
5.The clinical application of endoscopic thyroidectomy.

Objective To explore the method and result of endoscopic thyroidectomy. Methods Thyroidectomy was performed endoscopically in 4 cases of thyroidoma. Results 4 patients underwent endoscopic thyroidectomy, whose operation time was 52,63,70,75 minutes respectively. An average blood loss during operation was 35ml and no complication occurred. They were discharged 4 days~5 days after operation. Conclusions The endoscopic thyroidectomy has the advantages of minimal invasion, less bleeding, less complication and quicker recovery.
6.Effect of insulin combined with Edaravone therapy on cerebral vasospasm after experimental subarachnoid hemorrhage

Objective To investigate the effect of insulin combined with Edaravone therapy on delayed cerebral vasospasm (CVS) after experimental subarachnoid hemorrhage (SAH).Methods 40 New Zealand white rabbits were randomly separated into sham-operated group (n=8),SAH group (n=10),insulin group (n=10) and insulin+Edaravone group(n=12).SAH models were made by injecting blood twice into cisterna magna.30 min later of the first blood injected,the insulin 0.2 U/kg was subcutaneous injected in insulin group and insulin+Edaravone group,3 times daily for 7 d;while,the insulin+Edaravone group received Edaravone 0.5 mg/kg (intravenous injected in ear vein) twice a day for 7 d.7 d after the model made,the basilar artery cross sectional area was measured and the pathological changes were obseved to estimate the degree of CVS.The expression of insulin receptor(InRa) in vascular endothelial cell was detected by immunohistochemical staining.Results The basilar artery cross sectional area between insulin+Edaravone group[(0.46?0.3)mm2]and the sham-operated group[(0.48?0.4)mm2] was no significant difference;but they were obviously bigger than those in SAH group[(0.25?0.3) mm2] and the insulin group [(0.30?0.3)mm2](all P
7.Analysis on readmission by reason of postoperative complications after transurethral prostatectomy

Objective To study the reasons leading to readmission in patients with severe postoperative complications after transurethral prostatectomy. Methods Clinical data of 27 cases of readmission after transurethral resection of prostate (TURP, 14 cases), transurethral vaporization of prostate (TUVP, 3 cases), transurethral laser prostatectomy (TULP, 9 cases) or holmium laser enucleation of prostate (1 case) in this hospital from June 1998 to June 2003 were analyzed. Results Reason leading to readmission included: postoperative severe bleeding in bladder (4 cases), urinary retention (15 cases), urethral stricture (3 cases), bladder neck contracture (2 cases), severe urinary frequency (2 cases) and urinary incontinence (1 case). Conclusions More weight should be placed on the fact that various modes of transurethral prostatectomy may precipitate severe long-term complications.
8.Endoscopic thyroidectomy with bipolar electrocoagulation

Objective To study the feasibility of endoscopic thyroidectomy with bipolar electrocoagulation. Methods Endoscopic partial thyroidectomy via precordial approach, mainly by using bipolar electrocoagulation and scissors, was accomplished in 21 patients with thyroid tumors with a diameter of 0.7~5.5 cm. Results The operation was accomplished endoscopically in 20 patients, whereas a conversion to open thyroidectomy was conducted in 1 patient. The mean incision length was 22 mm, the operation time was 69?47 min, and the blood loss was 55?41 ml. Postoperative analgesic treatment was required in 2 patients and no serious complications happened. Conclusions Endoscopic thyroidectomy with bipolar electrocoagulation is feasible in the absence of ultrasonic scalpel.
9.Hand-assisted laparoscopic colorectal resection: An analysis of 21 cases

Objective To evaluate the feasibility of hand-assisted laparoscopic colorectal resection. Methods Clinical data of 21 cases of hand-assisted laparoscopic colorectal resection in this hospital from October 2003 to August 2004 were retrospectively reviewed. Results The hand-assisted laparoscopic resection was accomplished in 20 cases. The mean operation time was 144 min, the mean blood loss was 120 ml, the mean number of dissected lymph nodes was 8.5, and the mean time to intestinal function recovery, 69 h. Postoperative abdominal bleeding was found in 1 case, which was cured with conservative therapy. A conversion to open surgery was needed in 1 case. Conclusions Hand-assisted laparoscopic colorectal resection safe, feasible, and radical, with advantages of fewer complications and simplicity of performance.
10.Observation on Epigallocatechin Gallate Treating the Bladder Tumor Induced by N-methyl-N-nitrosourea in Rats

Objective To investigate the therapeutic effect of epigallocatechin gallate (EGCG) on the bladder neo-plasm induced by N-methyl-N-nitrosourea (MNU) in Wistar rats. Methods Seventy-two female rats were randomly divid-ed into two groups, treatment group and control group. The rat model of bladder tumor was established by intravesical instilla-tion of MNU (2mL/time, once every two weeks) inside bladder. Rats in treatment group were given EGCG (2 mL/time, once/day), and control group were given the same volume of distilled water. Six rats were executed respectively at the 3rd, 5th, 7th, 9th, 11th and 13th week, and the bladder histopathological changes were studied in two groups. Results The untypical hyper-plasia was observed after 5 weeks. The bladder neoplasm was observed at the 11th week in control group. The bladder neo-plasm was not found in treatment group. There was a significantly lower pathological score in treatment group than that of control group except for the first two weeks (P<0.05). Conclusion EGCG is effective for treatment of the bladder neo-plasm induced by MNU.
